1.Comparison of the Content of General Flavone and Elements in the Different Parts of Stenoloma Chusanum(L.)Ching

[Objective]Comparison of the content of general flavone and elements in the different parts of Stenoloma chusanum(L.)Ching.[Methods]Total flavone was determined by vis spectrophotometer.Elements were determined by inductively coupled plasma-atomic emission spectrometry(ICP-AES).[Results]The contents of general flavone in the different parts of Stenoloma chusanum(L.)Ching are respectively 11.89%,4.47%,3.46% and 11.75%.And there are some differences in the contents of elements between the different parts.[Conclusion]There are abundant elements and general flavone in the different parts of Stenoloma chusanum(L.)Ching.As the result reveals that Stenoloma chusanum(L.)Ching is very valuable.
2.Pectoralis major myocutaneous flap in the application of the head and neck surgery

Objective To summarize experience in pectoralis major myocutaneous (PMMC) flap reconstruction for patients undergoing head and neck surgeries. Methods We conducted a retrospective study to analyse the clinical data of 20 patients who had PMMC flap reconstruction in a head and neck surgery from March 2011 to June 2016. All patients were male,with the average age of 54 years(ranging from 27 to 71 years). The PMMC flaps of all patients survived after the surgery. Only one patient developed pharyngocutaneous fistula ,and was recovered after treatment with incision dressing. Among follow?ups for 2 to 24 months,all patients got well recovered,the swallowing and communication functions normal communication and the appearance a satisfactory. good looking. Conclusion PMMC renders effective flathe flap reconstruction with broad clinical application value is an optimal choice for patients underwent head and neck surgery with muscle or skin defect,especially for those who had postoperative radiotherapy because of its rich blood supply and it has a lot of affluent tissues,fmore flexibility inle application and higher survival rate. So it is highly applicable,especially for grass?roots hospitals.
3.Diagnosis and treatment of extra-adrenal pheochromocytoma

Objective To highlight the diagnostic and t herapeutic procedures for extra-adrenal pheochromocytoma. Methods Thirty-eight cases of extra-adrenal pheochromocytoma proven patholo gically from 1998 to 2002 were reviewed and studied.Hypertension was observed in 34 cases and intermittent macrohematuria in 4 patients.Another 2 cases were adm itted to hospital with abdomen masses through routinely physical examination.Uri nary VMA,serum and urinary catecholamine were measured in 33 cases.38 cases had B-model ultrasound examination and 34 cases took CT scan.The level of urine VMA elevated in 30 cases(91%) and serum or urinary catecholamine were elevated in 2 8 (85%).The positive rates of localization of B-ultrasound and CT were 92% and 100%,respectively.29 cases took ?-receptor blocker for at least 2 weeks preope ratively.There were 6 cases operated under epidural anesthesia and 32 general an esthesia.Six cases were operated transvesical.10 cases with lumber incision and 19 with abdominal incision.The other 3 cases operated laparoscopically. Results Tumor location,6 cases in bladder wall,7 in renal hilum, 9 in upper pole of kidney,4 in low pole,10 in anterio-lateral to the adrenal gl and,and 2 cases of multifoci.The specimens were from 2 to 9 cm in size.Among 34 cases with hypertension,the blood pressure of 26 patients return to normal and 2 cases were still hypertensive.10 cases were assayed as malignant pheochromocyto ma.There were 9 cases of recurrence or metastases and 5 patients died during fol lowed-up from 13 months to 10 years. Conclusions Urine VMA level is a useful marker to detect extra-adrenal pheochromocytoma and CT sc an is the most reliable in localizing the lesions. 131I-MIBG scintigra phy is of great value for the localization and quantitative for extra-renal phe ochromocytoma with high sensitivity and accuracy.It may be also used as a therap eutic method.Surgical manipulation of the tumor is the best management and metic ulous perioperative management is extremely important.Measuring the level of uri nary catecholamine can monitor the recurrence or metastasis of the tumor.
4.Treatment of cervical disc herniation using coblation technology

