1.Effect of comprehensive intervention on antimicrobial use and incidence of healthcare-associated infection in sports medicine department

Objective To explore the effect of comprehensive intervention measure on antimicrobial use and incidence of healthcare-associated infection(HAI)in patients in sports medicine department.Methods Antimicrobial use and incidence of HAI in hospitalized patients in the surgical departments (sports medicine department and other surgical departments)of a hospital between January 2006 and December 2010 were analyzed retrospectively(before intervention),from January 2011, comprehensive intervention based on clinical-centered multi-department cooperation was adopted by sports medicine depart-ment,the other surgical departments over the same period were not comprehensively intervened;antimicrobial use and inci-dence of HAI in patients in surgical departments between January 2012 and December 2013 were monitored prospectively (after intervention ),effectiveness of intervention was evaluated.Results Antimicrobial use and incidence of HAI in pa-tients in sports medicine department after intervention were both lower than before intervention (55.75% vs 95.26%,χ2=753.42;0.10% vs 0.32% ,χ2 =15.13,both P <0.01).Antimicrobial use in patients in other surgical departments after intervention was lower than that before intervention(65.63% vs 73.79% ,χ2 =251.57,P <0.01 );incidence of HAI was higher than that before intervention(0.55% vs 0.42% ,χ2 =19.04,P <0.01).Conclusion Comprehensive in-tervention measure based on clinical-centered multi-department cooperation is safe and effective,it can reduce antimicrobial use and incidence of HAI in patients in sports medicine department.
2.Expression of IL-12 family subunit genes in mice C6 glioma cells

Objective:To observe the expression of IL-12 family subunit genes by real-time quantitative PCR in mice C6 glioma cells,construct the basis of the brain glioma research on IL-12 family in the future.Methods:Mice C6 glioma RNA was abstracted and reversed transcription cDNA.The mice C6 glioma cells mRNA expression influence of IL-12 family subunit genes was compared and analyzed by real-time quantitative PCR.Results: In mice C6 glioma cells, high expression abundances in IL-23a, IL-12a, midlde expression abundances in EBI3, IL-27, low expression abundance in IL-12b.Conclusion: IL-12 families are closely related to the occurrence and development of glioma,IL-12,IL-23 are regarded as the most potential anti-glioma cytokines among them,research de-velopments will bring a new way of brain glioma immune therapy.
3.Survey on the cleanliness of health care workers' hands and uniforms in a hospital

Objective To understand the cleanliness of hands and uniforms of health care workers(HCWs)while they were working in a hospital.Methods Specimens of hands and uniforms of HCWs while they were working were collected and detected.Results A total of 342 specimens were collected, 173 were specimens from hands and 169 were from uniforms, the total qualified rate was 78.65%,qualified rates of hands and uniforms were 76.30% and 81.07% respectively.Qualified rates of hands and uniforms of different HCWs were compared respectively, differences were both statistically significant (both P<0.05);qualified rates of hands and uniforms of nurses were both highest (87.93% and 92.86% respectively), followed by doctors (75.86% and 87.72% respectively), while medical auxiliary persons were the lowest(64.91% and 62.50% respectively).Correlation analysis between qualified rates of hands and uniforms of HCWs in general wards showed that the correlation coefficient was 0.930 (P<0.01).Conclusion Cleanliness of hands and uniforms of HCWs needs to be further improved, especially medical auxiliary persons;cleaning frequency and cleanliness standard of HCWs' uniforms needed to be studied further.
4.Regulation of dopamine receptors on cocaine-induced MAPK signaling pathway and c-fos gene expression

AIM: To investigate the role of D1 and D3 dopamine receptor on MAPK signal transduction and c-fos gene expression after acute cocaine treatment. METHODS: Activations of extracellular signal-regulated kinase (ERK), the c-Jun N-terminal kinase (JNK), p38 activation and expression of c-fos in wild type and D1 and D3 receptor mutant mice after acute cocaine treatment were checked by Western blotting. RESULTS: ERK activation and c-fos induction was enhanced in D3 mutant mice and abolished in D1 mutant mice by acute cocaine treatment, while p38 and JNK activation was not obviously modulated by the D1 and D3 receptors by acute cocaine treatment. Meanwhile, c-fos induction was inhibited when SL327, a specific MEK inhibitor, was injected before cocaine treatment. CONCLUSION: D1 and D3 receptors play opposite roles in the regulation of ERK activation and c-fos gene expression after acute cocaine treatment. The expression of c-fos gene depends on ERK signal pathway after acute cocaine treatment.
5.IL-10 inhibits cardiac fibroblasts proliferation and phenotype transformation induced by TGF-β1 in rats

