1.Clinical analysis of 64 cases of diabetes complicated with multiple organ failure

Objective To analyze the inducement,clinical manifestations,diagnosis and treatment of diabetes patients with multiple organ failure(MOF).Methods A retrospective analysis of 64 diabetes patients with multiple organ failure was executed,the clinical data were statistically analyzed.Results Heart failure was the most common in 64 cases with a incidence of 100.0%,the rest common failure organ were kidney(92.2% ),brain( 32.8% ),lung ( 14.1% ),gastrointestinal tract (3.1% ),and liver( 1.6% ).Cases with the glucose level over 33.5mmol/L illustrated a significantly higher incidence than other groups ( P < 0.05 ),glucose level between 22.5 and 33.4mmol/L group was significantly higher than the 12.2 ~ 22.4mmol/L group( P < 0.05 ) The inducement of MOF were infection (76.6% ) medication terminate ( 9.4% ),ketoacidosis ( 7.8% ),cardiovascular and cerebrovascular accidents (6.3% ).Conclusion Positive control of blood glucose in the early stage of diabetes,prevent complications,eliminate the incentive could reduce the incidence of MOF,eventually reduce the mortality rate.
2.Study of virtual touch tissue quantification in noninvasive assessment of the effect of transjugular intrahepatic portosystemic stent shunt

Objective To explore the feasibility of virtual touch tissue quantification(VTQ) technique in noninvasive assessment of transjugular intrahepatic portosystemic stent shunt (TIPSS).Methods Twenty-four patients were enrolled in this study.Shear wave velocity(SWV) of the liver and spleen were obtained by VTQ technique on one day before TIPSS insertion and one week after TIPSS placement,and the portal vein pressure was measured while placing the TIPSS.The correlation between SWV of the liver and spleen and portal venous pressure were analysed.In addition,fourteen patients were followed up for one month after TIPSS placement.Results There was significant difference in portal vein pressure pre [(27.82 ± 4.07) mm Hg] and post [(16.26-± 3.10) mm Hg] TIPSS insertion.The SWV of the liver in patients pre and post TIPSS measured (2.43 ± 0.37)m/s and (2.40 ± 0.34)m/s,respectively.The SWV of the spleen in patients pre and post TIPSS were (3.49 ± 0.40)m/s and (3.08 ± 0.39)m/s,respectively,there was significant difference (P =0.000).However,SWV of the liver pre and post TIPSS placement was no significant difference(P =0.130).And as well as the patients which were followed up for one month after TIPSS placement.A significant,direct,linear correlation(r =0.602,P =0.000) was found between SWV of the spleen and portal vein pressure,but liver SWV was not correlated with the portal venous pressure.Conclusions SWV of the spleen measured by VTQ technique has a significant correlation with portal vein pressure.Hence,the spleen SWV based on VTQ technique can be used as a non-invasive method for quantitatively in monitoring the portal vein pressure as the function of the TIPSS.However,evidence to support a similar role for liver stiffness is lacking.
3.Influential factor and management of biliary tract complications after liver transplantation

Objective To explore the influential factor and management of biliary tract complica-tions after liver transplantation. Method Clinical data of 57 patients who underwent liver transplantation between May 2006 and November 2007 were studied retrospectively. Results Among the 57 patients, 8patients (14.04%) developed postoperative bililary tract complications,4 patients with biliary leakage, 2patients with anastomosis stricture, 1 patient with intrahepatic biloma, all above eases were fully recover, 1patient with anastomosis stricture and intrahepatie biliary tract casting mould received liver retransplantation.Conclusions The management of biliary tract complications after liver transplantation are difficult. To at-tach importance to influential factor and prevention, timely diagnosis and treatment will improve patients' sur-vival time and quality of life.
4.Isolation of tick-borne encephalitis virus Far-eastern subtype and Siberian subtype in the China-Kazakhstan border area in Xinjiang

To investigate the genotype and the biological characteristics of Tick-borne encephalitis virus (TBEV) in Charles Hilary endemic foci in the China-Kazakhstan border area in Xinjiang,ticks were collected by flagging during May to June in 2012 and 2014,and were stored in liquid nitrogen.TBEV strains were isolated from tick samples by inoculating BALB/c mice and BHK-21 cells.The FE gene fragments of TBEV-Far and the S gene fragments of TBEV-Sib were detected by RT-PCR from infected mice brain tissue and BHK cells,and then subjected to sequence alignment.Totally 16 TBEV strains were isolated from Ixodes persulcatus and Dermuceuter silvarum,among 13 strains were Far eastern subtype,three strains were Siberian subtype.It was first time that the TBEV-Sib was isolated in China.The Charles Hilary TBE natural foci were in the China-Kazakhstan border area,and both TBEV-Far and TBEV-Sib co-circulated.
5.Construction of pseudovirus reference standard containing five kinds of hemorrhagic fever virus

