1.Study on the change of ECG after chemotherapy and radiotherapy in treatment of patients with breast cancer

Objective To explore the change of ECG after chemotherapy and radiotherapy in treatment of patients with breast cancer. Methods 58 patients with breast cancer were treated with surgery,after operation the observation group were treated with chemotherapy and radiotherapy,and the control group was given chemotherapy singly.The change of ECG was compared between two groups. Results In the observation group,11 patients had abnormal ECG(37.93%),In the control group,2 patients had abnormal ECG(9.52%)(x2=8.03,P <0.05=.In addition,in the observation group the incidence of abnormal ECG in the patients with left breast cancer(7/15)was slightly higher than that with right breast cancer(4/14)(x2=1.01,P > 0.05). Conclusion The treatment of chemotherapy combined with radiation could significantly increase cardiac toxicity in the patients with breast cancer.

3.Acetyl-CoA carboxylase expression in the liver of quail with hyperuricemia and abdominal obesity

The quail model of hyperuricemia combined with abdominal obesity was induced by high purine diet.Body weight in model group showed no change.Serum uric acid level in model group was increased significantly on 7,14,21,and 28 d( P<0.05 or P<0.01 ).Abdominal fat index in model group increased significantly on 28d.On 7 d and 28 d,serum free fatty acid level was increased significantly.Acetyl-CoA carboxylase( ACC ) protein expression in the liver of model quail was increased as shown by ELISA and immunohistochemisty ( P<0.05 or P<0.01 ),suggesting that the alteration of ACC expression contributes to the pathogenesis of hyperuricemia combined with abdominal obesity.
4.Centralized Management of Medical Instrument

OBJECTIVE To conduct the centralized management of the medical instrument in the supply room,and to assure the quality and validation of the clinical nursing and the patient safety. METHODS The cleaning and packing procedures of the instrument were standardized.The reusable instrument would be concentrated to clean,pack,and sterilize.To create a photograph management system,and use the photos to distinguish specialized instrument. RESULTS The instrument was reprocessed in the supply room instead of the clinical departments by the method of the centralized management of the instrument,which provided better instrument care and reprocessing. CONCLUSIONS The supply room conducts the centralized management of the medical instrument and enhances the quality of the instrument reprocessing.
5.Effects of interactive health education on knowledge and behaviors in drug use and biochemistry indicators in hemodialysis patients

Objective To explore the effects of interactive health education on the knowledge and behaviors of medicine use and the biochemistry indicators in hemodialysis patients. Methods Forty-eight eligible hemodialysis patients were intervened with interactive health education once every month. After three times'intervention, phosphorus, hemoglobin and hematocrit level were compared before and after the intervention. Results After the intervention, phosphorus, levels of hemoglobin and hematocrit were all significantly improved ( P<0 . 001 ) compared with that before the intervention . Conclusion The interactive health education can improve the knowledge and compliance behaviors in medicine use, serum phosphate and symptoms of anemia in hemodialysis patients.
6.Value of HBP in diagnosis of catheter-related infections in maintenance hemodialysis patients

Objective To investigate the diagnostic value of Heparin-binding protein (HBP) in maintenance hemodialysis patients in catheter-related infections.Methods 75 patients with maintenance hemodialysis were enrolled in the study and divided into observation group(n=45,with catheter-related infections) and control group(n=45,without catheter-related infections).The serum HBP,procalcitonin (PCT) and C-reactive protein (CRP) levels were detected.The values in diagnostic of catheter-related infections and Gram-negative bacterial infections were analyzed by receiver operating characteristic(ROC) curve.Results There were 34 cases of patients infected by Gram-positive bacterial with the rate of 75.6% and 11 cases of patients infected by Gram-negative bacterial with rate of 24.4%.The serum HBP,PCT and CRP levels in observation group were significantly higher than those in control group and the serum HBP,PCT levels of patients infected by Gram-negative bacterial were higher than those of patients infected by Gram-positive bacterial (P<0.05).The area under curves of HBP,PCT,CRP in diagnosis of catheter-related infections were 0.955,0.890,0.871 with sensitivity of 86.7%,88.9%,62.2% and specificity of 93.3%,73.3%,93.3%.The value of HBP in diagnosis of Gram-negative bacteria infections was better than PCT and CRP.The area under ROC was 0.818 and the cut off value was HBP>27.52 μg/L with sensitivity of 100.0% and specificity of 61.8%.Conclusion Serum HBP,PCT and CRP may be the effective indicators for diagnosis of catheter-related infections,and the HBP has important reference value for diagnosis of Gram-negative bacterial infections.

