1.A study on constructing a clinical nursing model of happiness and natural affection in children

Objective To decrease the unhappiness of children and make them happy to the utmost in their process of seeking medical help by constructing a clinical nursing model of happiness and natural affection. Methods During the clinical process of setting up a suitable surrounding for children's hospi-tal visit and hospitalization, and finding a pleasant method to meet children's psychological needs, indexes of physiological response pre and post admission, discharge and painful operations including surgeries were recorded. Parameters of psychological reactions such as facial expressions, movement reactions and emo-tions were observed and changes of medical compliance were analyzed. We also evaluated variation of fear, time of disease rehabilitation, the satisfaction degree of parents and doctors. Results Child's fear was lowered obviously, cooperation and compliance of treatment were improved remarkably, time of disease re-habilitation was shortened to different degree, the satisfaction degree of patients and doctors had been im-proved to above 98%. Conclusions The clinical nursing model of happiness and natural affection for children can decrease the unhappiness of children and make them happy to the utmost in their process of seeking medical help.
2.Agrobacterium tumefaciens-mediated transformation of Aureobasidium pullulans and high-efficient screening for polymalic acid producing strain.

To develop a genetic transformation method of Aureobasidium pullulans and T-DNA insertion for high-efficient screening of polymalic acid (PMA) producing strain. Agrobacterium tumefaciens-AGL1, containing the selection genes encoding hygromycin B phosphotase or phosphinothricin acetyltranferase, was used to transform Aureobasidium pullulans CCTCC M2012223 and transformants were confirmed by colony PCR method. Transferred DNA (T-DNA) insertional mutants were cultured in microwell plate, and screened for high-titer PMA producing strain according to the pH response model. DNA walking was used to detect the insertion sites in the mutant. Results show that the selection markers could stably generated in the transformants, and 80 to 120 transformants could be found per 10(7) single cells. A high-titer PMA mutant H27 was obtained, giving a good PMA production caused by the disruption of phosphoglycerate mutase, that increased by 24.5% compared with the control. Agrobacterium tumefaciens-mediated transformation and high-efficient screening method were successfully developed, which will be helpful for genetic transformation of Aureobasidium pullulans and its functional genes discovery.

3.Mechanism responsible for pulmonary fibrosis induced by concomitant chronic smoke exposure and pentoxifylline administration

AIM:To investigate the impact of long-term administration of pentoxifylline (PTX) on morphology and inflammation of the lung in mouse models with chronic exposure of cigarette smoke. METHODS: Male BALB/c mice were randomized into the following four study groups: smoke-exposure only, shamed smoke-exposure, smoke-exposure and PTX administration, shamed smoke-exposure and PTX administration. Animals assigned to smoke-exposure were put inside a chamber twice a day for cigarette smoke exposure. The oral dose of PTX allocated to each mouse was about 20 mg?kg-1?d-1. Animals were sacrificed anaesthetically at day 120. Slices of lung were stained with H&E for pathological analysis. Modified ashcroft pulmonary fibrosis score (mAPFS) was estimated, and IFN-? (a Th1 cytokine), IL-4 (a Th2 cytokine) in broncho-alveolar lavage fluid (BALF) and hydroxyproline in mouse lung tissue were measured by commercial kits of ELISA assay. RESULTS: Lungs in smoke-exposure only group exhibited emphysema-like morphology, low mAPFS (median 1.50, 95%CI 1.25-3.75), lowest hydroxyproline (2.43?0.11) mg/L and lowest ratio of IL-4 to IFN-? (20.3?25.5), whereas lungs in smoke-exposure and PTX interference group exhibited interstitial fibrosis-like morphology, highest mAPFS (4.75, 4.09-5.71), highest hydroxyproline (5.57?0.55) mg/L and highest ratio of IL-4 to IFN-? 4.Investigation on degeneration of outer hair cells in guinea pig.

