1.Effects of Intensive Insulin Therapy on Related Indexes of Stress Diabetes Patients with Multiple Trauma

OBJECTIVE:To investigate the effects of intensive insulin therapy on related indexes of stress diabetes patients with multiple trauma. METHODS:82 stress diabetes patients with multiple trauma were randomly divided into observation group and control group,with 41 cases in each group. Two groups were given specific processing. Observation group was additionally giv-en biosynthetic human insulin therapy with 1-2 IU/h drops of micro pump,adjusting dripping speed of micro pump according to re-al-time blood glucose value. Control group was given metformin hydrochloride tablets,with a meal,the initial dose of 0.5 g,twice a day,adjusting the dose of glucose-lowering drugs according to blood glucose value,increasing the dose to 1 g,twice a day. Both groups were treated for 10 d. CRP,TNF-α,IL-1,IL-6,average blood glucose were observed in 2 groups before and after treat-ment. The occurrence of complications as local infection,sepsis and SIRS were compared between 2 groups. RESULTS:After treat-ment,the serum levels of CRP,TNF-α,IL-1,IL-6,average blood glucose in 2 groups were significantly lower than before,and the serum level of CRP,IL-1,IL-6 in observation group after 5,10 d of treatment and the serum level of TNF-α,average blood glu-cose after 1,5,10 d of treatment was significantly lower than control group at same time,with statistical significance(P<0.05). The incidence of complications in observation group after multiple trauma was significantly lower than control group,with statisti-cal significance (P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Based on routine treat-ment,intensive insulin therapy can more effectively reduce the levels of blood glucose and inflammatory factors and prevent the oc-currence of complications after multiple trauma,moreover,do not increase the occurrence of ADR.
2.5-azacytidine and butyrate inhibit in vitro invasion of human liver cancer cells

Objective We have recently shown that 5-azacytidine and butyrate can trigger apoptosis in human liver cancer cells. The purpose of our present study was to determine the effects of 5-azacytidine and butyrate on the invasion of liver cancer cells and the changes of the protein expression in the liver cancer cells during the treatment with these agents. Method Human liver cancer cell lines HepG2 and Hep3B were treated with either 5-azacytidine or butyrate; in vitro cell invasion, adhesion and proliferation, as well as the expression of the cell cycle inhibitory proteins p21waf1, p27kip1 and p53 were observed.Results Our results demonstrated that the combination of 5-azacytidine and butyrate effectively blocked in vitro invasion of these two liver cancer cell lines. Both agents inhibited the adhesion and proliferation of the liver cancer cells. The expression of p21waf1 and p27kip1 protein in the liver cancer cells was increased by the treatment with either agent. These two proteins may contribute to the inhibition of the liver cancer cells invasion.Conclusion Our finding suggests that the agents that specifically target the cell cycle inhibitor genes may clinically useful in the adjuvant treatment of liver cancers.
3.The clinical efficacy of capecitabine combined with docetaxel in treatment of patients with locally advanced breast cancer and its influence on immune function

Objective To investigate the efficacy of docetaxel combined with capecitabine in the treatment of patients with locally advanced breast cancer and its effect on immune function.Methods 160 patients with locally advanced breast cancer were randomly divided into observation group and control group.The control group was treated with capecitabine.The patients in the observation group were treated with docetaxel combined with capecitabine.The clinical effective rate, median time to progress, median survival time, one year survival rate and immunoglobulin protein expression levels were observed.Results The clinical effective rate and stable rate in the observation group were 88.75% and 95.00%, which were significantly higher than 68.75% and 87.50% in the control group, the differences were statistically significant (x2=5.89,4.17,all P<0.05).After treatment, the median time to progression, median survival time and one year survival rate of the observation group were significantly higher than those of the control group (t=3.43,5.09,6.23,all P<0.05).Compared with before treatment, after treatment, the IgG,IgM and IgA levels in the observation group significantly decreased(t=2.34,3.16,3.27,all P<0.05), while those of the control group were not significantly changed (t=0.79,0.22,0.27,all P>0.05).Conclusion Docetaxel combined with capecitabine in the treatment of patients with locally advanced breast cancer can improve the clinical efficacy and stability, prolong the survival time of patients.Compared with using single agent chemotherapy, combined use has no significant effect on human immune function, the effect is better than the use of capecitabine alone, which is worthy of promotion in clinical use.
4.Expression and Significance of I?B kinase-? mRNA in the Lung Tissues of Rabbits Following Hemorrhagic Shock

5.Experimental Study on the Preventive Mechanism of Anisodamine on Multiple Organ Dysfunction Syndrome

