1.Biological effect of antisense transforming growth factor beta 1 in inhibiting hyperplastic scar of rabbit's ears

BACKGROUND: Nowadays, it is thought that transforming growth factorβ (TGFβ) is closely related with cicatrization. TGFβ that is a key active molecule can affect each phase of cicatrization. Theoretically, to inhibit the biological effect of TGF β can reduce cicatrization.OBJECTIVE: To explore the inhibitive effect of antisense TGF β1 deoxy-oligonucleotide on generation of cicatricle in intention of animal models with hyperplastic scars and observe the effective route of administration of using antisense TGF β1.DESIGN: Own control and animal study.SETTING: Department of Plastic Surgery, Anning Hospital of General Hospital, Lanzhou Military Area Command of Chinese PLAMATERIALS: The experiment was performed at the laboratory of anatomy,Lanzhou Medical College from September 2002 to July 2003. Totally 20flap-eared Japanese rabbits were selected.METHODS: Blood vessels could be seen in ventral surface of each rabbits' ear getting out of the way along long axis to establish two 1.0 cm×2.5 cm oblong full-thickness cutaneous deficiency raw surfaces that interval for 1.5 cm, to the surface of cartilage, totally 80, so asto establish ventral surface of rabbits' ear models with hyperplastic scars. After epithelizatio of raw surfaces of rabbits' ear (20 days, averagely), 5μL(1 g/L) antisense TGF β1 deoxy-oligonucleotide was closely injected into local endepidermis of each raw surface of left ear of each rabbit with microinjector, which was regarded as TGF β1 group. 5 μL saline was injected into each raw surface of right ears, which was regarded as saline control group. After injection for3, 7, 11, 20, 30 days, cicatricial tissues were cut, 4 rabbits in each time phase. Hematoxylin-esoin (HE) staining, Masson staining and TGFβ1 mRNA, type Ⅰ and Ⅲ collagen mRNA in situ hybridization histochemistry staining were applied.MAIN OUTCOME MEASURES: Results of HE staining, Masson staining and in situ hybridization histochemistry staining.RESUTLS: A total of 20 animals were included in the result analysis. ①HE staining showed that inflammatory cell infiltration and significant infiltrative zone of leukocytes occurred in hyperplastic scars of right ears in each group. There was inflammatory cell infiltration, but no infiltrative zone of leukocytes in hyperplastic scars of left ears after intervention with antisense TGF β1. ②Masson staining suggested that collagen fibers with deep blue-stain occurred in hyperplastic scars of right ears from the 3rd week after injury, till the 7th week there still was blue-stain collagen fibers,which was bulky (width of about 8-10 μm) and arranged in a great mess.The blue-stain collagenous fibers also appeared in hyperplastic scars of left ears at the 3rd week after injury by the intervention of antisense TGF β1,but till the 6th and 7th weeks the blue-stain became light and thin (width of about 3-5 μm), arranged in order. ③In situ hybridization revealed that expressive rates of TGF β1 mRNA, type Ⅰ collagen mRNA, type Ⅲ collagen mRNA positive cells decreased obviously.CONCLUSION: Antisense TGF β1 can inhibit the proliferation of hyperplastic scars of rabbits' ears and lighten markedly the fibrosis of cicatricial tissue. The local injection with naked DNA is feasible in the treatment of cicatricle.
2.Investigation and analysis of theatre nurses′ awareness of knowledge related to malignant hyperthermia

Objective To formulate countermeasures of improving nurses′knowledge of malignant hyperthermia by investigating theatre nurses. Methods A self-designed questionnaire was used to investigate 120 theatre nurses for awareness, demand and access about malignant hyperthermia. Results Average correct response rate on the awareness of theatre nurses about malignant hyperthermia was (17.14±9.68)%.There are differences in different education, title and seniority(F=8.002,6.509,2.643,P<0.05).100.0%(120/120)theatre nurses are interested in studying knowledge related to malignant hyperthermia, 94.2%(113/120) theatre nurses are willing to learn knowledge related to malignant hyperthermia. Conclusions The awareness of theatre nurses about malignant hyperthermia is low. It is necessary to strengthen the training of theatre nurses about the knowledge of malignant hyperthermia. A variety of forms can be adopted.
3.Value of exercise stress test guiding aerobic exercise rehabilitation in patients with myocardial infarction