OBJECTIVE:
To investigate the degeneration mechanics of outer hair cells in guinea pig.
METHOD:
The mechanics of outer hair cells isolated by enzyme were observed under inverted microscope for 6-8 h continuously.
RESULT:
Over half of living outer hair cells could keep good conditions in 6 hours. During the degeneration there was always a longitudinal fold line from tip to base. Presence or absence of calcium, as well as lossing of stereociliary bundle, couldn't change the conditions of out hair cells.
CONCLUSION
Neither calcium nor stereociliary bundle is the decisive cause in keeping outer hair cells alive, and its degeneration may be basically related with something surrounding the cell.

5.Analysis of self-efficacy and influencing factors of male nurses

Objective:To investigate and analyze the current situation and influencing factors of professional decision-making self-efficacy of male nurses.Methods:Convenience sampling was used to select 133 male nurses from September to October 2020 in Shenzhen City as the research objects. The Self-efficacy Scale for Career Decision-making was used to conduct self-evaluation, career information collection, career goal selection, career planning formulation, and job-selection problem resolution.Results:Attitudes towards nursing majors, academic qualifications, reasons for applying for nursing majors, monthly family income, whether it is an only child, family residence, work status, number of job changes, mother′s education level, and married or not were the factors that affect male nurses′ professional self-efficacy ( t values were -1.989-12.523, F values were 7.476-325.316, P<0.05 or 0.01). Conclusion:Career decision-making self-efficacy has a good guiding role in the career selection, development and planning of male nurses. Medical units should formulate reasonable training methods or related training for new male nurses entering the clinic, so as to increase male nurses′ recognition of their occupations, thereby enhancing them Career decision-making self-efficacy.
6.Pathological changes of JNET classification of colorectal tumors

Objective:To investigate the pathological changes of JNET classification of colorectal tumors.Methods:Data of patients with colorectal neoplasms who underwent narrow-band imaging combined with magnifying endoscopy, and whose postoperative pathological specimens were obtained at the endoscopy center of the Affiliated Hospital of Guizhou Medical University from January 2015 to June 2018 were analyzed retrospectively. The endoscopic JNET diagnosis and pathological features (surface pit epithelial exfoliation, surface mucosal necrosis structure, surface mucosal ethmoid reticular structure, fibrous tissue reactive hyperplasia, mucosa muscle without residual and carcinomatous interstitial reaction) were analyzed.Results:A total of 81 patients with colorectal neoplasms (diameter >1.5 cm) were enrolled with 74 cases receiving endoscopic treatment and seven cases receiving surgical treatment. The frequency of surface mucosal sieve reticular structure in type 3 of JNET (20.00%) was lower than that in type 2B (42.86%), which was in turn higher than that in type 2A (2.78%), with significant differences ( P<0.05). The frequencies of fibrous tissue reactive proliferation, no residual mucosal muscles, and carcinomatous interstitial reaction in type 2A and 2B were lower than that of type 3, with significant differences (all P<0.05). There were significant differences in the consistency group and inconsistency group of JNET classification and pathological diagnosis in surface pit epithelial exfoliation and surface mucosal necrosis (all P<0.05). Conclusion:Type 2B of JNET classification indicates that the pathological features may be the surface mucosal reticular structure; type 3 indicates reactive hyperplasia of fibrous tissue, no residual mucosal muscles and cancerous interstitial reaction. The exfoliation of the surface pit epithelium and the necrosis of the surface mucosa may be the pathological interference factors affecting the accuracy of JNET classification diagnosis.
8.Risk factors for postoperative hypoxemia in patients with Stanford type A aortic dissection: A systematic review and meta-analysis

