1.Etiological Characteristic and Drug Resistance of Two Strains of Rhodotorula glutinis Found in ICU

OBJECTIVE To discuss the mycology characteristic of two strains of Rhodotorula glutinis causing infection in ICU,and conduct molecular biological identification and antifungal agent sensitivity experiment.METHODS The methods used included conventional mycology identification and sequencing the ITS region of strains,and the in vitro antifungal agent sensitivity experiment were done by glutinis liquid-based dilution method(M27-A).RESULTS The two strains of R.glutinis were confirmed after mycology identification and sequenced the ITS region of strains.They were both sensitive to amphotericin B and 5-flucytosine,and insensitive to fluconazole,itraconazole and voriconazole.CONCLUSIONS The reasons causing Rh.glutinis infection include catheter related infections,(total parenteral nutrition TPN) and immunosuppressives.Using amphotericin B and removing intravenous catheter can be the first choices in the treatment of Rh.glutinis infection.Azole shows insensitive to Rh.glutinis in the in vitro antifungal agent sensitivity experiment,therefore it should be used very carefully.
2.Assay of verbascoside in Shenqiyinao Capsules by HPLC

AIM: To determinate the content of verbascoside in Shenqiyinao Capsules (Fructus Lycii, Herba Cistanches, Rhizoma Polygonati, etc.). METHODS: Waters C 18 column was used and acetonitrile-methyl alcohol-1% acetic acid solution was used as a mobile phase. The detection wavelength was set at 334nm. RESULTS: The linear range was in the range of 0.206~1.85?g, and the average recovery was 96.6% and RSD was 2.5%, respectively. CONCLUSION: The method can prevent from impurities effectively and can be applied to the quality control of Shenqiyinao Capsules.
3.The surface characterization of ultrafine grained titanium after roughness treatment of different types

Objective:To investigate the surface characterization,cell adhension and proliferation of ultrafine grained titanium (UFG Ti) after different sand blasting and acid-etching(SLA).Methods:The billets of UFG Ti and commercially pure titanium Ti(CP Ti) were incised into cylindrical specimens with 7 mm in diameter and 2 mm in height.The specimens were sand blasted at the air pressure of 0.2,0.4,0.6 and 0.8 MPa respectively(n =10) and then acid-etched.The surface morphology,roughness and surface wettability of the specimens were examined.Rat embryo osteoblasts MC3T3-E1 were cultured on the speciments for 1 d,3 d and 5 d respectively,the cell morphology and cell density were observed.Results:The different hierarchical porous topographies were formed on the surface of UFG and CP Ti after modified by SLA.The sizes of blasted holes on the surface and the surface roughness of both materials increased with the increase of blast pressure,but the values of UFG Ti were lower than those of CP Ti correspondingly(P < 0.05).The surface wettability of them was also changed with the blast pressure,but the values of UFG Ti was significantly smaller than those of CP Ti(P < 0.05).When the blast pressure was 0.6 Mpa,the UFG Ti exhibited excellent wettability,the cell density was the highest,the morphology of MC3T3-E1 cells on UFG Ti was superior to that on CP Ti.Conclusion:UFG Ti exhibits proper surface morphology,roughness and excellent wettability,which is more appropriate for adhension and proliferation of MC3T3-E1 cells after modification by SLA at blast pressure of 0.6 Mpa.
4.A case of fructose-1, 6-bisphosphatase deficiency with delayed diagnosis for 7 years and review of literature

To report a case of fructose-1, 6-bisphosphatase deficiency child with delayed diagnosis for seven years, and review the literature. A 13-year-old boy presented with recurrent episodes of hypoglycemia after infection since 6 years old, he also had convulsion and short stature. The laboratory finding revealed ketotic hypoglycemia, lactic acidosis, transient elevated ALT and UA. The genetic analysis showed compound heterozygous mutations of c. 960-961insG and c. 355G> A in FBP1 gene. Among eighteen patients reported in China, 88.9% had convulsion, 16.7% had growth retardation, the average delayed diagnosis spent 2.8 years, which could result in permanent brain damage and high death risk. The most common mutation was c. 960-961insG, followed by c. 355G>A and c. 490G>A, these mutations maybe hot spot sites of FBPl gene in China.
5.Clinical study of Huzhang Polou Decoction combined with acupoint application for the children with mycoplasma pneumonia complicated with atelectasis and phlegm-heat obstruction

