2.Correlation between structural changes of facial muscle after denervation and rehabilitation of facial muscle function

BACKGROUND: It is of significance to imitate facial nerve paralysis of temporal trauma, establish the model of facial nerve paralysis, and study the histopathologic changes of facial muscle after denervation and the effect on rehabilitation of facial muscle function.OBJECTIVE: To investigate the relationship between rehabilitation of facial muscle function and the structural changes of mitochondria and succinate dehydrogenase (SDH) of oris muscles after denervation.DESIGN: A randomized controlled trail based on experimental animals.SETTING: Department of otolaryngology in a hospital of a university.MATERIALS: The experiments were conducted in the Center of Animal Experiment, the Department of Histoembryology, and the Second Electron Microscope(EM) Center, China Medical University. Sixty white guinea pigs provided by the Center of Animal Experiment, China Medical University [certification No: SCXK (liao) 2003-0009 ], were randomly divided into three groups, 5 s squeezed group( n = 15, 10 tested for the function of facial muscle, 5 for EM specimen 15 days after operation), 15 s squeezed group( n =20, 10 tested, 5 for EM specimen 15 days and 30 days after operation), and 30 s squeezed group( n = 25, 10 tested, 5 for EM specimen 15 days after operation and 10 for EM specimen 30 days after operation).INTERVENTIONS: Models of facial paralysis were established. The threshold of facial nerve was detected with electroneurogram(ENoG) before and after squeezed, and the duration of the functional recovery of facial muscle was observed with blink reflex. Orbicularis oris was taken for SDH cytochemical staining, and the ultrastructural changes of SDH positive reaction granules,mitochondria and muscle fibers were observed under transmission electroscope.MAIN OUTCOME MEASURES: ENoG threshold value, tine for recovery of facial muscle function, SDH positive reaction granules and ultrastructural changes of mitochondria and muscle fibers.RESULTS: In 5 s squeezed group, the average inhibition time of nerve excitement conduction was(35.2 ± 16.8) minutes, the structure of mitochondria and the positive reaction granules of SDH remained normal, and the muscle fiber had no pathologic changes. In 15 s squeezed group, it took an average of(47 ± 15) days for blink reflect to recover. In 15 s squeezed group of 15 days, the mitochondria showed slight bubble degeneration and the SDH granules decreased and the muscle fiber had slight pathologic changes, but recovered after 30 days. The mitochondria changed severely and SDH granules decreased significantly in 30 s squeezed group of 15 days and 30 days; no case of blink reflect recovered within' half a year.CONCLUSION: The longer the facial nerve is injured, the nore severely the function of facial muscles is damaged, and the more difficultly it will recover. Therefore, facial nerve decompression should be performed as early as possible.
3.Chromosomal abnormalities and adverse pregnancy outcome with maternal serum second trimester triple screening test for fetal Down syndrome in 4 860 Chinese women

Objective:To investigate the efficiency of maternal serum triple screening for the genetic abnormality in second-trimester and the morbidity of adverse pregnancy outcome in false positive results of the test. Methods: A total of 4 680 pregnant women with singleton pregnancies assigned in Obs & Gyn Hospital, Fudan University, underwent triple screening test (alpha fetoprotein, AFP; human chorionic gonadotropin, HCG and unconjugated estriol, uE3) by fluorescence enzyme immunoassay between 2003 and 2005. The valid MoM (Multiples of Median) value of mid-trimester serum AFP, uE3, and hCG and risk assessments was provided by Beckman Coulter Co. When applied in the prenatal Down syndrome screening service. The study compares the incidence of chromosomal abnormalities with Down syndrome in screen positive women and compares to the MoM value established in the literature. The risks of having a fetus with congenital abnormalities or of developing obstetric complications in the screen positive women with their matched controls.Results:The MoM values for the triple tests of our study are similar to established values of literature. Only 51.01% women with pregnancies agree to receive screening. Amniocentesis utilization rate was 55.12% in the screen-positive pregnancies. The false positive rate was 6.89% and the median of maternal age of the women was 28.13 (range 19 to 49) years old. Chromosomal abnormalities were identified in 21 pregnancies, including 9 cases of trisomy 21.The detection rate was 77.77%. Pregnancies with positive screening results had a significantly higher risk of adverse outcomes than those with negative results (P< 0.05). Whereas there was no difference in the incidences of fetal congenital appearance or skeleton abnormality. Conclusion: Adjusting MoM values of local unaffected populations is limited to increasing the detection rate. Because chromosomal defects have variable exhibitions, amniocentesis utilization is still a choice for screen-positive pregnancies. Screen-positive pregnancies had increased risk of chromosomal abnormalities.
4.Human synovial fluid promotes directed differentiation of bone marrow mesenchymal stem cells

