1.Arthroscopically assisted percutaneous screw fixation for 14 cases of patellar fractures

Objective To evaluate the effectiveness of arthroscopically assisted percutaneous screw fixation for treating patella fractures.Methods A total of 14 cases of patella fractures were treated by percutaneous reduction and internal fixation with screws under arthroscope from November 2001 to June 2004 in this hospital.The fracture was reduced by closed manipulation or percutaneous leverage force using the Kirschner wire.Then the patella was temporarily fixed by using a large-sized towel clamp or Kirschner wires.Under the guidance of knee arthroscopy or C-arm fluoroscopy,a micro-incision was made at the site of screw placement,the pilot holes were drilled at a proper depth,and the thread was configured.Two titanium screws or absorbable screws were inserted parallelly.Results Follow-up checkups for 6~18 months(mean,10 months) in 13 cases showed a satisfactory recovery of knee functions.According to the Xu Shaoding's standard,excellent effects were obtained in 11 cases(84.6%) and good effects,2 cases(15.4%).Conclusions Treatment of patellar fractures by percutaneous screw fixation under arthroscope offers advantages of minimal invasion,accurate reduction,reliable fixation,and excellent recovery of joint functions.
2.The role of interleukin-2 pathway in pathogenesis of nasal polyps.

OBJECTIVE:
To study the expression of Interleukin2 (IL-2) signaling pathway related factors and Treg cell in nasal tissue of nasal polyps, so that to investigate the possible mechanism of IL-2 signaling pathway in the progress of nasal polyps and the correlation between IL-2 pathway and Treg cell.
METHOD:
Thirty patients were enrolled for study, including the patients with nasal polyps and those with only deviated nasal septum as normal control. The nasal polyps tissue and the turbinate mucosa of the patients were collected during surgery. The expression levels of IL-2 and IL-2R were measured by means of ELISA. The level of pSTAT5 was evaluated by Western blot. We measured the level of Foxp3 mRNA in the tissue by real-time PCR, and the proportion of Treg cells was evaluated by flow cytometry. Finally. we analyzed the correlation between IL-2 pathway related factors and Treg cells in nasal polyps.
RESULT:
The expression levels of IL-2. IL-2R and pSTAT5 were significantly decreased in the nasal polyps compared with normal control (P < 0.05), and the level of Foxp3 mRNA and proportion of Treg cells in patients with nasal polyps were significantly lower than in normal control (P < 0.05). Moreover, there was positive correlation between the levels of IL-2 pathway related factors and the levels of Foxp3 mRNA and Treg cells proportion in nasal polyps.
CONCLUSION
The activated state of IL-2 signaling pathway got changed in nasal polyps tissue, the level of which was positively correlated with the expression of Treg cells, indicating that the IL-2 signaling pathway may play a crucial role in the development of nasal polyps, and the decreased level of Treg cells in nasal polyps may result from the downregulation of IL-2 signaling pathway.

3.The effect of burns on iron-related proteins in liver and spleen of different sex mice

Objective To investigate the changes of iron-related proteins in different sex mice before and after burns.Methods The burn model in mice were prepared with 90 ℃ vapor.Both liver and spleen iron contents were measured by colorimetric method.Hepatic hepcidin and Fpn1 mRNA content were determined with RT-PCR.The Fpn1 protein was determined with Western blot.Results After burns,the content of spleen iron was increased and the expression of hepatic hepcidin was decreased in male mice,and at the same time the Fpn1 of liver and spleen was reduced.The content of iron homeostasis abnormality in female mice was less serious.Conclusion The change of iron-related proteins after burns may be one cause of iron homeostasis abnormality.The differences of iron-related protein expression in different sex after burns suggest that estrogen has a potentially protective function.
4.Research of the mechanism of pre-B-cell colony-enhancing factor in pulmonary vascular endothelial permeability increasing after CPB

