BACKGROUND: Intravascular stent implantation is one of the main methods for intra-extracranial artery stenosis. The safety and efficacy of stent implantation has been elevated, and symptoms or ischemic advert events with artery stenosis has been reduced. However, risks of thrombosis, cerebralhemorrhage, restenosis, and hyperperfusion still existed. OBJECTIVE: To investigate the influence of atorvastatin on ischemic adverse events following cerebral middle artery stent implantation.METHODS: Twenty-four patients received cerebral middle artery stent implantation were randomly divided into 2 groups. In the treatment group, patients were received atorvastatin 40 mg + plavix 75 mg + aspirin enteric-coated tablet 300 mg, once per day; in the control group, patients were received plavix 75 mg + aspirin enteric-coated tablet 300 rag, once per day. Ischemic adverse events, such as in-stent restenosis, transient ischemic attack, cerebral infarction, or re-interventional therapy were observed by transcranial Doppler, digital subtraction angiography, nuclear magnetic resonance imaging, diffusion weighted imaging, blood lipid level and C-reactive protein level examinations prior to and at months 1, 3, 6 and 12 after stent implantation.RESULTS AND CONCLUSION: Compared with the control group, the levels of blood lipid and C-reactive protein were obviously decreased at months 1, 3, 6 and 12 after treatment (P<0.05, P<0.01 ), with dramatically declined ischemic adverse events (P<0.05, P<0.01 ). Compared with before operation, the levels of blood lipid and C-reactive protein were decreased after operation in the treatment group (P<0.05, P<0.01). No server adverse events occurred in patients treated by atorvastatin, which showed 40 mg was safe for patients. The results revealed that atorvastatin combined with antiplatelet therapy can prevent ischemic adverse events following cerebral middle artery stent implantation.

AIM: To observe the efficacy, feasibility and safety of endovascular stent-grafts in treating aortic dissection.METHODS: Six patients with aortic dissecting aneurysm (3 males and 3 females, aged 32-67 years, average of 50 years old) were selected from Department of Cardiology, First Affiliated Hospital of Gannan Medical College between March 2005 and March 2006. They were diagnosed by transthoracic magnetic resonance imaging (MRI) and aortic angiography, and as type Ⅲ aortic dissecting aneurysm according to Debakey typing. The tumor diameter was ranged from 3.2 to 4.3 cm. The informed consent was obtained from all patients. Two weeks after the onset, the blood pressure was control between 90-110 and 60-70 mm Hg and heart rate at 60-65 times/min, then the straight circular and bifurcated stents, the product of Yuhengjia Company, composed of single naked membrane, straight circular and bifurcated stents made of nickel-titanium memory alloy, and cover of domestic artificial blood vessel were implanted with the patients generally anesthetized. The aortic dissection and general condition of patients after operation were observed with angiography, and the patient recovery condition was examined by spiral CT at postoperative months 3 and 12.RESULTS: All 6 cases were involved in the result analysis.①The diameters of stents for the patients were appropriate. The angiography showed that the thoracic aortic dissection in 4 cases disappeared, and thrombogenesis was found in false lumen; the thoracic aortic dissections in 2 cases disappeared, however, a little contrast agent was found in false lumen, but the blood flow was very slow. The general condition of all patients was good with stable vital signs, all patients discharged after 7-10 days.②CT results at 3 month after operation suggested that the original false lumen significantly contracted, and the true lumen restored to normal level. In case 3, there was mural thrombus in the distal stent that connected with stent vessel wall, and the thrombus hyperplasia was not obvious during follow up. At 12 months after operation, there was no distinct alteration in the stent location and appearance, and the life quality of patients was significantly improved.CONCLUSION: Endovascular stent-grafts are effective and safe in treating aortic dissection.

