1.Pathogen distribution and resistance in childhood urinary tract infection

To explore the new trend of pathogens and antimicrobial resistance in children with urinary tract infection (UTI).Distribution of pathogens and their antimicrobial resistance in 85 culture-positive hospitalized children with UTI from 2008 to 2012 were analyzed.Urine culture was deemed positive with a pure growth 8 10S/ml (single organism).Among 85 strains of bacterium,gram-negative bacilli accounted for 92.9% (with 64.7% of E coli.),gram-positive cocci 2.4% and fungi 4.7%.The resistance rates of Gram-negative bacilli to ampicillin,trimethoprim-sulfamethoxazole and ampicillin/sulbactam were 82.3%,63.3% and 63.3% respectively.The resistance rates of E coli.to 3rd and 4th generation cephalosporin were both over 60%.And 0% and 5.5% of E coli.UTIs were resistant to imipenem and amikacin respectively.It indicated that E coli.remained a dominant pathogen in childhood UTI.The resistance patterns were grave.Every hospital should monitor the resistance patterns of urinary pathogens on a regular basis and use antibiotics with a low resistance.
2.Treatment of cervical disc herniation using coblation technology

[Objective]To retrospectively analyze the results of plasma-mediated coblation in treating cervical disc herniation.[Method]From January 2003 to April 2006,127 symptomatic cases(197 disc levels)with cervical contained disc herniation were treated with plasma-mediated coblation technology,including 81 males and 46 females,their average age was 49 years,the treated disc levels were 17 in C3、4,61 in C4、5,90 in C5、6 and 29 in C6、7 ,respectively.All patients were reported to have persistent cervical or unilateral arm pain for a minimum of three months and failed previous conservative treatment.MRI showed herniated cervical disc causing focal compression of the nerve root.An average percentage of symptom relief(VAS score)and subjective satisfaction were administered to measure symptoms at all visits.[Result]In the immediate post-operative period,124 patients reported significant amelioration of symptoms,3 patients complained of arm numbness did not change clinical status,one week after operation,percentage of symptom relief was 64%,perfect rate of subjective satisfaction was 91%.Patients were followed up for average 18 months(range 6~31 months)postoperatively.Percentage of symptom relief was 47%,perfect rate of subjective satisfaction was 62%.In the last follow-up,7 patients were observed a stabilization of clinical status over 2 years and in 15 patients the status was similar to preoperation.The other patients referred some residual symptoms and were still under follow-up.There were no complications observed such as hemorrhages,infections and nerve root injurys due to the procedure.[Conclusion]Our data indicate coblation procedure appearing to be less injury,safety,is a promising treatment option for patients with contained cervical disc herniation who have failed conservative therapies and are not considered candidates for open surgery,also supplementary therapy perioperatively in patients treated with cervical open surgery.
3.A study on the coordinated development strategy of medicine and health, medical security and social economy in China

The paper describes the short and medium term (within 2 to 5 years) priorities that any region in China right now faces in working out and implementing its regional health planning, viz. the coordinated development strategy of medicine and health, medical security and social economy. The authors hold that to achieve the coordinated growth of the health cause and society, it is imperative to overcome in the short run logically related obstacles in four aspects: ineffective solution to the apparent problems followed with interest by both the social and health sectors and lack of effective operating conditions accompanying medical insurance reform, both resulting in the lack of a driving force in the reform within hospitals; difficulty in achieving breakthroughts in the development of the health cause; and the probability of a regional health planning becoming a mere formality because of the above factors. In addition, the fact that reform of medical and health institutions in their setup and ownership of property rights lags behind macroscopic social economic reform obscures the explanation of and solution to the above problems. Based on the research results, the paper sets forth the priorities in short and medium term planning.
4.125I seed-loading stent vs. conventional stent for the treatment of middle-late stage esophageal cancer:a clinical control study