8.Effect of clinical nursing pathway on allergic rhinitis in patients with subcutaneous immunotherapy

Objective To explore the effect of clinical nursing pathway(CNP)on allergic rhinitis in patients with subcutaneous immunotherapy.Methods One hundred and twenty patients with allergic rhinitis treated with subcutaneous immunotherapy were equally randomized into the observation group and the control group.The former group were managed with clinical nursing pathway and the latter received only routine nursing care.The curative effect of subcutaneous immunotherapy was compared between the two groups including local adverse reactions,treatment compliance,patients' satisfaction and quality of life.Results The scores by symptom assessment at time points of half a year,years 1,2 and 3 showed the symptoms at different time points between the groups were significantly different by the comparison between time and the interactive effect of main effect(F=678.24,P<0.01),and the symptoms in the two groups were both alleviated with the extension of treatment due to the compared time effect at F=47.16, P<0.01.The scores on the symptoms in the observation group were significantly lower than those of the control group at different time points(P<0.01).The experiment group was better than the control group in terms of local adverse reactions,treatment compliance, quality of life and patient's satisfaction(P<0.01)Conclusion Clinical nursing pathway can improve patient's compliance,improve immunotherapy,reduce adverse reactions and improve the quality of life of patients as well as patients' satisfaction.
9.Beta-amyloid affects retinoid receptor-α exporting into cytoplasm

Objective To investigate the effect of Alzheimer' s beta-amyloid (Aβ) on the production and the translocation in cytoplasm of retinoid receptor-α (RXRα). MethodsN2awt cells were treated with Aβ peptide or amyloid protein precursor(APP695) transfection. The nucleus were separated from the cytoplasm by kit. The quantity of RXRα in the nucleus and cytoplasm was detected by Westernblot. The translocation of RXRα in the nucleus and cytoplasm of above N2awt cells or of the cortex cells in the brains of Alzheimer' s disease (AD) patients and their normal control groups was observed by immune fluorescence. ResultsIn N2awt cells, the increasing APP or Aβ had no significant effect on the production of RXRα but resulted in RXRα exporting into cytoplasm, the ratio of RXRα in cytoplasm increased from 3.2% (in control group) to 17.6% (in APP+ group) and from 3.8% (in control group) to 14.3% (in Aβ + group) respectively; compared with normal cortex cells, the translocation of RXRα in the cytoplasm of the cortex cells in the brains of AD increased significantly. ConclusionAβ may affect RXRα exporting into cytoplasm.
10.Effects of focal cerebral ischemia on the proliferation, differentiation and migration of intrinsic neural stem cells and progenitor cells

BACKGROUND: In the central nervous system(CNS) of normal adults there are neural stem cells or neural progenitor cells, which are capable of self-renewing and multiple differentiating. In normal physiological conditions, intrinsic neural stem cells are in a resting state. What state will they be in during cerebral ischemia?OBJECTIVE: To observe the distribution, proliferation and differentiation of intrinsic neural stem cells in focal transient ischemia in rats.DESIGN: A randomized controlled exploratory trial based on the rats.SETTING: Neurobiological department, histological and embryological department of a medical college.MATERIALS: The experiment was conducted in the Neurobiological Department of Luzhou Medical College from July 2001 to July 2002. Altogether 41 healthy adult SD rats of either gender, weighting 250 g - 300 g, were selected.INTERVENTIONS: The focal ischemia model was made by blocking middle cerebral artery(MCA) and reperfusing for 0.5 hour, 3 hours, 6 hours, 12 hours, 1 day, 2 days, 3 days, 5 days and 10 days. Sham-operation group was treated by the same method, but the filament was not long enough to block MCA, and normal rats served as control group. The rats were sacrificed at given time points, and their brains were made into cerebral slices. The single-and double-labeled immunohistochemical staining was employed to detect the proliferation, distribution and differentiation of intrinsic neural stem cells.MAIN OUTCOME MEASURES: The distribution, proliferation and differentiation of intrinsic neural stem cells.RESULTS: Immunoreactivity of proliferating cell nuclear antigen(PCNA)was present in most ependymal cells in ventricular zone(VZ), and PCNA-positive cells were sparsely distributed in the parenchyma in normal and sham-operation groups. At 3 hours of reperfusion, PCNA-labeled cells were first detected in rostral subventricular zone. At 12 hours of reperfusion and onward, PCNA-positive cells appeared in some choroid plexus cells in bilateral lateral VZ. At day 3 to day 10 of reperfusion, PCNA-labeled cells significantly increased in infarct boundary in preoptic area, striatum and deep layer of frontoparietal cortex. PCNA-labeled cells were first detected in subgranular zone of dentate gyrus 3 days after reperfusion, and increased with time. A very small number of double-positive cells expressed with PCNA and glial fibrillary acidic protein(GFAP) were first detected in infract boundary in preoptic area on day 3 and onward. No double-PCNA and NF-positive cells were detected within 10 days of reperfusion.CONCLUSION: Focal cerebral ischemia activates intrinsic neural stem cells, which proliferate and differentiate, and migrate toward ischemic striatum and frontoparietal cortex. This may help clarify the mechanism of functional recovery after ischemia.
