1.The results analysis of intervention on fall among the elderly in Niujie area of Beijing

Objective To explore the current status and influencing factors of fall among the elderly in Niujie area of Beijing so as to provide evidences for prevention. Methods Based on the basic survey of fall in 1703 elderly people, the people were divided into intervention group and control group according to different habitation areas using cluster sampling method. The elderly people in intervention group of 874 were accepted intervention management for one year, including cognitive behavioral intervention, fall-inducing diseases intervention and intervention of elderly static and dynamic behaviors that affected the balance function, so as to help the elderly to master the relevantfactors of fall and preventive measures. Results The annual occurrence rate of fall was 11.7% (200/1703) and the fall injury incidence was 9.0% (152/1703). The prevalence of fall was increased with aging, and it was high in people with balance dysfunction. After one year intervention, the incidence of fall was decreased to 5.2%, while it was 7.6% in control group. Conclusions Cognitive behavior intervention can effectively prevent fall in the elderly, which has an important significance for improving the life quality of the elderly.
2.Analysis of auxiliary therapeutic effect of community exercise intervention on hypertension and diabetes mellitus

Objective:To explore auxiliary therapeutic effect of exercise intervention on hypertension and/or diabetes mellitus (DM) .Methods :A total of 456 patients with hypertension and/or DM from community were selected and randomly equally divided into intervention group (n=228 ,wore exercise amount monitor ,and lifestyle disease inte‐grated control software was used to monitor exercise ,and unique person provided quantitative exercise guidance for them for three months and they were followed up for eight times ) and control group (n=228 ,did routine activity as usual) .Body weight ,BMI ,waist circumference (WC) ,blood pressure ,blood sugar ,blood lipid etc .and exercise a‐mount were measured and compared between two groups before and after intervention .Results:Compared with be‐fore intervention ,there were no significant changes in all above indexes in control group after intervention , P>0.05 all;there were significant reductions in body weight ,BMI ,WC ,blood pressure ,blood sugar ,blood lipid etc . in intervention group after intervention ( P< 0.01 all) .After intervention ,compared with control group , there were significant reductions in body weight [ (68.28 ± 8.43) kg vs . (65.93 ± 10.43) kg] ,blood pressure [ (124.32 ± 13.70 /77.81 ± 8.22) mmHg vs . (120.94 ± 8.35 /74.58 ± 5.76) mmHg] ,HbA1c [ (7.12 ± 1.53) mmol/L vs . (6.73 ± 1.04) mmol/L] ,LDL‐C [ (2.72 ± 0.40) mmol/L vs . (2.46 ± 0.67) mmol/L] ,and significant rose in total exercise amount [ (537.85 ± 338.63) kcal vs .(637.15 ± 447.65) kcal] ,effective exercise amount [ (229.46 ± 239.04) kcal vs . (323.56 ± 257.84) kcal] in intervention group ,P<0.01 all .Conclusion:Exercise can improve ex‐ercise capacity ,reduce blood pressure ,blood sugar ,blood lipid levels ,is help to disease control and quality of life improvement in patients with hypertension and/or diabetes mellitus .

α-cells, which synthesize glucagon, also support β-cell survival and have the capacity to transdifferentiate into β-cells. However, the role of α-cells in pathological conditions and their putative clinical applications remain elusive due in large part to the lack of mature α-cells. Here, we present a new technique to generate functional α-like cells. α-like cells (iAlpha cells) were generated from mouse fibroblasts by transduction of transcription factors, including Hhex, Foxa3, Gata4, Pdx1 and Pax4, which induce α-cell-specific gene expression and glucagon secretion in response to KCl and Arg stimulation. The cell functions in vivo and in vitro were evaluated. Lineage-specific and functional-related gene expression was tested by realtime PCR, insulin tolerance test (ITT), glucose tolerance test (GTT), Ki67 and glucagon immunohistochemistry analysis were done in iAlpha cells transplanted nude mice. iAlpha cells possess α-cell function in vitro and alter blood glucose levels in vivo. Transplantation of iAlpha cells into nude mice resulted in insulin resistance and increased β-cell proliferation. Taken together, we present a novel strategy to generate functional α-like cells for the purposes of disease modeling and regenerative medicine.

4.Optimization of extraction technology for couplet medicinals of Astragalus membranaceus-Puerariae lobata by multi-index comprehensive scoring method combined with orthogonal experiment

OBJECTIVE To optimize extraction technology of couplet medicinals of Astragalus membranaceus-Puerariae lobatae. METHODS With contents of puerarin,daidzin,calycosin-7-O-β-D-glucopyranoside,daidzein,calycosin and formononetin and the yield of dry extract as index,the analytic hierarchy method was used to determine the weight coefficient of each index and calculate the comprehensive score. The effects of solid-liquid ratio, extraction times and extraction time on the comprehensive score were investigated by single factor test. The level of each factor was determined. By multi-index comprehensive scoring method, using comprehensive scores of above 7 indexes as indexes,the extraction technology of couplet medicinals of A. membranaceus-P. lobata was optimized by orthogonal experiment,and the validation tests were conducted. RESULTS The weight coefficient for the contents of puerarin,daidzin,calycosin-7-O-β-D-glucopyranoside,daidzein,calycosin and formononetin and the yield of dry extract were respectively 0.304 7,0.065 2,0.185 8,0.185 8,0.107 8,0.107 8 and 0.042 7. The optimal extraction technology was determined as follows: solid-liquid of 1∶8(g/mL),extracting 3 times and for 1 h each time. RSD of each evaluation index in the validation test results was lower than 3.00% (n=3). CONCLUSIONS The optimized extraction technology for A. membranaceus-P. lobata is stable and feasible.

