1.The clinical effects of traditional Chinese medicine which supplements Qi and activates blood circulation on the X-ray radiotherapy-induced lung injury

ObjectiveTo observe the effects of traditional Chinese medicine which supplements Qi and activates blood circulation on the treatment of the X-ray radiotherapy-induced lung injury.MethodsEighty-four patients who were given three-dimensional conformal radiotherapy were randomized into treatment group and control group.The patients in treatment group were given traditional Chinese medicine, which supplements Qi and activates blood circulation, while the patients in control group were given antibiotics and hormone instead.ResultsThe outcome of treatment group was better than that of control.The response rates were 88.6% and 60.0%, respectively (x2 = 9.16, P < 0.05) .Compared with the patients in control group, the clinical manifestations and pulmonary CT reflected better response in the patients of the treatment group(x2 =12.22,P < 0.05) .ConclusionThe traditional Chinese medicine, which supplements Qi and activates blood circulation,has a good therapeutical effect on radiation pneumonitis and protective role for radiation-induced pulmonary fibrosis.
2.A Synchronistic Deficiency in Expression of HLA Class I Molecules and TAP/LMP Genes in Human Ovarian Cancer Cells

Objective: Full expression of class I HLA molecules on tumor cells is pivotal in priming the tumor antigen-specific CTLs for effective iramunotherapies. Tumor cells display abnonnal expression of HLA class I molecules in human ovarian carcinoma. Methods: In this study, the expression of class I molecules in human ovarian cancer cells was determined by using Western bloting, immunohistochemistry testing and flow cytometry. The mRNA of TAP (transporter associated with antigen processing) and LMP(low molecular weight polypeptide) were exannined at the same time by RT-PCR. Results: It has been shown that class I molecules were unable to be, or only weakly, expressed in five of eight ovarian cancer cell lines. On these cell lines abnormal in class I expression, no or only little mRNA were detected for TAP1, TAP2, LMP2 and(or) LMP7 genes. Class I expression, however, could be partially recovered by incubating the tumor cells at 25℃ when compared with those at 37℃ . Conclusion: The results suggested that the dysfunction of TAP and IMP genes, following by a defective expression of class I molecules, might be one of the mechanisms enable ovarian cancer cells to escape from immune surveillance.
3.Clinical effect of internal fixation of Hoffa fracture

Objective To approach the methods and effects of internal fixation for Hoffa fracture. Methods A total of 26 patients with 26 condylar Hoffa fractures ( medial condylar fractures in 13patients and lateral condylar fractures in 13) treated from August 1993 to February 2010 were retrospectively analyzed.According to the Letenneur classification,there were 16 patients with type Ⅰ fractures,four with type Ⅱ fractures and six with type Ⅲ fractures.Among them,two patients were with open fractures and 24 with closed fractures.Surgical approaches including screw fixation in 21 patients and lateral support plate fixation in five were selected based on the fracture types and affected sides. Results All patients were followed up for 12.5-48 months (average 18 months),which showed fracture healing in all the patients within 3-4 months (average 3.5 months).Two patients had slight shift together with knee joint pain,ie,one patient had ROM of knee for 95 °,and one failed the functional exercise because of pain and had ROM of knee for 60° during follow up.No complications including infection,delayed union or bone necrosis occurred.According to Letenneur' s functional assessment system,the postoperative outcomes were excellent and good in 24 condyles,fair in one and poor in one. Conclusions Surgical treatment for Hoffa fractures is safe and effective,but the key point is to choose correct screw fixation position and orientation according to the fracture types and fracture fragment size.
4.Relationship between clopidogrel resistance and inflammation in elder patients after coronary stenting