Objective: To explore value of exercise stress test guiding aerobic exercise rehabilitation in patients with myocardial infarction.Methods: A total of 102 patients with myocardial infarction in recovery were selected.According to random number table, they were randomly and equally divided into routine rehabilitation group (received routine rehabilitation guidance) and aerobic exercise rehabilitation group (received aerobic rehabilitation exercise in guidance of exercise stress test).Amount of exercise, hemodynamic parameters and psychological state were compared between two groups.Results: After treatment, amount of exercise: compared with routine rehabilitation group, there were significant rise in amount of exercise[(6.79±0.58)METs vs.(7.42±0.69)METs], maximum heart rate[(126.27±5.26) beats/min vs.(138.18±5.81) beats/min], heart rate blood pressure product[(221.87±21.46) vs.(244.85±23.09)]and exercise duration[(10.36±1.36) min vs.(11.18±1.45) min]in aerobic rehabilitation group, P<0.05 or <0.01;hemodynamic parameters: compared with routine rehabilitation group, there were significant rise in cardiac output[(5.36±0.57) L/min vs.(5.72±0.68) L/min], cardiac index[(2.96±0.53) L min-1 m-2 vs.(3.25±0.52) L min-1 m-2], stroke volume[(73.16±8.47) ml vs.(78.12±9.14) ml]in aerobic exercise rehabilitation group, P<0.05 all;psychological state: compared with routine rehabilitation group, there were significant reductions in scores of Hamilton rating scale for anxiety[HAMA, (9.74±3.26) scores vs.(6.35±1.18) scores]and Hamilton rating scale for depression[HAMD, (14.83±4.58) scores vs.(10.56±3.27) scores]in aerobic exercise rehabilitation group, P<0.05 both.Conclusion: Exercise stress test guiding aerobic exercise rehabilitation can help to improve exercise function and hemodynamic indexes, alleviate the negative mood, then promote patients recover.
4.Effect of primarily cultured human lung cancer-associated fibroblasts on radiosensitivity of lung cancer cells

Objective To investigate the effect of human lung cancer-associated fibroblasts (CAF) on the radiosensitivity of lung cancer cells when CAF is placed in direct contact co-culture with lung cancer cells.Methods Human lung CAF was obtained from fresh human lung adenocarcinoma tissue specimens by primary culture and subculture and was then identified by immunofluorescence staining.The CAF was placed in direct contact co-culture with lung cancer A549 and H1299 cells,and the effects of CAF on the radiosensitivity of A549 and H1299 cells were evaluated by colony-forming assay.Results The human lung CAF obtained by adherent culture could stably grow and proliferate,and it had specific expression of α-smooth muscle actin,vimentin,and fibroblast activation protein,but without expression of cytokeratin-18.The plating efficiency (PE,%) of A549 cells at 0 Gy irradiation was (20.0 ± 3.9) % when cultured alone versus (32.3 ± 5.5) % when co-cultured with CAF (t =3.16,P < 0.05),and the PE of H1299 cells at 0 Gy irradiation was (20.6 ± 3.1) % when cultured alone versus (35.2 ± 2.3) % when co-cultured with CAF (t =6.55,P <0.05).The cell survival rate at 2 Gy irradiation (SF2) of A549 cells was 0.727 ±0.061 when cultured alone versus 0.782 ± 0.089 when co-cultured with CAF (t =0.88,P > 0.05),and the SF2 of H1299 cells was 0.692 ±0.065 when cultured alone versus 0.782 ± 0.037 when co-cultured with CAF (t =2.08,P >0.05).The protection enhancement ratios of human lung CAF for A549 cells and H1299 cells were 1.29 and 1.25,respectively.Conclusions Human lung CAF reduces the radiosensitivity of lung cancer cells when placed in direct contact co-culture with them,and the radioprotective effect may be attributed to CAF promoting the proliferation of lung cancer cells.
5.Prevalence of metabolic syndrome in patients with schizophrenia and its related factors