[Objective]To retrospectively analyze the results of plasma-mediated coblation in treating cervical disc herniation.[Method]From January 2003 to April 2006,127 symptomatic cases(197 disc levels)with cervical contained disc herniation were treated with plasma-mediated coblation technology,including 81 males and 46 females,their average age was 49 years,the treated disc levels were 17 in C3、4,61 in C4、5,90 in C5、6 and 29 in C6、7 ,respectively.All patients were reported to have persistent cervical or unilateral arm pain for a minimum of three months and failed previous conservative treatment.MRI showed herniated cervical disc causing focal compression of the nerve root.An average percentage of symptom relief(VAS score)and subjective satisfaction were administered to measure symptoms at all visits.[Result]In the immediate post-operative period,124 patients reported significant amelioration of symptoms,3 patients complained of arm numbness did not change clinical status,one week after operation,percentage of symptom relief was 64%,perfect rate of subjective satisfaction was 91%.Patients were followed up for average 18 months(range 6~31 months)postoperatively.Percentage of symptom relief was 47%,perfect rate of subjective satisfaction was 62%.In the last follow-up,7 patients were observed a stabilization of clinical status over 2 years and in 15 patients the status was similar to preoperation.The other patients referred some residual symptoms and were still under follow-up.There were no complications observed such as hemorrhages,infections and nerve root injurys due to the procedure.[Conclusion]Our data indicate coblation procedure appearing to be less injury,safety,is a promising treatment option for patients with contained cervical disc herniation who have failed conservative therapies and are not considered candidates for open surgery,also supplementary therapy perioperatively in patients treated with cervical open surgery.
5.How to extend the thrombolytic time window for acute ischemic stroke?

Currently,thrombolytic therapy is the most effective treatment for acute ischemic stroke.Intravenous recombinant tissue plasminogen activator administered within 3 hours of symptom onset is the only medication approved by U.S.Food and Drug Administration for the treatment of acute ischemic stroke.However,because the time window for intravenous thrombolytic therapy is very short,only a very few patients can reach the hospital for intravenous thrombolytic therapy within 3 hours of onset.Therefore,how to extend the time window of thrombolytic therapy for more patients to have accessed to it and benefit from it have been the concern of researchers.This article reviews about the recent advances in research on extending the time window of thrombolytic therapy.
6.Effect of telmisartan on the expression of urate transporter protein in the renal tubule epithelial cells

Objective To explore the protective effect and underlying mechanism of telmisartan on hyperuricemic nephropathy.Methods (1)High level of uric acid (600 μmol/L) and telmisartan in different concentrations (10nmol/L,100 nmol/L,1000 nmol/L,10000 nmol/L) were added to renal tubule epithelial cells and cultured for 48 h,the expression of UAT,TGF-β1 and α-SMA were detected by Real-time PCR,RT-PCR,Western blotting or cell immunofluorescence.(2) Wister rats were randomly divided into normal control group(Con),high uric acid group (HU),and telmisartan treatment group (Tel).Four weeks later,Scr,BUN and serum uric acid of the rats were detected.The expression of UAT in rat kidney was detected by Western blotting.Results (1)In vitro,compared to control group,high uric acid (600 μmol/L) inhibited the expression of UAT (P < 0.01),and the inhibition could be alleviated by telmisartan; Telmisartan inhibited the upregulation of TGF-β1 and α-SMA induced by high uric acid(all P < 0.05); (2)In vivo,compared to high uric acid group rats,telmisartan group rats had significantly reduced serum uric acid levels (189.9 μmol/L vs 204.5 μmol/L,P<0.05),upregulated UAT and downregulated TGF-β1 expression in rat kidney (all P< 0.05).Conclusion Telmisartan significantly inhibits the upregulation of TGF-β1 and α-SMA induced by uricemia,which may prevent kidney from fibrosis.The protect effect of telmisartan may be related to the upregulation of UAT.
7.Clinical Observation of Acupuncture plus Paroxetine for Depression Due to Liver-qi Stagnation

Objective To observe the clinical efficacy of acupuncture plus Paroxetine in treating depression due to liver-qi stagnation, for seeking a more effective treatment for this disease.Method Sixty patients with depression due to liver-qi stagnation were randomized into a treatment group and a control group, 30 cases in each group. The treatment group was intervened by acupuncture plus Paroxetine, while the control group was by Paroxetine alone, successively for 8 weeks. Hamilton Depression Scale (HAMD), Chinese medicine syndrome and sign score, and central neurotransmitters were observed before and after the treatment, for evaluating the therapeutic efficacy.Result The total effective rate was 96.6% in the treatment group versus 71.4% in the control group, and the treatment group was superior to the control group (P<0.05).Conclusion Acupuncture plus Paroxetine is an effective method in treating depression due to liver-qi stagnation.
8.Investigation on home blood pressure monitoring among hypertensive patients

