1.Effect of extended nursing on puerperal melancholia

Objective To study the effect of extended nursing on puerperal melancholia incidence.Methods Two hundred perpetrates received interventions of extended nursing after delivery,including education about puerperal state,turning on hot line and creation of QQ group,follow-ups and supporting from family members.Result The incidence rate of puerperal melancholia since performance of extended nursing was significantly lower than what is after its performance(P<0.01).Conclusions The extended nursing can help perpetrates to receive nursing knowledge,alleviate their anxiety,strengthen the confidence to take care of baby.As the result,it can reduce the incidence of puerperal melancholia.
2.Detection of recombination-activating gene expression and T cell receptor gene recombination in human T cell leukemia cell lines

BACKGROUND: The lymphocyte-specific recombination-activating gene (RAG) 1 and 2 are essential for initiating V (D)J recombination events in both T and B cells. In addition to initiating V(D)J recombination, RAG-mediated transposition has also been proposed to contribute to chromosomal translocations and lymphoid malignancy, but the mechanism underlying this activity remains poorly understood.OBJECTIVE: To investigate RAG gene,DNA repair factors Ku70/Ku80 and terminal deoxynucleotidyl transferase(TdT)mRNA expression and T cell receptor(TCR) gene recombination in human leukemia and lymphoma cell lines.DESIGN: Repeated-determined experiment.SETTING: Institute of Molecular Immunology, College of Biotechnology,Southern Medical University.MATERIALS: Four human T leukemia and lymphoma cell lines (Jurkat,Molt-4, HuT-102, 6T-CEM) ,two Burkitt's lymphoma(Raji and Daudi) and two myelogenous leukemia cell lines (HL-60 , K-562) were cultured in complete growth medium (Hyclone,USA) , penicillin/streptomycin(50 U/mLand 50 mg/L) at 37 ℃ in a humidified atmosphere in 0.05 volume fraction of CO2. Jurkat and 6T-CEM were bought from SIBS,CAS(Institute of Biochemistry and Cell Biology,Shanghai Institutes for Biological Sciences, Chinese Academy of Sciences). The other cell lines were conserved by Institute of Molecular Immunology, College of Biotechnology, Southern Medical University.METHODS: The experiment was carried out in Institute of Molecular Immunology, College of Biotechnology, Southern Medical University from October 2005 to January 2006.Reverse transcription-polymerase chain reaction (RT-PCR)was performed to examine the expression of RAG1,RAG2,terminal deoxynucleotidyl transferase(TdT), Ku70 and Ku80 genes,reparative factors,in pathway of non-homologous end joining (NHEJ);Nested and semi-nested PCR reactions were performed with locus-specific primer sets to determine TCR Dβ-Jβ signal joint T-cell receptor excision DNA circles (sjTRECs); Ligation-mediated PCR(LM-PCR) assay was used to detect the recombinational intermediates ds RSS breaks of TCRβ locus with primers specific for the flanking sequences of both Dβ1 and Dβ2.Thus,gene expression during initiating V (D)J recombination and generation of TCR gene recombination intermedium could be known.MAIN OUTCOME MEASURES: RAG gene,DNA repair factors Ku70/Ku80 and TdT mRNA expression and TCR gene recombination in human leukemia and lymphoma cell lines.RESULTS: Determination results by RT-PCR showed that RAG1 was expressed in four T cell lines and was not expressed in two Burkitt's lymphoma and two myelogenous leukemia cell lines. RAG2 and TdT were only expressed in Jurkat,Molt-4 and 6T-CEM, but TdT expression in 6T-CEM was very low. Except that Ku80 expression was not detectable in HL-60,Ku70 and Ku80 were expressed in all the cell lines. However, Determination results of TCR gene in T cell lines recombination intermedium showed that the TCR gene recombination process was not occuring in all the RAG-expression T cell lines. Ongoing TCR gene recombination was only found in Jurkat, which represent a mature stage of T cell development. In addition,characteristic junctional diversity of signal joints was observed in DNA isoated from Jurkat.CONCLUSION: T leukemia and lymphoma may be more likely to have a relation with RAG.The results give a clue that Jurkat can probably provide an ideal cell line modle for studying RAG and T cell lymphomagenesis.
3.Investigation on the allocation status of neonatology nursing human resources in China's economically developed areas

