1.Investigation on professional identity of medical students with long education periods

Objective To explore the professional identity of medical students with long education periods and its influencing factors.Methods Survey on professional identity was conducted through self-designed questionnaire among 665 medical students with long education periods of every grade.Single factor analysis of variance and t test were applied to do statistical analysis.Results There were significantly statistical differences in influencing factors of professional identity among every grade of long periodical medical students (P < 0.05).There were significant statistical differences among influencing factors of sex,educational background of parents and residing(P < 0.05).Conclusions Because of characteristics of medical education and psychological changes of students,professional identity of students shows a first decline and then an increase.Personal and environmental factors exert influences on professional identity at varying degrees.
2.Placenta accreta treated with uterine arterial embolization (case report)

Objective To investigate the possibility of interventional treatment of placenta accreta. Methods One patient with placenta accreta was treated with uterine arterial infusion of cef and MTX followed by embolization. Results Her placenta discharged through vagina at the 27th day after uterine aterial embolizayiea. No remnant was found in the uterine by Doppler imaging. The patient uneventfully recovered.Conclusions Intervetional therapy with uterine arterial embolization is a new and reliable method for treatment of placenta accheta.

4.CE fingerprint of the compound Shengmai Powder

AIM: To investigate the CE fingerprint of the compound Shengmai Powder(Radix et Rhizoma Ginseng rubra,Radix Ophiopogonis,Fructus Schisandrae chinensis). METHODS: Sequential uniform design was used to optimize the separation conditions. A CE fingerprint for Shengmai Powder was established using buffer comprising 44 mmol/L borate and 34 mmol/L SDS at pH 9. 5,a running voltage of 25 kV,a capillary temperature of 25 ?C and a wavelength of 200 nm. The sample was injected at a pressure of 50 mbar for 100 s. RESULTS: From the fingerprints of eleven batches of sample solutions,twenty main common peaks were determined. four peaks came from Radix et Rhizoma Ginseng,six peaks from Radix Ophiopogonis,thirteen peaks from Fructus Schisandrae chinensis,three peaks shared by Radix et Rhizoma Ginseng and Radix Ophiopogonis,one peak shared by Radix Ophiopogonis and Fructus Schisandrae chinensis and one new constituent. CONCLUSION: The developed method is accurate and reliable,and the fingerprint analysis can be used for the quality assessment and control of compound Shengmai Powder.

Objective:To discuss and analyze the clinical efficacy and application value of transperineal ultrasound dynamic monitoring for the contraction function of pelvic floor muscle to guide training based on its feedback in the treatment of stress incontinence of female.Methods: 169 female patients with stress incontinence were enrolled this study. And all of them were treated by using the Koegel physical training of pelvic floor muscle under the guidance of doctor, at the same time, the relative contraction function indicators of pelvic floor muscle before and after treatment were observed and compared by applied transperineal ultrasound dynamic monitoring.Results: The differences between before and after treatment of physical training on pelvic floor muscle about the length of the levator hiatus, the acreage of pelvic diaphragm hiatus, and the perimeter of pelvic diaphragm hiatus in contraction and silent period were significant (t=-2.858,t=-2.949,t=-4.899,P<0.05), and that of after treatment was higher than that of before treatment. The horizontal distance of pubis combined with margo inferior, vertical distance and thickness of viscera on pubis of after treatment were significant higher than that of before treatment (t=3.674, t=-4.899,t=4.230,P<0.05). The posterior urethra intersection angle and the intersection angle of left and right levator after treatment were significantly higher than that of before treatment (t=-4.087,t=6.584, P<0.05).Conclusion: Transperineal ultrasound dynamic monitoring can correctly evaluate the clinical efficacy of the contraction function of pelvic floor muscle to guide training based on its feedback in the treatment of stress incontinence of female. And this method has important clinical value.
6.Correlation of XRCC1 Polymorphism with Radiotherapy Response in Squamous Cell Carcinoma of Cervix