Osteoporotic thoracolumbar fracture in the elderly will seriously reduce their quality of life and life expectancy. For osteoporotic thoracolumbar fracture in the elderly, spinal reconstruction is necessary, which should comprehensively consider factors such as the physical condition, fracture type, clinical characteristics and osteoporosis degree. While there lacks relevant clinical norms or guidelines on selection of spinal reconstruction strategies. In order to standardize the concept of spinal reconstruction for osteoporotic thoracolumbar fracture in the elderly, based on the principles of scientificity, practicality and progressiveness, the authors formulated the Clinical guideline for spinal reconstruction of osteoporotic thoracolumbar fracture in elderly patients ( version 2022), in which suggestions based on evidence of evidence-based medicine were put forward upon 10 important issues related to the fracture classification, non-operative treatment strategies and surgical treatment strategies in spinal reconstruction after osteoporosis thoracolumbar fracture in the elderly, hoping to provide a reference for clinical treatment.
6.Establishment of HPLC Fingerprint of Schisandra sphenanthera and S. chinensis and Analysis of Chemical Pat- tern Recognition

OBJECTIVE:To establish HPLC fingerprint of Schisandra sph enanthera and S. chinensis,and to analyze chemical pattern recognition. METHODS :HPLC method was adopted. Using schizandrin A as reference ,HPLC fingerprints of 10 batches of S. sphenanthera and S. chinensis (N1-N10,S1-S10) were drawn. Similarity Evaluation System of TCM Chromatographic Fingerprint(2012 edition)was adopted for similarity evaluation to determine the common peaks. SPSS 20.0 and SIMCA 14.1 software were used for HCA ,unsupervised madel of PCA ,supervised model of OPLS-DA. Using variable importance projection (VIP)value greater than 1 as the standard ,the differential markers that affected the quality of S. sphenanthera and S. chinensis were screened. RESULTS :S. sphenanthera and S. chinensis were identified 32 and 33 common peaks ,respectively. The similarity of 10 batches of S. sphenanthera and 10 batches of S. chinensis were all higher than 0.9,and the similarity of S. sphenanthera and S. chinensis was 0.05. A total of 19 characteristics peaks were identified ,among which five common peaks were identified as schisandraol A ,schisandraol B ,schisantherin A ,schizandrin A and schisandrin B by reference. HCA results showed that N 1-N10 were clustered into one category ,and S 1-S10 were clustered into one category ,of which N 1,N3,N8,and N 9 were clustered into one category ,and the rest were clustered into one category ;S1,S3,S6,and S 9 were grouped together ,and the rest were grouped together. The results unsupervised model of PCA showed that the cumulative variance contribution rate of the first two principal component factors was 87.20%. Supervised model of OPLS-DA showed that schizandrin A ,schisandraol A ,schisantherin A and schisandrin B were the differential markers that affected 、the quality of S. sphenanthera and S. chinensis (VIPs were 2.29,2.24,1.73,1.48,respectively). 7.Quality evaluation of Guiqi tongmai mixture based on fingerprint and chemical pattern recognition

OBJECTIVE To establish the fingerprint of Guiqi tongmai mixture and evaluate its quality in combination with chemical pattern recognition . METHODS Using narirutin as reference peak ,HPLC fingerprint of Guiqi tongmai mixture was established with Similarity Evaluation System of Chromatographic Fingerprint of TCM (2012 edition). Common peaks identification and similarity evaluation were conducted . Hierarchical cluster analysis (HCA),principle component analysis (PCA)and orthogonal partial least square -discriminate analysis (OPLS-DA)were performed ,and the value of variable importance in the projection (VIP) greater than 1 was taken as the standard to screen the differential markers affecting the quality of Guiqi tongmai mixture . RESULTS Nineteen common peaks in the HPLC fingerprints of thirteen batches of Guiqi tongmai mixture were calibrated ,and the similarities of them were 0.928-0.990;eight common peaks were identified ,including the peak 7(hydroxysafflor yellow A ),peak 8(albiflorin),peak 9(paeoniflorin),peak 10(calycosin-7-O-glucoside),peak 12(liquiritin),peak 14(ammonium glycyrrhizinate ), peak 16(narirutin)and peak 19(hesperidin). According to the results of HCA ,thirteen batches of samples could be clustered into three categories :S5,S9 and S 11-S13 were clustered into one category ;S4 and S 8 were clustered into one category ;S1-S3,S6- S7 and S 10 were clustered into one category . The results of PCA indicated that the accumulative variance contribution rate of the principle components 1-7 was 92.115%. The results of OPLS -DA showed that VIP values of narirutin ,paeoniflorin,liquiritin, ammonium glycyrrhizinate ,the components corresponding to peak 11 and peak 1 were greater than 1. CONCLUSIONS HPLC fingerprint of Guiqi tongmai mixture is established ,and it could be used for quality evaluation of Guiqi tongmai mixture by combining with chemical pattern recognition techniques . Six components such as narirutin are differential markers affecti ng the quality of Guiqi tongmai mixture .