Objective To study the status quo of nursing human resource configuration and structure of neonatology department in China's economically developed areas.Methods Using judgment sampling and multi-stage stratified sampling,investigations were conducted by the self-designed questionnaire in letter in 2012.Results In 2012,both the bed nurse ratio(1:0.79) and doctor nurse ratio (1: 1.22) met the Health Ministry's standards (1:0.58-0.61 and 1: 2) respectively.Age distribution of neonatology nursing staff was from 25 to 34 years old (55%).The distribution of wordng experience was average.Nurses with junior college degree as the main body took up 48%,with primary title as the main body 66%,and the appointment system accounted for 42%.The nursing personnel with young ages,senior skilled,higher education degree,and advanced titles were concentrated in Grade Ⅲ-A hospitals and public hospitals of women and children.In terms of distribution equity,two Gini coefficients of geographical and population distribution were both less than 0.35 and near zero,which indicated relatively perfect equity.Conclusions The stock and per capita of neonatology nursing human resources in China's economically developed areas were insufficient in the long run and showed imbalanced allocation among hospitals with different levels.The allocation structure failed to meet the needs of nursing disciplinary development.
4.Impact of transvaginal natural orifice transluminal endoscopic surgery-assisted laparoscopic nephrec-tomy on female sexual function and quality of life

Objective To investigate the impact of transvaginal natural orifice transluminal endo-scopic surgery(NOTES)-assisted laparoscopic nephrectomy on female sexual function and quality of life . Methods This was a prospective study on the change of female sexual function and quality of life of female patients who underwent transvaginal NOTES-assisted laparoscopic nephrectomy from May .2011 to Nov. 2012.A total of 42 cases were included in this study (28 of them with severe hydronephrosis , non-functio-ning kidney , 11 with pyelonephrosis , 1 with renal tuberculosis , 1 with duplex kidney complicated with hy-dronephrosis, and 1 with renal angiomyolipoma ).The mean age was 36.9±5.3 (26-45) years, and the mean body mass index was 21.7±2.6 (14.7 to 27.1) kg/m2.Twenty-four cases were operated on the left side, 18 cases on the right .The female sexual function and quality of life were assessed before and 4 months, 7 months and 1 year after surgery using the Female Sexual Function Index (FSFI) questionnaire and the MOS 36-item Short-Form Health Survey (SF-36), respectively. Results The mean FSFI of 42 cases preoperatively and 4 months, 7 months and 1 yr postoperatively were 27.74 ±4.34, 27.19 ±4.49, 28.54±4.23, and 28.68 ±4.19, respectively.There was no statistically significant difference among them (F=1.111, P=0.346).Compared with that of preoperation , the physical function, vitality, metal health, body pain, and general health of the patients were improved , but the role-physical, role-emotion and social function were not improved at postoperative month 4 and month 7 (P<0.05).Each item of SF-36 was im-proved after postoperative 1 year ( P<0.05) . Conclusions Transvaginal NOTES-assisted laparoscopic ne-phrectomy does not cause negative effect on the female sexual function .The quality of life can be improved after operation .The physical function is improved at early stage , and the psychological function as well .
5.Transvaginal NOTES-assisted laparoscopic nephrectomy

Objective To describe the initial clinical experience of transvaginal NOTES-assisted laparoscopic nephrectomy. Methods From May to June 2010, 4 female patients with non-functioning kidney and 1 female patient with renal tuberculosis underwent transvaginal NOTES-assisted laparoscopic nephrectomy. The median age was 41 years (range 36 to 63). Three lesions were on the right side, and 2 on the left. After induction of general anesthesia, the patients were positioned in lithotomy with ipsilateral lumbar at 60° angle to the operating table. A 5 mm Trocar and a 10 mm Trocar were placed through the umbilicus, and a 10 mm Trocar for 30° laparoscope was placed through the posterior vaginal fornix under direct vision. The kidneys were put into a home-made bag and removed via the incision of posterior vaginal fornix after there were completely resected. Results The 5 procedures were successfully finished. The median operative time was 190 (range 150 to 260)min. The median estimated blood loss was 185 (range 150 to 210) ml. There were no intra-operative or postoperative complications. The patients resumed ambulation 1 day after surgery, and resumed nutrition 2 days after surgery. The drainages were removed on postoperactive day 3. The patients were discharged on postoperative day 7. The incision in vagina healed well. Conclusions Transvaginal NOTES-assisted laparoscopic nephrectomy is feasible and reproducible. This novel technique may provide additional benefits related to postoperative pain, recovery, and cosmetic results even when compared to today's minimally invasive procedures.
6.Investigation of the cognitive status of cardiopulmonary resuscitation in a health checkup population

