1.Clinical Observations on Electroacupuncture at Huatuo Jiaji Points and Du Meridian Points for Spinal Cord Injury-induced Dysfunction

ObjectiveTo seek an effective protocol fortreating spinal cord injury-induced limb dysfunction by comparing the therapeutic effects of electroacupuncture at Huatuo jiaji points plus Du meridian points versus conventional electroacupuncture. MethodForty patients with spinal cord injury-induced limb dysfunction were allocated, using a random number table, to treatment and control groups, 20 cases each. Both groups were treated with ganglioside and routine rehabilitation therapy. In addition,the treatment group received electroacupuncture at Huatuo jiaji points and Du meridian points and the control group, conventional acupuncture. Lower limb function was assessed using ASIA Motor Score, Sensory Score and the MBI in the two groups of patients before treatment and after one and two courses. The clinical therapeutic effects were evaluated.ResultThere was a statistically significant pre-/post-treatment difference in lower limb dysfunction in both treatment and control groups (P<0.05). The improving effect was better in the treatment group than in the control group (P<0.05,P<0.01).ConclusionElectroacupuncture at Huatuo jiaji points and Du meridian points has a definite therapeutic effect on spinal cord injury-induced limb dysfunction. It is superior to conventional electroacupuncture.

The present research focused on general surgery that would be found by co-words cluster analysis, and stages of development about each topic are discussed by strategic coordinates based of cluster results. It is helpful to provide a decision-making reference for professionals and managers.
3.Analysis on subject's hot topics from scientific research grants using MetaMap

Objective Based on scientific research grants to explore the feasibility of analyzing the subiect's hot topics. Methotis The CRISP(Computer Retrieval of Information on Scientific Projects)database of National Institutes of Health was used to retrieve the grants related of gene mechanism of essential hypertension in 2006. We obtained concepts from the fields of title and abstract by MetaMap. and then analyzed the key concepts about genes that were found more than 3 times. Results There were 73 related grants about that from CRISP in 2006. After processing the samples by MetaMap, we got 45 high frequency concepts about genes. which were divided into seven groups. Conclusions It iS feasible to analyze and study subject hotspot by scientific research grants. and it can be used to the supplement of normal literature analysis. MetaMap is a better tool for natural language processing, but also has some limitations.
4.Clinical Observation of Acupuncture plus Paroxetine for Depression Due to Liver-qi Stagnation

Objective To observe the clinical efficacy of acupuncture plus Paroxetine in treating depression due to liver-qi stagnation, for seeking a more effective treatment for this disease.Method Sixty patients with depression due to liver-qi stagnation were randomized into a treatment group and a control group, 30 cases in each group. The treatment group was intervened by acupuncture plus Paroxetine, while the control group was by Paroxetine alone, successively for 8 weeks. Hamilton Depression Scale (HAMD), Chinese medicine syndrome and sign score, and central neurotransmitters were observed before and after the treatment, for evaluating the therapeutic efficacy.Result The total effective rate was 96.6% in the treatment group versus 71.4% in the control group, and the treatment group was superior to the control group (P<0.05).Conclusion Acupuncture plus Paroxetine is an effective method in treating depression due to liver-qi stagnation.
5.Clinical features of interstitial lung disease in patients with primary Sj(o)gren's syndrome