Objective To study the effects of bone marrow mononuclear cells (MNCs) transplantation on left ventricular (LV) remodeling and its potential mechanism in swine myocardial infarction models. Methods The left anterior descending coronary arteries of swines were obstructed by balloon to create myocardial infarction models. Three weeks later, MNCs(n= 7)or PBS(n=5) were injected into the infarction related coronary arteries through balloon catheter. The cardiac function were measured by echocardiography and ventriculargraphy. Collagen amount was also assessed at 4 weeks after transplantation. Results At 4 weeks after transplantation, LV end-diastolic dimension decreased in the BM-MNC group than before (40.40?4.51 mm vs. 45.88?4.15 mm, P=0.026), but increased in the control group (48.50?9.31 mm vs. 42.40?7.29 mm, P=0.328). Left ventricular function was improved from 41.16%?9.83% to 47.50%?9.07% in the BM-MNC group (P=0.020) but there was no significant change in the control group. Significant differences existed between the 2 groups in their absolute change before and after the procedure in both LV dimension and LV function (P=0.046 and P=0.030 respectively). The results showed reduction of collagen content in the border and the remote infarct regions in the BM-MNC group compared with the control (P=0.047 and P=0.034 respectively). Conclusion BM-MNCs transplantation regulates collagen content in heart and attenuates the degree of post-MI LV dilation and the development of infarction area. This effect of BM-MNCs transplantation may be one of the mechanisms which intervene ventricular remodeling following myocardial infarction.

Objecitve:To study the effects of ketamine on the large conductance Ca~(2+)-activated potassium (BK)channels in cultured newborn rat hippocampal neurons. Method:All experiments were carried out on 3-7 days old neurons. Single channel current signals were recorded in symmetrical high K~+ solution(140mM) under cell-attached patch and inside-out patch. Result:(1)In inside-out patch, with the unitary conductance being 176.5?16.5pS,the BK channel activity was markedly activated by internal Ca~(2+) but was blocked by potassium channel blocker (TEA). (2) In cellattached patch,when the concentration of ketamine was 0.01,0.05,0.20mmol/L and membrane potential kept at?60mV,the open probability (Po) decreased from 0.042?0.011 at baseline to 0.027?0.008,0.030?0.015,0.032?0.021 respectively(P0.05). Conclusion:Ketamine has dual effects on the BK channels,the inhibition may be related to its side effects,and the activation to the mechanism in anesthesia.

Objective A survey was conducted to learn the status of award winning for science and technology achievement among the medical institutions ranked level 2 and above in Sichuan province,to learn the research capacity in such medical institutions,and to provide the basis for research management and health decision.Methods Questionnaire survey was carried out among 555 hospitals ranked level 2 and above in Sichuan province,information on award winning for science and technology achievement during 2010 -2012 was collected and analyzed.Results Only 33％ of those medical institutions were awarded for their achievement in research with total number of 882 awards in three years:300,266,and 316 awards were obtained in 2010,2011 and 2012,respectively.Conclusions In the medical organization of Sichuan province,the number of awards of scientific and technological achievements showed no significant increase with an volatility state,and the overall level of award need to be improved.Awarded Research areas were mainly on clinical research-based,especially on surgery department.Most award winning hospitals are level 3,and the general hospitals win more award than specialized hospitals;furthermore,hospitals located in different regions are quite different in terms of award winning.

Objective To evaluate the apphcation of wireless mobile infusion system in the outpatients and emergency patients.Methods A total of 566 outpatients and emergency patients with infusion were divided into the observation group and the control group according to the order of infusion.The observation group with 297 cases was used wireless mobile infusion system,and the control group with 269 cases was used traditional infusion method.The reception time,infusion waiting time and satisfaction for nurses' job of patients between two groups were compared.The satisfaction for job of nurses among different work models were compared,too.Results The reception time,infusion waiting time of patients in the observation group were less than those in the control group:(2.07±0.26) min vs.(3.17±0.26) min,(9.89±0.31) min vs.(13.49± 0.21) min,and there were significant differences,P＜0.01.The satisfaction for nurses' job of patients in the observation group were better than those in the control group,x2=8.921,7.064,5.251,and there were significant differencs,P＜0.05.The satisfaction for job of nurses in the observation group was better than that in the control group,and there was significant difference,P＜0.05.Conclusion Wireless mobile infusion system can improve work efficiency,guarantee the safety of infusion,improve patients and nurses' satisfaction.