Objective To explore the knowledge and training needs of cardiopulmonary resuscitation (CPR) among a health checkup population, and to provide evidence for promoting cardiopulmonary resuscitation knowledge and skills at physical examination centers. Methods Using physical examination numbers, the random number table method was used to randomly select 1 000 people from a health checkup population, information collected including their basic information, cognitive status of cardiopulmonary resuscitation, and attitudes and behaviors toward cardiopulmonary resuscitation. The statistical analysis was performed using χ2test, a one-way ANOVA, and multivariate logistic regression analysis. Results Questionnaires was acquired from 997 consenting adult subjects. From which, 26% had received CPR training, and they demonstrated a low ratio of cognitive knowledge, scoring only 9.94 ± 6.36 (from a possible score of 30). Additionally, 92.1% had a willingness to learn CPR skills, and the CPR performance score was 23.12±3.27 (the highest score was 28). Unmarried people younger than 25 years old, professional and technical personnel, and military men had higher CPR scores than other groups. The CPR learning efficiency of military men, undergraduates, and people younger than 25 years old was higher than other groups. Older people had a lower willingness to learn cardiopulmonary knowledge and the better the feel-good status, the lower the willingness to learn CPR skills, of which OR values were 0.787 and 0.657, respectively (P<0.05). The rate of people learning CPR skills for relatives was higher than for strangers (P<0.05), and 83.8% were willing to receive medical training. Conclusion The health checkup population had a low cognitive level of CPR knowledge, and the willingness to learn CPR was high. To address this, free CPR training should be conducted at medical examination institutions.
7.Transvaginal natural orifice transluminal endoscopic surgery in female urologic diseases

Objective To describe our experience with the transvaginal natural orifice transluminal endoscopic surgery (NOTES) in female patients,and to evaluate its clinical value.Methods Between May 2010 and April 2017,a total of 289 female patients underwent transvaginal NOTES in our center,including 31cases of adrenal tumors,46 cases of renal tumors,188 cases of non-functional kidneys,5 cases of renal cysts,4 cases of duplex kidney,7 cases of renal tuberculosis and 8 cases of renal pelvis tumors.Among them,146 cases were on the left side,142 cases on the right side,and 1 case on both sides (renal cysts).The median age was 39.4 (range 23 to 76) years,and the median body mass index was 21.8(range 15.4 to 32.6) kg/m2.After general anesthesia,the patients were positioned in lithotomy with ipsilateral lumbar at 30°-60° angle to the floor.Hybrid and pure transvaginal NOTES were performed.The specimen was removed through an extended incision at the posterior vaginal fornix.Results Transvaginal NOTES was successfully completed in 281 patients.Two patients required conversion to suprapubic-assisted laparoendoscopic single-site surgery because of the rectal injury in pure transvaginal NOTES nephrectomy.Six patients underwent open conversion.The various transvaginal hybrid NOTES procedures included adrenalectomy (31 cases),nephrectomy (206 cases;simple 174 cases,radical 32 cases),partial nephrectomy (13 cases),heminephroureterctomy for duplex kidney (4 cases) and hybrid endoscopy for nephroureterectomy (7 cases).The mean operative time was 105 min (45-310 min),the mean estimated blood loss was 87 ml (20-800 ml),the mean visual analogue score (VAS) of 48 hours after operation was 2.5 (1-4) points and the mean hospitalization was 7.3 d (4-13 d) for transvaginal hybrid NOTES.The various pure transvaginal NOTES procedures included nephrectomy (22 cases;simple 21,radical 1),renal cyst excision (5 cases).The mean operative time was 170 min (60-320 min),the mean estimated blood loss was 140 ml (20-500 ml),the mean VAS of 48 hours after operation was 1.7 (1-3) points and the mean hospitalization was 5.5 d (3-10 d) for transvaginal pure NOTES.Complications occurred in 56 cases (19.38%),including 20 cases (6.92%) of severe complications,and no death occurred.After a mean follow-up of 51 (range 3 to 86) months,umbilicus scar was hidden.The incision in the vagina healed well.No infection in the abdominal or pelvic cavity or celiocele occurred.The umbilicus scar is not obvious after surgery,and 3 months after surgery.The median Patient Scar Assessment Questionnaire (PSAQ) score was 38 (31-58) and the median Female Sexual Function Index (FSFI) score was 28.2 (22.5-32.2),and the quality of life index was significantly higher than that of preoperation.Condusions Transvaginal NOTES could treat variousprimary diseases,and improve the postoperative quality of life.It does not increase the incidence of related complications,or cause negative effect on the female sexual function,pregnancy or fertility.It can lead to good cosmetic outcome and less pain,which is worth applying in clinical practice.
8.Characteristics of gut microbiota in hyperuricemia population based on full-length 16S rRNA sequencing

