Objective To study the cause of different prognosis in typical medullary carcinoma and atypical medullary carcinoma. Methods The immunohistochemical staining method was used to evaluate the positivity of E cadherin,? catenin, in 30 cases of atypical medullary carcinoma and 18 cases of typical medullary carcinoma and 10 cases of normal breast. 2.Effects of intravenous anesthetics on endotoxin-induced inflammatory responses in rabbits undergoing mechnical ventilation

Objective: To determine the effects of intravenous anesthetics (fentanyl,midazolam and propofol )on inflammatory responses. Method: We employed carrageenan-sensitized endotoxemic rabbit model. Thirty-two New Zealand rabbits were randomly assigned to one of four groups,control group(no anesthetic was used), fentanyl group, midazolam group and propofol group. Different anesthetics were used intravenously for anesthesia induction and maintenance according to the group classification. Meanwhile, all animals were mechanically ventilated, then, 2?g?kg~(-1) lipopolysaccharide(LPS) were injected for endotoxin challenge. Result:In all groups, serum tumor necrosis factor (TNF)activity began to increase at 30 min,reached its maximal level at 60 min,and tended to decrease at 120 min after (LPS) challenge,and the significant increase of TNF activity was associated with development of hypotension. Meanwhile,serum phospholipase A_2(PLA_2)activity began to increase at 60 min and persisted in going up within 2h. Those animals,with continuous infusion of these three different anesthetics and mechanical ventilation,had much lower peak level of TNF and higher mean arterial pressure (MAP)than control levels, but their changes of serum PLA_2 activity had no significant difference from control level. Conclusion: With mechanical ventilation, intravenous infusion of these anesthetics can reduce LPS induced injury in the acute phase of endotoxemia, probably by the inhibition of TNF production.
3.A preliminary study on the proliferation of endothelial cells of human hemangioma

Objective: To study the role of simian virus 40 large T antigen(SV40LTag) and telomeras in the proliferation of endothelial cells in hemangioma. Methods: The expression of SV40LTag was examined in 100 cases of hemangioma with immunohistochemistry, telomerase activity and telomeric length were measured by RT-PCR based telomeric repeat amplification protocol(TRAP) assay, telomeric restriction fragment (TRF) analysis and chemiluminescence in situ hybridization in 50 cases of hemangioma and 10 of squamous cell carcinoma. A sample of embryonic kidney endothelia was used as the contrdol. Results: SV40LTag expression was observed in 45 out off the 100 cases of hemangioma. Telomerase expression was found in 27 out off the 50 cases of hemangioma and all the 10 of squamous cell carcinoma. The telomeric length in hemangioma varied from 6.8 to 13.2 kb, 10.5-13.5 kb in squamous cell carcinoma and 12.4 kb in embryonic kidney endothelia. Conclusion: Transformation of endothelia by simian virus 40 may contribute to the occurring of hemangioma, telomerase may play a role in the proliferation of the endothelial cells in hemangioma.
4.Effect of Stigma Maydis Aqueous Extract on Glucolipid Metabolism and Oxidative Stress in Type 2 Diabetic Rats

Objective To investigate the effect of stigma maydis aqueous extract on glucolipid metabolism and oxidative stress in type 2 diabetic rats.Methods Male Sprague-Dawley rats were randomly divided into normal control group,model control group,positive control group(Metformin) and stigma maydis aqueous extract low,middle,high dose group.Type 2 diabetic rats model was induced by high-fat and high-sugar dietary feeds and streptozotocin(STZ) in all groups except the normal control group.After treatment of 8 weeks,automatic biochemistry instrument was applied to detecting the levels of fast blood glucose(FBG),triglyceride(TG),total cholesterol(TC) and low density lipoprotein cholesterol(LDL-C) in serum.The levels of free fat acid(FFA),malonaldehyde(MDA) and superoxide dismutase(SOD) in serum were assayed by uv-vis spectrophotometer.Results Compared with model control group,after stigma maydis aqueous extract treatment,middle and high dose groups remarkably lowered the levels of FBG,FFA,TG and TC in dose-dependent manner(P
5.Chronic invasive fungal rhinosinusitis complicated with infratemporal fossa fungal infection: 2 cases report.

Two cases of chronic invasive fungal rhinosinusitis were reported. One patient healed while another died with suspicious residual fungal infection. The clinical symptoms of infratemporal fungal infection include maxillofacial pain, with or without fever. Acute or chronic inflammation of soft tissue could be revealed pathologically. Proof of pathogen may not be found in histopathological slice while serological detection may reveal positive evidence. Surgical resection, good drainage and postoperative antifungal therapy could cure or promptly relieve the symptoms of the disease.

