OBJECTIVE:To provide reference for the rational use of narcotic drugs. METHODS:The utilization of narcotic drugs for inpatients in our hospital during 2012-2014 were collected from HIS,and analyzed statistically in respects of consumption amount,consumption sum,DDDs,B/A,DDC and consumption amount of the different departments,and so on. RESULTS:The quantity and consumption amount of Morphine sulfate sustained-release tablets,Oxycodone hydrochloride sustained-release tablets and Codeine phosphate tablet ranked the first 3 places during 2012-2014. Morphine sulfate sustained-release tablets,Oxycodone hy-drochloride sustained-release tablets and Fentanyl transdermal patch(8.4 mg)steadily occupied the first 3 places in the list of con-sumption sum. In the list of DDDs,Fentanyl transdermal patch and Morphine sulfate sustained-release tablets took up the first 3 places;Morphine hydrochloride injection,Bucinnazine hydrochloride injection and Meperidine hydrochloride injection occupied the last 3 places. B/A value of Morphine sulfate sustained-release tablets and Fentanyl transdermal patch(8.4 mg) in 2012,Oxycodo-ne hydrochloride sustained-release tablets in 2014,Fentanyl transdermal patch(2.5 mg)in 2013 and 2014 were all equal to 1 and synchronized well. Others were all biased. DDC of narcotic drugs kept stable in 3 years,and DDC of Oxycodone hydrochloride sus-tained-release tablets took up the first place,followed by Fentanyl transdermal patch (8.4 mg). Narcotic drugs were consumed in 19 departments,but mainly in the department of oncology and general surgery. In the department of oncology,the consumption amount and sum of oral formulation were both significantly higher than other dosage forms,accounting for more than 88% and 52% respectively. CONCLUSIONS:The variety and dosage form of narcotic drugs could meet the clinical needs and its application is basically reasonable. Besides,the utilization of narcotic drugs in the department of oncology consistent with the basic principles of WHO three steps cancer analgesia treatments.

Objective:To explore effects of early rehabilitation nursing on aged patients with diabetes mellitus (DM) complicated myocardial infarction (MI) .Methods :A total of 280 aged DM + MI patients in our hospital were se‐lected .According to random number table ,they were randomly divided into routine nursing group (n=140) and early rehabilitation nursing group (n=140) ,and nursing effects were compared between two groups .Results:Com‐pared with before nursing ,there were significant reductions in levels of fasting blood glucose (FBG) ,2h postprandi‐al blood glucose (2hPBG) and glycosylated hemoglobin A1c (HbA1c) after nursing in both groups , P< 0.05 all;compared with routine nursing group ,there were significant reductions in levels of FBG [ (7.9 ± 1.1) mmol/L vs . (6.8 ± 1.0) mmol/L] ,2hPBG [(9.9 ± 1.4) mmol/L vs .(8.7 ± 1.1) mmol/L] and HbA1c [(7.9 ± 0.7)% vs .(6.9 ± 0.8)% ] ,and significant rise in all item scores of quality of life and total satisfaction (85.0% vs .93.6% ) in early rehabilitation group , P< 0.05 all .Compared with routine nursing group ,there was significant reduction in inci‐dence rate of major adverse cardiovascular events (20.0% vs .9.3% ) within six-month follow-up in early rehabil‐itation nursing group , P<0.05. Conclusion:Early rehabilitation nursing could significantly reduce blood glucose lev‐el ,improve quality of life and satisfaction for nursing in aged DM + MI patients ,which is worth further extension .