Objective To construct the pseudovirus containing nucleic acid(NA)fragments of Marburg virus,Zaire Ebola virus,Sudan Ebola virus,Lassa fever virus and Yellow fever virus by using a lentiviral vector system in order to provide a reference standard for the detection of the five viruses.Methods The gene fragments of the above five viruses were synthesized in vitro,connected into a single gene by fusion PCR technique,and cloned into lentiviral vectors with its auxiliary vector.After co-transfecting into 293T cells,the supernatants were collected on 48 h and 72 h post transfection. The naked NA was cleaned from the supernatants using DNase and RNase digestion before pseudotype virus was purified and concentrated.After the NA of the pseudotype virus,were extracted normal PCR and real-time PCR were conducted. Results Sequence analysis showed that the five target genes in vitro synthesis were properly connected and inserted into lentivirus vectors.Using the NA of the pseudotype virus as the template,both normal PCR and real-time PCR could sensitively amplify the target gene with the primers and probes of the above five,viruses respectively.The result indicated that the pseudovirus particles containing the five kinds of hemorrhagic fever virus target genes were successfully packaged. Conclusion The pseudovirus particles containing gene fragments of five viruses are constructed,which can be used as a common reference standard for NA detection.
6.Surveillance of bacterial resistance in Tongling People′s Hospital during 2013

Objective To investigate the antimicrobial resistance of clinical isolates in Tongling People′s Hospital during 2013. Methods A total of 2 281 nonduplicate clinical isolates were collected.Kirby-Bauer disc diffusion method was employed to study the antimicrobial susceptibility.The data were analyzed with WHONET 5.6 software according to CLSI 2012 breakpoints. Results The top 5 most frequently isolated microorganisms were E.coli (479,21.0%),K.pneumoniae (360,15.8%),A. baumannii (271,11.9%),P .aeruginosa (240,10.5%),S.aureus (171,7.5%).Gram negative and gram positive microorganisms accounted for 76.5% and 23.5%,respectively.The prevalence of methicillin-resistant strains in S.aureus (MRSA)and coagulase negative Staphylococcus (MRCNS)was 38.6% and 73.1%,respectively.The resistance rates of MR strains to beta-lactams and other antimicrobial agents were much higher than those of MS strains.No staphylococcal strain was found resistant to vancomycin or teicoplanin.E.faecalis showed relatively lower resistance to penicillin,ampicillin and nitrofurantoin.E.faecium strains were more resistant than E.faecalis to most of the antibiotics tested.Approximately 50.5% of E.coli and 44.5% of Klebsiella isolates produced extended-spectrum beta-lactamases (ESBLs).The ESBLs-respectively.And 29.8% and 23.4% of the P .aeruginosa strains were resistant to imipenem and meropenem.Nearly all (94.0%)P .aeruginosa isolates were susceptible to amikacin.Conclusions There appears a trend of increasing resistance in the clinical bacterial isolates in this hospital,especially the carbapenem-resistant Enterobacteriaceae,which is of great concern.It is mandatory to take effective antibiotic policy and infection control measures.
7.Spectroscopic Study of Interaction of Harpagoside and Human Serum Albumin