ObjectiveTo investigate prevalence of metabolic syndrome (MS) and related factors in patients with schizophrenia.MethodsFive hundred and three inpatients with schizophrenia and 60 healthy volunteers were included in this study.The demographic information and medical data of these participants were recorded,and their physiological and biochemical parameters were measured,the MS was diagnosed according to CDand IDF criteria.ResultsThe MS prevalence in patients with schizophrenia was 12.1% according to CDS criteria and 28.4% based on IDF standard,significantly higher than control group; the prevalence of MS was higher among female patients ( 15.3% or 39.8% ) than male patients ( 10.4% or 22.4% ) ; the prevalence of MS was higher among patients aged 40 or over ( 14.3% or 30.9% ) than patients aged 40 below (6.4% or 22.1% ).Dyslipidaemia (elevated TG and reduced HDL-C),obesity (BMI and waist) and increased blood glucose were the most important risk factors ( OR value > 6.0),increased total cholesterol,high blood pressure,elevated LDL-C,increased uric acid,female,in marriage,and paranoid type were also important risk factors (OR value > 2.0).Stepwise discriminate analysis showed triglyceride (0.569),BMI (0.451 ),HDL-C ( -0.298 ),gender (0.297 ),age (0.294)and LDL-C (0.210) 6 factors had a separate identification role in MS.ConclusionThe prevalence of MS in patients with schizophrenia in significantly higher than that in the general population,females are higher than males in the patients with patients,and older patients are higher than younger patients.Dyslipidemia,obesity and elevated blood glucose are the most important risk factors and predictors of the MS.
6.The relationship between gastric intestinal metaplasia and helicobacter pylori infection in the elderly

Objective To evaluate the relationship between intestinal metaplasia (IM) and helicobacter pylori infection in elderly patients with chronic gastritis. Methods The patients over 60 years old and diagnosed as chronic gastritis by endoscopy from January 2008 to December 2009 were divided into two groups by pathological appearance. One group was chronic gastritis with IM (IM group), the other was chronic gastritis without IM (NIM group). The infection of helicobacter pylori, activity of inflammation, atrophy of gland, and folliculus lymphaticus in gastric mucosa were observed. Results The infection of helicobacter pylori, infiltration of neutrophil and atrophy ratio of gland were significantly higher in IM group than in NIM group (52.7% vs. 37.3%, 35.5% vs.24.0 %, 36.6 % vs. 3.5%, x2 = 10. 677, 7.641, 95. 606; P = 0. 001, 0. 006, 0. 000, respectively).There was no statistical difference in presentation of folliculus lymphaticus between IM group and NIM group (x2 = 0.003, P=0. 957). The infiltration of neutrophil and presentation of folliculus lymphaticus were significantly higher in helicobacter pylori positive patients than in helicobacter pylori negative patients (45.8% vs. 14.8%, 42.6% vs. 4.2%, both P=0.000). Conclusions The appearance of IM seems to be related with helicobacter pylori infection, activity inflammation and atrophy of gland. The presentation of folliculus lymphaticus seems to be closely related with helicobacter pylori infection. The eradication treatment of helicobacter pylori infection in the elderly chronic gastritis patients should be strengthened.
7.The characteristics of clinical manifestations in patients with anti-γ-aminobutyric acid B receptor encephalitis

Objective To analyze the clinical features and prognosis in adult Chinese patients with anti-γ-aminobutyric acid B receptor (GABA-BR) encephalitis.Methods We reviewed the clinical manifestations,cerebrospinal fluid (CSF) examinations,brain magnetic resonance imaging and prognosis of 12 patients who were diagnosed as anti-GABA-BR encephalitis in Capital Medical University Xuanwu Hospital from March 2013 to December 2015.Results The major clinical features of anti-GABA-BR encephalitis patients included seizures (12/12),cognitive disorder (10/12),psychiatric symptoms (10/12),sleep disorders (3/12),visual hallucination (2/12),involuntary movements (2/12),decreased consciousness (3/12),cerebellar signs (2/12),fever (2/12).GABA-BR-antibody was positive in CSF and serum of all the patients.Electroencephalogram revealed epileptic discharges in 4/12 patients.Brain MRI showed abnormal signal in up to 9/12 patients,located in the hippocampus,temporal lobes,thalamus and periventricular area.PET or SPECT indicated hypometabolism in 5/6 patients.After the average of fourteen months follow-up,9/12 patients had good prognosis,3 patients did not.Of 3 patients with poor outcome,2 had small cell lung cancer.Conclusions The predominant clinical features of the anti-GABA-BR encephalitis are seizures,cognitive disorder and psychiatric symptoms.The lesions are not only located in limbic system.Early diagnosis and immune modulation may provide a good outcome.
8.A control study on changes of serum proteins and related enzymes in patients with schizophrenia