Objective To assess the clinical value of the implantation of 125I seed-loading stent in treating middle-late stage esophageal cancer.Methods A total of 64 patients with middle-late stage esophageal cancer,who were treated with esophageal stent implantation during the period from July 2013 to December 2015,were included in this study.According to patient's own will,the patients were divided into group A (n=28,using conventional stent) and group B (n=36,using 125I seed stent).Based on the treatment planning system (TPS) and tumor morphology,conformal comprehensive isodose distribution of 125I seeds was formulated.The success rate of stent implantation,the complication rate,the improvement rate of dysphagia,the stent patency rate,the average hospitalization days,the hospitalization expenses and the survival time were compared between the two groups.Results In both groups,the success rate of stent implantation and the improvement rate of dysphagia were all 100%.The 12-month stent patency rate of group B was evidently higher than that of group A,and the difference was statistically significant (P<0.05).No statistically significant difference in the average hospitalization days existed between the two groups (P>O.05).The mean hospitalization expenses between the two groups was statistically significant (P<0.05),with the mean medical expense in group B being 13,769.57 RMB more than that in group A.Both the mean survival time and the median survival time of group B were longer than those of group A (P<0.05).Conclusion It is safe and effective to use 125I seed stent to treat middle-late stage esophageal cancer.This technique can evidently prolong the survival time of patients,although its medical cost is higher than that of the ordinary stent.
5.Clinical analysis of 108 cases with chronic kidney disease at stage 2 to 5 in children

Objective To analyze the pathogenesis,initially diagnosed symptoms and clinical manifestations of children with chronic kidney disease (CKD) at stage 2 to 5.Methods The data of 108 children who were hospitalized in Children's Hospital Affiliated to Capital Institute of Pediatrics from September 2007 to April 2016 with CKD stage 2 to 5 were retrospectively analyzed.The etiologies,clinical manifestations and examinations were summarized,and the clinical manifestations were compared between the congenital hereditary urinary diseases group and the acquired urinary diseases group.Results (1) In the 108 cases collected,66 cases were male,42 cases were female,aged from 3 months to 15 years and 1 month old.Twenty-four cases were diagnosed at stage 2,26 cases at stage 3,35 cases at stage 4,and 23 cases at stage 5.(2) Twenty-eight kinds of illness were involved in the cause of CKD.Among them,57 cases (52.8%) had congenital anomalies of the kidney and urinary tract,5 cases(4.6%) had hereditary kidney diseases,41 cases (38.0%) had other primary or secondary kidney diseases,and in 5 cases (4.6%) the causes of disease were unknown.(3) For the initially diagnosed symptoms,29 cases(26.9%) were due to complaints associated with kidney disease,36 cases (33.3%) were of other outside kidney symptoms,and 43 cases (39.8 %) were of negative symptoms.The results of urinary ultrasound were abnormal in 79 cases(73.1%) and 87 cases(80.6%) showed abnormality in urinary analysis.There were 105 cases (97.2%) with abnormal manifestations either in urinary tract ultrasound or in urinary analysis.(4)The ages on diagnosis as CKD in children with congenital hereditary urinary diseases(5.89 years old) were younger than that of children with acquired urinary diseases (9.20 years old),and the difference was significant(Z =-3.434,P =0.001).The frequency of cases with short stature or lower-weight in group of congenital hereditary urinary diseases[66.1% (41/622 cases),64.5% (40/62 cases)] were significantly higher than those of the acquired urinary diseases group[43.9% (18/41cases),43.9% (18/41 cases)],and the differences were statistically significant(x2 =4.983,4.263,P =0.026,0.039).Conclusions The causes of CKD are complicated,and the congenital anomalies of kidney and urinary tract are the major causes of CKD at stage 2 to 5 in the cases.The initially diagnosed symptoms of CKD are insidious and atypical.The children with congenital hereditary urinary diseases tend to have more serious growth retardation.Urinary analysis and ultrasound may have an important significance for early diagnosis of CKD in children.
6.Ancient literature on the heat control of umbilicus fumigation method and the modern clinical research.

