BACKGROUND:Sirolimus and Paclitaxel-eluting stents are commonly used for clinical application.Sirolimus-eluting stent have been proved safely and effectively to treat acute myocardial infarction.However,the comparison between those two eluting stents has been less reported yet.OBJECTIVE:To compare the security and long-term efficacy between Sirolimus- and Paclitaxel-eluting stents to the treatment of ST-segment acute myocardial infarction via percutaneous transluminal coronary intervention.METHODS:A total of 354 patients with ST-segment acute myocardial infarction,including 259 males and 95 females,were administrated with Sirolimus- and Paclitaxel-eluting stents.All cases were randomly divided into Sirolimus-elutin9 stent group (n=213) and Paclitaxel-eluting stent group (n=141).Major adverse cardiovascular events were compared between the two groups during 1-year following up.RESULTS AND CONCLUSION:One-year following up indicated that there was no significant difference in recurrent myocardial infarction (1.5% vs.1.5%) and cardiac death (2.5% vs.3.0%) between the two groups.Radiography showed that there was no significant difference in restenosis rate (5.0% vs.4.5%) between the two groups.Inner diameter lose was (0.19±0.34) mm in the Sirolimus-eluting stent group and (0.19±0.37) mm in the Paclitaxel-eluting stent group,and there was no significant difference.Additionally,there was also no significant difference in major adverse cardiovascular events between the two groups (8.9% vs.9.1%,P>0.05),suggesting that both Sirolimus-and Paclitaxel-eluting stents were safe and effective to the treatment of ST-segment acute myocardial infarction via percutaneous transluminal coronary intervention.

Congenital hydronephrosis is the main cause of childhood chronic renal disease and renal failure,which is also one of the most common anomalies in pediatric urology division.Although the operation is performed to treat obstruction,the renal function of patients get worse and worse,postoperatively.Up to date,there are lots of controversies in evaluation of hydronephrosis and treatment at prenatal and postnatal.According to recent articles,the recent examinations,evaluation system,follow-up and treatment strategies of hydronephrosis are introduced.

Teaching is very important for anesthesiology developmemt,so residents should be cultivated the teaching capacity as soon as possible.This article discussed how to cultivate the teaching capacity of residents in anesthesiology department.

Objective To investigate the clinical characteristics,treatment options and prognosis of duplex kidneys in children.Methods Data were retrospectively collected from all patients who were hospitalized at Shengjing Hospital Affiliated to China Medical University from January 2008 to May 201 5.The surgery to retain moie-ty nephron was performed in 30 cases:1 2 cases underwent non common sheath ureter reimplantation,4 cases of the patients had common sheath ureter reimplantation,1 2 patients underwent transurethral resection of the cyst,1 case of them had pyeloureterostomy,and the other patients underwent lower -pole pyeloplasty.Removal of renal moiety was performed in 52 cases:43 patients underwent upper -pole heminephrectomy,8 patients underwent upper pole hemi-nephrectomy and lower -pole ureter reimplantation,and 1 case of the patients had upper -pole heminephrectomy and lower -pole ureteroplasty.Results A total of 54 cases were followed up,the follow -up rate reached 65.9%,the follow -up time lasting 2 months to 4 years,with an average time of 9.53 months.All clinical symptoms disappeared after remove moiety operation.There were small ureteral cysts but compared with the preoperative condition,the symp-toms reduced significantly in 4 cases.All clinical symptoms disappeared in patients whose nephron was retained after operation,and hydronephrosis was alleviated.De novo 2 degree of vesicoureteral reflux was found in 3 out of 1 2 pa-tients who underwent transurethral puncture of the cyst.Conclusions The clinical manifestations of duplex kidney have some characteristics which include recurrent urinary tract infection,normal intermittent voiding urine dripping, abdominal pain,urethral orifice mass,abdominal mass and aggravation of hydronephrosis.Duplex kidney is often asso-ciated with cardiac malformation and cryptorchidism.The preoperative testicular examination and echocardiography should be performed.The choice of operation depends on the clinical manifestation,auxiliary examination results and comprehensive analysis.The prognosis of duplex kidney is well,but patients need to be further followed up af-ter operation.

Objective To investigate whether the interleukin-10 (IL-10) gene therapy has the effect of anti liver fibrosis in mice and its mechanism.Methods Liver fibrosis was induced by long-term thioacetamide administration in mice.Human IL-10 expression plasmid was delivered via electroporation after liver fibrosis was established.The immunohistochemistry was used to study the expression of IL-10 in liver.Sircol collagen determination method was used to detect the contents of collagen in the liver.The reverse transcription polymerase chain reaction (RT-PCR) was used to detect the expressions of liver fibrosis-related genes,including transforming growth factor-β1 (TGF-β1),collagen α1,tumor necrosis factor-α (TNF-α),intercellular adhesion molecule-1 (ICAM-1),fibronectin,vascular cell adhesion molecule 1 (VCAM-1),and matrix metalloproteinase-inhibiting factor (TIMP-1,TIMP-2).Results Immunohistochemical results showed IL-10 gene therapy reversed hepatic fibrosis.Sircol collagen assay showed that IL-10 gene therapy reduced the content of collagen fibers(P ＜ 0.05).RT-PCR revealed IL-10 gene therapy reduced liver TGF-β1,TNF-α,collagen α1,cell adhesion molecule,and TIMPs mRNA upregulation.Conclusions Electroporative IL-10 gene therapy might be an effective therapeutic reagent for liver fibrosis with potential future clinical applications.

