1.Combination of scFv-AFP with doxorubicin inhibits ceII proIiferation in hepato-ceIIuIar carcinoma ceII Huh7

This study was to investigate the inhibitory effects of single-chain variable fragment of alpha fetopro-tein (scFv-AFP)in combination with doxorubicin on the proliferation of human hepatocellular carcinoma cell lines Huh7.Huh7 cells were treated with different concentration of scFv-AFP or doxorubcin alone or their combi-nation.The inhibitory effects were detected by MTT assay,and cycle arrest and apoptosis of Huh7 cells were ana-lyzed by flow cytometry in different groups using PI and Annexin V /PI-staining respectively.Results showed that scFv-AFP,doxorubicin alone or in combinations dose-dependently inhibited the proliferation of Huh7,and a syn-ergistic effect was observed in their combined action.The combination treatment resulted in significantly higher apoptosis than those in other groups (P <0.05).scFv-AFP (40 μg/mL)markedly blocked the Huh7 cell pro-gression by arresting the cells in the G0 /G1 phase,and the percentage of cells in S phase decreased dramatically (P <0.05);and scFv-AFP combined with doxorubicin blocked the Huh7 cell progression by arresting the cells in G2 /Mphase (P <0.01).
2.Ultrafiltration and nanofiltration membrane separation technology in extracting and purifying oligosaccharides of Rehmannia glutinosa

Objective Oligosaccharides of Rehmannia glutinosa were separated and purified by different molecular weight cut-off ultrafiltration(UF) membranes,and then ultrafiltrate was concentrated and purified by nanofiltration(NF) membrane.Methods Different molecular weight cut-off UF unit first and then NF unit were used in process.Results Oligosaccharides were separated by twice UF,the optimum separation conditions:the concentration of feed solution was 13—132 mg/mL,the operation pressure was 0.25—0.275 MPa,and the temperature was 20—40 ℃.Then ultrafiltrate was concentrated and purified by NF membrane,the optimum separation conditions:the operation pressure was 0.59—0.79 MPa,the temperature was 20—40 ℃,and the concentration multiplegot to three.Under this condition,the total extraction rate of oligosaccharide products was 46.63% and the purity was above 93.3%,the molecular weights of oligosaccharide products determined by gel filtration were less than 6 000.Conclusion The technology is not only simple and feasible but also easy to separate and purify the oligosaccharides of R.glutinosa effectively.
3.Study on optical energy transmission in biotic tissues by Monte Carlo method.

Biotic tissues are a kind of highly scattering random media; studies on laser light propagation in biotic tissues play an important role in bio-medical diagnostics and therapeutics. The propagation and distribution of infinitely narrow photon beam in tissues are simulated by Monte Carlo method in this paper. Also presented are the energy distribution with regard to depths, light distribution in tissues, reflection and transmittance on the upper and lower surface. The optical parameters adopted in this study are g, albedo and microa, which have influence on energy distribution. The results show: The energy distribution decreases more quickly with the increase of depths and reveals a peak value close to the surface; g factor plays an important part in the lost energy on the upper surface and lower surface; the decrease of g factor causes weaking of the forward moving ability, so the penetration depth becomes smaller and the energy becomes dispersives variation of albedo has distinct effect on the shallow and deep tissues.

4. Application of "three-in-one" tutorial system in standardized residency training of general practice

The mentor system has been applied in the standardized residency training, currently the "one to one" ortwo mentors′ system are more common applied. However, the clinical training of general practitioners needs joint effort of mentors from the general practice department and specialists in hospital, as well as of mentors from primary health institutes. Thus, we proposed a "three-in-one" tutorial system, which consisted of one GP mentor, one specialists in the hospital and one primary physician for residency training of general practice. The implementation of the "three-in-one" system has motivated the enthusiasm of specialists and community instructors, and enhanced their responsibility. The mentors from different department can learn from each other and complement each other, so the overall teaching level can be raised and the quality of standardized training of general practitioners can be improved.
5.Evaluation of isotopic labeling of lysine residues of peptides for quantitative proteomics.

To evaluate the reagent 2-methoxy-4,5-dihydro-1H-imidazole used for isotopic labeling in quantitative proteomics, we synthesized 2-methoxy-4,5-dihydro-1H-imidazole and its tetradeuterated analog in three steps. Prior to tryptic cleavage, bovine serum albumin (BSA) was reduced and alkylated. Tryptic peptides were derivatized with an equal volume of either DO or D4 and D4-derivatized peptides were mixed with at variable ratio (from 10:1 to 1:5) prior to MS and MS/MS analysis. We used matrix assisted laser desorption/ionization-mass spectrometry (MALDI-MS) and Electro spray ionization-mass spectrometry (ESI-MS) to evaluate the quantitative capability of labeling. The specificity of the reagent is excellent: only lysine side chains were modified among tryptic peptides. MALDI and ESI ionization modes not only could achieve the quantification of differentially expressed proteins but also facilitate the de novo sequencing. This side-chain modification can be used for quantitative analysis with proteomic strategies involving liquid chromatography. Reverse phase liquid chromatography (RPLC) kept a good resolution, and the introduction of D atoms did not introduce a variation of retention time between heavy and light peptides in RPLC.