Objective To investigate changes of serum proteins and related enzymes in patients with schizophrenia and its relationship to metabolic syndrome.Methods Five hundred and three inpatients with schizophrenia and 60 healthy volunteer were selected in this study.The demographic information and medical history data of them were recorded and levels of serum proteins and enzymes were measured,and the metabolic syndrome (MS) was diagnosed according to IDF criteria.Results Serum protein contents of schizophrenic patients (total protein (71.14 ±6.13) g/L,albumin (44.56 ±4.24)g/L,globulin(26.35 ±5.04) g/L,apoprotein A(10.97 ± 0.27) g/L,and apoprotein B (0.69 ± 0.25) g/L) were significantly lower than control group (79.96 ± 6.10) g/L,(49.44 ±5.63) g/L,(30.52 ±4.00) g/L,(10.97 ±0.27) g/L,and(0.69 ±0.25)g/L (P<0.01).Urea nitrogen ((4.36± 1.36) mmol/L) was lower than control group ((5.22 ± 1.31) mmol/L) and C-reaction protein ((3.17 ± 6.58) mg/L) was higher than control group (1.35 ± 1.83) (P < 0.01).Alanine aminotransferase ((24.28 ± 32.76) IU/L),a-L Fucosidase((17.49 ± 4.83) U/L),adenosine dehydrogenase((17.81 ± 5.67) U/L),and creatine kinase isoenzyme((10.50 ±4.92)IU/L) were higher than control group (18.12 ±9.77)IU/L,(15.98 ± 3.58)U/L,(11.75 ±5.48) U/L,and (9.12 ±3.62)IU/L,P<0.01),but AST isoenzyme ((5.97 ±4.97) U/L)was lower than control group ((7.05 ± 6.72) U/L).Alanine Aminotransferase,alkaline phosphatase,γ-glutamyl transpeptidase,and a-L Fucosidase,AST isoenzyme and cystatin-c of schizophrenic patients with MS were significantly higher than non-MS patients(P < 0.05).Conclusion There are significant metabolic disorders of proteins and enzymes in patients with schizophrenia,and abnormal changes of many enzymes are significantly associated with metabolic syndrome.
9.Clients' evaluation on ethical behavior of their counselors or psychotherapists

Objective:To explore the clients' assessment for the ethical behavior of the counselors and psychotherapists.Methods:The main method used in this evaluation was questionnaire,and the self-made questionnaire of the ethical problems in psychotherapy and counseling(for the clients)were used as evaluation tools.Totally 1100 clients,from 12 cities in China,were investigated with 807 questionnaires returned.Results:84.5% of the clients were confident in the treatments,and 89.2% of them gave positive assessment for the ability of their counselors and psychotherapists.15.9% of clients said the counselors and psychotherapists showed impatience or talked to them about their stress sometimes.Besides,59.1% knew about the characters,names and charge of the psychotherapies and counseling,and 53.4% knew the background of the counselors and psychotherapists.91.3% of clients thought the privacy and treatment could be kept secret.7% of clients had intimate relationship with their counselors or psychotherapists,1.5% had sexual relations,and 18.3% made friends in lives.79% of clients could receive timely psychotherapies and counseling.Conclusion:From the assessment of the clients,the vast majorities of the counselors and psychotherapists do well in work ability,confidentiality and time-setting,but not so well in informed-consent and dual relations,which need to be paid attention to and improved.
10. Safety evaluation of early exercise out of bed in patients with partial hepatectomy

Objective:
To confirm early (6 hours) exercise in patients with partial hepatectomy is safe and feasible.
Methods:
The control group included 211 patients who qualified for partial liver resection before the accelerated rehabilitation surgery (ERAS), 218 patients with partial hepatectomy who met the entry criteria after the implementation of ERAS were taken as test group, get out of bed 24 hours after routine mission in control group, activity of 6 hours after operation in ERAS group. To observe the time of getting out of bed and the incidence of postoperative complications in 2 groups.
Results:
Comparison with control group, early exercise time of patients in ERAS group after surgery. The incidence of postoperative complications decreased was 14.3% (31/218) while 34.6%(73/211) of the control group. The difference was statistically significant (