ObjectiveTo investigate predictive value of matrix metalloproteinase-9 (MMP-9) and Interleukin-6 (IL-6) combined with cervical length in premature delivery.MethodsPlasma samples of 62 pregnant women were detected in this study.Samples were divided into three groups based on pregnancy outcomes.MMP-9 and IL-6 values were determined by ELISA.Cervical length was measured by transvagihal sonography.ResultsThe level of MMP-9 of the pregnant women in the premature delivery group was significantly higher than that of continuous pregnant group and that of normal pregnant women [(47.24 ±6.41) ng/ml,(523.24 ± 62.35) pg/ml vs (20.39 ±7.23 )ng/ml,(219.29 ± 80.23)pg/ml,(32.18 ±5.16)ng/ml,(406.18 ±71.52) pg/ml,P ＜0.01].The level of MMP-9 and IL-6 of the pregnant women in the continuous pregnant group was significantly higher than that of normal pregnant women [(32.18 ±5.16)ng/ml,(406.18 ±71.52) pg/ml vs (20.39 ±7.32) ng/ml,(219.29 ± 80.23) pg/ml,P ＜0.01].Cervical length of the pregnant women in the premature delivery group was significantly shorter than that of normal pregnant women and that of continuous pregnant group [( 15.62 ± 2.31 )mm vs ( 31.05 ± 1.48 )mm,(26.14 ±3.12)mm,P ＜0.01].ConclusionsDetection of level of MMP-9 and IL-6 combined with measurement of cervical length can be used as predicting index of premature delivery.

ObjectiveTo explore the inhibitory effects of r-k4k5 on retinal neovascularization.MethodsEighty-eight one-week-old C57BL/6J mice were put into the environment with 75% oxygen for 5 days to establish models of vascular proliferation retinopathy. One eye of each mouse received an intravitreal injection of 500 ng of r-k4k5 (large-dosage group) and of 250 ng of r-k4k5(small-dosage group), and the same volume of BSS was injected into the other eye of the mice both in these two groups as a control. The ADPase histochemical staining was used for retinal flatmount to observe changes of retinal vessels. The inhibitory effects of r-k4k5 on retinal neovascularization were evaluated by counting the endotheliocyte nuclei of new vessels extending from retina to vitreous in the tissue-slice.ResultsRegular distributions and reduced density of retinal blood vessels in eyes in the treatment group were found in retinal flatmount. Objective To study the effect of diabetic education on the patients using insulin treatment.Methods 41 cases of diabetes were trained in three different ways:group education,individual education and self education in the hospital for three to four weeks by a team of nurses.Results The results showed that the total average test score was increased by 4.44 after education.The average test scores was 9.37±0.45 after education and higher than before education (4.93±0.59) ( P <0.001).12.4% of the patients got full scores after education,34.1% of the patients got the excellent scores (≥8) before education.82.9% of patients got excellent scores after education.41.5% of the patients got low scores (<3) before education,and it was only 3.1% after education.For diabetic knowledge accumulating,the effects were significant ( P <0.001).The self care skill and knowledge such as self monitor of blood glucose and urine glucose,foot care were improved ( P <0.001).By education,diet control,exercise,oral hypoglycemic durgs and insulin treatment,FBS and PBS of patients were efficiently controled ( P <0.001).Conclusion Education is an important way for the patients to learn diabetic knowledge,improve the ability of self management

AIM To study the effects of recombinant human IL 1 receptor antagonist (IL 1ra) on isolated airway smooth muscle tissue. METHODS Changes of isolated organ's tension were monitored with force displacement transducer and MedLab recording system. RESULTS ①IL 1ra haddirect relaxant effect in normal and ovalbumin sensitized pulmonary as well as tracheal smooth muscle. The mean EC 50 values were 1 29?10 -8 mol?L -1 and 8 06?10 -8 mol?L -1 in normal pulmonary and tracheal smooth muscle, but the mean EC 50 values were 2 61?10 -7 mol?L -1 and 5 88?10 -7 mol?L -1 in ovalbumin sensitized pulmonary and tracheal smooth muscle. Objective To investigate the effect of stellate ganglion block combined with morphine sustainedrelease tablets for controlling pain of advanced lung cancer.Methods 90 cases with advanced lung cancer were randomly divided into the observation group and control group with 45 cases in each group.The control group was givenmorphine sustained-release tablets,the observation group received combination therapy with stellate ganglion block and morphine sustained-release tablets.The analgesic effects,quality of life scores and adverse reactions of two groups were compared.Results The effective rate of observation group was 93.33 ％ (42/45),which was significantly higher than that of control group 77.78％ (35/45),the difference was statistically significant (x2 =4.54,P ＜ 0.05).The morphine consumption of observation group was significantly less than that of control group(t =9.11,P ＜ 0.05),duration of analgesia was significantly longer than that of control group (t =6.72,P ＜ 0.05).Quality of life score in the observation group was significantly higher than that in the control group,the difference was statistically significant(Z =3.61,P ＜ 0.05).The incidence rate of adverse reactions was 17.78％ (8/45),which was significantly lower than that of the control group 46.67％ (21/45),the difference was statistically significant (x2 =4.54,P ＜ 0.05).Conclusion Stellate ganglion block combined with morphine sustained-release tablets for controlling pain of advanced lung cancer can significantly improve analgesia,reduce morphine consumption and adverse reactions,is worthy of clinical application.