Objective To investigate the correlation of XRCC1 Arg194Trp Arg399Gln Single nucleotide polymor-phism (SNP) with radiotherapy response of squamous cell carcinoma of cervix. Methods Patients with exogenous type cer-vical squamous cell carcinoma confirmed by histopathology were selected for our study. These include:patients in stageⅠ(4 cases), patients in stageⅡ(36 cases), patients in stageⅢ(30 cases), patients in stageⅣ (3 cases). There are 30 patients with tumor diameter less than 4 cm and 43 patients with tumor diameter over 4 cm in our test. There are 36 cases with dose point A less than 80 Gy and 37 cases with dose point A over 80 Gy . Radiotherapy outcomes showed 47 cases of complete re-mission and 26 cases of part remission. Polymorphisms Arg194Trp, Arg399Gln of XRCC1 gene in 73 cervical cancer pa-tients were analyzed by mismatch amplification polymerase chain reaction (MAMA-PCR). Results Arg/Arg, Arg/Trp, TrP/Trp of Arg194Trp genotype distribution were 31 (42.5%), 37 (50.7%), 5 (6.8%) respectively. Arg/Arg, Arg/Gln, Gln/Gln of Arg399Gln distribution were 6 (35.6%), 39 (53.4%), 8 (11.0%) respectively. The response to radiotherapy was not statistical-ly significant in three genotypes, Arg/Arg, Arg/Trp, TrP/Trp of XRCC1 at codon 194(P>0.05). Neither was XRCC1 at codon 399. Multivariate analysis showed that late clinical stage was a risk factor of part remission. Conclusion SNP of XRCC1 gene at codon 194 and codon 399 could not predict clinical response of patients with squamous cell carcinoma of cervix to ra-diotherapy. The patients with advanced cervical cancer had poor response to radiotherapy.
7.Epithelial-mesenchymal transition of human retinal pigment epithelial cells under the high glucose condition in vitro

Background Several types of cells participate in the formation of proliferative membrane in proliferative retinopathy (PVR),and the proliferation,migration and epithelial-mesenchymal transition (EMT) of retinal pigment epithelium (RPE) cells play an important role.Many studies have confirmed high blood glucose is the basic pathogenesis of diabetic retinopathy (DR).However,whether EMT could be induced in RPE cells under the high glucose condition has not been reported.Objective This study was to investigate the effects of high glucose on the migration and EMT of RPE cells in high glucose culture model in vitro.Methods Human RPE cell line D407 were cultured and passaged in DMEM/F12 medium with 10% fetal bovine serum,and 6-8 generations of cells were used in experiment.The cells were divided into 3 groups based on different glucose concentrations in medium.The glucose at the final concentration 5.5 mmol/L or 60.0 mmol/L was respectively used in the normal control group or high glucose group,and the DMEM with 5.5 mmol/L glucose and mannitol was used in the hypertonic control group.The migration rate of the cells were detected 0,24,48 and 72 hours after scratching by wound-scratch test.Real-time PCR was used to detect the relative expressions of zonula occludens-1 (ZO-1) and α-smooth muscle actin (α-SMA) in the cells.Results Cultured cells showed a polygon shape with the clear nucleolus and dense arrangement in the normal control group and the hypertonic control group,but the cells were larger and elongated with the lapse of culture time with the indistinct structure and loose arrangement.At 48 hours after scratching,migrating cells were seen in the scratching area,and the scratching area disappeared at 72 hours after scratching in the high glucose group,but the scratching area still was existed in the normal control group or hypertonic control group.The migrating rate of the cells was higher in the high glucose group than that in the normal control group or hypertonic control group,showing total differences among 3 groups and various time points (Fgroup =328.600,P =0.000 ; Ftime =773.270,P=0.000).Compared with the normal control group,the expression level of ZO-1 mRNA was significantly lower,and α-SMA mRNA level was higher 48 hours and 72 hours in the high glucose group than those in the normal control group (all at P<0.05).Conclusions High glucose induce the migration and EMT of RPE cells in vitro,which may be associated with the pathogenesis of proliferative diabetic retinopathy.