Objective To identify the cellular distribution of a my elin-associated protein (Nogo-A) in the central nervous system (CNS) of mice a nd explore its possible inhibition on the CNS axon regeneration after spinal cor d injury. Methods Brain, spinal cord, peripheral tissues and w eight-dropping injuried spinal cord from the adult C57BL/6 mice were studied. N ogo-A protein expression was localized immunohistochemically. Chick E12DRG neur ons were cultured and growth cone collapse assessed. Results N ogo-A protein expression detected was mainly in the oligodendrocyte cell body a nd the myelinated axons surrounded by cell processes rather than in the peripher al tissues. After spinal cord injury, Nogo-A was up-regulated at a moderate de gree in the area around the lesion. Objective:To explore C-terminal tensin-like(CTEN) gene in breast cancer tissue and its significance. Methods:Explore the prevalence and clinical significance of CTEN expression in invasive breast cancer cases ( n = 1409 ) using immunohistochemistry and tissue microarray. Results: The staining pattern for CTEN in breast tumour cells was homogenous cytoplasmic staining. Moderate to strong cytoplasmic immunoreactivity for CTEN was observed in 90% of the studied cases(1268 cases) . CTEN expression was significantly associated with poor prognostic variables including larger tumour size(χ2=4. 254,P=0. 044), higher histological grade(χ2=7. 555,P=0. 019),axillary nodal involvement(χ2=5. 842,P=0. 035),poor Nottingham Prognostic Index (χ2=7. 578,P=0. 016),Local recurrence(χ2=5. 211,P=0. 039),Regional recurrence(χ2=4. 778,P=0. 042),Distant metastases (χ2=4. 780,P=0. 041) and Histological type(χ2=7. 634,P=0. 012). Kaplan-Meier survival analysis demonstrated that patients with high CTEN ex-pression had significantly shorter Breast Cancer Specific Survival(P=0. 004) and Metastasis-Free Survival(P=0. 041) than those with low-CTEN expression. Multivariate analysis showed that CTEN was not an independent prognostic marker in breast cancer. Conclusion:CTEN expression increase is a cancer gene,its association with poor prognostic parameters,and the expression of CTEN associated with poor prognosis of breast cancer parameters.

Objective To evaluate the optimal degree of neuromuscular blockade for inhibiting obturator nerve-muscle responses induced by transurethral resection of bladder tumor (TURBT) with general anesthesia.Methods Ninety American Society of Anesthesiologists physical status Ⅰ-Ⅲ patients of both sexes,aged 26-64 yr,weighing 50-80 kg,scheduled for elective TURBT for lateral bladder wall tumors with general anesthesia,were divided into 3 groups (n =30 each) using a random number table:low-dose mivacurium group (group L),medium-dose mivacurium group (group M) and high-dose mivacurium group (group H).After mivacurium 0.15 mg/kg was injected intravenously during anesthesia induction,mivacurium was continuously infused at a rate of 0.2,0.3 and 0.4 mg · kg-1 · h-1 in L,M and H groups,respectively,until the end of operation.Neuromuscular blockade was continuously monitored during operation.When T1％ and TOF count (TOFC) disappeared,post tetanic count (PTC) was used.ROC curve was applied to analyze the relationship between the occurrence of obturator nerve-muscle responses and degree of neuromuscular blockade.Results T1％ and TOFC were recorded in 16 patients (15 cases in group L,1 case in group M) during the resection of tumor,and the obturator nerve-muscle response was observed in all of these patients.In the other 74 patients,T1％ and TOFC disappeared,and PTC recorded was 10.0±3.1 (group L,n=15),6.0± 3.5 (group M,n=29) and 4.0±2.2 (group H,n=30).Among the 74 patients,the obturator nerve-muscle response was found in 18 patients (8 cases in group L,10 cases in group M).The area under the ROC curve of PTC value in predicting the occurrence of obturator nerve-muscle responses was 0.882 with a PTC cut-off of 9 (P＜0.05).The sensitivity and specificity were 87.3％ and 72.2％,respectively.Conclusion In order to inhibit the obturator nerve-muscle response during TURBT with general anesthesia,the optimal degree of neuromuscular blockade should be kept not more than 9 for PTC.

Objective To investigate the effects of silencing of the human cerebral 21 .5 kDa myelin basic protein (MBP) gene on the proliferation and apoptosis of the glioma U251 cells .Methods The 21 .5 kDa MBP sequence‐specific short hair‐pin RNA (shR‐NA) recombinant plasmids pGenesil‐1‐MBP‐3 were transfected into the human glioma cell line(U251) ,the cells of U251 was used as MBP silencing group ,the cells transfected with negative control plasmids used as negative control group ,and the cells transfected with liposomes used as blank control group .Real‐time PCR and Westernblot were used to detect the expression levels of the 21 .5 kDa MBP mRNA and protein in each group ,and the cell proliferation curve was measured by CCK‐8 assay ,the apoptosis rate was a‐nalysised by Flow cytometry .Results Both the mRNA and the protein expression levels of the 21 .5 kDa MBP of MBP silencing group were significantly lower than those in the control groups (P<0 .05);the cellular proliferation activity of the MBP silencing group decreased significantly (P<0 .05)while the cellular apoptotic rate increased significantly (P<0 .05) .Conclusion Silencing of the human cerebral 21 .5 kDa MBP gene may playa dual role in the inhibition of proliferation and the promotion of apoptosis of the glioma U251 cells .
