1.Distribution and antibiotic resistance of pathogen isolated from intra-abdominal infection associated sepsis in surgical intensive care unit

Objective To investigate the distribution and antibiotic resistance of clinical pathogen isolated from patients of intra-abdominal infection associated sepsis in surgical intensive care unit (SICU ) during recent 5 years ,then instruct clinical application of antibiotics reasonably .Methods Abdominal drainage of 65 patients of intra-abdominal infection associated sepsis were collected from January 2008 to December 2012 .All the specimens were analyzed for pathogen distribution and drug susceptibility .Results A total of 151 strains of pathogenic bacteria was isolated ,in which Gram-negative bacteria were the most popular pathogen (104 strains ,68 .9% ) ,whereas Gram-positive bacteria and fungi accounted for 19 .2% (29 strains) and 11 .9% (18 strains) ,respectively . The five most common pathogens were Escherichia coli(23 .2% ) ,acinetobacter baumannii(15 .2% ) ,enterococcus faecium(13 .2% ) , pseudomonas aeruginosa(8 .6% ) ,andklebsiella pneumonia(7 .9% ) .The rate of the ESBL-producing strains of escherichia coli and pneumonia pneumonia were 80 .0% and 33 .3% .Both of them showed low resistance to carbapenem antibiotics .The proportion of multidrug resistantstrains and extensive drug resistant strains of acinetobacter baumannii was 56 .5% and 30 .4% ,respectively .Con-clusion The resistance of the pathogen isolated from the patients of intra-abdominal infection associated sepsis in SICU is seriously high .Early and rational using of antibiotics is of great importance to control the production and transmission of drug resistance .
2.Clinical manifestations and treatment of duplex kidneys in 82 children

Objective To investigate the clinical characteristics,treatment options and prognosis of duplex kidneys in children.Methods Data were retrospectively collected from all patients who were hospitalized at Shengjing Hospital Affiliated to China Medical University from January 2008 to May 201 5.The surgery to retain moie-ty nephron was performed in 30 cases:1 2 cases underwent non common sheath ureter reimplantation,4 cases of the patients had common sheath ureter reimplantation,1 2 patients underwent transurethral resection of the cyst,1 case of them had pyeloureterostomy,and the other patients underwent lower -pole pyeloplasty.Removal of renal moiety was performed in 52 cases:43 patients underwent upper -pole heminephrectomy,8 patients underwent upper pole hemi-nephrectomy and lower -pole ureter reimplantation,and 1 case of the patients had upper -pole heminephrectomy and lower -pole ureteroplasty.Results A total of 54 cases were followed up,the follow -up rate reached 65.9%,the follow -up time lasting 2 months to 4 years,with an average time of 9.53 months.All clinical symptoms disappeared after remove moiety operation.There were small ureteral cysts but compared with the preoperative condition,the symp-toms reduced significantly in 4 cases.All clinical symptoms disappeared in patients whose nephron was retained after operation,and hydronephrosis was alleviated.De novo 2 degree of vesicoureteral reflux was found in 3 out of 1 2 pa-tients who underwent transurethral puncture of the cyst.Conclusions The clinical manifestations of duplex kidney have some characteristics which include recurrent urinary tract infection,normal intermittent voiding urine dripping, abdominal pain,urethral orifice mass,abdominal mass and aggravation of hydronephrosis.Duplex kidney is often asso-ciated with cardiac malformation and cryptorchidism.The preoperative testicular examination and echocardiography should be performed.The choice of operation depends on the clinical manifestation,auxiliary examination results and comprehensive analysis.The prognosis of duplex kidney is well,but patients need to be further followed up af-ter operation.
3.Approach to parenteral nutrition in premature infants

After birth,the premature infants usually need a proper way for intestinal nutrition.The composition and configuration of nutrition admixture must meet the special requirements of the premature infants.In the first few days,because of invisible water lose,they should maintain a stable internal environment,and 1 week later,they need to gradually achieve a stable growth rate.Parenteral nutrition may lead to various complications,such as infection,metabolic complications,etc.monitoring the index,then adjusting the dosage,and achieving full enteral nutrition as soon as possible,may be effective prevention measures.
4.Evaluation and treatment of hydronephrosis in fetus and neonate

