1.Intrathecal injection of dexmedetomidine reduces neuropathic pain in rat model of chronic constriction injury

Objective To investigate the effect of intrathecal injection (IT) of dexmedetomidine targeting Toll-like receptor 3 (TLR3) on neuropathic pain and spinal cord levels of TLR3 mRNA,interleukin-1β(IL-1β),tumor necrosis factor-alpha (TNF-α) proteins in rat model of chronic constriction injury (CCI).Methods Male Sprague-Dawley rats were randomly divided into five groups(n =10):the sham group(intrathecal normal saline,IT NS),CCI group (CCI + IT NS),DEX-pre group (CCI + IT DEX pre 2ds),DEX-post group (CCl + IT DEX post 7ds) and DEX + ATIP group (CCI + IT DEX + ATIP).The lumbar intrathecal catheters were implanted in L5_6 of rats and CCI models were established as previously described.The thermal and mechanical nociceptive thresholds were assessed by paw withdrawal latency(PWL) to radiant heat and yon Frey filaments.The DEX was administered intrathecally for 7 days starting from 2 day before surgery or 7day after surgery.The spinal cord expression of TLR3 mRNA was assessed by real-time polymerase chain reaction (PCR).Levels of IL-1β,TNF-α in spinal cord were detected by enzyme-linked immunosorbent assay (ELISA).Results Compared with the sham group,animals in CCI group had significandy lower mechanical (F =12.73,P < 0.05) and thermal pain thresholds (F =14.65,P < 0.05),higher expression of TLR3 mRNA (F =11.03,P < 0.05) and levels of IL-1 β (F =9.67,P < 0.05),TNF-α(F =8.78,P < 0.05) in the spinal cord (P < 0.05).Rats in DEX-pre group had significantly higher mechanical (F =11.03,P < 0.05) and thermal pain thresholds (F =15.03,P < 0.05) and significantly lower expression of TLR3 mRNA (F =14.65,P <0.05) and levels of IL-1β (F =12.51,P<0.05,TNF-α (F =9.01,P <0.05) in the spinal cord compared with those in the CCI group (at any observed time points after ligation,but most significantly at 7 d).And the effects of DEX-pre group were abated by IT ATIP at the same time or 7days after surgery alone (P < 0.05).Conclusions Intrathecal injection of DEX can decrease the levels of inflammatory factors by decreasing the TLR3 mRNA in the spinal cord of rats and prophylactic relieve the neuropathic pain induced by CCI.

Objective To observe the effect of intrathecal injection (IT) of oxycodone hydrochloride on neuropathic pain and spinal cord level of microglial c-Jun N-terminal kinase/chemokine (C-X-C motif) ligand 1 (c-JNK/CXCL) 1signal in rat model of chronic constriction injury (CCI).Methods Male Sprague-Dawley rats were randomly divided into five groups (n =40 per group):sham group (intrathecal normal saline,IT NS),CCI group (CCI + IT NS),oxy group (CCI + IT 5 μg/30 μl oxy),mino group (CCI + IT 5 μg/30 μl Minocycline),and c-JNK inhibitor group (SP group,CCI + IT 5 μg/30 μl SP600125).The lumbar intrathecal catheters were implanted in L5-6 of rats and CCI models were established as previously described.The thermal and mechanical nociceptive thresholds were assessed by paw withdrawal latency (PWL) to radiant heat and von Frey filaments.The oxycodone,minocycline and SP600125 were administered intrathecally for 3 days before surgery.The spinal cord expression of Ⅰ ba-1,p-c-JNK and CXCL1 proteins assessed by Western blot.Immunofluorescence staining was performed to examine microglia morphology and the number of Ⅰ ba-1 positives cells in dorsal horn of injured spinal cord at 7 days post-IR.Results Compared to sham group,rats in CCI group had significantly lower mechanical and thermal pain thresholds,but higher spinal proteins expression of Ⅰ ba-1,and p-c-JNK and CXCL1 (P <0.05).Rats in oxy group,mino group and SP group had significantly higher mechanical and thermal pain thresholds and significantly lower proteins expression of Ⅰ ba-1,p-c-JNK and CXCL1 compared to those in CCI group (at any observed time-point after ligation,but most significantly at 7days,P < 0.05).At the 7days after surgery,microglial cells in CCI group transformed from the ramified shape to amoeboid macrophage-like shape by immunofluorescence staining with the increases of Ⅰ ba-1 positive cells;while the other three groups exhibited hypertrophic morphology with less number Ⅰ ba-1 positive cells (P < 0.05).There were no significant differences between these three groups at any observed time (P > 0.05).Conclusions Intrathecal injection of oxycodone hydrochloride can relieve CCI-induced neuropathic pain by down-regulation microglial c-JNK/CXCL1 signal in spinal cords.Provide new therapeutic targets for clinical treatment of neuropathic pain.
3.Intrathecal injection of dexmedetomidine attenuates neuron disruption induced by spinal cord ischemia-reperfusion injury in rabbits