Objective To investigate the mechanism of PBEF in pulmonary vascular endothelial permeability increasing after car-diopulmonory bypass ,in order to provide the basis for a better lung protective measures during cardiopulmonary bypass .Methods Animal models were established ,group A :the rats were transfected with the lentiviral AD-PBEFshRNA ;group B :the rats were took 30 min deep hypothermic circulatory arrest ;group C :the rats were took 30 min deep hypothermic circulatory arrest ,then trans-fected with the lentiviral AD-PBEFshRNA ;control group :the rats were anesthetized and established CPB tube ,without CPB by-pass .Lung tissue was detected with Western blotting and ELISA .Results PBEF ,phosphorylation of P38MAPK ,ERK ,MLC ,VE-cadherin ,FAK in group C had significant difference with group A ,B and the control group(P<0 .05) .VEGF ,MMP2 ,MMP9 ,W/D in group C had significant difference with group A ,B and the control group(P<0 .05) .The pathological results showed that rat lung tissue of the control group was normal ,while in group C ,it had severe pathological damage ,the pathological damage degree in group A ,B reduced compared to group C .Conclusion PBEF can through MAPK/PI3K-Akt/VEGF signaling pathway ,increase pulmonary vascular endothelial permeability .
5.An inveatigation on adsorptive effect of attapulgite with Tanshinone Ⅰand Tanshinone Ⅱ_A in traditional Chinese medicine decoction

Purpose To investigate the adsorptive effect of attapulgite on Tanshinone Ⅰ and TanshinoneⅡ_A in decoction of Radix et Rhizoma Salviae Mihiorrhizae and Composite Dansen Pill.Methods Using the amount of solid component and the content of Tanshinone Ⅰ and Tanshinone Ⅱ_A as index,the adsorptive effect of common attapulgite and acidified attapulgite on decoction of Radix et Rhizoma Salviae Miltiorrhizae and Composite Dansen Pill was investigated respectively and the content of Tanshinone Ⅰ and Tanshinone Ⅱ_A was determined by HPLC.Results Common attapulgite could not markedly decreased the amount of solid component and had a litfle effect on the content of Tanshinone Ⅰ and TanshinoneⅡ_A in decoction of Radix et Rhizoma Salviae Miltiorrhizae and Composite Dansen Pill.The acidified attapulgite could obviously decrease the amount of solid component and had a little effect on the content of Tanshinone Ⅰ and TanshinoneⅡ_A in decoction of Radix et Rhizoma Salviae Miltiorrhizae and Composite Dansen Pill.Conclusion Acidified attapulgite could be used as adsorptive clarifier in decoction of Radix et Rhizoma Salviae Miltiorrhizae and Composite Dansen Pill.
6.The effect of hyperbaric oxygen combined with drug treatment on the cell cycle of colon carcinoma Lovo cells

Objective To investigate the effect of hyperbaric oxygen(HBO)combined with 5-fluorouracil (5-FU)chemotherapy on colon carcinoma Lovo cells.Methods Lovo ceils were exposed to 0.20 MPa HBO combined with 5-FU at different concentrations.The cell cycle was monitored with flow cytometry,and cell proliferation was detected using a methylthiazol tetrazolium test(MTT).Results ①After exposure to HBO,phase accumulation of Lovo cells was significantly higher than in the control group,and the accumulation of Lovo cells exposed to HBO after 24 h was significantly higher than at 12 and 48 hours.②There was difference between HBO(0.20 MPa,post 24 h)+ 5-FU(≤8 pM)group and 5-FU(≤8 μM,12 h)group on the inhibition of cells proliferation(P <0.05),and significan difference could be seen between the two groups after treatment for 48 h(P < 0.01); ③There was no significant difference between HBO + 5-FU group and 5-FU group treatment with the concentration(16,32,64 μM)of 5-FU(P > 0.05)for 12 h ;however,the difference could be seen after treatment for 48 h between the two groups(P < 0.05).Conclusion The lethal effect on Lovo cells of 5-FU can be enhanced by exposure to 0.20 MPa HBO,especially with low concentrations of 5-FU.
7.Effects of hyperbaric oxygen on the expression of hypoxia inducible factor-1α mRNA and vascular endothelial growth factor mRNA in colon adenocarcinoma cells

