1.Clinical analysis of twelve cases with diabetic hemichorea

ObjectiveTo explore the clinical manifestations, pathogenesis, radiographic features and prognosis of Diabetic Hemichorea.Methods We conducted a retrospective analysis in 12 patients suffering Diabetic Hemichorea in terms of the clinical manifestations, radiographic features,laboratory results and treatment processeswho were hospitalized in our hospital from 2005 to 2010.Results Diabetic Hemichorea occurred mostly in the patients with diabetics with uncontrolled blood glucose level for a long term.Both ketosis and nonketosis patients can suffer this disease.Cranial CT and/or MRI imaging, especially those with high signal and clear border in MRIT1 WI but without edema shadow and mass effect, was of diagnostic value.The symptoms vanished 7 day to 4 years after the treatment.Conclusion Deepening the understanding of clinicians and imageological staff upon Diabetic Hemichorea can decrease the misdiagnosis and ensure timely treatment for this disease.
2.Effect of Leuprolide Preoperative Used in Patients with Uterine Fibroids

Objective:To study the effect of leuprolide preoperative used in the patients with uterine fibroids .Methods:Totally 70 patients with uterine fibroids were divided into the observation group and the control group with 35 ones in each.The 35 patients in the control group were with laparoscopic myomectomy , and those in the observation group was given leuprolide , 3.75 mg per time, once in a month for 3 months, and then undergone laparoscopic myomectomy .The serum hormone levels , menstruation situation and fi-broids and uterus volume changes in the observation group were observed and compared before the treatment and after the 3-month leu-prolide treatment .The situation during the operation and after the operation , and the recurrence rate of uterine fibroids between the two groups were compared .Results:After the pretreatment of leuprolide , the levels of luteinizing hormone ( LH) , follicle-stimulating hor-mone (FSH) and serum estradiol (E2) in the observation group were significantly lower than those before the treatment (P<0.05), and the menstrual blood volume , uterine volume and fibroid volume were significantly lower than those before the treatment , and the differences were statistically significant (P<0.05).The blood loss during the operation and pelvic drainage of the observation group were significantly lower than those of the control group , and the ambulation time and length of hospital stay of the observation group were significantly shorter than those of the control group with statistically significant differences (P<0.05).Conclusion:Leuprolide preoperative used in the patients with uterine fibroids can significantly reduce serum hormone levels and fibroid volume , which are ben-eficial to myomectomy with smaller surgery invasion and better prognosis .
3.Quality of life and its influencing factors in elderly patients with advanced cancer

Objective To assess the quality of life and its influencing factors in ellerly patients with advanced cancer.Methods The quality of life was evaluated with EORTC QLQ-C30 scale Chinese version in 103 elderly patients with advanced cancer in a hospital outpatient clinic of Beijing from July 2013 to July 2014.The overall health,physical function,role function,cognitive function,emotional function,social function and individual symptoms score were assessed.The general information and clinical data of patients were analyzed.Results The general score of quality of life in elderly advanced cancer patients was (35.77 ± 21.98).In the domain of function the role functional score was the lowest (36.60 ± 27.97);in the domain of symptoms the tired score was the highest(50.82 ± 30.16),the score of appetite loss was the highest in individual symptoms (39.78 ± 24.32).Educational level,marital status,living condition,medical insurance and the number of comorbidities were correlated with overall health in elderly patients with advanced cancer (P < 0.05).Multiple linear regression analysis showed that the educational level (t =2.447,P < 0.05),marital status (t =2.118,P < 0.05),medical insurance (t =3.770,P < 0.05) and overall health status were positively correlated with the quality of life scores;and number of eomorbidities (t =-7.157,P < 0.05) was negatively associated with the overall quality of life.Conclusion The quality of life was generally poor in elderly patients with advanced cancer;and educational level,marital status,medical insurance,comorbidities are important influencing factors.
4.Value of direct and indirect uitrasonographic signs in diagnosis of small breast cancer

