Objective To observe the clinical efficacy of Xuebijing injection ( XI) for the treatment of septic myocardial injury, and to study the possible therapeutic mechanism. Methods Thirty-nine qualified septic myocardial injury patients were randomly divided into XI group (N=24) and control group (N=15) . Both groups were treated with conventional therapy, including anti-inflammation treatment, transfusion for resuscitation, nutritional support, symptomatic and supportive treatment, and supportive treatment for keeping vital organ function. XI group was given intravenous drip of XI based on the conventional therapy. Before and after treatment, the peripheral blood concentrations of myocardial-specific isoenzyme of creatine kinase (CK-MB), cardiac troponin I (cTnI), brain natriuretic peptide ( BNP) and plasma calcitonin ( PCT) were detected, and the changes of electro-cardiogram ( ECG) were also observed. The mortality within 28 days was compared between XI group and the control group. Results ( 1) After treatment, cTnI, BNP and PCT concentrations in XI group were decreased significantly as compared with the control group, and there was statistical significance (P <0.05) . However, the decrease of CK-MB was not obvious in XI group ( P>0.05 compared with the control group) . ( 2) The difference of ECG was insignificant between the two groups after treatment ( P>0.05) . ( 3) The 28-day mortality was 8.33% in XI group, and was 13.33% in the control group, the difference being insignificant between the two groups (P>0.05) . (4) During the treatment, none case had adverse reaction. Conclusion XI can improve the myocardial injury, and relieve inflammation of septic myocardial injury patients.

Objective To investigate the correlation between serum homocysteine(Hcy) ,folic Acid ,vitamin B12 ,ferritin levels and cerebral infarction ,and provide a new way for prevention and treatment of cerebral infarction .Methods Forty -eight hospital‐ized patients with ischemic cerebrovascular disease who were in the neurology ward department of the hospital from April 2012 to November 2014 were enrolled in the study as cerebral infarction group .Meanwhile ,30 healthy elderly patients were recruited ran‐domly as a healthy control group during the same period .There was no significant difference in sex and age between the two groups .People with other diseases were excluded .Hospitalized patients were diagnosed within 24 -72 hours and received CT and MRI scan and other related auxiliary examination after being admitted to hospital .All these patients underwent plasma Hcy ,folic acid ,vitamin B12 ,ferritin level tests which were determined by using chemiluminescence and radioimmunoassay .SPSS17 .0 statistical software was used for statistical analysis ,and the correlation between Hcy ,folic acid ,vitamin B12 ,ferritin and cerebral infarction were analyzed .Results The plasma Hcy and ferritin levels in cerebral infarction group were higher than those in control group ,and the difference is significant(P<0 .05) .Meanwhile ,folic acid ,vitamin B12 levels in cerebral infarction group were lower than those in control group and the difference was significant(P<0 .05) .There were significant differences in levels of Hcy ,folic acid ,vitamin B12 ,ferritin between groups of different cerebral infarction levels .Conclusion Detections of serum Hcy ,folic acid ,vitamin B12 and ferritin could be of important clinical value in cerebral infarction .

Objective To explore the effect of auricular plaster therapy on associated symptoms and quality of life in patients with constipation induced by strong opioid drugs for pain.Methods A total of 60 patients selected were randomly divided into the intervention group and the control group with 30 cases in each group.The intervention group was treated with auriculotherapy based on the pattern/syndrome differentiation after detecting the auricular sensitive acupoints' distribution in patients with opioid-induced constipation after receiving opioid therapy.BFI,self-made scale on symptoms accompanied by constipation and PAC-QOL were used to collect data about constipation symptoms and the level of quality of life before or after the intervention,and still with a subset of the intervention group followed for more than two weeks.Results The BFI and PAC-QOL scores of the intervention and control group were lower after intervention,and the decline score of the two groups showed significant difference.But for the self-made scale,the score of each item was slightly lower compared with that before treatment,which showed no statistically significant difference between groups.A follow-up of 2 weeks showed that the long-term efficacy of auricular plaster therapy was significantly lower than before,but was less desirable than short-term.Conclusions Scientific and reasonable choice of acupoint prescription according to syndrome differentiation can effectively improve symptoms accompanied by constipation and patients' quality of life.