Objective To observe the changes of serum levels of inflammatory cytokines in the elder patients with clopidogrel resistance (CLR) after coronary stenting. Methods A total of 93 patients with unstable angina pectoris received coronary stenting were enrolled, and peripheral blood samples were taken before and 24 hours, 1 week and 1 month after surgery. The platelet aggregation (PAG) induced by adenosine diphosphate (ADP) were detected, and all patients were divided into CLR group (n=33) and normal response group (n=60) according to PAG response. At the same time, the levels of C-reactive protein (CRP), soluble fragment of CD40 ligand (sCD40L) and P-selectin were detected by enzyme-linked immunosorbent assay. Results The incidences of CLR were 35. 5% (33/93), 26. 9%(25/93) and 20. 4%(19/93) respectively 24 hours, 1 week and 1 month after surgery. The levels of CRP C(8. 8 ± 2. 5) mg/L at 24 hours, (5. 3 ± 2. 5) mg/L at 1 week], P-selectin [(73. 8±34)×10~(-3) ng/L at 24 hours, (70. 5±31. 6)×10~(-3) ng/L at 1 week, (66. 4±22. 3) ×10~(-3) ng/L at 1 month] and sCD40L C(7. 7 ±2. 3)×10~(-3) ng/L at 24 hours] after surgery in CLR group showed significant differences comparing with pre-surgery and normal response group (all P< 0.05). The CLR at 30 days after coronary stenting was significantly correlated with the level of P-selectin (r=1.334) and smoking (r= 1.053). Conclusions The levels of CRP, P-selectin and sCD40L in some elder patients after coronary stenting are increased and may be correlated with CLR. The levels of P-selectin and smoking are the predictors for CLR.
5.Effect of a triptolide-eluting stent on neointimal hyperplasia and the expression of VEGF and ICAM-1 in pigs

Objective To observe the effect of a triptolide-eluting stent(TES)on neointimal hyperplasia in response to vascular injury,inflammation and safety to prevent restenosis after angioplasty.Method Twelve pigs were randomly divided into three groups and received either a bare metal stent(BMS),a sirolimus-eluting stent (SES)or a TES.Each pig was treated with antiplatelet drugs after angioplasty.Biochemistry,vascular morphometry,histopathology and immunohistochemistry were analyzed at 12 weeks after angioplasty.Results The injury scores of the blood vessel were similar in all three groups.There were no differences in minimal lumen diameter or lumen area between the TES[(5.13 ±0.46)mm2;(2.65 ± 0.21)mm]and SES[(5.01±0.54)mm2;(2.65±0.25)mm]groups,but they were significantly(P<0.01)larger than those in the BMS group[(3.76±0.61)mm2;(2.15 ±0.18)mm].The neointimal area in the TES group was smaller than that in the BMS group,but was similar to that in the SES group.The expression of VEGF,ICAM-1 and α-actinin were significantly lower in the TES group than in the BMS group.In all groups,the proliferation on both edges of the stents was insignificant.No toxicity was found in the TES group.Conclusions TES inhibits neointimal proliferation and the expression of inflammatory factors in pigs.In this study,TES safely and effectively prevented restenosis for 12 weeks.
6.Preparation of water-soluble chitosan solid dispersion of daidzein.

OBJECTIVETo enhance the dissolution rate of daidzein with solid dispersion technique.
METHODSolid dispersions were prepared by the solvent method using water-solubility chitosan as a hydrophilic carrier. DSC, IR and X-ray methods were used to verify the formation of solid dispersion.
RESULTDissolution percentages of solid dispersions were more than 90 percent in the drug-carrier ratio of 1:5 and 1:9. But dissolution percentages of physical mixtures and pure drug were 40 and 38.4 percent respectively. Part of daidzein dispersed in solid dispersion in the form of microcrystalline.
CONCLUSIONWater-soluble chitosan solid dispersion can significantly increase dissolution rate of daidzein.

7.Investigating the effects of moxibustion on serum metabolism in healthy human body based on the 1H NMR metabolomics technology

Objective:To investigate the effects of moxibustion on the serum metabolism in healthy human body based on the 1H nuclear magnetic resonance (1H NMR) metabolomics technology, and to find the differences in metabolites, as well as to elucidate the effects of moxibustion on healthy human body from the viewpoint of global metabolism. Methods:Sixty subjects of healthy young men from the enrolled students were randomly divided into a moxibustion group and a control group using random number table, with 30 cases in each group. Subjects in the moxibustion group accepted mild moxibustion on the right Zusanli (ST 36), once a day, 15 min for each time, and continuous treatment for 10 d; those in the control group did not receive any intervention. There were 28 cases in the moxibustion group and 23 cases in the control group after interventions. On the 1st day, 5th day and 10th day of the intervention, serum samples were collected from subjects of the two groups, and metabolic spectra were obtained by the1H NMR technology. Results: Before and after the intervention, serum1H NMR of the moxibustion group was significantly different, while the difference was insignificant in the control group. Metabolite changes in the moxibustion group were mainly in low density lipoprotein (LDL)/very low density lipoprotein (VLDL), valine, isoleucine, leucine, lactic acid, glutamine, citric acid, polyunsaturated fatty acids, creatine, glycine, glycerol, glucose, tyrosine, histidine, formic acid, alanine, lysine, acetic acid, and glutamic acid. Conclusion:Moxibustion can cause changes of serum metabolic patterns in healthy human by influencing the concentrations of branched-chain amino acids, polyunsaturated fatty acids, and other metabolites to strengthen body's metabolisms of amino acids and fatty acid.