6.Intention survey of Shanghai medical workers with regard to reward for personal services

Objective To find out about the intention of medical workers with regard to reward for personal services and explore the value of their labor. Methods An intention survey through questionnaires was conducted by means of random sampling among 1 424 medical workers from 6 hospitals in Shanghai with regard to reward for personal services and a model of expected income was obtained via multivariate stepwise regression analysis. Results Only 2.9% of the medical workers felt satisfied with their current total income. Such factors as professional titles, educational backgrounds, working years, and specialties all exerted some impact on the medical workers expected income. The expected income was several times that of the actual income, and there was a large difference between the expected incomes of people at different levels. Conclusion Reward to medical workers for personal services in China needs to be enhanced and reform of the distribution system should keep step with the reform of the personnel system.

Objective: To analyze the effects of treatment of squamous cell carcinoma of tongue with N 0. Methods: 145 cases of squamous cell carcinoma of tongue with N 0 were retrospectively reviewed.22 cases underwent resection of primary tumor without elective neck dissection (END) (control group).123 cases underwent resection of primary tumor with END (END group).Follow-up was carried out for more than 3 years or till the death of patients.Results:Lymph node metastases was indentified in 33 patients in END group after operation and in 4 in control gruop during follow-up.The total occult lymph node metastases was found in 25.52% of the patients.4 (18.18%) died from neck or lung metastasis in control group and 7 (5.69%) in END group (P
8.Performance evaluation and results analysis of chemiluminescence method for detecting plasma BNP

Objective To analyze and evaluate the precision ,accuracy and linearity of the chemiluminescence method for detec‐ting plasma BNP .Methods According to the experimental schemes of CLSI EP15‐A2 ,EP6‐A files and other relevant documents , the precision ,accuracy and linearity of the Siemens Centaur XP chemiluminescence instrument for detecting BNP ,and the detection results were compared with the performance declared by manufacturer or the quality target formulated by laboratory .Results The imprecision of BNP detected by the chemiluminescence method was less than 1/3 TEa regulated by the Clinical Laboratory Center of Ministry of Health (allowable total error);the variation coefficient index (CVR) of internal quality control data was less than ± 2;the relative bias of the results of external quality control blind samples with the target values were less than Tea regulated by the Ministry of Health ;the linear evaluation results showed that BNP was once linearity in the range of 5 .3‐4696 .7 pg/mL .Conclusion The precision ,accuracy and linearity of the Siemens Centaur XP chemiluminescence instrument for detecting BNP can accord withthe quality objectives requirements and meet the clinical needs .
9.Practice of curriculum integration teaching reform for Traditional Chinese Orthopedics

To improve the quality of teaching, teaching reform and exploration on integration of Ttradition Chinese Orthopedics curriculum has been carried out by orthopedics teaching and research section in our school. Putting Traditional Chinese Orthopedics as the framework of curriculum knowledge, the im-portant and difficult points of Human Anatomy and Clinical Medical Science were combined, which builds the overall teaching mode of normal-disease-cases and get some results. The evaluation of teaching effect shows thatthe integration of Traditional Chinese Orthopedicscourse will enable students to lay a solid the-oretical foundation and improve their ability.
10.Clinical study on human kallikrein gene 5 messenger RNA expression in breast cancer

Objective The purpose of the current study was to develop a real-time quantitative reverse transcription-PCR method for detection of human kallikrein gene 5(KLK5) mRNA expression,quantify the expression of KLK5 mRNA in malignant and benign breast tissues,and statistically analyze whether KLK5 expression levels correlate with clinicopathologic variables in the patients with breast cancer.Methods Paired breast tissue samples from cancerous and corresponding noncancerous tissues were obtained from 48 patients with breast cancer who underwent surgical resection.The relationship of KLK5 mRNA expression status with various clinicopathologic variables were quantitativly analyzed by real-time RT-PCR.Results KLK5 mRNA expression in cancerous tissues was decreased in 38 of 48(79%) breast cancer patients compared with normal counterparts.The mean expression level of KLK5 mRNA in cancerous tissues was significantly lower than that in noncancerous tissues(P0.05).Conclusion The results of this study indicated that KLK5 mRNA expression was significantly lower in cancerous tissues than that in noncancerous breast tissues,and low expression of KLK5 mRNA correlated with TMN stage,metastasis and ER status.
