Post management should replace capacity management in the reform of the personnel system in the health sector. The authors expound in the first place the connotations and the necessity of post management, and then discuss respectively three key links, viz. change of ideological concepts of people at various levels in the implementation of post management; establishment of necessary setups and mechanisms for post management; and organic combination of responsibilities, powers and interests in post management. Finally the authors put forward the following viewpoints in light of Tongren Hospital's practice in adopting post management: ①setting up a primary personnel system based on double engagements; ②setting up a system of selecting key members for posts based on a mechanism of post reward, such as the classification of chief experts into three classes; ③setting up a managerial mechanism conducive to the rational flow of personnel; ④conducting checks so as to provide basis for distributing appropriate salaries, with the elements of management, expertise and responsibilities taken into consideration in the distribution. The effects are also discussed.
2.Relationship between serum visfatin and glucose metabolism disorder in patients with polycystic ovary syndrome

Objective To investigate the relationship between serum visfatin and glucose metabolism disorder in patients with polycystic ovary syndrome.Methods Seventy eight patients with polycystic ovary syndrome (PCOS) were selected,and,according to the results of glucose tolerance test and insulin release test (OGTT),were divided into normal OGTT without insulin resistance (NONIR) group (n =32 cases),OGTT normal with hyper insulinemia (HI) group (n =25 cases),and OGTT abnormalities that may be associated with hyper insulinemia (also known as abnormal glucose metabolism (AGM) group; n =21 cases).Sixteen cases of tubal infertility women were selected as control group in the same period.The relationship between serum visfatin and glucose metabolism was analyzed and related to various factors.The changes of relevant indicators of PCOS patients who were treated before and after treatment were observed in AGM group.Results In AGM Group,PCOS patient's menstrual cycle,testosterone (T),luteinizing hormone (LH)/follicle stimulating hormone (FSH),visfatin,triglyceride (TG),homeostasis model assessment of insulin resistance (HOMAIR) were higher than control group (P < 0.05).The age at menarche,menstrual cycles,infertility time,waist-hip ratio (WHR),systolic blood pressure,T,estradiol (E2),and LH/FSH were no significant difference between AGM group versus NONIR group or HI group (P >0.05).However,the body mass index (BMI),waist circumference,diastolic blood pressure,HOMA-IR,and TG in AGM group were significantly higher than NONIR group,and older than HI group; visfatin in AGM group was higher than NONIR group or HI group (P < 0.05).Visfatin was positively correlated with BMI,waist circumference,fasting insulin (FINS),fasting plasma glucose (FPG),HOMA-IR,and TG (r =0.423 ~ 0.847,P =0.01 ~ 0.04).After 3 months of treatment,8 cases AGM patients' serum visfatin,HOMA-IR,and blood pressure were significantly decreased (P < 0.05).Conclusions High serum visfatin may be one predictor of glucose metabolism in patients with PCOS.Especially,the patients with obesity should be highly vigilant glucose metabolism disorder occurring and should be given positive joint programs treatment for weight control.
3.A analysis of the proteome of HL60 cell line induced by a new steroidal drug as a monocytic inducer

Aim The potentiality as a differentiation inducer of the new steroidal drug(NSC67657) had been studied.Then the expression differences of protein between treated and untreated HL60 cell line could be analysed.Method Firstly,the expression patterns of C/EBP alpha gene and protein were observed between treated and untreated HL60 cell line.Then the proliferation of HL60 cell line could be investigated by MTT.At the same time cellular chemical staining could be employed to investigate which direction HL60 cell line would be induced by NSC67657.Then the flow cytometry(FCM) could be employed to detect the profile of differentiation of HL60 cell line induced in different time and at different drug concentrations,by which the most suitable drug concentration and inducing time could be found. Following that,the information of cellular cycle and ultramicrostructure could be analysed by FCM and electronmicro scope,by which whether the apoptosis had happened or not under the drug treatment could also be found. Finally,the protein of these two group HL60 cell lines could be separated by modified two-dimensional electrophoresis (2-DE).Results The expression of C/EBP alpha gene and protein could be promoted under the treatment of NSC67657.Then the proliferation of HL60 cell line was inhibited significantly.From cellular chemical staining,the monocytic differentiation could be easily found and the perfect inducing time and drug concentration were defined as 10 ?mol?L-1NSC67657 and constantly inducing HL60 cell line within 5 days.The cellular number of G0~G1 was increased and hardly any apoptosis which might be happened during drug inducing ability could be seen.The protein of HL60 cell lines were separated by modified 2-DE technology.Then there were 14 protein spots which could only be found in the differentiated gels,on the other hand,20 protein spots could only be found in the undifferentiated gels,which would have been analyzed by MALDI-TOF.Conclusions HL60 cell line could be induced to monocyte by NSC67657 which could also stimulate the C/EBP? in the early stage.2-DE could separate the protein directly which expressed differentially,from which some proteins essential in cellular differentiation might be found.
4.A Comparative Study on Auditory Processing Abilities between Children with and without Learning Difficulties

