1.The curative effect observation of combined octreotide and omeprazole for the treatment of esophageal variceal bleeding

Objective To explore the clinical effect of combined octreotide and omeprazole for the treatment of bleeding of esophageal and gastric varices(BEV),analyze its security and provide objective basis for clinical treatment.
Methods 92 patients with BEV were randomly selected .According to the treatment methods , they were randomly divided into the control group and observation group .45 cases in the control group were treated with octreotide ,47 cases in observation group were treated with omeprazole on the basis of the control group .The main indexes were the treatment effect,48h rebleeding rate,bleeding time and volume of blood transfusion and adverse reaction .Results The total effective rate of the observation group was 93.62%,more than that of control group 82.22%,the difference was statistically significant(χ2 =6.24,P<0.05).The effective rate of the observation group was 6.38%,lower than that of the control group 17.78%,the difference was statistically significant (χ2 =6.17,P<0.05).The bleeding time of the observation group was (30.41 ±1.82) h,less than that of the control group (41.83 ±2.69) h,the difference was statistically significant (t =9.47,P <0.05).The blood transfusion of the observation group was (374.16 ± 28.30)mL,less than the control group(582.33 ±36.19)mL,the difference was statistically significant (t=12.31, P<0.05).48h the rebleeding rate of the observation group was 6.38%,lower than that of the control group 20.00%,the difference was statistically significant (χ2 =5.90,P<0.05).The adverse reactions of two groups had no significant difference(χ2 =2.78,P>0.05).Conclusion There is the better for clinical effect of combined octreoti-de and omeprazole for the treatment of BEV , which could reduce the rebleeding rate , shorten the bleeding time , decrease the volume of blood transfusion ,and there was no obvious adverse reactions .
2.Clinical efficacy and safety of TAPP surgery in inguinal hernia repair for the elderly

Objective To investigate the clinical efficacy and safety of TAPP surgery in inguinal hernia repair for the elderly by different mesh materials.Methods 24 elderly patients (32 cases) with inguinal hernia were randomly divided into the two groups.The control group was repaired by the braid mesh,while the other 16 cases in the observation group were repaired by the three dimension mesh.The operative indicators,the VAS pain score,the incidence rate of complications and recrudesce rate were compared between the two groups.Results The operative time,time length of staying and costs of the two groups were close,no statistical differences were found (all P > 0.05) ; VAS scores of postoperative 24h,48h,72h of the observation group were (0.97 ± 0.76) points,(0.62 ± 0.55) points,(0.49 ± 0.37) points respectively,which were significantly lower than those of the control group (t =6.331,6.031,5.882,all P <0.05).The incidence rate of complication in control group was 16.67% (2/12),no statistical difference was found between two groups (x2 =0.000,P > 0.05).No recurrence was found in two groups.Conclusion The three dimension mesh and the braid mesh in TAPP are safe and effective,the three dimension mesh has better performance in pain.
3.Effect of health management on morbidity risk of hypertension in population with eNOS gene G894T mutation

Objective:To explore the effect of health management on morbidity risk of hypertension in population with endotheliocyte nitric oxide synthetase (eNOS)gene G894T mutation.Methods:A total of 360 cases,who were 45~60 years old and possessed eNOS gene G894T mutation but without hypertension,were screened using gene chips detection from people who received physical examination and recreation in Qingdao sanatorium and patients visiting to affiliated hospital of medical college of Qingdao University from Jun 2010 to Jun 2013. According to their compli-ance condition,they were divided into health management group (n=163)and control group (n=197,continued their intrinsical lifestyle).Both groups were followed up for two years,and their incidence rates of hypertension were compared.Results:After two-year follow-up,a total of 17 cases (10.43%)were diagnosed as hypertension a-mong 163 cases of health management group;it was significantly lower than that in control group (24.87%,χ2 =12.43,P =0.001).Conclusion:Scientific health management can effectively reduce incidence rate of hypertension in population with eNOS gene G894T mutation.
4.Posture and the Bedrest Time after Percutaneous Coronary Intervention (review)

The patients must lie down on the sickbed for a period after percutaneous coronary intervention (PCI). Most patients will feel uncomfortable such as lumbago, backache, abdominal distension, insomnia, dysuria and etc. Therefore measures including adjusting posture, starting exercise as early as possible and preventing bleeding should be taken.
5.Comparison of different concentrations of ropivacaine combined with sufentanyl in labor analgesia

