1.Analysis on antibiotic prophylaxis in transurethral prostate resection of old patients with sterile urine

Objective To study the efficacy of antibiotic prophylaxis in old patients with negative urine cultures,who were submitted to transurethral prostate resection(TURP).Methods One hundred and twenty old benign prostatic hyperplasia(BPH) patients with sterile urine were divided into two groups(60 patients in each group) randomly.Antibiotic group received empirical venous antibiotics 4 days postoperative.Control group received 5% glucose saline as placebo.Their postoperative infection-associated parameters were compared.Results There were no significant differences between two groups in bacteriuria and infection-associated complications at the 28th day postoperatively.The ratio of bacteriuria in antibiotic group decreased significantly at postoperative at the 4th day when compared with that of control group.The bacteriuria at postoperative at the 4th day had no specific correlation with the long-term bacteriuria,the clinical observation of bacteremia and urinal tract infection,or the culture of prostatic tissue chip.Conclusions Antibiotic prophylaxis reduces the early postoperative bacteriuria for old patients with sterile urine before TURP,but its effectiveness for postoperative infectious complications is debatable.The antibiotics according to empirical validity are not specifically sensitive to the bacteria from various sources.
2.The working principle and application of SK-600I infusion pump

Objective: The theory and repair summary of infusion pump were introduced to improve its maintenance and usage by taking SK-600I infusion pump as an example. Methods:With the cases from practical works including faults such as under-pressure alarm, bubble alarm, block alarms and rate accuracy problem, true or fault respectively, the principle, maintenance, attention issues, fault analyses and repairing methods of infusion pump were presented. Results:To help operators and technicians deal with all kinds of pump failures. Some advices on improving theSK-600I infusion pump design were provided. Conclusion: Infusion pump is a high intelligent medical device, but improper use, mechanical and electrical fault will put adverse impact on its application. Mastering the basic principles, summarizing practical experiences, strengthen operator training, can make better use of infusion pump.
3.DETERMINATION OF FOLIC ACID IN FORTIFIED INFANT MILK POWDER BY HPLC

The fortified milk powder is dissolved in 0.05M K2HPO4 buffer solution, adjust pH to 4.5, and centrifuge for 20 minutes. The supernatant is then adjusted to pH 4.0, go through CGS resin column using 10ml KC1-K2HPO4 as eluant. The eluate is collected as testing solution and injected to HPLC. Column: ODS; mobile phase; 0.05M K2HPO4 : CH3OH (91:9); ultraviolet detector: 285nm. CV% is 3.3% and recovery 87%.
4.Gallbladder emptying and gastrointestinal hormones in functional dyspepsia

Objective To study the role of plasma levels of gastrointestinal hormones and gallbladder kinetics in the pathogenesis of functional dyspepsia. Methods Thirty two patients with functional dyspepsia (FD) and 20 normal subjects as controls were studied. Gallbladder ejection fractions (GBEF) were determined with 99m Tc radionucleography, and plasma levels of motilin (MTL)、cholecystokinin (CCK)、 vasoactive intestinal peptide (VIP) and somatostatin (SS) were measured with radioimmunoassay. Results Both preprandial and postprandial GBEF and MTL levels were markedly lower in FD group than those in control group ( P 0.05), while the levels of postprandial CCK were significantly decreased in FD group ( P 0.05). Conclusion (1) Patients with FD might have a decreased level of preprandial and postprandial gallbladder emptying. The decrease of plasma levels of MTL、CCK and the elevation of VIP might be the cause of gallbladder slow emptying in FD. (2) Radionucleography is non invasive and safe. It can be used in the quantitative analysis and dynamic study of gallbladder and Oddi sphincter functions, and provide accurate, physiological results.

