1.Expression of placental glucose transporters in pregnant women with abnormal glycometabolism

Objective To investigate the expression of glucose transporters(GLUTs)in placentas of pregnant women with abnormal glyeometabolism,and to explore its effect on glucose transport between mother and fetus and its relation with the birth weight.Methods Placentas of 41 pregnant women with abnormal glycometabolism(7 cases of DM,10 GDM A1,10 GIGT and 14 GDMA2)and 15 normal pregnant women as control were selected.The expression of GLUT1 and GLUT3was detected by immunohistochemistry.The birth weight was measured at delivery.Results GLUT1 was expressed in the syncytiotrophoblasts and cytotrophoblasts,whereas GLUT3 in some endothelial cells.The expressions of GLUT1 and GLUT3 were significantly different among the five groups(P<0.01).Positive correlation was shown between the GLUT1 expression and the birth weight(rs=0.532,P<0.01),but not in GLUT3 expression.Conclusions The expression of GLUT1 and GLUT3 in placentas of pregnancy with abnormal glycometabolism is enhanced,and GLUT1 may play a predominant role in the fetal glucose uptake.

Epstein-Barr(EB)virus is the first carcinogenic virus discovered in 1964.EBV infection is common in pediatrics and most of the infected children are asymptomatic.When the immune function of children becomes low or deficient, EBV may cause a series of benign or malignant lymphoproliferative diseases, such as infectious mononucleosis, chronic active EBV infection, hemophagocytic lymphohistiocytosis, post-transplant lymphoproliferative disorders, X-linked lymphoproliferative syndrome and so on.Most of these diseases have complex pathogenesis, diverse clinical manifestations, poor prognosis and high mortality.The only effective treatment is allogeneic hematopoietic stem cell transplantation.
4.Clinical Observation on Treating Menopause Syndrome with Acupuncture Therapy

Objective:To study the clinical effect and the endocrine mechanism of acupuncture therapy on menopause syndrome.Method:There were 47 cases with menopause syndrome,which were divided into two groups,32 cases in the acupuncture group,treated by acupuncture with main acupoins of Shenshu (BL 23),Zusanli (ST 36) and Sanyinjiao (SP 6),and 15 cases in the control group,treated by orally taking the Oryzanol.The Kupperman index of the two groups was observed before and after treatment for evaluating the therapeutic effect.The changes of blood-serum F2,T,FSH,LH of the 25 cases in the acupuncture group were tested by radioimmunoassay before and after the treatment.Results:The acupuncture therapy can relieve the clinical symptoms of menopause syndrome,and its therapeutic effect was better than Oryzanol.After being treated by acupuncture therapy,the content of blood-serum E2 obviously increased,while the contents of LH and T declined,and the FSH also had a tendency to lower,but it was not statistically significant.Conclusion:The therapeutic effect of acupuncture therapy for menopause is confirmed.The acupuncture therapy can improve the function of hypophysial-gonadal axis of the patient with menopause syndrome,which may be the main mechanism of the acupuncture therapy for treating menopause syndrome.
5.Cloning,prokaryotic expression,purification and identification of the transpeptidase domain of penicillin binding protein 2a of methicillin-resistant Staphylococcus aureus

Objective To construct the prokaryontic expression vector of the gene fragment which encodes the transpeptidase domain of penicillin binding protein 2a(PBP2a) of methicillin‐resistant Staphylococcus aureus(MRSA) ,and to express ,purify and i‐dentify the objective protein .Methods Strains of MRSA were isolated and identified from clinical samples ,according to the se‐quence of mecA gene recorded in GenBank ,the primers of mecA fragment which encoded the transpeptidase domain of PBP2a was designed .The gene fragment from MRSA was amplified by using polymerase chain reaction(PCR) and cloned into pET28a(+ ) plasmid .After being identified by enzyme digestion and sequencing ,the recombinant plasmid was transformed into the strain of Escherichia coli BL21(DE3)plysS .The expression of transpeptidase domain of PBP2a was induced by 0 .7 mmol/L IPTG ,the ex‐pressed products were purified by using Ni afinity chromatography ,then were analyzed by using Western blot .Results The recom‐binant expression vector was digested by BamHⅠ and EcoRⅠ ,and the products were at the expected size .The result of sequencing showed two bases undergoing mutation ,while there were no frameshift mutations .The expressed protein was identified by using SDS‐PAGE and Western blot ,a new protein band was visible at the relative molecular mass of 38 × 103 .Conclusion The corre‐sponding prokaryotic expression vector is successfully constructed ,and the transpeptidase domain of PBP2a is successfully ex‐pressed and purified .
6.Causes and Prevention of Recurrent Pain after Laparoscopic Cholecystectomy