Objective To explore the clinical,laboratory,and pathologic manifestations of the interstitial lung disease(ILD)associated with primary Sj(o)gren's syndrome(pSS,pSS-ILD).Methods Clinical data of 15 patients with pSS-ILD admitted to our hospital from 2006 to 2008 were collected and the different features were compared with 18 patients with pSS without ILD.Eight patients with pSS-ILD were followed up and observed the changes of high-resolution computed tomography (HRCT).Results The age at onset was later in pSS-ILD[(57±11)years]than in pSS without ILD[(43±11)years](P<0.01).The initial symptoms in six of patients with pSS-ILD were cough,expectoration,chest distress or dyspnea upon exertion.The respiratory clinical manifestations,circulationsystem involvement and pulmonary artery hypertension (PAH) were more common in pSS-ILD than in pSS without ILD(P<0.01).Compared with patients with pSS without ILD,patients with pSS-ILD had significantly higher serum IgG level(P<0.01).The common findings of HRCT included reticular changes,irregular linear hyper-attenuating areas,and nodules.Pleural involvement was found in 8 patients,honeycomb change in 5 patients and PAH in 3 patients.After treated for 6 months of 8 patients,radiological findings improved in 4 patients,stable in 2 patients,and worse in 2 patients.The comlnon histological findings included focal fibrosis in alveolar wall and alveolus with and alveolar space inflammatory cell infiltration,interstitial inflammation with mulifocal lymphocyte infiltration.One patient had the pathological changes of nonspecific interstitial pneumonia(NSIP).Conclusion The age at onset of pSS-ILD is late and tends to produce respiratory symptoms and prone to have circulationsystem involvement such as PAH and elevated serum IgG level.Honeycomb change in HRCT can be seen in one third of patients and most patients can improve after treatment,however.NSIP can be observed in histopathologieal
6.The association between concentration of particulate matt(PM2.5) and asthma attack in children: a Meta-analysis

Objective To evaluate the association between the concentration of particulate matter (PM2.5)and asthma attack in children.Methods The computer and databases were used to search the key wordsAsthma andChild were searched by means of the subject headings ( Asthma andChild) and free words (Asthma * , Child *andPediatric * etc.),moreover PM2.5 was searched on the basis of free words such asPM2.5, Particulate Matter *which were linked withAND in PubMed and Medline database.Furthermore,the key words asthma AND (Child OR Pediatric OR Infant) AND (PM2.5 OR Particulate Matter OR Inhalable Particles) for keywords were searched in CBM,CNKI,VIP and Wanfang databases.The literatures included those from the initial computer database up to those of December 2014 from computer database.Meanwhile manual research was added to screen literatures according to the standards of inclusion and exclusion.Newcastle-Ottawa Scale was employed to assess the quality of the included literatures including selection,comparability and outcomc.Data wcre synthesized and analyzed by Rcvman5.3 and Stata12.0 version for statistical analysis,heterogeneity test,sensitivity analysis,subgroup analysis,and so on.Results A total of 17 literatures involving 1326573 children were included.The findings from quality assessment involved 2 studies for 8 scores,2 for 7 scores,7 for 6 scores,5 for 5 scores,and 1 for 4 scores,respectively.The result of Meta-analysis showed that the combined the odds ratio (OR) of PM2.5 with childhood asthma attack was 1.06,and 95% confidence interval(CI) was 1.03-1.08.Subgroup analysis showed that the OR on the asthma hospital visit equaled to 1.03 (95% CI 1.01-1.06),and the OR on the asthma symptom equaled to 1.11 (95% CI 1.06-1.16),moreover,subgroup analysis between developed and developing countries showed that the OR of the asthma on developing countries corresponded to 1.04 (95% CI 1.02-1.06),and the OR of the asthma on developed countries corresponded to 1.06 (95% CI 1.03-1.09).Conclusion There is a positive correlation between concentration of PM2.5 and asthma attack in children.
7.Effect of Electro-acupuncture at Scalp-acupoints on Motor Function and Oxidative Stress in Mice with Parkinson's Disease