The author holds that the development of e-health should observe ethical principles such as inform-concent, safety, justice, risk-benefit and developing step by step, and puts forward regulating recommendations in many aspects.
6.Clinical Significance of Serum Chemerin and hs-CRP in the Diagnosis of Subclinical Atherosclerosis in Patients with Type 2 Diabetes

Objective To investigate the clinical significance of serum chemokine and hs-CRP levels in patients with type 2 diabetes mellitus with asymptomatic subclinical atherosclerosis.Methods The clinicpathological and follow-up data of 55 patients with Type 2 diabetic from 2012.1 to 2015.12 were collected and reviewed.At the same time,55 patients with non type 2 diabetes were taken as control.Determination of FPG,HbA1c,HDL-C,TC,TG,LDL-C,INS and other indicators by automatic biochemical analyzer,the application of color Doppler ultrasound equipment measurement C-IMT.Correlation using Pearson correlation analysis,risk factor analysis using multiple linear stepwise regression analysis.Results Compared with the control group,the two groups in BMI,WHR,systolic blood pressure,ankle brachial index,FPG,HbA1c,HDL-C,hsCRP,C-IMT,HOMA2-IR and serum chemokines were significantly different,with statistical significance (t =-6.31 ~5.79,P≤0.01 ~ 0.03).In 110 cases of experimental group and control group,the levels of serum chemokines were positively correlated with WHR,HOMA2-IR,C-IMT and hs-CRP (r=0.24~0.29,P=0.01~0.04).C-IMT and age,WHR,systolic blood pressure,diastolic blood pressure,FPG,HbA1c,diabetes duration,hs-CRP was positively correlated (r=0.15 ~0.68,P≤0.01~0.0.04),and the ankle brachial index was negatively correlated (r=-0.32~0.29,P≤0.01).Hs-CRP was positively correlated with HbA1c,HOMA2 IR,serum chemokine,C-IMT (r=0.25~0.32,P≤0.01~0.04),and was negatively correlated with TC and HDL-C (r=-0.27~-0.25,P all 0.02).Cox proportional hazard regression model for multivariate analysis showed that high serum chemokines,hs-CRP and HbA1c were the risk factors of C-IMT (β=0.026~0.658,SE=0.015~0.033,t=2.532~3.421,P≤0.01~0.04).Conclusion High serum levels of chemokines and hs-CRP are the risk factors of C-IMT,and it has important clinical significance for the diagnosis of asymptomatic subclinical atherosclerosis in patients with type 2 diabetes mellitus.

The effects of different technicological processes on the dissolubility of enteric coating aspirin tablets were studied. With testing in vitro, significant differences were found among three kinds of tablets from four factories in dissoluble feature. The length of time needed for release and that for disintegration of tablets from Factory C were 90 minutes and 5 minutes repectively, which were the shortest,and those from Factocy B, 210 minutes and 120 minutes repectively,which were the longest among three kinds of tablets. The average dissoluble amount of aspirin contained in tablets from Factory C was 74.07%;Factory D,74. 51%; Factory A, 66.6%; and Factory B, 62.51%. 10.Combined application of per perineal high-frequency ultrasound and per rectal endoscopic ultrasound examination in diagnosis of anorectal diseases

Objective To evaluate the diagnostic value of per perineal high -frequency ultrasound and/or per rectal endoscopic ultrasound in anorectal diseases. Methods 54 patients suffered from anorectul diseases, including 22 patients with anal fistula, 14 patients with perianal abscess, 12 patients with rectal cancer, and 6 patients with perianorectal inflammation, were performed with per perineal high-frequency ultrasound and/or per rectal endoscopic ultrasound examination. The diagnostic results were confirmed by clinical diagnosis, including operative and pathological detection. Results Except for the misdiagnosis of 4 cases of orifice location and 1 case of rectal cancer, all the other cases were correctly diagnosed. Conclusion Combined application of per perineal high-frequency ultrasound and per rectal endoscopic ultrasound examination has great practical value for ascertaining the trend and internal orifice location of anal fistula, location of perianul abscess, the infiltration depth of rectal cancer, and lymph node metastases.
