1.Safety of delivering nitric oxide through nitrous oxide path in anesthesia machine

To evaluate the feasibility of delivering NO through N_2O path in anesthesia machine,NO at 800 ppm was delivered through nitrous oxide path in anesthesia machine SIEMENS Ventilator 710 or NARKOMED 2B,to ventilate a test lung. The minute volume of ventilation (MV) was set from 2 to 6L/min, and NO concentration from 10 to 80 ppm. NO and NO_2 concentrations were measured by hemiluminescence and electro-chemical fuel cell technique. SIEMENS Ventilator 710:Non-rebreathing model,its inspiratory limb was prolonged to 7 meters,the NO level was adjusted at 80ppm before passing through soda lime, NO and NO_2 samples were taken before absorber,after absorber, 1, 2,4 and 6 meter site in inspiratory limb. NARKOMED 2B:Oxygen flows were at the rates epual to the MV mentioned above, NO concentration was at 10,20,40,60 and 80 ppm,the inspiratory gas mixture was sampled immediately passing through absorber, NO_2 levels were less than 5.0ppm in all conditions listed above. With SIEMENS Ventilator 710, the highest level of NO_2 was 3.45 ppm before absorber and 2.43 ppm after absorber,with NARKOMED 2B,that was 3.6 ppm. Objective:To assess the clinical and microbiologic effects of ultrasonic scaling with simultanous irrigation with chlorhexidine(CHX), conventional ultrasonic scaling or subgingival irrigation. Methods: 20 teeth in 7 patients with periodontitis were equally divided into test and control group. Patients in the test group were treated by irrigation with CHX during ultrasonic scaling(Master400),those in the control with irrigating after scaling(MiNi ). Clinical assessments of bleeding on probing (BOP),probing depth (PD) were made just before and 7 and 14 days after treatment. 5.Development of the KPI system for hospital's capital projects based on key performance indicators

Objective To build a performance indicators measurement system for hospital's capital projects for the purpose of objective comments on project performance.Methods Based on theories and practice of key performance indicators,building the KPI system for hospital's capital projects with the construction project of a hospital in another place.Results Level-1 indicators are set as business indicators and financial ones,the business indicators include three level-2 indicators and 20 level-3 indicators; the financial indicators also include one level-2 indicator and three level-3 indicators.These indicators are designed for scientific and objective appraisal of performance of the projects.Conclusion Such a KPI system can present objective appraisal of the project performance,and the performance management.
6.Expression and purification of recombinant human Cosmc extracellular domain in Sf-9 insect cells

Objective:In the present study,Bac-to-Bac baculovirus expression system was used to obtain recombinant human Cosmc extracellular domain protein,which can lay the foundation for the research about the structure and function of Cosmc protein in vitro,and simultaneously provide ideas for the research of O-glycosylation and related diseases. Methods: The Cosmc extracellular domain ( Cosmc-ED) gene was cloned into a transfer vector pFastBac1 to form the recombinant donor plasmid pFastBac1-Cosmc ED, which was transformed into competent cells DH10Bac. By using blue-white selection and PCR analysis,we could obtain recombinant shuttle vector rBacmid-Cosmc ED. Then, the recombinant gene DNA of rBacmid-Cosmc ED was used to transfect Sf-9 mediated by cationic lipid formulation,and the recombinant baculovirus bacmid was obtained,which was further used to infect the serum-free cell Sf-9 to express Cosmc-ED in the supernatant. Then the protein of interest was detected by SDS-PAGE and Western blot and purified with Ni-NTA affinity column. Results:SDS-PAGE and Western blot analysis showed a specific band about 33 kD,consistent with the interest protein. Mass spectrometry results further prove that the protein was Cosmc extracellular domain protein. Conclusion: Human Cosmc-ED protein can be successfully expressed in Sf-9 insect cells and laid basis for subsequent studies.
7.Comparison of analgesic effect of ultrasound-guided continuous femoral nerve block after total knee ath-roplasty