Objective :
To investigate the status and influencing factors of home blood pressure monitoring (HBPM) among hypertensive patients, so as to provide the evidence for building and maintaining HBPM among hypertensive patients.
Methods:
Hypertensive patients hospitalized in the First Affiliated Hospital of Guangdong Pharmaceutical University were sampled from July to December 2022, and subjects' general data, HBPM behaviors and cognition were collected using self-designed questionnaires. In addition, factors affecting regular HBPM were identified using a multivariable logistic regression model.
Results:
Totally 440 questionnaires were allocated, and 422 valid questionnaires were recovered, with an effective recovery rate of 95.91%. The respondents included 234 males (55.45%) and 188 females (44.55%), and had a median age of 70 (interquartile range, 15) years. There were 239 respondents with regular HBPM (56.64%). Of 422 respondents, 68 had good cognition of blood pressure monitoring (16.11%), and 79.15% did not think regular changes of their blood pressure within 24 hours, while 72.04% did not think it necessary to measure blood pressure more than twice a day. Multivariable logistic regression analysis showed that recommendation of regular blood pressure monitoring by healthcare workers (OR=4.341, 95%CI: 2.493-7.560), number of blood pressure measurements according to real circumstances (OR=3.858, 95%CI: 1.358-10.961), recording of measurement results (OR=4.945, 95%CI: 1.863-13.129), provision of data to doctors at admission (OR=2.023, 95%CI: 1.173-3.488) and good cognition of blood pressure monitoring (good, OR=11.939, 95%CI: 3.972-35.886; general, OR=9.681, 95%CI: 5.157-18.172) resulted in a high possibility of regular HBPM among respondents.
Conclusion
Hypertensive patients with recommendation of regular blood pressure monitoring by healthcare workers, number of blood pressure measurements according to real conditions, recording of blood pressure measurement results, provision of blood pressure to doctors at admission and good cognition of blood pressure monitoring are more likely to have regular HBPM.
9.Efficacy analysis of ketamine on the patients with depression of modified electric convulsive therapy

Objective To observe the effects of ketamine on the patients with depression re-ceiving modified electric conulsive therapy (MECT).Methods Sixty patients with depression were randomly divided into ketamine group and propofol group (n =30 each group).Atropine 0.5-1.0 mg, propofol 1.0 mg/kg or ketamine 0.8 mg/kg i.v.were given before MECT,Scoline 0.7-1.0 mg/kg i. v.was given after the eyelash reflex disappeared.Hamilton Depression Rating Scale (HAMD)was completed after the 2 nd ,4 th and 6 th MECT,the time of convulsion,twitch index,energy percentage, respiratory recovery time and adverse reactions were recorded.Results The total score of HAMD was significantly decreased with the increasing times of MECT in both groups,compared with propo-fol group,ketamine group's HAMD total score decreased faster,especially after the 4th MECT,the score decreased significantly in ketamine group (P <0.05).The time of convulsion,twitch index,en-ergy percentage, respiratory recovery time, adverse reactions all had no statistical significance between the two groups.Conclusion Compered with propofol,ketamine,as an anesthetic of MECT, can effectively lower the score of HAMD.
10.Stent angioplasty for the treatment of symptomatic stenosis of middle cerebral artery

Objective To investigate the safety and feasibility of endovascular stent angioplasty in treating symptomatic stenosis of middle cerebral artery.Methods Endovascular angioplasty with coronary stents was performed in 27 patients with symptomatic stenosis of middle cerebral artery.The clinical results were reviewed and analyzed.Results Of the total 27 patients,successful placement of the coronary stents Was achieved in 24.Angiography immediately after the procedure showed that the stenotic degree of the diseased artery was markedly decreased from preoperative (80±19)%to postoperative (8±4)%,the improvement was very obvious.Percutaneous transcatheter angioplasty had to be employed in two cases because of the failure of stent placement.A mean follow-up period of 18 months was carried out.During the following up period no transient cerebral ischemia attack occurred in 25 patients and no newly-developed cerebral infarction in region fed by the responsible vessels occurred either.Re=irrigation cerebral hemorrhage was seen in one patient,which occurred three hours after the placement of the stent.In one case the placed stent fell off and immigrated into the siphon of internal carotid artery,and the displaced stent Was took out later with a catching apparatus.In another case re-stenosis occurred six months after the stenting.Conclusion Percutaneous endovaacular stent angioplasty is a safe and effective treatment for symptomatic stenosis of middle cerebral artery,although its long-term results need to be further evaluated.
