1.Research on the Evaluation Model Concerning Teaching Quality of Courses for Medical Postgraduates

Based on the relevant theories and methods concerning the evaluation of higher education, authors used student questionnaires to evaluate the teaching quality of the 23 courses in the postgraduate curriculum, including basic theoretic courses, specialty-related basic courses and courses of methodology for scientific research. The methods used involve 'evaluation by students' and the integrated evaluation of both the quantitative and qualitative aspects of various factors. The easy-to-use student's questionnaires make it possible to find, with respect to students, teachers, and courses, many factors affecting the teaching quality, and at the same time, to provide the close following-up of the process of postgraduate training.
2.Clinical analysis of 64 cases of closed abdominal trauma

Objective To investigate the clinical diagnosis and treatment of blunt abdominal trauma(BAT).Methods The clinical data of 64 patients with BAT were retrospectively analyzed.Results 49 patients treated by surgery were cured and 12 cases treated by non-surgery were cured.3 patients died,two of them died for multi-organ dysfunction and one of them died for brain trauma.Conclusion Because the BAT is severe and complicate,so it shoald be diagnosed and treated as soon so possible.
3.The Practices and Effects of Objective Management in Hospital Meticulous Management

Objective: Through managing objectives in our hospital, promote the hospital management. Methods:Use expert advice method to establish the target assessment index system, with the weighted index method to calculate the final score. Results: According to the characteristics of the department, develop differentiated, personalized indicators; objective management system consists of 3 level indicators medical examination standard (680 points), scientific research and teaching standards (220 points), departmental management assessment criteria(100 points), and 45 secondary indicator of internal medicine, surgical 49 second-level indicators. Conclusion: Objective management is the starting point of the hospital and meticulous management, and implementation of objective management enhance the awareness of the collective participation of the medical staffs at all levels, so that the department changes from passive management to active management, and it enhances department execution and promotes the hospital science directly.
4.Clinical research of Pulmonary surfactant on neonatal with acute respiratory distress syndrome

Objective To investigate the effect of pulmonary surfactant on neonatal with acute respiratory distress syndrome.Methods98 cases with neonatal respiratory distress syndrome inthe fourth hospital of Ningbo were randomly divided into experimental group and control group, 49 cases in each group.All patients were given warm, anti infection, and maintain the internal environment stability, prevention of bleeding and other conventional treatment.The control group were treated with mechanical CPAP, the experimental group were given pulmonary surfactant 70mg/kg, concentration is 35mg/mL.Pulmonary surfactant was injected slowly by tracheal intubation at supine, lateral(left, right) and semi recumbent position.The drug was distributed evenly in the lung of the patients who were given the drug 1-2 times.Respiratory frequency (RR), pH, oxygen partial pressure (PaO2), partial pressure of carbon dioxide (PaCO2) levels and the incidence of complications, clinical effective rate of the tthe two groups were observed and compared.ResultsCompared with pre-treatment, RR and PaCO2 levels were decreased, pH and PaO2 levels were increased after treatment in the two groups, the differences were statistically significant (P<0.05);compared with the control group, RR, PaCO2 level were lower, pH and PaO2 levels were higher in the experimental group, the differences has statistical significance (P<0.05);compared with the control group, the experimental groupwith a low incidence of complications, clinical effective rate is higher, the difference was statistically significant (P<0.05).ConclusionPulmonary surfactant can reduce the respiratory frequency in neonatal with acute respiratory distress syndrome, improve arterial blood gas levels, which get better clinical curative effect.
5.Study of infusion methods of postoperative enteral nutrition in patients with gastric cancer

Objective:To investigate the infusion methods of postoperative enteral nutrition in patients with gastric cancer.Methods:86 patients with gastric cancer received radical operationin our hospital from January 2014 and December 2015 were enrolled for the study and randomly divided into continuous infusion group and periodic infusion group,each group with 43 cases.Patients in continuous infusion group were given postoperative continuous enteral nutrition for 24 h in one day,while cases in periodic infusion group were given postoperative continuous enteral nutrition for 15-17 h in one day.Results of enteral nutrition,physical indicators including upper arm skinfold,diameter of midpoint of upper arm and grip strength,nutritional indexes including total protein (TP),hemoglobin (Hb),albumin (AIb),prealbumin (PA) and transferrin(TF),immune indicators including immunoglobulin G(IgG),immunoglobulin M (IgM),immunoglobulin A (IgA),CD4+,CD8+ and CD4+/CD8+were compared between two groups before and after operation.Results:Intolerance of enteral nutrition occurred in 1 case(2.3%) of continuous infusion group and 3 cases(7.0%) of periodic infusiongroup,and there was no significant difference between them (P>0.05).Time of transition to full enteral nutrition and taking off the tube to self-feeding incontinuous infusion group were significantly smaller than that in periodic infusion group (P<0.05).There were no significant difference in baseline data,physical indicators,nutritional indexes and immune indicators between them before operation (P>0.05).PA level at 5 d,10 d after operation in continuous infusion group were significantly higher than that in periodic infusion group (P<0.05),and there were no significant difference in physical indicators,other nutritional indexes and immune indicators between them after operation(P>0.05).Conclusion:Compared with periodic infusion,continuous infusion method could shorten the time of transition to full enteral nutrition and taking off the tube to self-feeding in patients after radical operation of gastric cancer,and had more significant effect of improvement of nutritional status,worthy of clinical popularization and application.
6.Clinical application of CT guided percutaneous for lung with 60 cases analysis