Objective To investigate the beneficial effects and possible mechanisms of intrathecal injection of dexmedetomidine (DEX) in protection of spinal cord ischemia-reperfusion injury.Methods Spinal cord ischemia-reperfusion injury was induced in rabbits by infrarenal aortic occlusion for 30 minutes.Twenty-four rabbits were randomly divided into four groups:sham group,I/R group,DEX group (1 μg/kg dexmedetomidine by intrathecal injection),DEX + ATIP group (1 μg/kg dexmedetomidine and 0.5 μg/kg atipamezole (ATIP) by intrathecal injection).Physiological indices were assessed and motor neurons in the ventral gray matter were counted by histological examination.The expression of total extracellular signal-related kinase (t-ERK),phospho-extracellular signal-related kinase (p-ERK),and caspase 3 were assessed by Western blotting and real-time polymerase chain reaction (RT-PCR).The water content in spines was also measured.Results Intrathecal injection of dexmedetomidine minimized the neuromotor dysfunction and histopathological deficits(F =14.32,P < 0.05) and attenuated the influences of peroxidation at 24 hours after spinal cord ischemia-reperfusion injury.The physical indices during experiment were comparable among all groups.In addition,intrathecal injection of dexmedetomidine reduced the excessive expression of caspase 3 mediated by ERK signal pathway(F =15.45,P < 0.05).Conclusions Pre-emptive intrathecal injection of dexmedetomidine produced beneficial effects on spinal cord after spinal cord ischemia-reperfusion injury in a rabbit model of transient aortic occlusion.This beneficial effect was partly attributed to the inhibition of caspase 3 mediated by ERK signal pathway,which represents a potential therapeutic approach to mitigate spinal cord injury after aortic occlusion.
4.Mechanism and effects of NMDA receptor NR1 subunit on chronic constriction injury-induced neuropathic pain in a rat model

Objective To investigate the effects of intrathecal injection (IT) of agonist and antagonist of N-methyl-D-aspartate receptor subunit 1 (NMDA NR1) on pain threshold and spinal levels of glutamate and glutamate transporter-1 (GLT-1) in rat model of chronic constriction injury (CCI) induced neuropathic pain.Methods Sprague-Dawley (SD) rats were randomly divided into 4 groups (Sham,CCI,NM-DA,and Humanin).The CCI model was established by right sciatic nerve constriction.The thermal and mechanical thresholds were assessed by paw withdrawal latency (PWL) to radiant heat and yon Frey filaments at 1,3,5,7,10 and 14 days as well as behavior after CCI.The NMDA and Humanin groups were administered intrathecally for 3 days before surgery.Spinal expression of GLT-1 assessed by Weston blotting and levels of glutamate were measured by enzyme-linked immunosorbent assay (ELISA).Results Compared to Sham group,the rats in CCI group gradually appeared the symptoms of toe close together,foot valgus and repeated licking to the operative side of the hind legs after surgery,and the paw withdrawal threshold (PWT),and PWL were greatly decreased,reaching the lowest level on the 7th day.Meanwhile,the glutamate content was increased,and the GLT-1 expression was decreased in spinal cord (P < 0.05).Compared to CCI group,the rats in NMDA group were observed with further decrease in PWT,PWL and GLT-1 expressions at all observed timepoints (P < 0.05),but increase in glutamate content in spine cord (P <0.05).Compared to CCI group,the rats in Humanin group were observed with increase in PWT,PWL and GLT-1 expressions at all observed timepoint (P < 0.05),but decrease in glutamate content in spine cord (P < 0.05).Conclusions NMDA receptor NR1 subunit participated in regulating neuropathic pain,inhibiting NMDA receptor NR1 subunit can alleviate neuropathic pain by down-regulating of glutamate and GLT-1.

During the 17 days of relief efforts, the team treated and cured close to 3 000 victims, winning high praise from the government of the disaster-hit country, We have learned from organizing the relief work that effective organization and coordination is the key to smooth preparation, powerful ideological work is the guarantee of fulfilling the task, careful training and technical support is the basis of ensuring the success of the task, sufficient material preparation is the essential condition for completing the task, and unimpeded communication is beneficial to understanding the situation and seeking countermeasures in a limely manner.
6.THE IMMUNOCYTOCHEMICAL OBSERVATION OF NEUROENDOCRINE CHARACTERISTIC IN MICE THYMIC NURSE CELLS

The arginine vasopressin (AVP) and oxytocin (OT) in thymic nurse cells (TNCs) of mice were stained by the method of immunogold and indfrect immunofluorescence, PAP immunocytochemical technique. TNCs of mice were shown immunocytochemical reactivity with antfserum of AVP and OT. The posttive areas were much denser near the cells membrane and around TNC-lymphocytes (TNC-Ls). It was implicated that TNCs might serve as neuroendocrine cells and might play a role in TNC-Ls differentiation via secreting AVP and OT.
7.95 % effective target plasma concentration of remifentanil for tracheal tube tolerance during recovery period from anesthesia following cervical spine surgery