Objective To observe the effect of penetrating electro-acupuncture at scalp-acupoints on the motor function and oxidative stress action of mice with Parkinson's disease (PD).Methods Mouse model of PD was established by 5-day intraperitoneal injection of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP,30 mg/kg),and then PD mice were given electro-acupuncture set at different parameters.The effect of the electroacupuncture on the behavior and expression of tyrosine hydroxylase (TH) of mice were observed to screen out the appropriate treatment parameters.Healthy C57BL/6 mice were randomly divided into blank group,model group,positive group (madopar,6.25 mg/kg) and electro-acupuncture group.The mice except for blank group were given intraperitoneal injection of MPTP to induce PD,and then the PD mice of positive group and electroacupuncture group were treated with madopar(6.25 mg/kg) and electro-acupuncture respectively from the 6th day for 7 continuous days.Behavioral test was carried out on the 5~ modeling day and on the Th electro-acupuncture treatment day to evaluate the motor function of the mice.After the mice were killed and their brains were isolated,mitochondrial complex Ⅰ ~Ⅳ activities,reactive oxygen species(ROS) level,malondialdehyde (MDA) content,and total activities of superoxide dismutase (SOD) in the cerebral mitochondria were determined to evaluate the effect of electro-acupuncture at acupoints on anti-oxidative stress of PD mice.Results The motor function of PD mice was improved,the activity of mitochondrial complex Ⅰ was increased,the contents of ROS and MDA in cerebral mitochondria were decreased,and the activity of SOD was increased in the electro-acupuncture group,the differences being significant compared with those of the model group (P < 0.05 or P < 0.01).Conclusion Penetrating electro-acupuncture at scalp-acupoints can increase the anti-oxidative ability of the cerebral mitochondria and improve the motor function of PD mice.
8.Pedicle screw fixation for thoracolumbar fracture with spinal cord injury:a comparison of the selective injury segment and the whole bone graft fusion

BACKGROUND:Spinal canal decompression is needed in posterior pedicle screw fixation surgery for thoracolumbar burst fractures combine with spinal cord injury. The structure of posterior spine is often damaged. The posterolateral bone fusion in al fixed segment is stil the main surgery. In order to further reduce fusion segment and maintain motor unit, it is necessary to perform selective segmental bone graft fusion during fixation and decompression.
OBJECTIVE: To discuss the advantages of selective posterolateral vertebral fusion for thoracolumbar fracture with spinal injury through comparing with posterolateral vertebral fusion.
METHODS: Data of 83 thoracolumbar burst fracture cases, who received posterior lumbar decompression and short segment fixation with pedicle screws and bone graft through injured vertebra from January 2006 to July 2013, were analyzed retrospectively. According to fusion segments, above patients were divided into selective posterolateral vertebral fusion group (n=42) and the whole posterolateral vertebral fusion group (n=41). Perioperative index, internal fixation, vertebral height loss rate, Cobb angle, spinal nerve recovery and Oswestry Disability Index were compared between the two groups.
RESULTS AND CONCLUSION:(1) Al cases were folowed up for 25-32 months. (2) There was no statistical significance in operation time, intraoperative blood loss and ambulation time between the two groups (P > 0.05). Postoperative drainage volume was less in the selective posterolateral vertebral fusion group than in the whole posterolateral vertebral fusion group (P < 0.05). Before removal of fixator, there was no loosened fixator or breakage of screw or stick. (3) There were improvements in the rate of vertebral front height loss and Cobb angle in both groups at various time points after operation (P < 0.05). There was no statistical significance in the rate of vertebral front height loss and Cobb angle in both groups (P > 0.05). (4) There was no significant difference in fusion rate at 6 months after treatment between the two groups (P > 0.05). Fusion was achieved in both groups before removal of the fixator. (5) Spinal nerve recovery was found after treatment in both groups. No significant difference in Oswestry Disability Index was detected in final folow-up (P > 0.05). (6) Results verified that compared with the whole posterolateral vertebral fusion, selective posterolateral vertebral fusion can obtain a good vertebral height and prevent Cobb angle loss again, reduce the internal fixation loosening and breakage. After removal of the fixator, selective posterolateral vertebral fusion can reduce spinal motion unit lost, and decrease the adjacent vertebral degeneration.
9.Clinical and laboratory studies of patients with low body mass index and chronic obstructive pulmonary disease