Objective To investigate the causes and preventive measures of recurrent pain after laparoscopic cholecystectomy (LC). Methods From January 2000 to January 2006,168 patients developed recurrent pain after LC. The pain was caused by non-incisional wound,postoperative complications,relapse of preoperative symptoms,cholecystitis complicated with gallbladder stones,or other diseases. The data of the cases were reviewed in this study. Results Among the cases,24 patients had neck and shoulder pain,and 5 cases complained of puncture pain; 71 cases showed consistent abdominal pain,among which 28 had bile leak; 23 developed secondary common bile duct stones,6 suffered from acute pancreatitis,and 8 had residual cholecystitis or cholecystic stones; inferior hepatic space inflammatory fluid was detected in 5 cases,duodenal bulbar ulcer in 1 case; and 32 cases suffered from paroxysmal pain in the abdominal cavity,9 of them were diagnosed as bowel hyperperistalsis,and 23 had biliary dyskinesia; 31 patients had jaundice complicated with abdominal pain,right focal dilation of the intrahepatic bile duct was detected in 7 of them,residual stones in the extra hepatic bile duct in 11 cases,ampullary carcinoma in 2 cases,carcinoma in the inferior bile duct in 1 case,and ascariasis of the biliary tract in 2 cases; two patients developed abdominal pain because of abdominal distension,and one was induced by severe nausea and vomiting. In this series,the symptoms were relieved in all of the cases,86 of them were cured by a second operation or ERCP plus EST,and the other 82 received conversational therapy. The patient was followed up for 1 to 36 months,during the period,no complications occurred. Conclusions The causes of recurrent pain after LC involve in each component of perioperation procedures.
7.Quality Analysis of Measles Breakout Events and Relative Cases in China in 2006

Objective To analyze the quality of measles breakout events and relative cases in order to offer the integrated,accurate,basic data for infectious disease anticipation and control.Methods Associate the measles breakout events with cases,the cases with organization type,and users'information with breakout events.As a result,the integrated data construct is created in order to analyze the index in detail. Results Measles breakout events substitute ration is 33.93%,measles breakout events without relative cases reporting rate is 12.50%.Relative cases reporting rate in time is 75.51% and substitute ratio is 12.07%.Conclusions There are still many problems in measles breakout events reporting and relative cases reporting.The local CDCs and hospitals should pay much attention to the data quality especially in the infectious disease outbreak and guarantee the data quality through more methods of quality control and evaluation.
8.Diagnostic value of thoracoscopy routine pleural biopsy combined with frozen biopsy for pleural effusion

Objective To investigate the diagnostic value and safety of thoracoscopy routine pleural biopsy combined with frozen biopsy for pleural effusion. Methods A retrospective analysis was made on the pathological diagnosis rate of pleural effusion. Results 120 cases in thoracoscopy, 103 cases were confirmed with routine biopsy specimens (85.8%), 16 cases found in the lesions with conventional clamp not satisfactory tissue specimens, combined with frozen cut obtained satisfactory specimens, the diagnostic accuracy rate of 16 cases of cryobiopsy was 100.0%, and the total diagnostic accuracy rate of medical thoracoscopy combined with pleural biopsy and cryobiopsy was 95.0%. There was significant difference between conventional biopsy and cryobiopsy (P < 0.05). Conclusion Medical thoracoscopy combined with pleural biopsy and cryobiopsy can achieve a higher rate of pathological diagnosis, and the complications are mild, so it is worthy of clinical promoting.
9.Analysis on risk factors for deep vein thrombosis after bone trauma

Objective To investigate the risk factors for deep vein thrombosis (DVT) after bone trauma.Methods The study involved 118 patients with traumatic fractures (traumatic fracture group),21 DVT patients diagnosed by color Doppler (DVT group) and 56 healthy patients (control group).Anti-cardiolipin antibody (ACA) was determined by ELISA method.D-dimer and fibrinogen (Fib) were detected by coagulation analyzer and C-reactive protein (CRP) by rate nephelometry.Results Levels of D-dimer,Fib and CRP in traumatic fracture group were significantly increased,but were lower than those in DVT group.ACA positive rate in DVT group presented significant increase and three patients with positive ACA in traumatic fracture group all suffered from DVT.The positive rates of Fib,D-dimer and CRP in lower limb fracture group,multiple fracture group and pelvic fracture group were higher than those in upper limb fracture group (P < 0.05).Levels of Fib and D-dimer showed gradual rise with growth of age,but their levels in DVT group had different degree of reduction after thrombolytic therapy.Conclusions Positive ACA and enhancement of D-dimer,Fib and CRP are risk factors for DVT after bone trauma.Levels of Fib and D-dimer in patients with bone trauma are related with age and therefore risk of posttraumatic DVT increases with age.
10.Progress in non-invasive ventilation for the treatment of neonatal respiratory disease

Mechanicalventilation in newborns can be divided into invasive mechanical ventilation and non-invasive me-chanical ventilation. In order to reduce invasive ventilation-induced lung injury, more and more non-invasive ventilation patterns have been used clinically. Early application of nasal intermittent positive pressure ventilation (NIPPV) has become the main mode of non-invasive ventilation in many neonatal intensive care units (NICU). At the same time, the combination with selective pulmonary surfactant can reduce the incidence of chronic lung disease. This article reviews the use and prospect of non-invasive respiratory support modes which include continuous positive airway pressure (CPAP) and bi-level CPAP mode (BiPAP) such as SiPAP and NIPPV. Some modes which are in the research stage were also reviewed including synchronized nasal intermittent positive pressure ventilation, neurally adjusted ventilatory assist and non-invasive high-frequency oscillatory ventilation.