Objective:To analyze the important controllable factors which affect the glycemic control of diabetes.
Methods:A cross-sectional study was carried out to examine the role of relevant characteristics in glycemic control by a sampling investigation of 430 diabetic patients in Hunan, China. A questionnaire was designed for personal interviews to collect data. Univariate regression analysis and multiple linear regression analysis were used to evaluate the effects of various factors on glycated hemoglobin A1c (HbA1c) control.
Results:hTe level of HbA1c in 430 patients was (8.7±2.6)%, and the value in 34%patients among them was ≤7.0%. Base on univariate regression analysis some factors were associated with good HbA1c control, including age, diabetic education, self monitoring of blood glucose, knowledge of blood sugar control standard, living environment, and self-owned glucometer. However, the upgraded treatment was associated with poor control. Based on multiple linear regression analysis, the ifrst four factors mentioned above were protective factors for HbA1c while upgraded treatment was risk factor for HbA1c.
Conclusion:Knowledge of blood sugar control standard, diabetic education and self monitoring of blood glucose are important controllable factors for better glycemic control of diabetes.

OBJECTIVE To analyze the disease spectrum and the epidemiologic characteristics of fever patients,providing effective and precise medical service for them.METHODS The disease spectrum and the epidemiologic characteristics of 9376 cases of fever or intestinal disease patients from Oct 2007 to Mar 2008 were analyzed retrospectively.RESULTS Among those patients with fever,there were 57.99% patients with respiratory infection,followed by 18.60% cases with gastroenteritis,12.74% cases with infection at other sites and 4.59% cases with tonsillitis.The fist priority was given to the patients with severe syndromes and acute onset.CONCLUSIONS Fever cases usually have complicated disease history,with an easy-to-change character of disease status. This study will provide some clues to improve our future medical service for fever patients.

Hyperhomocysteinemia, which is an independent risk factor for atherosclerosis, may cause aberrant methylation and dysregulation of gene expression, but the characteristics of the aberrant methylation and its key links involved in its pathogenic mechanisms are still poorly understood. The effect of hyperhomocysteine on DNA methylation in vascular smooth muscle cells, its characteristics and the underlying mechanism of Hcy-induced changing in DNA methylation patterns were investigated. Clinical relevant concentrations of homocysteine was added into the cultured vascular smooth muscle cells of the Homo sapien umbilical vein for 24 h. The level of SAM and SAH was detected by HPLC. The activity of SAH Hydrolase was detected by real-time quantitative reverse transcription-PCR and Western blotting analysis. The level and patterns of DNA methylation were measured by endogenous C-5 DNA methyltransferase(C-5 MT-ase) activity and capacity of genomic DNA to accept methyl groups and methylation-dependent restriction analysis. The results indicated that an increased Hcy concentration induced elevated SAH, declined SAM and the ratio of SAM/SAH, reduced expression of SAH Hydrolase, but increased activity of C-5MT-ase. The methylation status of gDNA analyzed by methyl-accepting capacity of gDNA uncovered a demethylation process in gDNA, or homocysteine-caused hypomethylation in gDNA.With different methylation-dependent restriction endonucleases, the aberrant demethylation was found to prefer C↓CGG sequences to CpG islands. The impacts of different dosage of Hcy showed that the varied detrimental effects of Hcy could be attributed to different concentrations via different mechanisms. In mild and moderate hyperhomocysteinemia, the Hcy may primarily influence the epigenetic regulation of gene expression through the interference of transferring methyl-group metabolism, while in more higher Hcy concentration, the notorious impacts may be more directly caused via oxidative stress, apoptosis, inflammation etc.