Purpose:By ascessing the CT guided percutaneaus biopsy to improve clinical diag- nostic rate and reduce complications.Materials and methods:60 cases of biopsy with complete materials have been reviewed,and the methods of“retain anesthetic tracing needle”has been proposed-to obtain tissues for cytologic and histologic exams with different biopsy needles.Results:Successful rate of the puncfure 100%.39 cases were identified as lung cancer,8 cases as tuberculosis,7 cases as pneumonia. Successful biopsy was 90%.Conclusion:“Retain anesthetic tracing needle”could reduce the times of adjusting direction,improve the one-time successful rate and possess the characteristics of positioning safe,reliable and accurate diagnosis.
7.Transpedicular Fixation Combined with Mixed Transpedicular Structural and Cancellous Bone Graft for Severe Compressed or Burst Thoracolumbar Spinal Fractures

Objective To explore the efficacy of transpedicular fixation combined with mixed transpedicular structural and cancellous bone granule graft for the treatment of severe compressed or burst thoracolumbar spinal fractures.Methods From January 2001 to January 2008,18 cases of severe compressed or burst thoracolumbar spinal fractures was treated with transpedicular fixation of spondylolisthesis and fracture(SF) or atlas fixation(AF) combined with mixed transpedicular structural and cancellous bone granule graft in the compressed vertebral body.Results In this group,no injuries to the spinal cord or nerves occurred.The mean blood loss was 50-300 ml(average 80 ml).The mean operation time was 100-180 min(mean 120 min),and the mean postoperative hospital stay was 12 to 30 days(14 days on average).Follow-up was available in 18 cases for a mean of 24 months(5-72 months),all the patients achieved satisfying recovery of the height and physiological curve of the vertebral body: 17 of the cases had normal height and physiological curve;in the other patient,the natural curve was partly destroyed;no patient had loosing and disrupted internal fixation.According to preoperative Frankel evaluation,1 case was classified as Frankel A,1 Frankel B;5 Frankel C,3 Frankel D,and 8 Frankel E before the operation;while postoperative classification showed 1 case of Frankel B;1 cases of Frankel C,and 16 cases of Frankel E.Conclusions Transpedicular fixation combined with mixed transpedicular structural and cancellous bone granule graft is safe and effective for the treatment of severe compressed or burst thoracolumbar spinal fractures.
8.Value of Doppler echocardiography in evaluation of left ventricular myocardial remodel and dysfunction in neonates with hypoxia-induced pulmonary artery hypertension

Objective To investigate the application of the Doppler echocardiography on the clinical value on the left ventricular myocardial remodel and dysfunction in neonates with hypoxia-induced pulmonary artery hypertension. Methods The Doppler echocardiography (ECHO) datas of 24 neonates of pulmonary diseases with secondary pulmonary artery hypertension were identified by color Doppler echocardiography,and compared with 14 neonates with pulmonary diseases without pulmonary artery hypertension. Results Pulmonary artery systolic pressrure (PASP) (38.23?3.26) mm Hg increased ,the diameter of right ventricular(11.92?2.18)mm vs (7.34?0.93) mm and pulmonary artery(10.20?1.21)mm vs (6.89?0.22) mm and diameter of left ventricular end-diastolic(19.74?0.71)mm vs (14.42?0.32) mm in PAH groups increased significantly ( P 0.05). 9.Preparation and Quality Control of Weishu Emulsion

OBJECTIVE:To prepare weishu emulsion and to establish its quality control method.METHODS:Weishu emulsion was prepared with sodium carboxymethylcellulose,etc.as the adjuvants and the content of dyclonine hydrochoride was determined by UV-spectrophotometry.RESULTS:The linear range of dyclonine hydrochloride was2.0~12.0?g/ml(r=0.9999)and the average recovery rate was99.38%,RSD=0.47%,(n=6).CONCLUSION:The emulsion is feasible in preparation method and controllable in quality.
