1.Community-acquired bacteria bloodstream sepsis in children:Analysis of pathogens and clinical characteristics in PICU

Objective To explore the relationship between pathogen and disease characteristics of community-acquired bacterial bloodstream sepsis in PICU.Methods The clinical data of children diagnosed as sepsis admitted to the PICU of Shengjing Hospital of China Medical University between June 2011 and June 2016 were retrospectively analyzed.Medical records were screened to confirm the diagnosis of commu-nity-acquired bacteria bloodstream sepsis.Results One hundred and ninety-one children(108 males and 83 females) had confirmed community-acquired bacteria bloodstream sepsis with the following characteristics:167 patients(87.4%) with the age less than 36 months,50 patients(26.2%) with co-existing disease,40 pa-tients(20.9%) with shock,of which 33 patients had septic shock,and 41 patients(21.5%) died.Gram-posi-tive bacteria was the predominant pathogen(73.3%,140/191).Streptococcus pneumoniae was the primary pathogen(19.9%,38/140). Conclusion Community-acquired bacteria bloodstream sepsis in PICU most commonly affects children less than 36 months of age.Gram-positive bacteria are the predominant pathogenic bacteria,the most common pathogens are Streptococcus pneumoniae,Staphylococcus epidermidis,Staphylo-coccus aureus,Streptococcus pyogenes and Escherichia coli.The highest mortality rate in septic children is caused by Pseudomonas aeruginosa.Children with co-existing disease,sepsis complicated with shock and pa-tients requiring mechanical ventilation are risk factors for death.
2.Advances of long noncoding RNAs in myocardial fibrosis

Accumulating studies have recently shown that long noncoding RNAs (lncRNAs) are involved in the initiation and progression of myocardial fibrosis,a common histological characteristic of heart conditions and prominent global health issues. LncRNAs are prominently served as regulatory molecules via interaction with DNA,RNA and proteins in transcriptional and post-transcriptional processes. They can change morphological structure and biochemical metabolism of cardiac cells and regulate homeostasis of the cardiac extracellular matrix. Therefore,lncRNAs show great potential as diagnostic and prognostic biomarkers and therapeutic targets for anti-fibrotic treatment.
3.Research progress of mesh-related visceral complications after tension-free inguinal hernia repair

Mesh-related visceral complications caused by mesh erosion after tension-free inguinal hernia repair are one kind of rare long-term complications, but they are easily neglected. Interval time from initial hernia repair to mesh-related visceral complications by preperitoneal and laparoscopic repair is short. Rutkow and transabdominal preperitoneal repair have the highest reported rate. Lichtenstein has the longest interval time and the lowest reported rate. The most frequently eroded organs are sigmoid colon, bladder and small intestine. The common clinical manifestations of sigmoid colon erosion are hematochezia, abdominal wall fistula and colitis, hematuria and recurrent urinary tract infection in bladder erosion cases, intestinal obstruction and abdominal wall fistula in intestinal erosion case, sigmoid-bladder fistula and intestinal-bladder fistula in multiple organ erosion cases. Resection or repair of corresponding organs with mesh removal have good efficacies in most patients. The authors summarize and analyze researches on mesh-related visceral complications after tension-free inguinal hernia repair from 1994 to 2021, review their advances, in order to raise awareness of such complications in clinicians.
4.Electro-acupuncture promotes repair of rotator cuff injury in rats.

OBJECTIVE:
To observe the therapeutic effect of electro-acupuncture on tendon healing and functional recovery of rotator cuff injury in rats and explore the therapeutic mechanism of electro-acupuncture.
METHODS:
Ninety SD rats were randomly divided into electro-acupuncture group, model group and blank control group, and models of rotator cuff injury were established in the former two groups.The rats in electro-acupuncture group was treated with electro-acupuncture after the operation, and those in the other two groups received no treatment.The right forefoot thermal withdrawal latency (TWL), the contents of IL-1β, IL-6 and TNF-α in the synovial fluid and the maximum tension load of supraspinatus tendon were measured at 2, 4 and 8 weeks after the operation.
RESULTS:
TWL in the model group was significantly lower than that in the blank control group and electro-acupuncture group at 2, 4 and 8 weeks after the operation (
CONCLUSIONS
Electro-acupuncture treatment not only effectively reduces the expression of inflammatory factors to relieve pain, but also promotes the repair of damaged tissue to improve the biomechanical properties of rotator cuff in the rat models.
