1.Analysis on clinical diagnosis and treatment of 120 cases with cesarean uterine scar defects

Objective To analyze the clinical treatment and prevention methods of uterine scar defects after cesarean section.Methods Retrospectively analyzed the clinical data of 120 patients with scar defect after cesarean section,it was mainly applied in the hysteroscopic uterine scar electrocoagulation resection,after anterior fornix inci-sion scar repair and hysteroscopy determine scar position underwent laparoscopic uterine scar repair such as surgery. The surgical treatment effect and the quality of life scores before and after the surgery were observed.Results The patients after surgery, six months after surgery expert hysteroscopy could be observed in patients with uterine scar defects compared with preoperation significantly shrink or disappear.Gradually returned to normal menstruation,pelvic pain effectively alleviated,effective treatment of 90 cases,the effective rate was 75.00%.Pre-operative quality of life score was (32.54 ±5.63)points,quality of life scores after surgery was (48.65 ±7.85)points,the difference was statistically significant (t=5.632,P<0.05).Conclusion The cesarean section scar defect is a common complica-tion of cesarean,surgery is effective treatment.We need to strictly abide by cesarean section surgery when practices and scientific selection of cesarean section and uterine incision to improve skills, which can effectively reduce the cesarean scar defect rate.
2.Expression and clinical significance of survivin, bcl-2 and mdr1 in acute leukemia in children

Objective: To explore the expression of survivin,bcl-2 and mdr1 in acute leukemia in children and its clinical significance.Methods: The mRNA of survivin,bcl-2 and mdr1 in all patients were detected by RT-PCR.Results: The positive expression rates of survivin,bcl-2 and mdr1 in the newly diagnosed group were 60.47%(26/43N),60.47%(26/43N) and 34.88%(15/43N),respectively,all of which were higher than those in the control group(P0.05).The positive expression rates of survivin and bcl-2 in the CR group were significant lower than those in the newly diagnosed group(P0.05).Conclusions: survivin,bcl-2 and mdr1 have higher expression in children with AL,which may contribute to the pathogenesis and the progress of the leukemia.The expression of survivin and bcl-2 is not related to the type of leukemia and the WBC.The co-expression of survivin bcl-2 and mdrl may indicate the poor prognosis of leukemia in children.
3.Effect of polysaccharide from spirulina platensis on hematopoietic recovery and related cytokines in mice with transplant tumor after treated by chemotherapy

AIM: To evaluate the effect of polysaccharide from spirulina platensis (PSP) on hematopoietic recovery and related cytokines in mice with transplant tumor after treated by chemotherapy. METHODS: The tumor cells were subcutaneously injected in the right forefoot to induce transplant solid tumor in mice. The mice were ig with PSP for 10 d and injected intraperitoneally with cyclophosphamide (CTX) for 3 d. On d 11, the count of peripheral blood cell, nucleated cell in bone marrow and CFU-S (Colony Forming Unit-Spleen) was observed; The content of DNA in bone marrow was inspected by UV-spectrophotometer; The cytokines in serums were inspected by double antibody sandwich ELISA. RESULTS: CTX could induce evident myelosuppression. But PSP could elevate the level of peripheral blood cell, increase nucleated cell and DNA in bone marrow, and promote CFU-S formation. In addition, PSP could also increase the content of IL-1, IL-3, GM-CSF, and TNF-? in serum. CONCLUSION: It is probably that PSP accelerates the hematopoietic recovery in mice with transplant tumor treated by chemotherapy by promoting endogenous cytokines secretion.
4.Relation of pain threshold and effect of postoperative analgesia with preoperative anxiety in patients with hysteromyoma

Objective: To evaluate the relation between preoperative anxiety and pain threshold in patients with hysteromyoma, and to investigate the effect of anxiety on postoperative analgesia. Methods: Fifty patients with hysteromyoma were included and evaluated the anxiety level with the Self-Rating Anxiety Scale (SAS) 1 day before hysterectomy. Threshold of heat pain and cold pain of all patients were also assessed by quantitative sensory testing (QST) at the same time. All patients received hysterectomy under the combined spinal epidural anesthesia,and 2 mg morphine were injected to their epidural spaces 10 min before the end of surgery. Visual analgesia scores (VAS, 0~10) were performed to evaluate the level of pain at 0 h, 2 h, 6 h and 12 h after the operation. The time of first ambulation after operation were also recorded to help assess the effect of postoperative analgesia. Results:The average SAS scores of all these 50 patients was (48.7±10.3) . And 13 cases (26%) were with mild anxiety (SAS=50-59), whereas 5 cases (10%) were with moderate or severe anxiety (SAS≥60) . The threshold of heat pain and cold pain were (37.8±3.1)℃ and (15.8±3.6)℃.VAS at all investigated time points was 0,(1.4±1.1), (3.3±1.6) and (4.4±2.1) . There was significantly negative correlation between the SAS scores and the pain threshold of all patients with anxiety (r=-0.67, -0.56; Ps<0.001). But for these patients, the correlations between the SAS scores and VAS scores at 6h and 12h after the operation were positive (r=0.48,0.55, Ps<0.001) . Furthermore, the average time of first ambulation after operation in those anxious patients was later than that in the patients with no anxiety (SAS<50) [(46.6±7.1) h vs.(39.8±9.3) h, P <0.05].Conclusion: The higher level of preoperative anxiety in patients with hysteromyoma is correlated to higher level ofpain threshold, less effect of analgesia and longer time to recovery. It indicates that it is necessary to assess preoper-ative anxiety in patients with hysteromyoma.
5.Aplastic anemia and clonal evolution:germline and somatic genetics