Objective To observe the analgesic effect of ultrasound-guided and nerve stimulator after artificial total knee athroplasty(TKA).Methods Forty elective cases receiving TKA under gen-eral anesthesia were randomly allocated into the ultrasound group (group C)and stimulator group (group S).The time for nerve block,onset time and complications were recorded in both groups. Results Compared to group S,the time for nerve block and onset time was significantly shortened in group C (P <0.05).There was no statistical difference in times of pressing analgesic pump and VAS score in postoperative 48 h.One patient suffered from nerve injury and two underwent vascular dam-age and hematoma in group S,while no complication was found in group C.Conclusion Compared to nerve stimulator,ultrasound-guided continuous femoral nerve block(CFNB)may reduce nerve block time and onset time and decrease complications,so that to increase safety of CFNB.
8.Double-equilibration technique for measuring liquid/gas partition coefficients of isoflurane

This study was designed to establish methods to measure partition coefficients of inhaled anesthetics at 1 atmosphere and 37 C by using syringe-flask double equilibration technique. Olive oil/gas (O/G), saline/gas (S/G), and water/gas (W/G) partition coefficients of isoflurane were 90.20?2.01, 0.6256?0.109, and 0. 741?0.025, respectively. Even though O/G was 150 folds of S/G,coefficients of variation for both O/G and S/G were less than 3%. The results indicate that the established double-equilibration technique has a very high repeatability and is applicable to wide ranges of inhaled anesthetics' solubilities.
9.Influence of washable type subglottic attract on incidence of ventilator- associated pneumonia in patients with mechanical ventilation

ObjectiveTo explore the influence of washable type subglottic attract on incidence of ventilator-associated pneumonia (VAP) in patients with mechanical ventilation. Methods356 cases in need of mechanical ventilation from July 2009 to July 2010 in our hospital were chosen as the research object.They were randomly divided into the observation group and the control group with 178 cases in each group.The control group was treated with conventional endotracheal tube for mechanical ventilation.The observation group used washable type subglottic endotracheal tube to attract for mechanical ventilation.The VAP rate for the two groups occurred during mechanical ventilation in ventilator was compared.Migration of the dominant bacteria of the subglottic secretion was detected,underwent pathogenic examination and drug sensitivity test. ResultsThe VAP rate was significantly lower in the observation group than that of the control group.186 bacteria strains were detected from subglottic secretion,and 169 were detected from lower respiratory tract. ConclusionsWashable type subglottic attract for mechanical ventilation can reduce the risk of ventilator-associated pneumonia for patients with mechanical ventilation significantly,and it is worthy of clinical application.
10.Effects of localized hyperthermia on Langerhans cells in HPV-infected skin

Objective To observe the morphology, quantity and migration of Langerhans cells in normal or HPV-infected skin treated with localized hyperthermia. Methods Tissue specimens obtained from 16 patients with condyloma accuminatum (CA) and 15 normal controls were divided into three equal parts, and irradiated by a self-made hyperthermia equipment at 37℃, 42℃ and 45℃ respectively for 30 minutes. Immunohistochemistry and flow cytometry were applied to detect the morphology, quantity and migration of Langerhans cells (LCs), respectively, in these treated specimens. Results With a rise in temperature, the number of LCs in epidermis decreased, the dendrites shortened, decreased in number or even disappeared. After exposure to hyperthermia at 37℃, 42℃ and 45℃, the number of LCs was 782.40±114.8, 649.44±119.40 and 510.88±118.64 per square millimeter respectively, in normal tissue, 646.04±135.67, 489.38±118.19 and 387.93±110.15 per square millimeter respectively in HPV-infected skin tissue.The percentage of migratory LCs expressing CD1a was 0.19%±0.18%, 0.89%±0.19% and 1.59%±0.28% in normal skin tissue treated with hyperthermia at 37℃, 42℃ and 45℃ respectively, 0.62%±0.31%,2.31%±0.54% and 6.33%±0.98%, respectively, in HPV-infected skin tissue; the differences were significant among these different temperatures. Furthermore, the migration of LCs from tissue into culture was enhanced by the treatment with hyperthermia. Conclusions Hyperthermia can promote the migration of LCs, and accordingly enhance the antigen presenting effect of these cells in immune response.