6.Low-dose computed tomography screening for lung cancer in Tianjin: a preliminary clinical analysis of baseline screening and follow-up results

Objective:To explore the efficacy of low-dose computed tomography (LDCT) baseline and follow-up scans of lung cancer screening and to analyze lung nodules and other thoracic lesions detected from baseline and follow-up. Methods:A total of 650 sub-jects were enrolled in the LDCT lung cancer screening program, and investigators mainly focused on the analysis of 548 subjects who participated in the follow-up scan. The investigators recorded the nodules and other lesions of baseline screening, compared them with the follow-up images, and recorded their progress. Results:A total of 101 subjects were positive in the baseline screening, with a positivity rate of 18.4%. Six cases of lung cancer were confirmed by pathology, with a detection rate of 0.92%(6/650). The detection rate of lung cancer in female non-smokers (1.59%) was higher than that in male smokers (1.04%) without significant difference (P=0.624). Detected in the follow-up scan were 19 cases of new nodule-positive subjects. The positive rate for new nodules was 3.5%(19/548). The difference between the three-and two-dimensional levels was statistically significant. Conclusion:The effect of LDCT screen-ing for early lung cancer is significant. The detection rate in female non-smokers was not significantly higher than that in male smok-ers. Thus, LDCT lung cancer screening is equally significant for both sexes. The computer-aided detection (CAD) volume measurement technique is better to evaluate the progress of nodules during the follow-up interval.
7.Influence of rmhTNF on the Chemotherapy Treatment of Small Cell Lung Cancer

The efficacy and safety of the recombinant mutant human tumor necrosis factor (rmhTNF) combined with chemotherapy vs chemotherapy alone in the treatment of patients with small cell lung cancer (SCLC) were evaluated in this study. The selected 37 patients with SCLC were divided into experimental group (n = 18) and control group (n = 19). Bothgroups were subjected to EP regimen. While in the experimental group, a regimen of 4 × 106 U/m2 rmhTNF intramuscular injection was given once a day from the 1st to 7th day and 11th to 17th day on the chemotherapy cycle.Twenty-one days were as a chemotherapy cycle and all patients received treatment with 2 cycles.The response rate was 83.3 % (15/18) in the experimental group and 63.2 % (12/19) in the control group respectively (P<0.05). The KPS score after treatment was 78.4±9.6 in the experimental group and 71.2±9.7 in the control group with the difference being significant (P<0.05).No severe adverse effects occurred in the two groups. It was concluded that the curative effectiveness of the rmhTNF combined with chemotherapy in the treatment of SCLC was more satisfactory than chemotherapy alone. The former could obviously improve the quality of life of the patients with SCLC.
8.Values of serum human epididymis protein 4, endothelial cell specific molecule-1 and epidermal growth factor receptor in the diagnosis of lung cancer

Objective:To investigate the diagnostic values of human epididymis protein 4 (HE4), endothelial cell specific molecule-1 (ESM-1) and epidermal growth factor receptor (EGFR) for lung cancer.Methods:The clinical data of 90 patients with lung cancer and 50 patients with benign lung diseases diagnosed by the pathological examination in Tangshan People's Hospital from December 2019 to January 2021 were retrospectively analyzed, and 40 healthy physical examiners in the same period were selected as the controls. The serum HE4 levels were detected by electrochemiluminescence method. The serum ESM-1 and EGFR levels were tested by enzyme-linked immunosorbent assay. The differences in serum HE4, ESM-1 and EGFR levels between the three groups were compared; logistic regression analysis was used to screen out the effective indicators for the diagnosis of lung cancer and to construct a prediction model for the diagnosis of lung cancer. Using pathological diagnosis result as the gold standard, the receiver operating characteristic (ROC) curve was drawn, and the diagnostic efficacy of indicators for lung cancer was evaluated.Results:The levels of serum HE4 in lung cancer group, benign lung diseases group and healthy control group were 119.55 pmol/L (82.06 pmol/L, 189.00 pmol/L), 58.84 pmol/L (45.62 pmol/L, 69.41 pmol/L) and 42.67 pmol/L (37.09 pmol/L, 51.84 pmol/L), the levels of ESM-1 were 33.00 ng/ml (25.85 ng/ml, 47.40 ng/ml), 20.14 ng/ml (11.93 ng/ml, 28.90 ng/ml) and 15.39 ng/ml (11.84 ng/ml, 20.19 ng/ml), and the levels of EGFR were 46.60 pg/ml (37.45 pg/ml, 58.98 pg/ml), 32.77 pg/ml (26.27 pg/ml, 40.86 pg/ml) and 30.43 pg/ml (27.54 pg/ml, 35.75 pg/ml), and the differences in each indicator among the three groups were statistically significant (all P < 0.001). The levels of serum HE4, ESM-1 and EGFR in lung cancer group were higher than those in benign lung diseases group and healthy control group. In patients with lung cancer, logistic regression analysis was performed with HE4 (X 1), ESM-1 (X 2) and EGFR (X 3) as the independent variables and pathological diagnosis as the dependent variable, and a lung cancer prediction regression model was established: P = 0.171X 1+0.351X 2+0.184X 3-24.660. The accuracy of this model in predicting lung cancer could reach 98.5%, and serum HE4, ESM-1 and EGFR were risk factors for the occurrence of lung cancer (all P < 0.05). The area under ROC curve from high to low was HE4 (0.960), ESM-1 (0.942) and EGFR (0.859). The diagnostic sensitivity of serum HE4 63.67 pmol/L for lung cancer was 86.7%, and the specificity was 97.5%. Both serum HE4 ( r = 0.304, P = 0.004) and ESM-1 ( r = 0.416, P < 0.001) were correlated with EGFR. Conclusions:Serum HE4, ESM-1 and EGFR can be used as effective indicators for the diagnosis of lung cancer, and the prediction model established based on the three serum tumor markers is of good value for the diagnosis and prediction of lung cancer.
9.Postoperative paravertebral muscle degeneration and its correlations with health related quality of life in patients undergoing minimally invasive surgery-transforaminal lumbar interbody fusion