Objective:To evaluate the clinical efficacy of Huzhang Polou Decoction combined with acupoint application in the treatment of children with mycoplasma pneumoniae pneumonia (MPP) complicated with atelectasis.Methods:A total of 105 MPP children with atelectasis and phlegm-heat obstructing lung syndrome in our hospital from August 2019 to March 2022, who met the inclusion criteria, were divided into control group (53 cases) and observation group (52 cases), by random number table method. The control group was given azithromycin sequential therapy on the basis of conventional western medicine treatment, and the observation group was given Huzhang Polou Decoction and acupoint application on the basis of the control group. Both groups were treated for 19 days. TCM syndromes were scored before and after treatment. The FEV1 and peak expiratory flow rate (PEF) were measured by pulmonary function measurement instrument. The CRP was measured by immunoturbidimetry and procalcitonin (PCT) was measured by ELISA. The recovery time of fever, disappearance time of cough and sputum, disappearance time of lung rales, lung recruitment time, and adverse reactions during treatment were recorded, and the clinical efficacy was evaluated.Results:The total effective rate was 96.15% (50/52) in the observation group and 84.91% (45/53) in the control group, and the difference between the two groups was statistically significant ( χ2=3.85, P=0.050). After treatment, the TCM syndrome score of the observation group was significantly lower than that of the control group ( t=22.78, P<0.01). FEV1 [(1.87±0.29) L vs. (1.54±0.28) L, t=5.93] and PEF [(79.82±6.29) L/min vs. (74.32±6.30) L/min, t=4.48] were significantly higher than those in the control group ( P<0.01). The levels of serum CRP [(9.18±2.98) mg/L vs. (12.34±3.00) mg/L, t=5.42] and PCT [(0.60±0.15) ng/L vs. (0.96±0.21) ng/L, t=9.93] were significantly lower than those in the control group ( P<0.01). The recovery time of fever, disappearance time of cough and sputum, disappearance time of lung rale and lung recruitment time in the observation group were significantly earlier than those in the control group ( t=7.27, 6.84, 3.76, 5.87, all Ps<0.01). During treatment, the incidence of adverse reactions was 3.77% (2/53) in the control group and 1.92% (1/52) in the observation group, and there was no significant difference between the two groups ( χ2=0.32, P=0.569). Conclusion:The Huzhang Polou Decoction combined with acupoint application can improve the pulmonary function of children with MPP complicated with atelectasis, reduce the level of serum inflammatory cytokines, improve the clinical efficacy safely.
6.Genetic analysis of a juvenile with maturity-onset diabetes of the young type 12.

OBJECTIVE:
To explore the genetic basis for a juvenile with maturity-onset diabetes of the young type 12(MODY12).
METHODS:
High-throughput sequencing was carried out to screen for the variants. Candidate variant was verified by Sanger sequencing. Pathogenity of the variant was predicted by searching the genetic databases and analysis by using bioinformatic software.
RESULTS:
Genetic testing indicated that the patient and his mother have both carried a heterozygous c.3976G>A variant (p.Glu1326Lys) in exon 32 of the ABCC8 gene. Prediction of the protein structure suggested the variant to be deleterious. Based on the guidelines of the American College of Medical Genetics and Genomics, the variant was predicted to be uncertain significance.
CONCLUSION
Whether the c.3976G>A variant of the ABCC8 gene is the cause of the disease in this patient or not depends on the functional studies and more case data. Above finding has enriched the spectrum of ABCC8 gene variants.

7.Mechanism study of impaired homocysteine transsulfuration metabolism in sepsis

Objective:To investigate the clinical significance and possible mechanisms of elevated homocysteine(Hcy) levels in peripheral blood of children with sepsis.Methods:The clinical data of 51 children with sepsis (sepsis group) admitted to PICU at Xuzhou Children′s Hospital from January 2019 to December 2019 were analyzed, and the levels of Hcy in plasma were compared with 50 non-septic children (common infection group) and 50 healthy children (healthy control group) during the same period.The possible mechanism of metabolic disorders about Hcy was analyzed by detecting the levels of the key rate-limiting enzymes cystathionine-β-synthase(CBS) and cystathionine-γ-lyase(CSE), which were in the downstream of metabolism in septic mouse model induced by lipopolysaccharide.Results:The level of Hcy in plasma was (12.62±5.46)μmol/L in sepsis group, which was significantly higher than those in common infection group[(9.42±2.28) μmol/L] and healthy control group[(8.14±1.60) μmol/L]( P<0.05). The level of Hcy in plasma of 12 children with acute kidney injury in sepsis group was significantly higher than that of 39 children without acute kidney injury in sepsis group[(16.48±5.87)μmol/L vs.(11.62±4.74) μmol/L, P<0.05]. The level of Hcy in plasma of six children with acute liver failure in sepsis group was significant higher than that of 45 children without acute liver failure in sepsis group[(18.35±7.10) μmol/L vs.(11.84±4.78) μmol/L, P<0.05]. The level of Hcy in serum significantly increased in septic mouse models ( P<0.01). The transcription and protein expression levels of key rate-limiting Hcy transcription enzymes CBS and CSE in liver and kidney tissues of septic mouse were significantly down-regulated ( P<0.05). Conclusion:The level of Hcy in peripheral blood of children with sepsis increases, which is more obviously in children with acute kidney injury or acute liver injury.When patients developed sepsis, the expression of CBS and CSE will be restrained, leading to disorders related to transsulfuration metabolism and elevated level of Hcy in peripheral blood.