Objective Construct the reasonably effective performance assessment mode of clinical departments,which is suitable for the continuous development of modern hospital.Method Use of balanced scorecard (BSC) principle,using the analytic hierarchy process (AHP) and Rank sum ratio method (RSR)to establish a performance assessment system. Results Constructed the performance assessment mode of clinical departments,which can be dynamically adjusted based on management objectives strategy.The mode takes the BSC as the baseline,the strategy of hospital development as the guide,performance assessment index system as the key element. Conclusion The performance assessment mode of clinical departments,base on BSC,achieve the strategic performance management,and is conducive to the hospital for continuous improvement of medical care quality,in line with the long-term strategic needs of the hospital.

OBJECTIVE: To observe whether curcumin could inhibit the accumulation of the collagen IV and fibronectin in the glomeruli in nephrotoxi sera nephritis rats. METHODS: Seventy-two healthy male Sprague-Dawley rats were divided into three groups, with 24 animals in each group. For normal control group, normal saline (0.5 ml/d) was injected through intra-caudal-vein for two days, and at the same time normal saline (0.5 ml/kg) was also daily administered intraperitoneally. For nephrotoxic sera nephritis group, nephrotoxic sera (0.5 ml/d) was injected through the tail vein for two days and dimethyl sulfoxide (0.5 ml/kg) was given intraperitoneally daily. For curcumin group, nephrotoxic sera was injected as above and meanwhile curcumin (50 mg.kg(-1).d(-1)) was administered intraperitoneally every day. Six rats in each group were killed on the 3rd, 7th, 14th and 28th day. Their renal tissue was fixed in 10% formalin for examining the expression of collagen IV and fibronectin. RESULTS: Minimal staining of collagen IV and fibronectin was detected in the basement membrane of normal control rats glomeruli. In the nephrotoxic sera nephritis rats and curcumin treated nephrotoxic sera nephritis rats, the accumulation of collagen IV and fibronectin was increased progressively, with significant difference in the accumulation of collagen IV (P<0.01) between these two groups at the same time points, while the significant difference in fibronectin accumulation (P<0.05) appeared only after the 7th days. CONCLUSION: Curcumin can reduce the accumulation of collagen IV and fibronectin in the glomeruli. Hence we postulated that curcumin might have beneficial effect for retarding glomerulosclerosis.

Objective To investigate the level of nurses' work engagement and analyze the relevance to the professional values in tertiary level 1st class general hospitals of Shandong province.Methods Totally 960 nurses from 7 tertiary level 1st class hospitals of Shandong province were surveyed by the questionnaires,including the Utrecht Work Engagement Scale,Nurses Professional Values Scale and the nurses' general information.Results The total score of nurses' work engagement was (33.91 ± 11.06); Professional values was correlated with work engagement.Conclusions Nurses' work engagement in tertiary level 1st class hospitals of Shandong province is at middle level.Professional values is the main influencing factor and ward atmosphere,chance of promotion and the professional titles can also influence the work engagement.

To study the abnormal distribution of type Ⅳ collagen and laminin chains in glomerular basement membrane (GBM) in membranous nephropathy (MN).Methods 52 cases of MN were collected and staged according to electron microscopic morphological characteristics,and 10 cases of kidney tissues of minimal change disease were used as normal GBM control.Distribution pattern of or5 (Ⅳ) chain,laminin α5and β2 chains,and laminin α2 and β1 chains were detected using immunofluorescence method.Results In minimal change disease,α5 (Ⅳ) chain,laminin α5 and β2 chains all showed continuously linear positive expression along GBM,and laminin α2 and β1 chains were negatively expressed in GBM.In stage Ⅰ MN,α5 (Ⅳ) chain,laminin α.5 and β2 chains all showed continuous linear positive expression along GBM.In stage ⅡMN,the expression of α5 (Ⅳ) chain was increased and showed abundant spikes on the basis of continuous linear positive staining along GBM,and the expression of laminin α5 and β2chains was increased,and segmental spikes were seen on the basis of continuous linear positive staining along GBM.In stage ⅢMN,the expression of α5 (Ⅳ),laminin α5 and β2 chains was also enhanced and segmental double tracks were seen.The expression of laminin α2 chain was negative in GBM in stage ⅠMN,but granular positive expression along GBM was seen in stage Ⅱ and stage Ⅲ MN.No positive expression of laminin β1chain was seen in GBM in different stages in MN.Conclusion The GBM thickness in MN originates not only from intrinsic type Ⅳ collagen chains and laminin chains,but also from laminin α2chain,which only exist in glomerulus mesangium in normal condition.