In order to improve the clinical efficacy of the umbilicus fumigation method, the ancient literature with the heat control of umbilicus fumigation method involved is collected extensively and analyzed systematically, and the heat control, precautions and contraindications of this method are discussed. In association with the cases and the present clinical experience, the main factors to the heat control are introduced, such as preparation of doughnuts, filling quantity, size of moxa cone and numbers of moxa cones so that the clinical application of the umbilicus fumigation method can be promoted and enhanced.

7.The relationship between cerebral artery stenosis and carotid artery elasticity in the ischemia stroke patients by echo-tracking technique

Objective To study the relationship between the cerebral artery stenosis and the carotid artery elasticity in the ischemia stroke patients by echo-tracking technique.Methods Ninety-two ischemia stroke patients were divided into both the mild cerebral arteries stenosis and moderate-severe cerebral arteries stenosis groups.CT angiography was performed to evaluate the stenosis of cerebral artery.The changes of the carotid diameter during cardiac cycles were obtained by echo-tracking technique and the quantitative parameters of pressure-strain elastic modulus (Ep),stiffness (β) and arterial compliance (AC) were calculated automatically. Results①There were significant differences (P<0.05) between the ischemia stroke patient group and the normal adults group in β,Ep and AC.② There were significant differences (P<0.05) between the mild cerebral arteries stenosis groups and the moderate-severe cerebral arteries stenosis groups in β and Ep.AC was lower in patients with the mild cerebral arteries stenosis groups than that in the moderate-severe cerebral arteries stenosis groups.Conclusions The carotid artery elasticity in ischemia stroke patients were significantly different from normal adults.In the ischemia stroke patients,the stiffer the carotid artery was,the higher the likelihood of moderate-severe cerebral stenosis was.
8.Establishment and functional analysis of a cell model expressing the C-type lectin receptor Langerin

Objective To establish a cell model expressing the Langerhans cell-specific C type lectin receptor Langerin in vitro.Methods The cDNA of Langerin was obtained by PCR and cloned into a eukauotic green fluorescent protein (EGFP) expression vector pEGFP-C 1 with EGFP located in the N terminal region of the Langerin gene.Then,the recombinant plasmid was transfected into a human embryonic kidney carcinoma cell line HEK293T.Subsequently,laser confocal microscopy was performed to observe the expression of EGFP-Langerin fusion protein,and flow cytometry to measure the expression of Langerin.Laser confocal microscopy was also conducted to visualize the recognition and endocytosis of dust mite antigen (nDer p 2) by Langerin.Results PCR and Western blot confirmed the successful transfection of HEK293T cells with the recombinant plasmid as well as the expression of Langerin in the transfected cells.As flow cytometry revealed,the expression level of Langerin in transfected HEK293T cells was increased by 43% compared with untransfected cells.Laser confocal microscopy showed that green fluorescein-labeled Langerin was successfully expressed,and could bind to and endocytose the red fluorescein-labeled antigen nDer p 2.Conclusions The fusion protein EGFP-Langerin expressed in HEK293T cells showed the distribution characteristic of cell surface receptors,and could bind to and endocytose allergens.

Aim To study the content and clinical significance of serum endothelin-1(ET-1)in patient with malignant tumors. Methods The contents of serum ET-1 in 85 patients with malignant tumors,30 patients with benign diseases and 30 normal indivaduals were detected by radioimmunoassay. Results (1)The mean concentrations of serum ET-1 in normal individuals and in non-tumor patients were (46.9± 23.1)μ g/L and (51.1± 30.9)μ g/L,respectively, while in patients with malignant tumors it was (190.1± 135.2~ 382.4± 190.1)μ g/L. (2)There was no significant difference between the mean serum contents of ET-1 in normal individuals and non-tumors pstients(P∧ 0.05),but the level of serum ET-1 in patients with malignant tumors were significantly higher than that in above two groups(P∨ 0.001). (3)When the mean content plus twice standard deviation of serum ET-1 in normal individuals was as a threshold value,that is ,the value was defined as the positive clinical value to diagnose cancer, the positive rates of patients with different malignant tumours were 72% ~ 100% . Conclusion Serum ET-1 contents could be used as a better tumor index for maligant tumor screening and accessory diagnosis.
