Circulating microparticles (MPs) derive from a variety of cells,including platelets,leukocytes,endothelial cells and erythrocytes.In addition,tumor cells release MPs into blood.MPs are formed as a result of membrane lipid remodeling and proteolytic cleavage of cytoskeleton.Exposure of phosphatidylserine (PS) and tissue factor (TF) provide a surface for the assembly of Xase and prothrombianse to activate coagulation and induce thrombosis.Considerable levels of platelet-derived PS+ MPs are detected in healthy subjects.However,the level of PS+ and TF+ MPs,the latter of which predominantly derived from monocytes,greatly increased in thrombotic diseases,such as sepsis,cancer,and myocardial infarction.Therefore,MPs could be a useful biomarker for the evaluation of thrombosis risk.

ObjectiveBy reviewing TCM research projects in Nanjing municipal hospitals from 2006 to 2010 (eleventh five-year plan), this study aimed to recognize the existing problems, put forward corresponding solutions, and help Nanjing municipal hospitals improve scientific research level.Methods Through field investigations and interviews with experts, this study set 139 research projects from 2006 to 2010 in 10 municipal hospitals in Nanjing as study samples. ResultsDuring 2006-2010, current situations in TCM research projects of Nanjing municipal hospitals were as follows:the number of TCM research projects was small, the research level was low, and the fund was little. TCM research resources were unevenly distributed;the overall quality of TCM human resources were not high;talent structure was unreasonable;great differences existed among different types of hospitals.ConclusionsDuring the eleventh five-year plan period, there were significant differences between TCM research projects and WM research projects in Nanjing municipal hospitals, such as the differences in number, level, fund and project channel. The study proposed that TCM research materials should be rationally allocated, related mechanisms should be perfected, the industrial structure should be optimized, and the pace of training and introducing TCM research human resources should be quickened, with a purpose of making contributions to TCM development in Nanjing.

The characteristics Included the extensive wounds, high incidence of shock and the blind wounds. The remote effect was often seen in these cases. And also the treatment principles of these wounds were dicussod in this paper.

322 eyes(290 cases)of retinal detachment(RD) and 31 eyes(28 cases)of retinal rhegma treated with laser were analyzed to study the effect of myopia on the incidence and therapeutic prognosis of RD.Among 200 eyes of nontraumatic rhegmatogenic RD,154 eyes(77.0%)were myopic,the majority of which,109 eyes(70.8%),were highly myopic. Among 77 eyes of traumatic rhegmatogenic RD, myopia accounted for 32 eyes(41.6%).Among 32 cases of bilateral RD,28 cases(87.5%)were myo- pes.The onset of RD in myopic eyes was not corre- lated with age.In operations of nontraumatic rhegma- togenic RD,the rate of surgical success on the first attempt was 78.3% for the nonmyopes,significantly higher than that(61.0%)for the myopes.The complication of proliferative vitreoretinopathy(PVR) was also much more severe in the myopic eyes than in the nonmyopic eyes,except in the group of contusive RD with PVR D_1-D_3.

Objective To observe the effect of clopidogrel in combination with aspirin to treat senile coro-nary heart disease and its influence on relevant indicators.Methods Clinical data of 200 elderly patients with coro-nary heart disease(CHD)were retrospectively analyzed,according to different therapeutic methods,they were divided into the two groups,100 cases in each group,the two groups on the basis of conventional stable plaques,expand coro-nary targeted treatment,the control group treated with oral aspirin enteric -coated metformin hydrochloride,the observation group treated with aspirin combined clopidogrel treatment.The clinical efficacy of the two groups,the cardiac function indexes before and after treatment,blood coagulation indexes,as well as the adverse reaction of the two groups of patients were observed.Results The total effective rate of the observation group was 93.0%,which was significantly higher than 77.8%of the control group(χ2 =4.142,P<0.05);In the observation group after treatment, the cardiac function indexes LVEF,SV,CO were significantly higher than the control group(t=2.475,2.397,3.052, all P<0.05);The MAADP,platelet inhibition rate after treatment in both groups before treatment and platelet aggre-gation rate and other indicators and comparative differences were statistically significant(t=8.293,12.285,10.384, all P<0.05 );In the observation group after treatment,the MAADP,platelet inhibition rate and platelet aggregation rate index difference compared with the control group after treatment were statistically significant(t=6.682,8.668, 8.05 1 ,all P<0.05 );In the two groups,the FIB levels after treatment compared with before treatment differences were statistically significant(t=2.375,4.393,all P<0.05);In the observation group after treatment,the plasma FIB level compared with the control group after treatment differences was statistically significant(t=2.186,P<0.05);In the two groups after treatment,the indexes of TC,TG,HGL and LDL levels compared with before treatment differences were statistically significant(t=3.226,2.834,2.448,2.834,3.006,2.927,2.446,2.927,all P<0.05);Again hair the comparison of the number of coronary heart disease,readmission,the observation group was better than that of control group(χ2 =4.126,8.256,all P<0.05 ).Conclusion Clopidogrel in combination with aspirin can more effectively by platelet aggregation and inhibit blood coagulation function to achieve fast and efficient antithrombotic effect,improve the treatment of coronary artery disease clinical curative effect and heart function,and short-term use safety is good.

Objective To investigate the relationship between women's reproductive tract mycoplasma infec-tion and tubal obstruction effusion .Methods A retrospective analysis of 60 cases infertility infertility clinic patients and healthy control were detected in our hospital from Junuary 2013 to December 2014.The cervical secretion sam-ples were used for detection of Mycoplasma ( leucorrhea , Ureaplasma urealyticum ( Uu ) and Mycoplasma hominis ( Mh) lipiodol angiography and fallopian tube , and two groups of patients with mycoplasma infection and tubal cir-cumstance tube obstruction were compared .Results The two groups infertile patients with Cervical Mycoplasma ex-amination with Uu positive were 41 cases, accounting for 68.3%of the patients in the control group .Uu was positive in 16 cases, accounting for 26.6%; Infertile patients with Uu +Mh positive were 15 cases, accounting for 25%, while the control group were no cases were positive for Uu +Mh;Infertility group patients detected negative for Myco-plasma in 4 cases, accounting for 6.7%, while the control group were negative for the detection of Mycoplasma in 44 cases, accounting for 73.3%.The results showed that infertility group and Uu in patients with Mh infection was high-er than that of the control group, and the difference is statistically significant (p<0.05).The infertile patients with fallopian tube patency were 12 cases, accounting for 20%; The control group of tubal patency were 33 cases, ac-counting for 55%, and it was accounted for 19.3% of 21 cases of tubal infertility group , obstruction.The control group of 13 cases of tubal obstruction patients , accounting for 21.7%;27 cases had complete obstruction of fallopian tube infertility patients group , accounting for 45.0%, and the control group had complete tubal obstruction in 14 ca-ses, accounting for 23.3%.The results showed that infertile patients with fallopian tube obstruction was significantly higher than that in the control group , and the difference was statistically significant (p<0.05).Conclusion Infer-tility women with genital tract mycoplasma infection occurrence are closely related .We should make a comprehensive inspection to discover occult infection of Mycoplasma timely and treat actively in women with infertility treatment .

The application of case-based teaching in medical curriculum can enhance discussion, motivate students learning interest,enhance their abilities of clinical practice and clinical thinking.To highlight the key points of course,the application of problem-based learning(PBL) is helpful.The combination of PBL and case-based teaching will reduce redundant content and improve the quality of clinical teaching.