Objective To explore the effect and value of sufentanyl combined with different concentrations of ropivacaine in labor analgesia.Methods 120 analgesia childbirth pregnant women were divided into 3 groups(group A 0.5g/L,group B 1.0g/L,group C 2.0g/L)by random number table method,40 cases in each group,they were giv-en sufentanyl combined with different concentrations of ropivacaine in labor analgesia.The labor analgesia effect of three groups of pregnant women was recorded.Results The postpartum 2 h blood loss and total labor time of group A were (147.55 ±25.39)mL and (727.54 ±56.38)min,which of group B were (131.03 ±18.73)mL,(573.53 ± 40.09)min,which of group C were (120.03 ±14.84)mL,(493.82 ±34.51)min,there were statistically significant differences among the three groups(F =3.342 7,7.551 7,all P <0.05).The VAS pain score,oxygen partial pressure and CO2 partial pressure of group A were (3.15 ±1.14)points,(17.11 ±1.43)mmHg,(52.73 ±2.59)mmHg, which of group B were (2.01 ±0.76)points,(18.24 ±2.03)mmHg,(50.01 ±1.68)mmHg,which of group C were (1.51 ±0.35)points,(19.41 ±2.76)mmHg,(47.21 ±1.02)mmHg,there were statistically significant differences among the three groups(F =4.651 4,2.852 5,5.681 4,all P <0.05).The incidence rate of complications in group A was 7.50%,which in group B was 10.00%,which in group C was 15.00%,there was statistically significant difference among the three groups(χ2 =0.4571,P >0.05).Conclusion Using 1.5g/L ropivacaine combined with sufentanyl in labor analgesia is remarkable,high safety,neonatal and maternal prognosis is good,and therefore should be preferred to 1.5g/L ropivacaine combined with sufentanyl in labor analgesia.
6.Determination of three components in Cang'ebiyan Tablets

AIM: To establish method of determing baicalin,chlorogenic acid and chlorpheniramine maleate in Cang'ebiyan Tablets(Fructus Xanthii,Herba Cemtipedae,Radix Astragali,Flos Chrysanthemi indici,etc). METHODS: The VP-ODS C_(18) column(4.6 mm?250 mm,5 ?m) was used with a mobile phase of acetonitrile-0.4%H_3PO_4 0-9 min(14∶86),9-20 min(28∶72) gradient elution,the wavelength of detector was set at 0-9 min 327 nm for determining chlorogenic acid content,9-20 min 280 nm for determining baicalin.The VP-ODS C_(18) column(4.6 mm?250 mm,5 ?m) was used with a mobile phase of acetonitrile-0.5% sodium dodecylsulfate-H_3PO_4,the wavelength of detector was set at 262 nm for determining chlorpheniramine maleate. RESULTS: The calibration curves were linear in the range of 0.21-3.14 ?g/mL(r=0.999 9) for baicalin,in the range of(0.05-0.81 ?g/mL)(r=0.999 9) for chlorogenic acid,in the range of 0.10-1.57 ?g/mL(r=0.999 9) for chlorpheniramine maleate,respectively.The average recoveries were 102.44%,RSD=0.57%(n=6);(98.68%,)RSD=0.30%(n=6);99.46%,RSD=0.70%(n=6),respectively. CONCLUSION: The method is simple,realizable and reproducible and can be used effectively for the quality control of Cang'ebiyan Tablets.
7.Microwave coagulation in combination with submucous 5-fluorouracil injection for early-stage lower rectal cancer

Objective To explore clinical indications and curative effects of microwave coagulation combined with submucous 5-fluorouracil (5-FU) injection in the management of early-stage lower rectal cancer. Methods Nine patients with early-stage lower rectal cancer underwent microwave coagulation with submucous 5-FU injection from January 1996 to December 2002. After the microwave treatment, a retention enteroclysis with 100~200 ml normal saline containing 1.0 g 5-FU was performed once daily for 3 days. Results In 3 patients, levels of carcinoembryonic antigen (CEA), which were preoperatively increased, were reduced below normal limits after microwave treatment. No hemorrhage or other intra- or post-operative complications was observed. Follow-up observations to January 2006 showed that all the 9 patients survived. One patient survived for 3 years and 5 months, 1 patient survived for 3 years and 8 months, and 1 patient experienced a tumor relapse at 1 year and 4 months and survived for 2 years and 9 months after a Miles procedure. Among 6 patients who survived longer than 5 years, 1 survived for 5 years and 1 month, 1 for 6 years and 3 months, 1 for 6 years and 8 months, 1 for 7 years and 6 months, and 2 for 9 years. Conclusions Surgical indications for microwave coagulation combined with submucous 5-FU injection for rectal cancer included early-stage cancer, lesions located within 7 cm away from the anus, the mass type cancer, tumor diameter at 0.5~3.0 cm or less than one third of the rectum’s circumference, well differentiated tumor, and patient’s strong anus-sparing demand. This method is minimally invasive, safe, reliable, and cost-effective.
8.Effect of low power ultrasound on ovarian angiogenesis of rabbit

10.Investigation on prostate cancer associated gene expression pattern

Objective To investigate the genes differentially expressed in prostate cancer and to determine some prostate cancer associated genes for diagnosis and treatment prostate cancer. Methods cDNA microarray chips containing 4 366 human genes in full length were used to investigate the differential gene expression patterns of samples from human prostatie cancers and normal prostate tissues. Results A total of 287 genes differentially expressed with prostatie cancers and normal prostate were screened out,comprising 165 unknown genes and 122 known genes.Among the known genes we further identified 56 genes which were markedly differentially expressed (20 up-regulated and 36 down-regulated genes). Conclusions cDNA microarray chips for analysis of genes expression patterns is a powerful method for identification of novel prostatie cancer associated genes.