Objective To investigate the effects of hyperbaric oxygen (HBO) on the expression of hypoxia inducible factor-1α (HIF-1α) mRNA and vascular endothelial growth factor (VEGF) mRNA in colon adenocarcinoma cells. Methods SW480 colon cancer cells were divided into a control group and an HBO group, which was in turn divided into various subgroups according to the various HBO pressures (0.15 MPa, O. 20 MPa, 0.25 MPa) and exposure times (12, 24 and 36 h) tested. The expression of HIF-1α and VEGF mRNA was evaluated with RT-PCR after the various treatments. Results The expression of HIF-1α mRNA was lower in the SW480 cells than in the controls after exposure to 0.25 MPa HBO for 12 h. No expression of HIF-1α mRNA was detected after exposure to 0.25 MPa HBO for 24 h. The expression of VEGF mRNA was lower after exposure to 0.25 MPa HBO for 36 h than in the controls. Conclusion Absence of HIF-1α mRNA expression and decreased expression of VEGF mRNA in SW480 can be observed after exposure to 0.25 MPa HBO for 24 h, suggesting that HBO may inhibit the formation of tumor blood vessels through down-regulating the expression of these mRNAs.
8.Vascular intervention plus arteriovenous fistula for the treatment of acute deep vein thrombosis of the lower extremities

Objective To explore the appropriate method for mechanical thrombectomy, selection of vena cava filter and its duration in place. Method Fifty-five cases of severe acute deep vein thrombosis of the lower limbs were enrolled. Vena cava filter was first placed, then thrombectomy was performed with Amplatz thrombectomy device (ATD). The iliac vein lesions were dilated by balloon followed by temporary arterivenous fistula construction. Result Procedures were all successful in 52 cases, iliac vein stenosis was found in 47 cases and dilated by balloon. Permanent vena cava filters were placed in 20 cases while temporary in 32 cases. Fifty cases were followed-up for 3~30 monthes. Tumefaction disappeared in 46 cases leaving no after thrombosis syndrome. Conclusion This comprehensive modality is effective, less traumatic and of quick recovery. During the procedure, possible residual thrombi should always be under close attention and stenosis in iliac veins be confirmed by repeated venography. In most cases, placing a temporary vena cava filter was the best choice.
9.Risk factors leading to pulmonary embolism in patients of lower extremity deep vein thrombosis with iliac vein compression

Objective To evaluate risks of symptomatic pulmonary embolism (PE) in deep vein thrombosis (DVT) patients complicating iliac vein compression syndrome (IVCS).Methods Between January 2010 and January 2015,patients diagnosed with lower extremity DVT and IVCS at our institution were included.The odds of symptomatic PE versus lower extremity DVT complicated with IVCS were assessed using Logistic regression models.Its association with age,gender,thrombophilic risk factors,degree of stenosis,filter implantation and symptomatic PE were assessed.Results Of 759 patients,410 patients had inferior vena cava (IVC) filters implanted and 349 had not.In filter placement group,3 patients suffered from symptomatic PE among 32 with thrombi intercepted in IVC filters.In the group with no IVC filters placement,39 cases suffered from symptomatic PE.Symptomatic PE was in positive correlation to respiratory disease and right lower extremity DVT,and negative correlation to IVC filter implantation.Conclusions In lower extremity DVT patients complicated with IVCS,the right lower extrenity DVT and respiratory disease are risk factors of symptomatic PE.IVC filter implantation prevents symptomatic PE from happening in most patients.
10.Study of rural doctors′social mentality based on relative deprivation theory

Objective To analyze rural doctors′ social mentality with the theory of relative deprivation.Methods 642 rural doctors from 225 villages of 1 5 counties in Shandong were selected for a questionnaire survey.The factor analysis method was used to reduce the scale dimension and simplify the scale.Analysis of rural doctors′vertical and horizontal relative deprivation was made based on different reference groups.Results Rural doctors tend to identify themselves with rural teachers and doctors in township hospitals,and 60.0% identify themselves as the low income group.39.9% of them found a higher income,while 33.5% of them found a higher social status.76.3% of them found their social contributions are higher.91.4% of them hold that villagers benefit from the new healthcare reform policies,while 65.9% of them hold themselves as benefiting from such policies.This indicates a low sense of achievement.Conclusions Compared with the reference groups,rural doctors feel an obvious sense of relative deprivation comparing both horizontally and vertically.They hold themselves as underpaid and have little sense of achievement for the policies.In this consideration,the government should increase subsidies to lift pay for rural doctors,and to identify their legitimacy,in order to stabilize these medical workers.