Objective:To analyze the differences in gut microbiota between patients with hyperuricemia (HUA) and healthy population for better understanding the correlation between gut microflora and high uric acid.Methods:This study recruited 63 adult volunteers, including 25 HUA patients (HUA group) and 38 healthy people (control group), who underwent physical examination in the PLA Strategic Support Force Characteristic Medical Center in 2021. Their fecal samples were collected and analyzed by 16S rRNA high-throughput full-length gene sequencing to analyze the composition of gut microbiota.Results:The overall composition of gut microbiota was different between HUA group and control group. The α diversity index decreased significantly in HUA group and β diversity analysis showed that there were significant differences between the two groups. HUA group showed increased Bacteroidetes and decreased Firmicutes. LEfSe analysis indicated a unique microbiota structure in HUA group, characterized by significantly decreased short-chain fatty acid (SCFA)-producing bacteria represented by Faecalibacterium prausnitzii and significantly increased Streptococcus salivarius, Bacteroides fragilis, Fusobacterium hwasookii, Flavonifractor plautii, Mycobacterium mucogenicum B and Blautia sp003287895. In addition, functional prediction through PICRUSt2 showed that the metabolism related to gut microbiota SCFA pathway in HUA group was decreased, which was consistent to the unique microbiota structure. Conclusions:Compared with healthy population, patients with hyperuricemia had different composition of gut microbiota and metabolic feature.
9.Characterization of gut microbiota based on full-length 16S rRNA gene sequencing in high diamine oxidase population

Objective:To investigate the correlation between gut microbiota and serum diamine oxidase (DAO) level and to analyze the differences in gut microbiota between high DAO (DAO-H) and normal DAO populations.Methods:This study recruited 62 adult volunteers (31 in DAO-H group and 31 in normal control group) taking health examination in the Strategic Support Forces Special Medical Center in 2021. Their stool samples were collected to analyze the composition of gut microbiota in the two populations by full-length 16S rRNA gene sequencing.Results:No significant difference in the alpha diversity of gut microbiota was found between the DAO-H group and the normal control group, but the structure and function of gut microbiota varied. In the DAO-H group, commensal bacteria decreased, such as Phocaeicola and Bacteroidetes, while potential pathogenic bacteria increased, such as Klebsiella pneumoniae. There were changes in the metabolism of gut microbiota in the DAO-H group, including inhibited sphingolipid metabolism and enhanced biosynthesis of vancomycin group antibiotics, one carbon pool by folate pathway, terpenoid backbone biosynthesis, cell cycle-Caulobacter, protein export, base excision repair and nitrogen metabolism.Conclusions:Compared with the people with normal DAO, the population with high DAO had unique characteristics in gut microbiota composition and metabolism.