Objective To examine the effects of IL-10 on cardiac fibroblasts ( CFBs) proliferation and phenotype transformation to myofibroblasts (MyoFbs) induced by transforming growth factor-β1 (TGF-β1);and to investigate the regulating pathways .Methods Cardiac fibroblasts were isolated from cardiac ventricles of neonatal SD rats . The passage 2~4 were used and divided into the following groups for treatment:1) control group, 2) IL-10 reac-tion group, 3) TGF-β1 reaction group, and 4) IL-10 plus TGF-β1 reaction group (TGF-β1 treatment followed with IL-10 pretreatment ) .Cells proliferation was assessed by MTT assay and immunocytochemistry staining for prolifera-ting cell nuclear antigen (PCNA);the phenotype transformation into MyoFbs was assessed by immunocytochemistry of α-smooth muscle actin (α-SMA);extracellular signal related kinase ( ERK1/2) and P38 kinase pathways were assessed by western-blot.Results TGF-β1 (10 μg/L) treatment boosted the proliferation and the expression ofα-SMA significantly (P<0.01), while IL-10 (10, 50 or 100 μg/L) plus TGF-β1 co-treatment induced lower cell proliferation and expression of α-SMA than treating with TGF-β1 alone ( P<0.05 ) , with the inhibitory effect of IL-10 being concentration dependent .TGF-β1 could significantly stimulate the ERK 1/2 and P38 kinase phospho-rylation ( P<0.01 ) , however IL-10 (100 μg/L) plus TGF-β1 co-treatment failed to down-regulated the phospho-rylation of ERK1/2 and P38 kinase compared with TGF-β1 alone ( ERK1/2:P<0.05;P38:P<0.01 ) .Conclu-sions IL-10 can attenuate TGF-β1-induced CFBs proliferation and phenotype transformation to MyoFbs .The in-hibitory effects may explained by a mechanism of inhibiting the activation of ERK 1/2 and P38 kinase .
6.Salvage therapy by hemicortical excision and reconstruction for low-grade malignant bone tumour

Objective: To treat low-grade limb malignant tumors with hemicortical excision and reconstruction of the defect with recycled pasteurized autogratf. Methods:From January 2005 to January 2011, 10 patients [age (27.0±6.5) years] suffering from low-grade limb malignant tumors were treated with hemicortical excision and reconstruction of the defect with recycled pasteurized autogratf:7 were histopathologically diagnosed as parosteal osteosarcoma, and 3 as peripheral chondrosarcoma. According to the Enneking staging system, 8 were in Stage IA and 2 were in IB. Results:All the patients were followed-up for (50±23) months. All the patients had bony union at the last follow-up. The length of tumor bone resection was (9.5±2.7) cm, operation time was (150±45) min, intraoperative blood loss was (1000±350) mL, and drainage volume was (450±200) mL. hTe only fracture in the host bone during the operation was treated by internal ifxation. Local recurrence occurred in 1 patient was treated by a wide operative margin and reconstruction with a prosthesis. There was no distal metastasis. A ccording to the Enneking system, 9 patients had excellent results and 1 had good results. Conclusion:Hemicortical excision for low-grade limb malignant tumors is safe and effective.
7.En bloc excision and reconstruction of the defect with pasteurized autograft for femoral primary malignant bone tumor in childen

Objective:To evaluate the efficacy of femoral primary malignant bone tumors treated by limb salvage, with wide en bloc excision and reconstruction of the defect with recycled pasteurized autogratf.
Methods: From January 2008 to January 2011, 11 patients (7 males, 4 females), aged (11.0±2.5) years suffering from femoral primary malignant bone tumors were treated with en bloc excision and reconstruction of the defect with recycled pasteurized autograft. Ten patients were histopathologically diagnosed with high-grade osteosarcoma, and 1 with Ewing’s sarcoma.According to the Enneking staging system, all patients were in Stage IIb.
Results:All patients were followed-up for 24-65(42±20) months and all showed bony union at the last follow-up. hTe length of tumor bone resection was (17.5±3.2) cm, the operation time was (180±35) min, the intraoperative blood loss was (1200±250) mL, and drainage volume was (650±125) mL. Local skin necrosis occurred in 1 patient (9.1%), which was resolved by debridement. Nonunion occurred in 3 patients (27.3%), who were treated by secondary iliac crest cancellous bone gratfing.Pulmonary metastasis occurred in 1 patient (9.1%) who died 35 months post-operatively. According to the function assessment by the Enneking system, 5 patients had excellent results, 4 had good results, 1 fair and 1 poor results, with a satisfaction rate of 81.2%.
Conclusion:A pasteurized autograft can be an easily accessible and economical alternative for children’s malignant bone tumor of femurs.
8.Study on autofluoresecence spectrum of gastric juice in clinical screening for gastric cancer