Congenital hydronephrosis is the main cause of childhood chronic renal disease and renal failure,which is also one of the most common anomalies in pediatric urology division.Although the operation is performed to treat obstruction,the renal function of patients get worse and worse,postoperatively.Up to date,there are lots of controversies in evaluation of hydronephrosis and treatment at prenatal and postnatal.According to recent articles,the recent examinations,evaluation system,follow-up and treatment strategies of hydronephrosis are introduced.
5.Impact of Conditioned Medium of Decidual Stromal Cell Cluture on Proliferation,Apoptosis and Invasion of Trophoblast Cells

Objective:To investigate the impact of conditioned medium of decidual stromal cell culture(DSCM) on proliferation,apoptosis and invasion of trophoblast cells.Methods:lsolution,culturing and indentifying the purity of decidual stromal cells from healthy women of early pregnancy to prepare different concentrations of DSCM (5%,20%,50% and 100%);Proliferation was evaluated by MTT assay;Cell apoptosis was examined by flow cytometry using annexin V-FITC/PI staining;Cell invasion was assayed using a Transwell chamber;RT-PCR and gelatin zymography were used to explore the mechanism of cell invasion.Results:Using this method we obtained a purity over 90% of decidual stromal cells.With the concentration of DSCM increased(5%,20%,50%,100%),the OD value of the MTT assay has elevated and the rate of apoptosis declined,the infiltrating cell increased as well (P < 0.05);Compared with control group,DSCM treatment led to a significant increase in the mRNA and protein expression of MMP-2/9(P <0.05).Conclusions:Decidual microenviroment may promote trophoblast proliferation and inhibite its apoptosis besides could enhance its ability of invasion by altering the expression of MMP-2 and MMP-9.
6.Expression of NF-κB and Vimentin in bladder urothelium carcinoma and their significance

Objective To discuss the expressions of NF-κB and Vimentin in bladder urothelium carcinoma and their significance. Methods 60 bladder urothelium carcinomas and 20 controls were evaluated and the presence of NF-κB and Vimentin were examined by inmunohistochemical SP method.Results Both NF-κB and Vimentin in bladder urothelium carcinoma and nomal bladder tissue had significant differences (x2 =21.8,P < 0.01; x2 =10.45,P < 0.01).The positive rates of NF-κB expression in different grade of bladder urothelium carcinoma:grade Ⅰ 32 % (5/16),grade Ⅱ 68 % (13/19),grade Ⅲ 96 %(24/25) (x2 =4.8,P < 0.05,x2 =4.24,P < 0.05).The positive rates of NF-κB expression in different stage:Ta-T1 stage 57 % (16/28),T2-T4 stage 81% (26/32) (x2 =4.13,P < 0.05).The positive rates of NF-κB expression in lymph node metastasis or not:87 % (21/24) and 58 % (21/36) (x2 =5.83,P < 0.05).The positive rates of Vimentin expression in different grade of bladder urothelium carcinoma:grade Ⅰ 19 %(3/13),grade Ⅱ 53 % (10/19),grade Ⅲ 84 % (21/25) (x2 =4.25,P < 0.05,x2 =4.94,P < 0.05).The positive rates of Vimentin expression in different stage:Ta-T1 stage 36 % (10/28),T2-T4 stage 75 %(24/32)(x2 =9.38,P < 0.05).The positive rates of NF-κB expression in lymph node metastasis or not:50 % (12/24)and 61% (22/36) (x2 =0.72,P > 0.05).The expression of NF-κB increased was positively correlated with expression of Vimentin (x2 =12.4,P < 0.005,r =0.42).Conclusion Both NF-κB and Vimentin in bladder urothelium carcinoma and normal bladder tissue have significant difference.The expression of NF-κB is positively correlated with tumor grade,clinical stage and lymph node metastasis,but not with age and sex.The expression of Vimentin is positively correlated with tumor grade,and clinical stage,but not with age,sex and lymph node metastasis.The expression of NF-κB is positively correlated with the expression of Vimentin.
7.Effect of simvastatin on IL-6 and adiponectin secretion and mRNA expression in 3T3-L1 adipocytes.

