1.Clinical analysis of 64 cases of diabetes complicated with multiple organ failure

Objective To analyze the inducement,clinical manifestations,diagnosis and treatment of diabetes patients with multiple organ failure(MOF).Methods A retrospective analysis of 64 diabetes patients with multiple organ failure was executed,the clinical data were statistically analyzed.Results Heart failure was the most common in 64 cases with a incidence of 100.0%,the rest common failure organ were kidney(92.2% ),brain( 32.8% ),lung ( 14.1% ),gastrointestinal tract (3.1% ),and liver( 1.6% ).Cases with the glucose level over 33.5mmol/L illustrated a significantly higher incidence than other groups ( P < 0.05 ),glucose level between 22.5 and 33.4mmol/L group was significantly higher than the 12.2 ~ 22.4mmol/L group( P < 0.05 ) The inducement of MOF were infection (76.6% ) medication terminate ( 9.4% ),ketoacidosis ( 7.8% ),cardiovascular and cerebrovascular accidents (6.3% ).Conclusion Positive control of blood glucose in the early stage of diabetes,prevent complications,eliminate the incentive could reduce the incidence of MOF,eventually reduce the mortality rate.
2.Nursing for patients with blepharocoloboma reconstruction with island flap of frontal branch of superficial temporal artery

Objective To evaluate the perioperative nursing and its efficacy in the blepharocoloboma reconstruction with island flap of frontal branch of superficial temporal artery. Methods A retrospective nursing experience was summarized in 5 patients undergoing blepharocoloboma reconstruction with island flap of frontal branch of superficial temporal artery. We paid attention to the mental nursing of patients besides the routine nursing.The growth of skin flap was scrupulously oberved and gave selective disposal when untoward problems occurred. Results The skin flap survived in four cases and epidermis necrosis with flap basis survive took place in one case.The color and quality of survived skin flap was similar to adjacent skin which could effectively protect eyeballs.No severe complications occurred. Conclusions Effective mental nursing and health instruction before operation and careful observation and nursing of skin flap after operation proved to be pivotal for successful operation.
3.Comparison of the intrusion effects on the maxillary incisors between implants and conventional method:a Meta-analysis

Objective To objectively evaluate the intrusion effects on the maxillary incisors of implants versus conventional method(J-hook headgear ,utility arch) .Methods Such online databases as Cochrane library ,PubMed ,EMbase ,VIP ,Wanfang and CNKI were searched by randomized control test and prospective case controlled study about implants vs conventional anchorage for the maxillary incisor intrusion .After study selection ,assessment and data extraction conducted ,meta-analyses were performed by u-sing the RevMan 5 .1 software .Results Totally 11 studies were included ,6 studies involving 154 patients were included in the J-hook headgear group ,5 studies involving 127 patients were included in the utility arch group .The results of meta-analyses showed that :(1)Compared with the J-hook headgear ,implants had more decrement in the distance of upper center incisior cutting to palatal plane(P<0 .001) ,more decrement in overbite(P=0 .001) ,shorter treatment time (P<0 .001) ,less increase in SN-OP angle(P=0 .004) ,less decrement in distance of maxillary molar cusp to palatal plane (P=0 .020);There was no statistically significant differ-ence in overjet ,root resorption(P>0 .05) .(2)Compared with the utility arch ,implants had more decrement in the distance of the center of resistance of the upper incisor to palatal plane (P=0 .010) ,less increase in U6-PP angle (P<0 .001);There was no statis-tically significant difference in the distance of upper center incisior cutting to palatal plane ,U1-PP angle ,the distance of upper first Molar cutting to palatal plane ,sagittal position of the mesial tip of the upper first Molar ,overbite ,overjet ,course of treatment ,root resorption(P>0 .05) .Conclusion The implants anchorage intruded the maxillary incisor was better than conventional method (J-hook headgear ,utility arch) ,it has more incisor intrusion ,shorter treatment time and more comfortable for the patients ,there was no difference in root resorption between the two groups .
4.Assessment of risk factors for acute lower-limb deep venous thrombosis

Objective To observe the changes of coagulation function in patients with acute lowerlimb deep venous thrombosis(DVT)and evaluate the risk factors for DVT. Methods Plasma APTT.PT,TT,D-dimer and fibrinogen(Fbg)were detected by an automated coagulation analyzer in 62 acute lower-limb DVT patients and 70 controls:Retrospective studies on the clinical data of all patients were done by binary logistic regression analysis.Results (1)In DVT group,plasma APTT,PT and TT,the levels of D-dimer and fibrinogen.and D-dimer/fibrinogen ratio(D/F ratio)were higher when compared with control group(au P<0.01);(2)There were positive correlations between D-dimer and fibrinogen both in DVT and control groups(r=0.475,P<0.01;r=0.564,P<0.01,respectively);(3)Logistic analysis indicated that acute lower-limb DVT was associated with the presence of hypertension and increased plasma level of fibrinogen(OR=24.99,P<0.01: OR=4.346.P<0.01,respectively).Conclusions Hypertension and elevated plasma level of fibrinogen are independent risk factors for acute lower-limb DVT.
5.Differentiating true progession from pseudoprogression in patients with gliobastomas using dynamic contrast-enhanced MRI