Objective To evaluate autofluorescence spectrum determination of gastric juice in clinical screening for gastric cancer. Methods A total number of 1506 gastric juice specimens were collected from the patients underwent gastroscopy. The specimens were diluted by 62.5 times with buffer solution to achieve pH9, then the fluorescence spectrums were detected using MpF 4 fluorescence spectrophotometer. Experimental data were double input using Epi6 software and a database was established after checking. Results The wavelength of excitation was 285 nm, the bandwidth was 3 nm, and the wavelength of emission ranged from 300 800 nm.Two peaks were found in the autofluorescence spectrum of gastric juice from all patients. The first peak was distinctively higher in gastric cancer than that in benign lesion. Single analysis was carried out with SPSS software, and the best critical point P 1FI≥76.5 on receiver operator characteristic curve(ROC) was selected as diagnostic standard for gastric cancer. The sensitivity, specificity, and accuracy of diluted gastric juice's autofluorescence spectrum for diagnosing gastric cancer were 83.2%, 80.7%, and 82.0%, respectively. Synthetic analysis was carried out with CART software to set up the diagnostic model for gastric malignant diseases by P 1FI≥78.5. The model's sensitivity, specificity, and accuracy for prior probability were 93.6%, 79.5% and 80.7%, respectively; for posterior probability were 83.2%, 81.3%, and 81.4%, respectively. Conclusions Autofluorescence spectrum determination of gastric juice had high sensitivity, specificity and accuracy for clinical screening of gastric cancer, so it might be a safe and simple, economical and effective method for clinical screening of gastric cancer.
9.Construction and Expression of RNase-Resisting His-Tagged Virus-Like Particles Containing FluA/B mRNA.

To prepare virus-like particles containing FluA/B mRNA as RNA standard and control in Influenza RNA detection, the genes coding the coat protein and maturase of E. coli bacteriophage MS2 were amplified and cloned into D-pET32a vector. Then we inserted 6 histidines to MS2 coat protein by QuikChange Site-Directed Mutagenesis Kit to construct the universal expressing vector D-pET32a-CP-His. In addition, the partial gene fragments of FluA and FluB were cloned to the down-stream of expressing vector. The recombinant plasmid D-pET32a-CP-His-FluA/B was transformed to BL21 with induction by IPTG. The virus-like particles were purified by Ni+ chromatography. The virus-like particles can be detected by RT-PCR, but not PCR. They can be conserved stably for at least 3 months at both 4 degrees C and -20 degrees C. His-tagged virus-like particles are more stable and easier to purification. It can be used as RNA standard and control in Influenza virus RNA detection.

10.Investigation of the Habits and Understanding Situation and Demand of Knowledge of Drug Use among Pa-tients in Our Hospital

OBJECTIVE:To provide reference for the development and improvement of pharmaceutical care in our hospital. METHODS:By questionnaire investigation,habits and understarding situation and demand of knowledge of drug use among pa-tients in our hospital were randomly collected by face-to-face interview or network platform. A statistical analysis was carried out on obtained results. RESULTS:In this questionnaire investigation,289 questionnaires were sent out face-to-face,and 282 valid ques-tionnaires were returned with effective feedback rate of 97.58%. Totally 51 valid questionnaires were returned through network plat-form. A total of 333 valid questionnaires were returned through two ways. The results of questionnaire investigation showed that 44.14% of the surveyed patients would read the drug instructions before taking drugs;41.14% of the surveyed patients would pay more attention to ADR and cautions stated in drug instructions. More than 60% of the surveyed patients had various bad habits of drug use. When getting better,66.07% of the surveyed patients would stop using drugs or reduce the dosage. When having not im-proved,26.73% of them would change drugs or increase dosage.Consulting with the medical staff was the most common source(69.07%)of drug use knowledge,and it was also the most trusted source(84.08%). Among the knowledge of drug use, surveyed patients most expected to understand theADR and side effects(65.46%),and the selectionindication and main us-er,cautions,drug interaction in multiple useandusage and dosagewere also selected frequently,and the choices of some options were significantly affected by the characteristics of people,such as age and education degree(P<0.05). The most expected way to acquire drug use knowledge wasface-to-face consultation with medical staff (72.97%),followed bytelephone consulta-tionandnetwork consultation,and the choices of some options were significantly affected by the characteristics of people,such as age and education (P<0.05). CONCLUSIONS:Patients in our hospital haven't pay enough attention to drug instructions and their content,and bad habits of drug use still exist. Sources of drug use knowledge are diverse,but confidence of other sources is not enough except for source of medical staff. The demands for the knowledge of drug use are different,however,the present form and content of pharmaceutical care in our hospital cannot fully meet the needs of patients.