Objective To explore the value of direct and indirect ultrasonographic signs in diagnosis of small breast cancer. Methods The sonographic features of 103 small breast lesions (47 malignant, 56 benign) were retrospectively analyzed and divided into direct and indirect signs according to the pathology.The results of ultrasonography were compared with pathology. Agreement was measured with K statistics between ultrasonographie signs and pathology. Results Compared with the malignant and benign small breast noplasms, there was statistical significance of the direact signs of micro calcifications, grade Ⅱ- Ⅲ flow,resistance index (RI)≥0.7 and the indireact signs of changes of superficial fascia (P<0.01).Agreement was moderate for micro calcifications, grade Ⅱ-Ⅲ flow, RI≥0.7, changes of superficial fascia (0.7> K≥0.4) ,and substantial for the combination of direct and indirect signs(K=0.726). Conclusions Combination of direct and indirect ultrasonographie signs was significant in diagnosis of small breast cancer.
5.Post management in hospitals

Post management should replace capacity management in the reform of the personnel system in the health sector. The authors expound in the first place the connotations and the necessity of post management, and then discuss respectively three key links, viz. change of ideological concepts of people at various levels in the implementation of post management; establishment of necessary setups and mechanisms for post management; and organic combination of responsibilities, powers and interests in post management. Finally the authors put forward the following viewpoints in light of Tongren Hospital's practice in adopting post management: ①setting up a primary personnel system based on double engagements; ②setting up a system of selecting key members for posts based on a mechanism of post reward, such as the classification of chief experts into three classes; ③setting up a managerial mechanism conducive to the rational flow of personnel; ④conducting checks so as to provide basis for distributing appropriate salaries, with the elements of management, expertise and responsibilities taken into consideration in the distribution. The effects are also discussed.
6.Investigation and analysis of theatre nurses′ awareness of knowledge related to malignant hyperthermia

Objective To formulate countermeasures of improving nurses′knowledge of malignant hyperthermia by investigating theatre nurses. Methods A self-designed questionnaire was used to investigate 120 theatre nurses for awareness, demand and access about malignant hyperthermia. Results Average correct response rate on the awareness of theatre nurses about malignant hyperthermia was (17.14±9.68)%.There are differences in different education, title and seniority(F=8.002,6.509,2.643,P<0.05).100.0%(120/120)theatre nurses are interested in studying knowledge related to malignant hyperthermia, 94.2%(113/120) theatre nurses are willing to learn knowledge related to malignant hyperthermia. Conclusions The awareness of theatre nurses about malignant hyperthermia is low. It is necessary to strengthen the training of theatre nurses about the knowledge of malignant hyperthermia. A variety of forms can be adopted.
7.Effect of tetramethylpyrazine on Nissl bodies in cerebral neurons of rats with epilepsy

BACKGROUND: Tetramethylpyrazine has the protective effect against the central nervous system injury. The structural changes in Nissl bodies were regarded as a marker of neuron injury.OBJECTIVE: To investigate the effect of tetramethylpyrazine on the structure and the quantity of Nissl bodies of cerebral neurons in rat with epilepsy.DESIGN: A comparative study.SETTING: It was conducted at the Physiological Department of Medical School of Xianning College.MATERIALS: From September 2004 to March 2005, it was completed at the Anatomy Department of Tongji Medical College, Huazhong University of Science and Technology. Forty healthy SD rats aging 3-4 months, weighing (250±50) g and regardless of their gender, were selected.METHODS: Rats underwent anesthesia and craniotomy. Then their cerebral cortex were exposed for placing BL-410 Experimental System of Biological Function (TME, China) to record the bilateral EEG of the brain and the seizure in rats with penicillin-induced epilepsy group and the 10 mg/kg tetramethylpyrazine group, 20 mg/kg tetramethylpyrazine group and 40 mg/kg tetramethylpyrazine group. In control groups, the brains of rats were taken out at 1 hour after craniotomy. In penicillin-induced epilepsy group, their brains were taken out at 1 hour after penicillin-induced epilepsy. In 10 mg/kg tetramethylpyrazine group, 20 mg/kg tetramethylpyrazine group and 40 mg/kg tetramethylpyrazine group, after stable penicillin-induced epilepsy, tetramethylpyrazine was injected intraperitoneally at a dose of 10 mg/kg, 20 mg/kg and 40 mg/kg, respectively.When the greatest protective effect of tetramethylpyrazine appeared, the rats' brains were taken out. Brain sections were sliced. Nissl bodies were stained by thionine staining. Under light microscope, structures of Nissl bodies were observed and the images of Nissl bodies were quantitatively analyzed by HPIAS-1000 high acuity color pathologic diagram-writing analyzing system. In each group, the average absorbency of 15 fields was regarded as the average absorbency of Nissl bodies in that group.MAIN OUTCOME MEASURES: In all the groups, the structure and the quantity of Nissl bodies of cortical neurons in rats were studied.of the structure of Nissl bodies in external granular layer cells and external pyramidal layer cells. In control group, multi-layer blue-stained and clumplike or granule-like Nissl bodies could be observed. In penicillin-induced epilepsy group, Nissl bodies were completely resolved and disappeared. In 10 mg/kg tetramethylpyrazine group, Nissl bodies were partly or completely resolved. In 20 mg/kg tetramethylpyrazine group, the quantity of Nissl bodies was significantly increased as compared with those in penicillin-induced epilepsy group and the 10 mg/kg tetramethylpyrazine group. In 40 mg/kg tetramethylpyrazine group, the quantity and the structure of of the average absorbency of stained Nissl bodies in external granular layer cells and external pyramidal layer cells in rat cortex among all the groups.In penicillin-induced epilepsy group, the average absorbency were dramatically lower than that in control group (0.033±0.002, 0.756±0.035, t=4.93,P < 0.01). In 20 mg/kg tetramethylpyrazine group and 40 mg/kg tetramethylpyrazine group, the average absorbency were significantly higher than that in penicillin-induced epilepsy group (0.435±0.011, 0.658±0.029, t=2.98,5.32, P < 0.01). In 40 mg/kg tetramethylpyrazine group, the average absorbency were similar to that in control group (t=1.75, P > 0.05).CONCLUSION: Tetramethylpyrazine can significantly elevated the concentration of Nissl bodies of neurons in rats with epilepsy. Changes in the structure and quantity of Nissl bodies of cerebral neurons may be closely associated with seizure, and tetramethylpyrazine can restore the structure and the quantity of Nissl bodies of neurons, regulate their functions and hereby, inhibit seizures.
8.Hollow periodic mesoporous organosilicas nanoparticles for efficient ultrasound-based imaging and ultrasound-mediated therapy