Objective:To analyze the postoperative paravertebral muscle degeneration and its correlations with health related quality of life (HRQL) in patients undergoing minimally invasive surgery-transforaminal lumbar interbody fusion (MIS-TLIF).Methods:The clinical data of the 50 patients were retrospectively analyzed who had undergone single-segmental MIS-TLIF at Department of Spinal Cord Surgery, The People's Hospital of Henan Province from January 2019 to December 2021. The relative volumes of lumbar posterior muscle (LM), the relative volumes of the psoas major (PM), and the rates of fatty degeneration (FD) of the fused segment and its adjacent segments were compared respectively between preoperation, 6 and 12 months postoperation. The correlations were analyzed between the HRQL scores [visual analog scale (VAS) for pain and Oswestry disability index (ODI)] and the relative LM volumes, the relative PM volumes, and the FD rates of the fused segment and its adjacent segments at 12 months postoperation.Results:Compared with the preoperative values, the relative LM volumes and the relative PM volumes of the fused segment and its adjacent segments at 6 and 12 months postoperation were significantly reduced while the FD rates significantly increased. However, the FD rate of the fused segment at 12 months postoperation (20.6% ± 6.1%) was significantly lower than that at 6 months postoperation (29.7% ± 8.2%) ( P < 0.05). The VAS score was strongly negatively or positively correlated with the relative LM volume ( r = -0.819, P < 0.001) and the FD rate ( r = 0.86, P < 0.001) of the fused segment, and moderately negatively correlated with the relative PM volume ( r = -0.435, P = 0.016). The ODI index was moderately negatively correlated with the relative LM volume ( r = -0.512, P = 0.004) and the relative PM volume ( r = -0.402, P = 0.020) of the fused segment, but moderately positively correlated with the FD rate of the fused segment ( r = 0.565, P = 0.001). There was a moderate negative correlation between the ODI index and the relative LM volume of the adjacent segments ( r = -0.478, P = 0.012). Conclusions:After MIS-TLIF, the volume of the paravertebral muscles decreases and the dorsal muscles develop fatty degeneration. The improvement of LM fatty degeneration may be observed by 12-month follow-up in the fused segment, but not in the adjacent segments. The LM volume and the FD rate of the fused segment are the most closely related to the postoperative HRQL.
10.Application of in situ simulation and standardized patient in residency training of general practice

The effects and practicability of in situ simulation and standardized patient (SP) in residency training of general practice was evaluated in Hangzhou First People′s Hospital. From September 2017 to December 2019, typical cases were collected in accordance with the teaching objectives, the teaching plans of in situ simulation were established and simulation scenarios were set up; the SPs were recruited from nursing staff and retirees in the community. Forty general practice residents were randomly divided into two groups with 20 in each group. The traditional teaching group was given theoretical teaching, while the combined teaching group was taught by in-situ simulation combined with SP teaching. The satisfaction of residents, the scores of theory and skill were observed and compared between two groups.Results showed that the satisfaction of teaching method( t=-2.11, P<0.05), teaching effect( t=-4.25, P<0.01) and scores of history collection( t=-3.21, P<0.01) in the combination teaching group were significantly higher than those in the traditional teaching group. It is suggested that in situ simulation combined with SP teaching method can improve the satisfaction and teaching effect of the general practice residents.