Objective To investigate the preventive mechanism of Anisodamine (654-2) on multiple organ dysfunction syndrome (MODS) of rabbits. Methods Rabbit model of MODS induced by hemorrhagic shock and endotoxin was used in this study. Twenty-four rabbits were randomly divided into the control group (C group) , hemorrhagic shock plus endotoxin group (M group) and 654-2 treatment group (T group). The expression of IKK-? of pulmonary alveolar macrophage (PAM) and kuffer cell(KC), the NF-?B activity of nuclear protein extracted from PAM and KC and the concentration of tumor necrosis factor-? Objective To explore the status of adult attachment and its relationship with cognitions, emotions, behaviors, and personal relationship of undergraduate from rural areas with left-behind experience( URLB ).Methods This study investigated 4271 rural college students through questionnaires ( Experiences in Close Relationships Inventory Questionnaire, Simplified Coping Style, the Self-Esteem Scale, Center for Epidimiologic Studies Depression Scale, State-Trait Anxiety Inventory, and College Student's Personal Relationship Synthetical Diagnostic Scale), and 1062 URLB were screened.Results 1.URLB have significant difference from those without in attachment avoidance, anxiety, depression, active coping style, self-esteem, and personal relationship ( t = 2.970,8.063,2.192,1.969,-6.605,-2.913,4.065 respectively, P < 0.05 ).2.URLB with higher adult attachment avoidance showed obvious interpersonal difficulties( t =-3.415, P<0.01 ); those with higher attachment anxiety showed dramatic rise in depression, state anxiety, trait anxiety and interpersonal difficulties ( t = 7.436, 7.961,10.131,2.633,6.764,10.598 respectively, all P < 0.01 )Conclusion The psychological behavior features of URLB were affected by adult attachment.There are needed appropriate psychological interventions to URLB with high attachment avoidence and high attachment anxiety.

Objective To explore the infiltration and apoptosis of eosinophilia(EOS)and the influence of death receptor(Fas) and B cell lymphoma-2 (Bcl-2) expression after Belamcanda and Ephedra Decoction on asthma rats.Methods40 healthy rats of Holland were selected as the research subjects,they were divided into four groups: control group,asthma model group, dexamethasone group,Belamcanda and Ephedra Decoction group with 10 rats in each group.Rat asthma model was prepared by repeated stimulation of egg protein,the symptoms of asthma in each group were evaluated;the number of EOS infiltration in lung tissue was detected by staining method;EOS apoptosis was detected by methyl thiazolyl tetrazolium(MTT) method;Blot Western was used to detect the expression of Fas and Bcl-2 protein;the flow cytometry was used to detect the apoptosis of EOS in rat.ResultsCompared with the rats in the model group, dexamethasone group, Belamcanda and Ephedra Decoction group rats asthma symptoms were significantly reduced, scores were significantly lower (P< 0.05),there was no significant difference between Dexamethasone group and sheganmahuang Decoction group.In the asthma model group,the number of EOS infiltration and EOS% in venous blood and lung tissue were significantly higher than those in control group,EOS apoptosis rate was significantly lower than that of the control group (P< 0.05).In Dexamethasone group,Belamcanda and Ephedra Decoction group infiltration number of EOS and EOS% were significantly lower than that of model group but higher than the control group, the apoptosis rate of EOS protein was significantly higher than the model group and the control group (P< 0.05),there was no significant difference between Dexamethasone group and sheganmahuang Decoction group.The expression of Fas in dexamethasone group,Belamcanda and Ephedra Decoction group was significantly higher than the control group and model group (P< 0.05),Bcl-2 protein was significantly lower than the control group and model group (P< 0.05),there was no significant difference between Dexamethasone group and sheganmahuang Decoction group.Cell flow cytometry showed that the control group had no cell apoptosis,a large number of apoptotic cells were appeared after Belamcanda and Ephedra Decoction treat with.ConclusionBelamcanda and Ephedra decoction can induce apoptosis of EOS expression,it may be associated with the upregulation of Fas, downregulation of Bcl-2 expression.

Objective To explore the analgesic effect of ropivacaine combined with sufentanil in epidural anesthesia on walk on painless labor.Methods 134 cases of maternal pregnancy were selected and randomly divided into two groups.61 cases were treated with conventional delivery,73 cases in the experiment group were treated with walk on painless labor.The visual analogue scale and improved Bromage classification score were compared at the first stage of labor,the second stage of labor,the third stage of labor.Results Compared with the control group,the rate of cesarean section was higher(P<0.05),The visual analogue scale was higher at the first stage of labor,the second stage of labor,the third stage of labor,the duration of labor were lower at the first stage of labor,the second stage of labor,the third stage of labor(P<0.05),and there was no significant difference between the two groups in the motor nerve block.Conclusion Ambulatory labor analgesia on maternal effect significantly,can significantly reduce the pain degree of maternal birth process and deliverry time,postpartum less complications.

10.Enantiomeric separation of zolmitriptan by CE with a sulfated β-cyclodextrin chiral selector

Aim:To develop a practical chiral CE method for the quantitative determination of the unwanted enantiomer[( R )-enantiomer]presented in zolmitriptan. Methods:The background electrolyte was 20 mmol/L sodium dihydrogenphosphate solution with 1% S-β-CD,adjusted to pH 3.50 with phosphoric acid. A fused-silica capillary(60 cm×50 μm ID,effective length 51.5 cm)was used at 20 ℃ for the separation. The applied voltage was -30 kV. The samples were loaded by hydrodynamic injection(50 mbar pressure,6 s). UV was measured at 220 nm. Results:Zolmitriptan and its chiral impurity were baseline resolved ( R s=6.66). The linearity was good over the concentration range from 4 to 80 μg/mL( r =0.999 8) of ( R )-enantiomer. The injection precision (expressed as CV%) was 2.83%. The average recovery was 99.97%( n =9). The limit of detection was 1.5 μg/mL. The host-guest complex binding constants were 964 and 905 mol-1 for ( R )-enantiomer and zolmitriptan,respectively. Conclusion:The method is suitable for the determination of ( R )-enantiomer in zolmitriptan and binding constants of zolmitriptan enantiomers to S-β-CD.