Objective To Study the application of ultrasound mediated drug-loading ultrasound contrast agents in auxiliary cancer chemotherapy.Methods Carrying paclitaxel (PTX) hollow periodic mesoporous organosilicas(HPMOs) were prepared by Structural differences selective etching method.The drug release kinetics of ultrasound mediated in vitro were evaluated at different time points of ultrasound radiation to PTX-HPMOs.The cell survival were evaluated by CCK8 experiment.Results ① The drug loading of PTX-HPMOs was 91 mg/g.②The achieved HMONs showed the high stability and excellent contrast enhancement for ultrasound imaging both in vitro.HPMOs showed the same good effect of contrastenhanced ultrasound imaging as SonoVue.③Drug-loaded HMONs exhibit ultrasound-responsive drug release performance.The ultrasonic stimulation can increase the release of 18 % of PTX each time.④ After ultrasonic stimulation,PTX-HPMOs showed good antitumor effect in vitro.Conclusions The new type of drug loading ultrasound contrast agents show good ultrasonic imaging and can help the cancer chemotherapy at the same time.
10.Study on genic expression of ampC in Serratia marcescens

Objectives To investigate the properties and distributions of ampC gene among different drug-resistant strains of Serratia marcescens,and the relationship of control gene ampR with AmpC enzymes′ expressions.Methods According to the results of inducting experiment with 1/2 MIC of beta-lactam antibiotics (CTX),three-dimensional testing and isoelectric focusing electrophoresis testing,143 strains of S.marcescens were classified into three groups:including induction group, continuous low-production group and hyperproduction group. In each group, the sequences of ampC and ampR genes were amplified using the method of PCR. The products of PCR were analyzed. The plasmid-mediated beta-lactamases were detected using the method of conjugation experiment.Results Among 143 strains of S.marcescens, the continuous low -production strains, induction strains and hyperproduction strains were 14,103,and 18, respectively.125 and 99 strains were ampC and ampR gene positive, respectively.The detection rate of ampR in hyperproduction group was lower than other groups.5 sites of ampC genes and 4 sites in the Open Reading Frame (ORF) of ampR gene were easily mutated in 5 induction strains and 2 hyperproduction strains.Conclusions The production of inducing drug-resistance of some S.marcescens might be related to mutation of ampC gene encoding AmpC beta-lactamases and the ORF mutation in ampR. The continuous hyperproduction drug-resistance had something to do with deletion mutation in ampR in segmental hyperproduction strains.The plasmid-mediated AmpC enzymes hadn′t been found in S.marcescens.