Objective To study the auditory processing abilities of children with learning difficulty and deter-mine the proportion of the children with auditory processing disorder.Methods A single factor completely random-ized experimental design was used.Seventeen children with learning difficulties and 31 normal children from grade 2 to grade 4 in Beijing were induded in this study.The abilities of low-pass filtered speech,competing sentences,di-chotic digits and frequency patterns were tested through auditory processing disorder screening test software.Results There were significant differences in the abilities of listening sentences and digits in competitive environment and recognizing frequency patterns between children with learning difficulty and without.The auditory processing ability of children with learning difficulties was obviously poorer than normal children.The incidence of auditory processing disorder among the children with learning difficulties was 41.176%~58.824%.Conclusion The auditory process-ing ability in the competitive environment of children with learning difficulties was obviously poorer and they need in-tervention specifically.
5.Intravitreal lucentis injections combined with laser photocoagulation for Coats disease

Objective To observe the therapeutic effect of intravitreal lucentis injections combined with laser photocoagulation for Coats disease.Methods This was a retrospective case series study.From August 2012 to December 2015,18 cases (18 eyes) were diagnosed as Coats disease with macular edema by FFA and OCT.All patients were treated at baseline using intravitreal lucentis injections.One week later,photocoagulation was performed by the guiding of FFA.Follow-up treatment was performed as necessary.The primary BCVA and central macular thickness (CMT) was measured postoperatively.The follow-up time was 16-24 months (mean 18.8 months).Results The mean number of lucentis injections was 2.6,and the mean number of laser treatment sessions was 1.8.The mean preoperative BCVA was (0.81 ±0.28) LogMAR,while the final visit was (0.76 ± 0.37) LogMAR (P =0.396).In 3 patients (16.7 %),BCVA had improved by more than 3 lines,and 13 patients (72.2%)showed stable BCVA (changes within 2 lines of visual acuity),while in 2 patients,BCVA decreased by more than 3 lines (11.1%).The mean CMT improved significantly from (341.11 ±67.97) μm preoperatively to (277.83 ±51.59) μm at the fmal visit (P =0.030).Final BCVA was significantly correlated with preoperative BCVA (r =0.817,P <0.001).The final BCVA of patients with subfoveal hard exudates preoperatively was significantly worse than that of patients without such exudates (P =0.044).Conclusion Intravitreal lucentis injection combined with laser photocoagulation may be an effective treatment for Coats disease.It may stablize the visual acuity,decrease the CMT of Coats disease.The final BCVA is associated with preoperative BCVA and occurrence of subfoveal hard exudates.
6.Effective antiviral therapy with entecavir in chronic hepatitis B virus carriers.

OBJECTIVETo evaluate the short-term effect and safety of entecavir for the treatment of chronic hepatitis B (CHB) virus carriers.
METHODSNinety-three cases of CHB virus infection (hepatitis B surface antigen (HBsAg)-positive, hepatitis B e antigen (HBeAg)-positive, hepatitis B core antibody (HBcAb)-positive, HBV DNA≥1x10(5) copies/mL) were divided into two groups: CHB virus carrier (47 cases) and CHB (46 cases). All of the 93 cases were given 0.5 mg entecavir orally once a day for 48 weeks. Virology, serology and biochemistry tests were perrmed at treatment weeks 0, 4, 12, 24 and 48. Side effects of entecavir and the incidence of liver cirrhosis and hepatocellular carcinoma were recorded.
RESULTSThe CHB virus carrier and CHB group had complete virological response rates of 14.9% and 17.4% at week 4, 51.1% and 63.0% at week 12, 76.6% and 89.1% at week 24, and 97.9% and 100% at week 48, respectively; there was no significant difference between the two groups (P>0.05). The CHB virus carrier and CHB group had partial virological response rates of 42.6% and 47.8% at week 4, 57.44% and 65.2% at week 12, 85.0% and 89.1% at week 24, and 100% and 100% at week 48, respectively; there was no significant difference between the two groups (P>0.05). No cases in either group experienced virologic breakthrough during the treatment course. The CHB virus carrier and CHB group had serological response (HBeAg-negative) rates of 0 and 4.3% at week 4, 2.1% and 8.7% at week 12, 4.3% and 13.0% at week 24, and 8.5% and 21.7% at week 48, respectively; there was no significant difference between the two groups (P>0.05). The CHB virus carrier and CHB group had HBeAg seroconversion rates of 0 and 0 at week 4, 0 and 4.4% at week 12, 2.1% and 10.9% at week 24, and 6.4% and 17.4% at week 48, respectively; there was no significant difference between the two groups (P>0.05). No case in either group showed HBsAg-negativity and seroconversion during the treatment course. The CHB group had a biochemical response (alanine aminotransferase normalization) rate of 26.1% at week 4, 65.2% at week 12, 91.3% at week 24, and 97.8% at week 48.No case in either group showed biochemical breakthrough during the treatment course. There were no cases of liver cirrhosis or hepatocellular carcinoma in either group. There were no side effects of the entecavir treatment experienced in either group.
CONCLUSIONAntiviral therapy with entecavir is effective, safe and well tolerated in CHB virus carriers.