Clonal progression to myelodysplastic syndromes (MDS) or acute myeloid leukemia (AML) remains a dreaded complication for a subset of patients with bone marrow failure (BMF). Recognizing risk factors for the development of MDS or AML would inform individualized treatment decisions and identify patients who may benefit from early or upfront hematopoietic stem cell transplantation. Now that next-generation DNA sequencing is available in the clinical laboratory, research has focused on the implications of germline and somatic mutations for diagnosing and monitoring patients with BMF. Most germline genetic BMF disorders are characterized by a high propensity to develop MDS or AML. Recent studies of somatic variants in marrow failure revealed a high frequency of clonal hematopoiesis with the acquisition of mutations in genes associated with MDS or AML. The evaluation and implications of germline and somatic mutations for the development of clonal disorders in patients with BMF and challenges of clinical genetic testing will be summarized in this paper based on the reports from the 58th American Society of Hematology (ASH) Annual Meeting.
6.Leptin and chronic renal failure

Leptin,a polypeptide hormone coded by Ob-gene,has the functions of depressing appetite,reducing caloric intake and enhancing caloric consumption,etc.Leptin has a close relationship with kidney,for it is metabolized and eliminated via kidney.Meanwhile,leptin can also directly affect kidney functions,influencing the process of chronic renal failure.Furthermore,hyper-leptinemia is associated with anorexia,malnutrition and cardiovascular diseases.Different dialysis patterns may also play different roles in cleaning leptin.For example,common dialysis could not clear leptin,but other dialysis modes such as high permeability dialysis,hemodiafiltration,and mastic absorption hemoperfusion can alleviate hyper-leptinemia through cleaning leptin partly.For kidney transplantation patients,hyper-leptinemia may affect the immune balance which has been maintained for a long time.

9.Perioperative Application of Antibacterials in 142 Surgery Patients in Our Hospital

OBJECTIVE: To investigate the perioperative application of antibacterials in our hospital.METHODS: The application of antibacterial drugs in 142 surgery patients sampled randomly from our hospital in 2006 was investigated in respect of the variety of drugs,medication time and drug combination etc.RESULTS: 100% of the total 142 cases received antibacterials for prophylaxis purpose for 6.8 days on average.58 cases(40.8%) received antibacterials before operation,8 cases(5.6%) during operation and 76 cases(53.5%) after operation.35.8% were rational on the whole and 64.2% irrational in drug use.CONCLUSIONS: The application of antibacterials in our hospital is worrisome,therefore,it is urgent to tighten control on the clinical application of antibacterials.
10.Effects of oxytocin and prostaglandins on oxytocin receptor expression of human myometrial smooth muscle cell

Purpose To investigate the effects of oxytocin(OT) and prostaglandins intervention on oxytocin receptor(OTR) expression of primary culture of human myometrial smooth muscle cell.Methods Using respectively oxytocin, prostaglandin E2(PGE2),and prostaglandin F2alpha(PGF2alpha) intervened on human myometrial smooth muscle cell.The expression of OTR mRNA and protein of cell hemogenate was examined.Results The expression of OTR between oxytocin intervention group and untreated group was similar.The expression of OTR in cell was significantly higher in the PGE2 or PGF2alpha intervention group than that in the untreated group.The expression of OTR in cell was significantly higher in the PGE2 and PGF2alpha joint intervention group than that in the untreated group and than that in the PGE2 or PGF2alpha individual intervention group.Conclusion Oxytocin didn′t increse the expression of OTR in human myometrial smooth muscle cell.PGE2 and PGF2alpha incresed the expression of oxytocin receptor in human myometrial smooth muscle cell.Furthermore PGE2 and PGF2alpha joint intervention more significantly increased the expression of oxytocin receptor than PGE2 or PGF2alpha individual intervention in human myometrial smooth muscle cell.