Objective To systematically evaluate the risk factors for hypoxemia after Stanford type A aortic dissection (TAAD) surgery. Methods Electronic databases including PubMed, EMbase, The Cochrane Library, Web of Science, CNKI, Wanfang Data, VIP and CBM were searched by computer to collect studies about risk factors for hypoxemia after TAAD published from inception to November 2021. Two authors independently assessed the studies' quality, and a meta-analysis was performed by RevMan 5.3 software. Results A total of 19 case-control studies involving 2 686 patients and among them 1 085 patients suffered hypoxemia, included 21 predictive risk factors. The score of Newcastle-Ottawa scale≥7 points in 16 studies. Meta-analysis showed that: age (OR=1.10, 95%CI 1.06 to 1.14, P<0.000 01), body mass index (OR=1.87, 95%CI 1.49 to 2.34, P<0.000 01), preoperative partial pressure of oxygen in arterial blood/fractional concentration of inspiratory oxygen (PaO2/FiO2)≤300 mm Hg (OR=7.13, 95%CI 3.48 to 14.61, P<0.000 01), preoperative white blood cell count (OR=1.34, 95%CI 1.18 to 1.53, P<0.000 1), deep hypothermic circulatory arrest time (OR=1.33, 95%CI 1.14 to 1.57, P=0.000 4), perioperative blood transfusion (OR=1.89, 95%CI 1.49 to 2.41, P<0.000 01), cardiopulmonary bypass time (OR=1.02, 95%CI 1.00 to 1.03, P=0.02) were independent risk factors for hypoxemia after TAAD surgery. Preoperative serum creatinine, preoperative myoglobin, preoperative alanine aminotransferase were not associated with postoperative hypoxemia. Conclusion Current evidence shows that age, body mass index, preoperative PaO2/FiO2≤300 mm Hg, preoperative white blood cell count, deep hypothermic circulatory arrest time, perioperative blood transfusion, cardiopulmonary bypass time are risk factors for hypoxemia after TAAD surgery. These factors can be used to identify high-risk patients, and provide guidance for medical staff to develop perioperative preventive strategy to reduce the incidence of hypoxemia. The results should be validated by higher quality researches.

Group A Streptococcus (GAS) is a very important pathogen, especially for children.On a global scale, GAS is an important cause of morbidity and mortality.But the burden of disease caused by GAS is still unknown in China and also has not obtained enough attention.For this purpose, the expert consensus is comprehensively described in diagnosis, treatment and prevention of GAS diseases in children, covering related aspects of pneumology, infectiology, immunology, microbiology, cardiology, nephrology, critical care medicine and preventive medicine.Accordingly, the consensus document was intended to improve management strategies of GAS disease in Chinese children.
10.Self-sufficient nanoparticles with dual-enzyme activity trigger radical storms and activate cascade-amplified antitumor immunologic responses.

Radiotherapy (RT) can potentially induce systemic immune responses by initiating immunogenic cell death (ICD) of tumor cells. However, RT-induced antitumor immunologic responses are sporadic and insufficient against cancer metastases. Herein, we construct multifunctional self-sufficient nanoparticles (MARS) with dual-enzyme activity (GOx and peroxidase-like) to trigger radical storms and activate the cascade-amplified systemic immune responses to suppress both local tumors and metastatic relapse. In addition to limiting the Warburg effect to actualize starvation therapy, MARS catalyzes glucose to produce hydrogen peroxide (H2O2), which is then used in the Cu+-mediated Fenton-like reaction and RT sensitization. RT and chemodynamic therapy produce reactive oxygen species in the form of radical storms, which have a robust ICD impact on mobilizing the immune system. Thus, when MARS is combined with RT, potent systemic antitumor immunity can be generated by activating antigen-presenting cells, promoting dendritic cells maturation, increasing the infiltration of cytotoxic T lymphocytes, and reprogramming the immunosuppressive tumor microenvironment. Furthermore, the synergistic therapy of RT and MARS effectively suppresses local tumor growth, increases mouse longevity, and results in a 90% reduction in lung metastasis and postoperative recurrence. Overall, we provide a viable approach to treating cancer by inducing radical storms and activating cascade-amplified systemic immunity.