7.Establishment and identification of HUVEC cell strains with over-expression and low expression of RACK 1

Objective To establish several human umbilical vein endothelial cell ( HUVEC ) strains with over-ex-pression or low expression of receptor for activated C kinase 1 ( RACK1 ) , which will provide an effective tool for future studying the function of RACK1 in arrhythmia.Methods The full-length cDNA sequence of RACK1 gene was amplified and inserted into pIRES2-EGFP.At the same time, designed and synthesised complementary DNA sequences of 3 pairs of short hairpin structure and a pair of negative control sequence , then subcloned into the plas-mid pGenesil-1 .The HUVEC cells were transfected with these plasmids and screened by using G 418 .And the expression of RACK1 mRNA and protein in the cells were assayed by qRT-PCR and Western blot , respectively . Results RACK1 eukaryotic expression vector and siRNA expression vectors of RACK 1 were constructed success-fully.After a 48 h transfection of HUVEC cells with the recombinant vectors and G 418 selection, the positive cell clones were obtained .qRT-PCR and Western blot showed that over-expression vector and interference vectors could effectively enhanced and knocked-down RACK1 expression in HUVEC strains .Conclusions HUVEC cell strains with over-expression and low expression of RACK 1 have been successfully established .
8.Sharp marginal ridge affects the fitness of the metal full crown cast for the abutment tooth

Objective: To study the effect of the sharp marginal ridge of the abutment on the casting of the fit metal full crown in dental preparation.Methods: We established the models of the designed crown-based-teeth(American Dental Association style,No2 trail) with a sharp or smooth marginal ridge,and cast a metal crown for each model.We injected silicone into the crown and immediately fixed it onto each model.Then we took out the solidified silicone and measured its thickness between the crown and the occlusal face of each model.The thinner the thickness,the better the fitness.Results: The average silicone thickness was 250 ?m in the smooth marginal ridge group and 1 660 ?m in the sharp marginal ridge group,with significant difference in between(P

Objective:To investigate the diagnostic value of metagenomic next-generation sequencing (mNGS) in AIDS patients complicated with Pneumocystis jirovecii ( P. jirovecii) infection. Methods:This is a retrospective study. From January 2019 to June 2021, the respiratory tract and other body fluid samples of 236 cases of AIDS co-infected patients diagnosed in the AIDS Department of Changsha First Hospital were collected, along with corresponding medical histories. Traditional etiological hexamine silver staining and serum 1,3-β-D glucan (BDG) were performed simultaneously with mNGS detection, and Fisher′s exact test was used to analyze the results and compare the diagnostic performances of mNGS with those of hexamine silver staining and serum G test.Results:A total of 236 cases of AIDS patients with pulmonary infection were collected and tested. Seventy-seven cases were clinically diagnosed with Pneumocystis jiroveci pneumonia and 159 cases with non- Pneumocystis jiroveci pneumonia. Among the 236 AIDS patients with pulmonary infection, mNGS detected 77 [32.63%(77/236)] positive cases of Pneumocystis jiroveci, while hexamine silver staining detected 10[4.24%(10/236)] and serum BDG detected 146 [61.86% (146/236). Based on these clinical diagnostic results, the sensitivity of mNGS detection was 100% (77/77) for the 77 patients with Pneumocystis pneumoniae, significantly higher than that of silver hexamine staining [12.99% (10/77), P=0.046] and serum BDG [58.44% (45/77), P=0.038]. The mNGS showed good specificity, which was the same as that of hexamine silver staining [100% (159/159)] and significantly higher than that of serum BDG [36.48% (58/159), P=0.026]. With therapeutic clinical diagnosis as the reference method, the accuracy of mNGS detection was 100% (236/236). Conclusions:This study evaluated the diagnostic value of mNGS detection in AIDS patients with Pneumocystis jirovecii infection. The results showed that the sensitivity and specificity of mNGS detection were high, and it had exceptional clinical application value in the pathogenic detection of infectious diseases.
10.A study of the relationship between MICA gene and systemic lupus erythematosus.

OBJECTIVETo investigate the relationship between major histocompatibility complex class I chain-related A(MICA) gene and systemic lupus erythematosus (SLE).
METHODSThe alleles and frequencies of exons 4 and 5 of MICA gene were determined in 70 cases of SLE and 152 controls of Yunnan Hans by STR genotyping, polymerase chain reaction, single strand conformation polymorphism and bidirection DNA sequencing.
RESULTSFive alleles of exon 5 and 10 alleles of exon 4 were found in this study. The frequency of each allele was determined in patients and controls. There was no significant difference between the two groups in exons 4 and 5 of MICA gene.
CONCLUSIONExons 4 and 5 of MICA were not related to SLE in Yunnan Hans.