Harpagoside (HAR) is believed to be a main compound in Scrophularia ningpoensis which possess a broad of biological activities.Human serum albumin (HSA) has important physiological roles in transportation, distribution and metabolism of many endogenous and exogenous substances in body.It is great significance in pharmacology to investigate the interaction mechanism of HAR and HSA.In this work, the interaction between HAR and HSA was investigated by fluorescence and ultraviolet absorption spectroscopy at different pH (pH=4.0, 7.4, and 9.0) and temperatures (297, 310 and 323 K).The experimental results showed that the HAR could cause the fluorescence quenching of HSA through a static quenching procedure, showing that the HAR regularly quenched the intrinsic fluorescence of HSA, and a decrease in the quenching constant was observed with an increase in temperature.Under different conditions, all the magnitude of binding constants (KA) was larger than 105 L/mol and the number of binding sites (n) in the binary system were approximate to 1.Base on the magnitude of enthalpy and entropy changes, the negative values of ΔG, ΔH and ΔS revealed that the binding of HAR with HSA was spontaneous and exothermic process, and the main interaction forces of the HAR with HAR were van der Waals forces and/or hydrogen bonding interaction.The binding distance (r) between the HAR and HSA was calculated to be about 4.2 nm based on the theory of F(o)rster′s nonradiation energy transfer, which indicated that the energy transfer from HSA to HAR occurred with high possibility.What was more, the synchronous florescence spectroscopy confirmed the conformational changes of HSA during the binding reaction.
8.Study on the combined application of laparoscopic cholecystectomy and endoscopic sphincterotomy in treatment of cholecystolithiasis and secondary choledocholithiasis

Objective To study the therapeutic effects of combined use of laparoscopic cholecystetomy and endoscopic sphincterotomy for cholecystolithiasis with secondary choledocholithiasis.Methods Thirty-five patients were diagnosed as cholecystolithiasis with secondary choledocholithiasis by B-ultrasonography and magnetic resonance cholangiopancreatography.Of them,in 28 cases,laparoscopic cholecystetomy was performed first,and ERCP and endoscopic sphincterotomy were done one week later;in 7 cases,endoscopic sphincterotomy were performed before laparoscopic cholecystectomy.Results The outcome of all the thirty-five cases was satisfactory without severe complications or conversion into open procedure.Conclusions The method of combined laparoscopic cholecystomy and endoscopic sphincterotomy,for cholecystolithiasis with secondy choledocholithiasis,especially for cases in whom the diameter of the common bile duct stone is ≤1cm,can give good therapeutic results and has advantages of minimal invasiveness,few complications and quick recovery.
9.Study on Hydrolyzed Peptides of Red Cubilose and White Cubilose by HPLC-ESI-MS

HPLC-ESI-MS technology was applied to determine the hydrolyzed peptides of red cubilose and white Abscubilose for the comparison of similarities and differences between different kinds of cubiloses' hydrolysis polypeptide primary structure.SDS-PAGE,capillary high-performance liquid chromatography and ESI-MS were used to detect the purity and peptide mapping of samples.Mascot 2.2 sof tware was used to search the database.The basepeak figure in mass spectrometry analysis showed good consistency of hydrolyzed peptides between red cubilose and white cubilose.The Mascot 2.2 sof tware was used to search the poultry database which had been downloaded from the UniProt.A total of 73 kinds of proteins from white cubilose were identified.And a total of 51 kinds of proteins from red cubilose were identified.There were 30 kinds of same proteins from both cubilose.It was concluded that the hydrolyzed peptides had relatively high similarity between red cubilose and white cubilose.The same proteins from both red and white cubilose can be used as the quality control characteristics of cubilose products.
10.Clinical analysis of early application of nimodipine combined with lumbar cistern drainage in the treatment of severe traumatic brain injury

Objective To explore the clinical value of early application of nimodipine combined with lumbar cistern drainage in the treatment of severe traumatic brain injury .Methods a total of 68 cases elderly patients with severe craniocerebral injury in our hospital from 2015 to 2017 were selected ,which were divided into control group and observation group according to random number table method .The control group was given routine medical treatment ,and the observation group was given nimodipine combined with lumbar cistern drainage on the basis of the control group .The Glasgow Coma Scale (GCS) score ,cerebral CT and blood flow velocity measurement of middle cerebral artery (MCA) were was performed ,and the recovery and prognosis were compared in the two groups .Results The GOS score and MCA blood flow velocity of the observation group at 7 ,14 and 21d after treatment were significantly higher than those in the control group(P<0 .05);the decline rate of intracranial pressure in the observation group at 7 ,10 and 14d after treatment was significantly higher than that in the control group (P<0 .05);the probability of infection in the observation group was significantly lower than that in the control group (P<0 .05);the prognosis rate of the observation group was significantly better than that of the control group(P<0 .01) .Conclusion Early use of nimodipine combined with lumbar continuous drainage in the treatment of severe traumatic brain injury can effectively improve the clearance rate of subarachnoid hemorrhage ,re-duce the occurrence of secondary brain injury ,avoid cerebral vasospasm ,and improve the prognosis of patients.