Objective To explore the effect of 10％ and 25％ oral glucose on neonatal pain during blood sampling through radial artery.Methods 90 newborns were randomly divided into group A,B and C,each group with 30 cases.2 min before the blood sampling,group A was offered 2ml 10％ oral glucose,group B was offered 2ml 25％ oral glucose,and group C was offered 2ml plain boiled water by oral intake.Pain was evaluated based on Neonatal Facial Coding System (NFCS),Neonatal Infant Pain Scale(NIPS),raise of heart rate and crying cases.Results The scores of NIPS during puncture,the scores of NFCS and NIPS after puncture,the raise of heart rate and crying cases were significantly different between three groups.Group B were lower than group A and group C,while differences between group A and group C had no statistical significance.Conclusions During blood sampling through radial artery,offering newboms 2ml 25％ oral glucose can reduce the pain,while 2ml 10％ oral glucose was ineffective.

Objective To investigate the influential factors of brachial-ankle artery pulse wave velocity (baPWV) in healthy individuals.Methods Total 716 healthy subjects including 503 males and 213 females,who underwent health check up in 2010,were enrolled in the study Bilateral baPWV was examined in all subjects and multiple factor linear regression was applied to evaluate the correlations of baPWV with sex,age,body mass index,arterial blood pressure,ankle-brachial index and biochemical variables.Results The mean baPWV of 716 healthy subjecs was ( 1224 ± 162) cm/s,and there was significant difference between men and women [ ( 1257 ± 159) cm/s vs.( 1144 ± 143) cm/s].There was a significant linear correlation of baPWV with sex,age,mean arterial blood pressure and plasma homocysteine concentration ; and the linear regression equation was:mean PWV =6.635 × age + 7.285 × mean arterial blood pressure + 2.682 × homocysteine concentration - 64.334 × sex + 347.500 ( sex:man =0,woman =1 ).Conclusion Age,sex,mean arterial blood pressure and plasma homocysteine concentration are independent influential factors of baPWV.

Objective To study the effectiveness of ultrasonography in thyroidnodule disease detection and malignant/benign lesion differentiation during health check-up.Method Thyroid examination data of Peking Union Medical College Hospital(PUMCH)staff over the last 6 years were retrospectively reviewed.The results of thyroidnodule scanning by using ultrasonography were compared with the findings of physical examinations.Results Among 6348 health check-up receivers from 2006 to 2010,450 were found to have thyroidnodule(detection rate 7.09％).Twenty malignant lesions were confirmed in histological examinations(mean detection rate per year 0.38％).In 2011,1706 thyroid glands were scanned by both physical examination and ultrasonography,and 525 thyroidnodules were identified(detection rate 30.77％).Surgical operations were performed for 22 suspected malignant tumors,and 18 malignant lesions were finally confirmed(detection rate 1.06％).Conclusion Ultrasonography can improve the detection of thyroidnodule.Irregular shape,microcalcification,and rich blood-flow may indicate malignant nodules.

Objectives To evaluate the association between serum cystatin C and carotid atherosclerosis.Methods Medical data,including physical examinations,serum levels of cystatin C,bilirubin,uric acid and other biochemical markers and duplex ultrasonographic scanning for the carotid artery,of 1741 healthy adults (1193 men and 548 women) who sought physical check-up at Peking Union Medical College Hospital during January 2010 and December 2011 were collected,and those with hypertension,diabetes and liver or renal function failure were excluded.Based on the results of carotid duplex uhrasonography,all the adults checked were assigned to two groups,one with thickened intima-media and atherosclerotic plaque (group A),the other with non-thickened intima-media and non-atherosclerotic plaque (group B).Difference in serum cystatin C,bilirubin,uric acid and other biochemical markers between the two groups was analyzed by using Student's t test.The correlation between the markers and carotid plaque was analyzed by using Spearman correlation analysis.The correlated factors of the carotid thickness and plaque appearance were analyzed by using unconditional Logistic regression analysis.Results Serum level of cystatin C was significantly increased in group A (t =-8.85,P ＜ 0.05).Thickness of carotid intima was positively correlated with serum cystatin C (r =0.21,P ＜ 0.05).In multivariate Logistic regression analysis,high level of serum cystatin C was not an independent risk factor of carotid atherosclerosis.Conclusion Relationship between serum cyatatin C and carotid atherosclerosis was not confirmed in our populations,which should be explored further in cohort studies.