In order to investigate the effects of simvastatin on secretion and mRNA expression of interleukin-6 (IL-6) and adiponectin in 3T3-L1 adipocytes, mouse 3T3-L1 adipocytes were stimulated with lipopolysaccharide (LPS). Production and mRNA expression of IL-6 and adiponectin in 3T3-L1 adipocytes were measured using enzyme-linked immunosorbent assay (ELISA) and reverse transcriptase polymerase chain reaction (RT-PCR), respectively. The results showed that simvastatin could significantly suppress LPS-induced IL-6 production and mRNA expression in adipocytes (P<0.05), but increase the LPS-induced adiponectin secretion and mRNA expression in a dose-dependent manner (P<0.05). It was suggested that simvastatin could exert beneficial effects on prevention of obesity-induced metabolic changes in adipocytes.
8.Response of EDAG knockout mice to low-dose radiation-induced damage

Objective To generate the erythroid differentiation associated gene(EDAG) knockout mice and analyze their sensitivity to low dose radiation-induced damage.Methods Zinc finger nuclease technology ( ZFNs ) was used to produce the EDAG knockout mice.The low dose radiation-induced damage was evaluated by peripheral blood cell counts, DNA damage and colony formation of bone marrow cells.Wild-type and EDAG knockout mice were irradiated with 0.31 Gy/min X-ray, one minute per day for seven consecutive days, and the cumulative radiation dose was 2.17 Gy(n=7).The blood cell counts were measured by an automated hemocytometer.DNA damage was detected by immunofluorescence assay with a DNA damage marker p-H2A.x antibody (n=3).The colony formation ability of bone marrow cells was evaluated with a semi-solid culture medium(n=3).Results A model of EDAG knockout mice was established.Compared to wide type mice, white blood cell counts of EDAG knockout mice decreased significantly while the DNA damage marker p-H2A.x expression was increased on the third day after X-ray irradiation.The ability of colony-forming was reduced after 7 days of X-ray irradiation.Conclusion Our present study found that EDAG knockout mice are more sensitive to low dose radiation-induced damage as shown by decreased peripheral blood cells counts, reduced colony-forming ability of bone marrow cells, and increased DNA damage.These results suggest that EDAG knockout mice can serve as a powerful tool for evaluation of the biological effects of low-dose radiation damage.
9.Intranasal Immunization with Mucosal Complex Vaccine Protects Mice Against Toxoplasma gondii

Objective To study the mucosal and systemic immune response after intranasal immunization with mucosal complex vaccine for Toxoplasma gondii,and to observe the protective effect on mice. Methods The mucosal complex vaccine was made of soluble tachyzoite antigen (STAg) and cholera toxin (CT),which were mixed and dissolved in PBS (1 ml PBS containing 1 mg STAg and 50 ?g CT). Fifty-two BALB/c mice were randomly divided into two groups: immunized group and control. Mice were intranasally immunized with 20 ?l mucosal complex vaccine (20 ?g STAg and 1?g CT) per mouse twice at an interval of two weeks,while the control mice were given PBS solution instead. Six mice of each group were killed by dislocation of cervical vertebra on day 14 after the last immunization. The specific IgG antibodies in serum and IgA in feces were detected by ELISA. Lymphocytes in spleen,Peyer's patches (PP) and intestinal intraepithelial lymphocyte(IEL) were isolated and counted. Percentage of CD4+ and CD8+ T cells was determined by immunocytochemistry. Other mice were challenged intragastrically each with 4?104 tachyzoites of RH strain Toxoplasma gondii on day 14 after the last immunization. Objective To observe dynamically the location and time of attachment and invasion of Toxoplasma gondii tachyzoites to murine intestinal mucosa by chromogenic in situ hybridization targeting SAG2 mRNA. Methods Thirty 7- to 8-week-old BALB/c mice were randomly divided into experiment group(24 mice)and control group(6 mice). Each animal in the experiment group was given 2?104 tachyzoites of RH stain in 0.2 ml PBS by intragastric administration and that in the control group was given 0.2 ml PBS. Four mice in the experiment group and one in the control group were sacrificed at 15 min,30 min,1 h,2 h,4 h and 8 h after infection,respectively,and paraffin sections of duodenum,jejunum and ileum were prepared to perform the in situ hybridization with Dig-labeled oligonucleotide probe complementary to SAG2 mRNA of T. gondii.