OBJECTIVE:To make clear the antimicrobials used for inpatients in our hospital from September 1999 to March 2000 METHODS:We sampled the records of discharged patients at the ratio of 1/3 of sickbeds in our hospital,and filled in "the questionnaire of antimicrobials used in clinical department" item by item,finally,we carried out comprehensive analysis RESU_LTS:More than half was administration of single and basic antimicrobial Combined use of antimicrobials for severe and mixed infections was rational The rate of using antimicrobials was high(79 64%),especially in preventing perioperative infections,reached nearly 100% The starting-point was high in choice of drug and the newly-developed,special and expensive drugs occupied the front place in order of consumption Combined use of drugs was unduly prevalent Most of treatment depended on experience The rate of bacterial drug resistance test was only performed in 10% of patients CONCLUSION:Rational use of drugs is the main trend,however,the measure directed against existing problem should be worked out
9.Studies on origin, morphology and histology of Galla Cinnamomi Camphorae specially produced in Jiangxi Province

Object To study the origin, morphology and histology of Galla Cinnamomi Camphorae(GCC). Methods By the literature investigation and the survey in the production area, GCC was identified by microscopic technique. Results The origin and the medicinal part of GCC were defined. Conclusion GCC is the abnormal fruit of Cinnamomum camphora (L.) Presl which has been infected by Exobasidium sawadae Yamada and formed with hymenium and fruit. The characters in morphology and histology of GCC provide the scientific reference of identification of the crude drug.

Background Sphingosine-l-phospate (S1P) is a bioactive lipid and important messenger molecule in cells.It participates in the regulation of many biological processes,such as cell proliferation,migration,survival,differentiation,apoptosis,etc.Hypoxia is a trigger factor of choriod neovascularization (CNV) and pathological basis of many diseases,and retinal pigment epithelial (RPE) cells are involved in formation of CNV.However,the effects of S1P on proliferation and apoptosis of RPE cells are below understood.Objective This study was to investigate the influence of S1P on proliferation and apoptosis of human RPE cells under hypoxic conditions.Methods Human RPE cells line-D407 cells were cultured and passaged and generation 3-5 cells were used and divided into 6 groups.The cells were regularly cultured in the blank control group using DMEM containing 10% fetal bovine serum.CoCl2(200.00 μmol/L) was added into the colture medium for 2 hours in the hypooxic group.S1P of different concentrations (0.01,0.10,1.00,10.00 μmol/L) were added in culture medium 2 hours after the affection of 200.00 μmol/L CoCl2.The proliferative values of the cells were detected using WST-1 method as the absorbance (A value) and the proliferative rate of different groups were calculated.The apoptosis of the cells was assayed by Hoechst staining.The results were compared among different groups.Results Cultured cells showed the round-like in shape with clear nuclei and pigment.The proliferative values (A value) was 0.91 ±0.08,0.37±0.09,0.46±0.08,0.52±0.09,0.61 ±0.06,0.70±0.10 in the blank control group,hypoxic group and 0.01,0.10,1.00,10.00 μmol/L S1P groups,respectively,with a significant difference among the groups (F=21.104,P=0.000),and A values in various S1P groups were higher than those in the hypoxiac group (all at P<0.05).The proliferative rate was gradually raised with the increase of dose of S1P.Hoechst staining exhibited a few apoptosis cells in the blank control group,but in the hypoxic group,a lots of apoptosis cells were seen with the light-blue nuclei and condensable chromatin.However,the number of apoptosis cells was significantly decreased in various concentrations of S 1 P groups.The apoptosis rates were (1.21 ±0.08) %,(8.99 ±0.09) %,(6.60 ±0.08) %,(5.95 ±0.09) %,(4.81 ± 0.06)% and (3.96±0.10)% in the blank control group,hypoxic group and the 0.01,0.10,1.00,10.00 μmol/L S1P groups,respectively,with a significant difference among the groups (F =25.070,P =0.000).Compared with the hypoxia group,the cellular apoptosis rates of various S1P groups were lower (all at P<0.05).Conclusions Under the hypoxia condition,S1P can promote the proliferation of human RPE cells and inhibit apoptosis.