9.Frequency of double population of Rh blood group C and E antigen after transfusion of allogeneic red blood cells and its influence on blood transfusion efficacy

【Objective】 To prove the necessity of precise red blood cell transfusion by studying the frequency of double population of Rh blood group C and E antigen after allogeneic red blood cell transfusion in our hospital and its influence on blood transfusion efficacy. 【Methods】 From June 2021 to March 2022, 296 blood recipients, transfused with ABO homogeneous but RhC/E phenotypes unknown 2U-RBC suspension, were enrolled from Beijing Jishuitan Hospital, and the ABO, RhD, RhC, RhE blood type before transfusion and double population frequency of C and E antigens after transfusion were detected. Patients with RhC and E not changed in comparison with pre-transfusion were set as group A(n=135), with RhC becoming double population as group B (n=18), with RhE becoming double population as group C (n=116), and with both RhC and RhE changed to double population as group D(n=27). The levels of red blood cell (RBC), haemoglobin (Hb) and hematocrit (HCT) before and after blood transfusion were compared among the four groups. 【Results】 The negative rates of RhD, RhC and RhE in 296 patients were 0.34%(1/296), 20.27%(60/296) and 67.57%(200/296), respectively. There were 17 cases out of positive screening for unexpected antibodies, among which 7 cases were of anti-E, with the highest proportion. After RBCs transfusion, the double population frequencies of C and E antigens were 15.20%(45/296) and 48.31%(143/296), respectively. The levels of RBC, Hb and HCT in group A, B, C and D were significantly increased, and the difference was statistically significant (P<0.05). There were significant differences in the elevations of RBC and HCT among the four groups (P<0.05), and the elevations of RBC, Hb and HCT were all group A > group C > group B> group D. 【Conclusion】 The Rh blood group C and E antigen double population has an significant influence on the efficacy of blood transfusion.
10.Demands and needs of patients signing family doctor service in Nanjing Gulou district

Objective To investigate the demands and needs of patients contracting for family doctor service in Nanjing Gulou district. Methods A questionnaire survey was conducted between July and December 2017 among residents in Nanjing Gulou district. The questionnaire included basic information of residents, willingness to sign service contract with family doctors, annual outpatient needs, demands of service mode, expectation for family doctor service, and willingness to pay. Results A total of 420 questionnaires were distributed and 415 questionnaires were valid. Multivariate analysis of variance suggested that age, income, and understanding of signing family doctor service were independent factors influencing signing intention (P<0.05).The variance analysis trend test indicated that the healthy people had highest willingness of signing, followed by the patients and the pregnant women (F=6.82, P<0.01). The number of annual outpatient service needs for healthy population, hypertensive patients, diabetic patients and pregnant women was(2.64±3.48),(7.02±4.80),(9.98±5.37)and(11.20±4.33)respectively (F=77.94, P<0.01). The average need of outpatient visit for health males was higher than that for females (P<0.01). There were significant differences in outpatient needs between patients with non-complicated hypertensionand complicated hypertension (P<0.01). The most anticipated services for healthy people were health records (74.26%,101/136), medical appointments (56.62%,77/136), and physical check-up (56.62%,77/136);those were management of blood glucose, blood pressure and body weight (70.13%, 108/154), regular testing (66.88%,103/154), follow-up supervision (66.23%,102/154) for chronic patients;those were regular physical examination (80.26%, 61/76), follow-up supervision (50.00%, 38/76) and health records (46.05%, 35/76) for pregnant women. The most popular health communication channel is WeChat, accounting for 44.10%(183/415), followed by outpatient clinic, telephone and internet services, accounting for 37.11%(154/415), 31.57%(131/415)and 11.08%(46/415)respectively. Among those who chose WeChat, 81.4%(149/183) were under 45 years old. And 86.27%(358/415)of people were willing to pay for corresponding signing fees. Conclusion Age, income, and understanding of general practice will affect residents′ willingness to sign the service contract with family doctors. Gender, disease spectrum and number of illnesses affect residents′outpatient service needs. Individualized family doctor service should be provided for people with different health needs.