Objective To investigate the clinical application and manifestation of dynamic contrastenhanced MRI (DCE-MRI) in differentiating true progession from pseudoprogression in patients with gliobastomas.Methods Twenty five glioma patients were treated with postoperative concurrent chemoradiotherapy and enrolled in this study.All patients were underwent DCE-MRI using a 1.5T scanner.Fifteen patients were confimmed by secondary pathology or clinical and imaging follow-up of patients with gliomas true progession (TP),10 patients were pseudoprogress (PP).Nonparametric Mann-Whitney test was used to compare perfusion parameters between two groups (TP and PP),were used for receiver operating characteristic (ROC) curve analysis to clear if these parameters can be the indicators to differentiate true progession from pseudoprogression.Results Ktrans (volume transfer constant),Ve (fractional volume of extravascular extracellular) values between TP and PP glioma groups were statistically significant,K and Ve values were significantly higher in the TP group than in the PP group (P < 0.05).The areas under the ROC curve are 0.990 and 0.847,respectively.Kep (efflux rate constant) value,Vp (fractional volume of plasma) value in the identification of glioma TP group and PP group was not statistically significant (P > 0.05).Conclusions DCE-MRI can be used to identify glioma TP and PP,Ktrans value and Ve value have clinical significance.

Objective Expression of recombinant human matrix metalloproteinase 9 (MMP9) protein in E coli,To establish a sandwich ELISA method for detecting MMP9 in sera and analyses of the difference of serum MMP9 in health population and the patient with hepatocellular carcinoma (HCC). Methods A fragment MMP9 sequence from nt 721-1 156 was amplified with PCR from the synthesized cDNAs of human liver tissues and inserted into the prokaryotic-expression plasmid pQE30.HIS-MMP9 fusion protein was expressed and purified from E coli MIS cells.The specific antibodies were elicited in rabbits and guinea pigs by immunization of the purified HIS-MMP9.After purifing antibodies with CNBr-activated Sepharose 4B,a sandwich ELISA technique was established.The serum MMP9 proteins were evaluated in 227 health adults and 193 HCC patients.Results SDS-PAGE displayed that the molecular-weight of the expressed fusion protein was about 17 000.The prepared antisera were able to recognize both recombinant and endogenously expressed MMP9 from human neutrophil and HepG2 cells.It was found that the average level of MMP9 proteins in HCC patients was higher than that in health control,showing significantly statistic difference.Conclusions The established method could reflect the level of serum MMPg.The data in this study supply scientific basis of generating methodology for screening metastasis and reoccurrence of HCC, using serum MMP9 as index.
7.Preparation of Peppermint Oil Moisturizing Microemulsion for Nasal Mucosa and Study on Its Mucosal Adhesion and Cilia Toxicity

OBJECTIVE: To prepare the Peppermint oil moisturizing microemulsion for nasal mucosa and survey its mucosal adhesion and cilia toxicity. METHODS: The polyoxyethylene hydrogenated castor oil was used as emulsifier to prepare the Peppermint oil moisturizing microemulsion for nasal mucosa, and the preparation technology was optimized on the basis of comprehensive score by orthogonal design. The microemulsion was characterized and the menthol content was determined by GC. The mucosal adhesion was evaluated by measuring the transport rate by cilia in vivo, and the cilia toxicity of microemulsion was evaluated by measuring the sustained movement time of cilia in vitro. RESULTS: The optimal preparation technology of self-made microemulsion was to firstly disperse the peppermint oil and the emulsifier, then add anhydrous ethanol, edible glycerin and distilled water, and stir at 1 200 r/min for 2 h. The average contents of menthol in the three batches of the microemulsion were 2.682, 2.507 and 2.496 mg/mL (RSD=2.89%,n=3), respectively. The cilia transport rates in vivo were (0.65±0.01), (0.78±0.03)and (0.92±0.04) cm/min in high-dose, medium-dose, and low-dose groups of self-made microemulsion (2.561, 0.256, 0.128 mg/mL of menthol) respectively, which were significantly lower than normal saline group and compound menthol nasal droups (P<0.05). The cilia movement time in vitro were(206.7±4.9), (226.0±13.5), (269.3±12.9)min, which were significantly longer than sodium deoxycholate group (P<0.05). CONCLUSIONS: The preparation technology of self-made microemulsion is easy-to-handle and controllable in quality. The prepared microemulsion shows good mucosal adhesion without cilia toxicity.
8.Content Determination of Pulegone in Schizonepeta tenuifolia and Its Compound Preparation by Hollow Fiber Liquid-phase Microextraction Coupled with HPLC