Objective:To evaluate the uncertainty in 6-methylthiopurine (6-MMP) determination by LC-MS/MS. Methods:The uncertainty sources during the whole process of 6-MMP determination were established and evaluated. The combined and expanded un-certainties were also calculated. Results:The expanded uncertainty at low (2 995 pmol) and high (140 900 pmol) concentration of 6-MMP was 275. 6 pmol and 11 396 pmol, respectively (P=95%, k=2). Conclusion: The uncertainty in 6-MMP determination by LC-MS/MS is mainly caused by recovery, matrix effect and sample preparation at low concentration, and by recovery and sample prep-aration at high concentration.

Objective To investigate the effects of sepsis bundles in China.Method An observational study of 43 patients with severe pneumonia and septic shock admitted to the respiratory intertive care unit(1/11/2006-31/12/2007)was carried out.The selection criteria were in accordance with criteria set by International Conference On Sepsis in 2001.Implementation of 6 hours and 24 hours sepsis bundles was divided into 3 continu-ous phases consisting of education,trial,and application phase.A cohort of 43 patients with matched disease his-tory(1/1/2004-31/10/2006)was enrolled as control group.The percentages for categorical variables and mean±SD for continuous variables were reported.Chi-Square test.unpaired Student's t -test.paired-samples t test,univariate and multivariate logistic regression models were used.Statistical significance was defined as P＜0.05.Results There were very little significant differences in basic characteristics of patients between the two groups.Compared with control group,the differences in serrum lactate,fluid resuscitation and fluid volume infused within 6 hours and blood glucose control in shock subgroup were significant(P values were 0.024,0.009,0.045,and 0.000,respectively).Compared with control group,the differences in respiratory rate and oxygenation index of bundles group at 72 hours later were significant(P values were 0.033 and 0.041,respectively).Compared with control group,the differences in APACHE Ⅱ score and predicted mortality in shock subgroup of bundles were sig-nificant(P values were 0.017 and 0.040,respectively).Compared with control group,the reduction in absolute mortality was 23.30% in bundles group(P=0.019).Conclusions Implementation of sepsis bundles con-tributes noticeably to the significant reduction in mortality of patients with severe pneumonia and septic shock.

OBJECTIVE: To filtrate and prove the different microRNAs (miRs) profiles in nasopharyngeal carcinoma. METHOD: Screening the different expressions of miRs between nasopharyngeal carcinoma and the inflammatory tissues by the application of expression profiling of chip high-throughput and large-scale microarray analysis. Then we used RT-QPCR technology to prove the accuracy of screening results. RESULT: There were significant expression differences of miRs between nasopharyngeal carcinoma and the control tissues, 144 human miRs had 2 or more fold the difference ratio. Compared with the inflammatory tissues, we have found that miRs-34b, miRs-449b and miRs-7-1 significantly low expressed in nasopharyngeal carcinoma, yet miRs-125b, miRs-184, miRs-196b, miRs-205 and miRs-24-1 expressed high. The results were consistent with the microarray analysis. CONCLUSION The difference expressed miRs might be closely related to the process of nasopharyngeal carcinoma, and the research on miRs profiles maybe provide a powerful target basis for early diagnosis and therapy of nasopharyngeal carcinoma.