Objective To study the characteristics of patients with low body mass index (BMI) chronic obstructive pulmonary disease(COPD). Methods A total of 38 clinically stable patients with moderate-to-severe COPD were enrolled. They were divided into two groups: underweight (UW) group (n=16,BMI<20);normal weight(NW) group(n=22, 20≤BMI<26). Body height and weight, smoking indexs, and six minutes walk distance (6MWD) were assayed. The British Medical Research Council (MRC) dyspnea scale was used to assess the degree of dyspnea. St. George's Respiratory Questionnaire (SGRQ) and Short Form 36 item Questionnaire (SF-36) were used for health-related quality of life (HRQoL) evaluation. The serum concentrations of leptin and ghrelin were detected by enzyme-linked immunosorbent assay (ELISA). Results Compared with the NW group, the inspiratory eapacity(IC), forced expiratory volume in one second (FEV), vital capacity (VC) ,most ventilate volume (MVV) and peak expiratory flow(PEF) were lower(P<0. 05) in the UW group. Residual volume-to-total lung capacity ratio (RV/TLC), smoking indexs and MRC score were higher (all P<0. 05) and 6MWD was significantly lower (P<0. 05) in the UW group than in NW group. Activity scores,impact scores and total scores of SGRQ showed significant deterioration in the UW group (P<0. 05). SF-36 also showed significantly worse scores for the parameters of the emotional and social functioning (P < 0. 05 ). Serum leptin was significantly lower ( P< 0.01 ) and ghrelin was higher in UW group than in NW group (P<0. 05). Stepwise multiple regression analyse showed that lC,mental health(MH) and physical function (PF) of SF-36, leptin,6MWD and smoking indexs were independently correlated with BMI. Conchtsions The pulmonary function, nutritional status, PF and life quality of COPD patients with low BMI were more deteriorative. The most significant influencing factor for BMI in COPD patients was IC. M H,exercise capacity,leptin level and smoking indexs were independently correlated with BMI in COPD patients. It is important to retrieve low BMI in the management of COPD patients.
10.Effect of simvastatin on bone formation and osteogenic differentiation of bone marrow stromal cells in young rats

BACKGROUND: Recently simvastatin has been shown to stimulate osteogenic differentiation and bone formation, but there is no report about the effect of simvastatin on the bone development of young rats.OBJECTIVE: To evaluate the effects of simvastatin on osteogenic relative genes of proximal tibia trabecular bone and osteogenic differentiation of bone marrow stromal cells (BMSCs).METHODS: Twenty 1-week-old Spragua-Dawley young rats were randomly and equally divided into simvastatin and control groups. Rats in the simvastatin group were treated with a subcutaneous injection of simvastatin[5 mg/(kg·d)] for 2 weeks, while rats in the control group were treated with placebo for 2 weeks. The expressions of bone morphogenetic protein-2 (BMP-2), matrix metalloproteinase-13 (MMP-13), and vascular endothelial growth factor (VEGF) of trabecular bone in the tibia were analyzed by mmunohistochemicel staining. BMSCs harvested from the rat femur were osteogenic-differentiation cultured. Alkaline phosphatase (ALP) staining was performed on day 14, real-time PCR analysis was applied to investigate the BMP2, RUNX2,Osterix, MSX2, DLX3, DLX5 mRNA expressions during osteogenic differentiation in vitro on day 21, and von Kossa staining was detected on day 28.RESULTS AND CONCLUSION: ① There was no significant difference in the expressions of BMP-2, MMP-13, and VEGF between simvastatin and control groups. ② The percentages of ALP positive-stained cells were about 30% and there was no significant difference between the two groups (P > 0.05). ③There was no significant difference in the expressions of BMP-2,RUNX2, Osterix, MSX2, DLX3, DLX5 mRNA in osteoganic differentiation-induced BMSCs. ④ von Kossa staining demonstrated that dark brown calcified spots in various sizes were observed, but there was no significant difference in size and density between simvastatin and control groups. A subcutaneous injection of simvastatin[5 mg/(kg·d)] for 2 weeks could not remarkably affect osteogenic relative genes of bone trabecula and osteogenic differentiation of BMSCs.