OBJECTIVE:To establish t he metho d for the content determination of pulegone in Schizonepetae tenuifolia decoction pieces and its compound preparation. METHODS :Hollow fiber liquid-phase microextraction coupled with HPLC (HF-LPME-HPLC) was adopted. Based on single factor tests ,HF-LPME condition of S. tenuifolia decoction pieces and its compound preparation (taking Compound S. tenuifolia granule as an expample ) was optimized by central composite design-response surface methodology using pulegone enrichment multiple as index ,with the concentration of sample phase solution (NaCl),extraction time and stirring speed as factors. Validation test was conducted. HPLC method was adopted to determine the content of pulegone. The determination was performed on Hypersil C 18 column with mobile phase consisted of methanol- 0.3% phosphoric acid (gradient elution )at the flow rate of 1.0 mL/min. The detection wavelength was set at 252 nm,the column temperature was 25 ℃. The sample size was 20 μL. The feasibility of HF-LPME-HPLC method established in this study was validated by using HPLC method stated in the item of S. tenuifolia decoction pieces in 2020 edition of Chinese Pharmacopoeia (part Ⅰ)as reference. RESULTS :The optimum HF-LPME conditions included n-nonanol as the extraction solvent ,sample phase solution with 11% NaCl and pH value of 7,stirring speed of 800 r/min,extraction time of 36 min. Results of HPLC methodology investigation showed that linear range of pulegone were 0.05-5 μg/mL(r=0.999 0). The limits of detection and quantitation were 0.4 and 1.3 ng/mL,respectively. RSDs of intra-day and inter-day precision were 1.8%-4.0% and 1.5%-4.1%(n=3),respectively. RSDs of reproducibility and stability tests (24 h)were all lower than 8%(n=6). Average recoveries of S. tenuifolia decoction pieces and Compound S. tenuifolia granule were 102.6%-105.1% and 97.2%-102.3%,respectively;RSDs were not higher than 4.1% and 6.2%(n=3). The average contents of pulegone in S. tenuifolia decoction pieces determined by pharmacopoeia method and established method were 0.84 mg/g(RSD=4.3% ,n=3)and 0.87 mg/g(RSD=5.5% ,n=3),respectively,with no significant difference (P>0.05). CONCLUSIONS :The established HF-LPME-HPLC method can enrich and concentrate pulegone , shows strong purification ability and high sensitivity ,and can be used to determine the contents of pulegone in S. tenuifolia decoction pieces and its compound preparation.

Beijing has been one of the epicenters attacked most severely by the SARS-CoV (severe acute respiratory syndrome-associated coronavirus) since the first patient was diagnosed in one of the city's hospitals. We now report complete genome sequences of the BJ Group, including four isolates (Isolates BJ01, BJ02, BJ03, and BJ04) of the SARS-CoV. It is remarkable that all members of the BJ Group share a common haplotype, consisting of seven loci that differentiate the group from other isolates published to date. Among 42 substitutions uniquely identified from the BJ group, 32 are non-synonymous changes at the amino acid level. Rooted phylogenetic trees, proposed on the basis of haplotypes and other sequence variations of SARS-CoV isolates from Canada, USA, Singapore, and China, gave rise to different paradigms but positioned the BJ Group, together with the newly discovered GD01 (GD-Ins29) in the same clade, followed by the H-U Group (from Hong Kong to USA) and the H-T Group (from Hong Kong to Toronto), leaving the SP Group (Singapore) more distant. This result appears to suggest a possible transmission path from Guangdong to Beijing/Hong Kong, then to other countries and regions.

10.Effect of soft tissue thickness of upper lip on lip retraction in orthodontical-treated Class II, Division 1 females.

OBJECTIVE:
To establish the Chinese Han national criteria for the thickness of upper lip in Chongqing and to investigate the effect of lip thickness on lip retraction in orthodontic treatment.
METHODS:
A total of 240 adult patients (male=120, female=120) of Chinese Han nationality aged 18-35 years in Chongqing were treated with cephalograms and measured the thickness of upper lip. We established the Chinese Han national criteria for the thickness of upper lip in Chongqing according to the results. Sixty-eight female patients with Angle II 1 from Department of Orthodontics Stomatological Hospital of Chongqing Medical University were treated with maxillary premolars extraction, and pre- and post-treated with lateral cephalograms, then divided into 3 groups according to our criteria: a thin lips group, a normal lips group and a thick lips group. The ratio of the incisors retraction and the upper lip retraction were measured and calculated. The correlation between incisors retraction and the upper lip retraction was analyzed by the Pearson correlation method.
RESULTS:
There was statistical significance in the thickness of upper lip between males and females (P<0.01). The ratio of maxillary inscior retraction to the upper lip soft tissue retraction was 1.6:1, 2.2:1 and 2.9:1 in the thin lips group, the normal lips group and the thick lips group, respectively.
CONCLUSION
Gender differences exist in the thickness of upper lip. Upper lip retraction with the incisors was negatively correlated with the soft tissue thickness of the upper lip in females with Angle II 1.