BACKGROUND:Nowadays, growth factors are commonly used to induce bone marrow mesenchymal stem cells. However, this is a high-cost method with a great amount of growth factors. In addition, the chondrogenic potential of bone marrow mesenchymal stem cells wil decrease significantly with increasing times of culture. OBJECTIVE:To observe the directed differentiation of bone marrow mesenchymal stem cells co-cultured with human synovial fluid. METHODS:Human bone marrow mesenchymal stem cells were isolated and cultured by adherence screening method. The synovial fluid of the knee was aspirated from healthy volunteers by aseptic operation. Passage 3 human bone marrow mesenchymal stem cells were co-cultured with the fol owing media:synovial fluid+complete medium;synovial fluid+bone marrow mesenchymal stem cells+complete medium;bone marrow mesenchymal stem cells+complete medium. The morphology and growth of the cells were observed under an inverted microscope every day. At days 7, 14 and 21 of induction, toluidine blue staining and immunocytochemical staining were performed.
RESULTS AND CONCLUSION:After co-culture with human synovial fluid, human bone marrow mesenchymal stem cells proliferated slowly, and varied from fusiform to oval or polygonal;toluidine blue and col agen II staining were positive. These findings indicate that the synovial fluid has a positive role in the chondrogenic differentiation of bone marrow mesenchymal stem cells. The synovial fluid may contain substances that promote the chondrogenic differentiation of bone marrow mesenchymal stem cells.

Qinghaosu I and III are new sesquiterpenes. Five main constituents, camphene, iso-artemisia ketone, 1-camphor, β-carophyllene, and β-pinene were identified from the volatile oil of this herb.

7.Treatment of medium and late stage esophageal carcinoma with combined endoscopic metal stenting and radiotherapy.

OBJECTIVETo evaluate clinical feasibility and efficacy of endoscopic metal stenting combined with radiotherapy for treatment of medium and late stages of esophageal carcinoma.
METHODSThirty-four patients of late stage esophageal carcinoma were treated with endoscopic stent implantation in combination with radiotherapy. Evaluations of CES stainless steel metal stent on the effect of radiation, clinical symptom alleviation and complication and survival rates in both groups were made.
RESULTS(1) CES stainless steel metal stent had no effect on radiation dosage distribution. (2) Dysphagia was markedly alleviated in both groups and different complication rates were observed between the two groups. (3) Six, nine and twelve month survival rates of inpatients treated with combined therapies were significantly higher than in simple stent implantation patients.
CONCLUSIONEndoscopic metal stenting in combination with radiotherapy was a feasible and practical management in treating medium and late stages esophageal carcinoma and was superior to simple metal stent implantation.