Objective To determine the 95 % effective target plasma concentration (EC95) of remifentanil for tracheal tube tolerance during the recovery period from anesthesia following cervical spine surgery.Methods Thirty ASA Ⅰ or Ⅱ patients,aged 18-60 yr,weighing 50-80 kg,scheduled for elective cervical spine surgery under total intravenous anesthesia,were enrolled in this study.Anesthesia was induced with iv injection of propofol,sufentanil and rocuronium.The patients were tracheal intubated and mechanically ventilated.Anesthesia was maintained with iv infusion of propofol and target-controlled infusion of remifentanil.The target plasma concentration (Cp) of remifentanil was set at 4-6 μg/L.BIS value was maintained at 40-60.Infusion of propofol was stopped at the end of surgery.Participants were allocated to a dose of remifentanil by 3-patient cohorts.Six Cps were selected from 1.0-3.5 μg/L before beginning and they were 1.0,1.5,2.0,2.5,3.0,3.5 μg/L.The Cp of remifentanil was 3.0 μg/L in the first cohort.After completion of the trial in each cohort,the posterior probability of each concentration was calculated according to the condition of sedation/analgesia and anterior probability of each concentration.The concentration with the posterior probability closest to 95 % was chosen as Cp in the next cohort.The concentration-probability curve was made according to the posterior probability of each concentration,and then EC95 and 95 % confidence interval of remifentanil were calculated.Results The EC95 and 95 % confidence interval of remifentanil were 2.77μg/L (2.65-2.83 μg/L) for tracheal tube tolerance during the recovery period from anesthesia following cervical spine surgery.Conclusion The EC95 of remifentanil for tracheal tube tolerance during the recovery period from anesthesia is 2.77 μg/L in patients undergoing cervical spine surgery.

This report presents microscopic observation on morphological and immuno- cytochemical characteristics of the trophoblast cells of the human decidua at the implantation site in the first trimester pregnancy .The infiltration of a unique type of large cells different from cytotrophoblast (CT) and syncytotrophoblast (ST) was found in the decidua at the implantation site. These unique cells are termed inter- mediate trophoblast (IT). They are typically mononucleate, but may be binucleate or multinucleate. The mononucleate cells vary in shape from round, polyhedral spindle shaped. Their cytoplasm is typically abundant and eosinophilic or amphophilic. The nuclei may vary in size and shape. These cells usually distribute around the spiral arterioles, differse blood vessel wall, penetrate into the lumen, and even replace the blood vessel wall totally. Immunocytochemically, both these cells and ST are stained positive for HCG and HPL. However, the HCG-stained Cell population of IT.is much lower than that of ST,while the of HPL stained cell population of IT is significantly higher than that of ST.On the other hand,neither HCG nor HPL are positive in CT. The results of SP-1 ?-HCG usually go with those of HCG.in CT, ST and IT.The PAPP-A gives non specific staining result. It is believed that IT,with its distinct morphological and immunocytochemical feats,is regarded as a transitional form in the shift from CT to ST.
10.Mechanism and Effects of Cannabinoid Receptor 1 on Sciatic Nerve Ligation-induced Neuropathic Pain in Rat Model

Objective To observe the effects of intrathecal injection(IT)of agonist and antagonist of cannabinoid receptor 1 on pain threshold in rat model of sciatic nerve ligation(SNL)induced neuropathic pain,and investigate the role and mechanism of CB1 in neuropathic pain. Methods Male Sprague-Dawley rats were randomly divided into 4 groups:sham group( intrathecal normal saline,IT DMSO),SNL group(SNL+IT 30μL DMSO),AM841 group(SNL+IT 5μg AM841,dissolved in 30μL of DMSO)and AM281 group(SNL+IT 5μg AM281,dissolved in 30μL of DMSO). IT was given 3 days before surgery. Sham group only had sciatic nerve exposure but without ligation. SNL model in the other three groups were established by right sciatic nerve ligation. The thermal and mechanical thresholds were assessed by paw withdrawal latency(PWL)to radiant heat and von Frey filaments at 1,3,5,7,10 and 14 days as well as behavior after SNL. Spinal expressions of CB1 were assessed by West-ern blotting. Results Compared with the sham group,symptoms of rats in SNL group,such as heat hyperalgesia,mechanical allodynia and poste-rior paws prone to close together,were gradually appeared in the observation time points,with lower spinal proteins expression of CB1(P<0.05). AM841 group exhibited increased proteins expression of CB1 and inhibited SNL-induced heat hyperalgesia and mechanical allodynia(P<0.05). AM281 group further decreased expression of CB1 and amplified the pain abnormality(P<0.05). Conclusion Spinal CB1 participates in the regulation of neuropathic pain,and exogenous cannabinoid CB1 agonists can alleviate the SNL-induced neuropathic pain.