8.Clinical characteristics and risk factors of dialysis catheter-related infection in CRRT patients

Objective To explore the clinical characteristics and risk factors of catheterrelated infection in continuous renal replacement therapy (CRRT) patients.Methods The demographic and clinical data of CRRT patients who inserted with double-lumen non-cuffed dialysis catheter at the First Affiliated Hospital of Sun Yat-sen University from January 1,2016 to December 31,2016 were collected.According to the presence or absence of catheter-related infections,they were divided into infected group and uninfected group.Statistics and analysis of the incidence and pathogenic characteristics of catheter-related infections;Comparison of clinical features of infected and uninfected groups;A multivariate Cox proportional hazard model was used to analyze risk factors for catheter-related infections.Results A total of 364 patients with CRRT (437 cases of central venous catheterization) were enrolled in the study.Catheter-related bloodstream infection (CRBSI) and catheterrelated colonization (CRCOL) rates were 3.565 and 2.228 events per 1000 catheter-days.These catheters were associated with higher proportion of inserted in ICU (P=0.007),immunosuppression (P=0.002),receive catecholamine inotropes therapy (P=0.001) and shock (P=0.030).The infection catheters also had shorter indwelling time (P=0.032) and lower level of blood hemoglobin (P=0.017),serum creatinine (P=0.004),blood brain natriuretic peptide (P=0.005) pericatheter use.The most common pathogens were Gram-negative bacteria,especially Acinetobacter baumannii,which caused 37.5% CRBSI and 20.0% CRCOL.Multivariate Cox regression model showed female (P=0.029,HR=2.151),diabetes (P=0.016,HR=2.807),receive catecholamine inotropes therapy (P=0.012,HR=2.655),immunosuppression (P=0.037,HR=2.203) were independent risk factors associated with catheterrelated infection.Conclusions The incidence of CRBSI and CRCOL is 3.565 and 2.228 events per 1000 catheter-days CRRT patients in our hospital.The most common pathogen of catherter-related infection is Gram-negative bacteria.Female,diabetes,received catecholamine inotropic drugs,and immunosuppression were independent risk factors associated with catheter-related infection.
9.Content determinnation of chlorogenic acid and linarin in Yejuhua Granules

Objective:To establish a method for determintation of chlorogenic acid and linarin in Yejuhua granules by HPLC.Methods:We applied HPLC methods. The Kromasil 100-5 C18 column (250 mm×4.6 mm,5 μm) was used, the mobile phase was acetonitrile-0.4%H 3PO 4 solution (gradient elution), the flow rate was 1.0 ml/min, the dection wavelenghth was 334 nm and the column temperture was 32 ℃. Results:Chlorogenic acid and buddleoside had good linearity in the ranges of 0.30-1.50 μg ( r2=0.999 1) and 0.12-0.62 μg ( r2=0.999 8), respectively. The average recoveries were 99.70% and 96.67%, with RSD<2%, respectively. Conclusion:The method is simple, rapid, reliable, efficient, and can be used for determination of chlorogenic acid and buddleoside in Yejuhua Granules.
10.Expression and clinical significance of breast cancer metastasis suppressor 1 mRNA in supraglottic laryngeal carcinoma.

OBJECTIVE:
To investigate the expression of breast cancer metastasis suppressor 1 (BRMS1) mRNA in supraglottic cancer and to evaluate its clinical significance.
METHOD:
The expression of BRMS1 mRNA was examined by using RT-PCR method which take beta-actin mRNA as reference template in 66 cases of supraglottic cancer tissues and their adjacent normal mucosa tissues (ANT).
RESULT:
The expression of BRMS1 mRNA in the tissues of supraglottic cancer is lower significantly than that in the tissues of ANT ( P<0.05). There is correlation between BRMS1 mRNA expression and the clinical stage, differentiation and cervical lymph node metastasis in the laryngeal supraglottic cancers (P<0.05). There is no correlation between BRMS1 mRNA expression and sex and age.
CONCLUSION
Expression of BRMS1 mRNA in supraglottic cancer is lower than that in adjacent normal mucosa. The decrease of BRMS1 mRNA expression may be related to clinical stage and low differentiation and lymph node metastasis of supraglottic laryngeal cancer.
