1.Technology Verification for Irbesartan and Hydrochlorothiazide Tablets

Objective: To verify the feasibility and reliability of the pilot process of irbesartan and hydrochlorothiazide tablets. Methods:Three batches of the tablets were produced according to the determined formula and process, and the key parameters in the pilot production were verified. Results:All the indices for each batch of irbesartan and hydrochlorothiazide tablets conformed to the re-quirements of the quality standard. Conclusion: The pilot process of irbesartan and hydrochlorothiazide tablets is feasible, and the technological process and parameters can ensure the product quality with promising reliability and stability.
2.Clinical Significance of blood lactic acid dynamic monitoring in the prognosis evaluation of critically ill pediatric

Objective To study the correlation between blood lactic acid level and PCIS scroing system.To study the relationship between dynamic lactic acid monitoring indexes and prognosis of critical patients.Methods Differences of mortality,incidence of MODS and shock,PCIS score between two groups were compared.The patients were divided into 3 groups by PCIS score to compare pathogenetic conditions prognosis, then to study the were divided into death group and survival group by the hospital prognosis. Differences of their lactic indexes and other clinical indicators which reflected organ/system status were compared, prognostic-significant lactic indexes were selected by mu fti-variable logistic analysis. Results Patients with elevated lactic acid level had more incidence of shock and MODS, higher PCIS scores and higher mortality, the differences had statistical significance compared with the normal lactic acid group. There were significant linear correlation between ICU lactic acid level, peak lactic acid level and PCIS scores respectively. For the patients with elevated lactic acid level, peak lactic acid level, 12h lactic clearance had significant correlation with prognosis. Conclusion PCIS scoring system had a significant linear correlation with lactic acid level. For the patients with elevated lactic acid level,peak lactic acid level,12h lactic clearance were good indicators to evaluate patients' prognosis.
3.Characteristic of hyperuricemia in patients with type 2 diabetes and association between serum uric acid level and components of the metabolic syndrome

Objective To evaluate the clinical and biochemical characteristics of type 2 diabetics with hyperuricemia and to investi-gate the association between serum uric acid and the number of metabolic syndrome components. Method In the cross-sectional study of 547 type 2 diabetics, we recorded body mass index (BMI), systolic blood pressure(SBP), diastolic blood pressure (DBP), and collected blood samples to measure serum uric acid (UA), fasting plasma glucose (FPG), glycosylated hemoglobin (HbAlc), creatinine (Cr) and blood lipid profile, including tfiglyceride (TG), total cholesterol (TC), low-density cholesterol LDL-C) and high-density cholesterol (HDL-C). Results Compared with the normal UA group, the diabetic patients in the high UA group had significantly increased levels of BMI, SBP, DBP, FPG, Cr, TG, TC and HbA1c (P <0.05, P <0.01, P <0.05, P <0.01, P <0.01), and decreased level of HDL-C(P < 0.05). There was no significant difference in the level of LDL-C between the two groups (P > 0.05). According to their possession of 1, 2, 3 or 4 components of metabolic syndrome, 547 cases were divided into four groups (MSI, MS2,MS3 and MS4). There were significant difference in the levels of UA, BMI, SBP, DBP, Cr, TG, TC, LDL-C and HDL-C among MSI, MS2, MS3 and MS4 (All P < 0.05). Multiple regression analysis showed that UA concentration was positively correlated with sex, TG and Cr (Beta:0. 088,0. 350, 0. 124; P < 0.05, P <0.01, P <0.01) and negatively correlated with HDL-C and HbA1 c(Beta: -0.107, -0.124 ; P <0.01, P <0.01). The con-centration of UA was strongly correlated with serum TG. Conclusions Serum UA level in type 2 diabetics was significantly elevated as the number of metabolic components increased. Abnormal TG had great influence on serum UA. Hyperuricemia was considered to be a dangerous factor of the metabolic syndrome.
4.Factors Related with Understanding of Stroke in Patients

Objective To investigate the factors that influence the patients to understand the stroke they suffering from.Methods 61 stroke patients were investigated with a questionnaire designed by ourselves.ResultsThe age,sex and educated level affected the stroke patients' understanding,but the time after onset,the degree of locomotory injured and hemiplegic side did not.ConclusionDistinguishing direction of rehabilitation should be established according to the age,sex and educated level of patients.
5.Stereotactic Radiosurgery by X-knife for the Treatment of Brain Metastatic Tumors:Analysis of 520 Cases

Objective To explore the efficacy and postoperative survival times of stereotactic radiosurgery by X-knife in the treatment of brain metastatic tumors.Methods Between March 1996 and March 2008,520 patients with brain metastatic tumors were treated in our hospital by stereotactic radiosurgery using X-knife.Among the cases,336 had single metastatic lesions,and 184 presented with multifocal tumors(2 lesions in 51 patients,3-5 lesions in 63,and 6-10 lesions in 70).The tumors sized 5 to 35 mm in diameter with a mean of 16.4 mm.Stereotactic radiosurgery was carried out in the cases with a mean central dose of 22 Gy(20 to 25 Gy),and mean marginal dose of 12 Gy(10 to 14 Gy).Over 80% of the lesions were encircled with the dosage.Results The 520 patients were followed up for 8 to 36 months(mean,20 months).CT and MRI performed in the 6th months postoperation showed complete response in 218 patients(41.9%),partial response in 182(35.0%),no response in 78(15.0%),and progressed disease in 42(8.1%).The overall control rate of the tumors were 91.9%(478/520).In this series,the 6-months survival rate was 85.0%(442/520),12-month survival rate was 68.1%(354/520),and the 1-year and 3-year survival rates were 40% and 5.5% respectively.The medium survival time was 12.6 months.Conclusions Stereotactic radiosurgery is an effective method for brain metastatic tumors.It can improve the life quality and survival time of the patients.
6.Antibiotic Resistance among Gram-negative Bacilli in Intensive Care Unit Patients with Lower Respiratory Tract Infection

OBJECTIVE To investigate the antimicrobial resistances of Gram-negative bacilli isolated from patients with lower respiratory infection in intensive care unit(ICU),so as to provide reasonable selections for antibiotic in clinical treatment.METHODS The phlegm specimens from ICU inpatients with lower respiratory tract infection were collected from 2004 to 2007,and drug sensitivity test was performed according to NCCLS standards.RESULTS Gram-negative bacilli were accounted for 75.2% of total strains.The most predominant pathogens were Pseudomonas aeruginosa(42.7%),Acinetobacter baumannii(16.3%),Sternotrophomonas maltophilia(10.4%),Escherichia coli(10.0%),and Klebsiella pneumoniae(6.7%).The prevalence of ESBLs in E.coli and K.pneumoniae increased from 54.5% and 37.5 % in 2004 to 65.6 % and 52.7% in 2007.ESBLs non-producing strains showed lower resistance rates to the aminoglycosides,quinolones and the third and fourth generation cephalosporin than those of ESBLs producing strains.Nonfermentative Gram-negative bacilli were very highly resistant to most antimicrobial agents.CONCLUSIONS The drug resistance rates of Gram-negative bacilli of lower respiratory tract are serious.It is important to investigate the antimicrobial resistances of pathogenic bacteria and rationally use of antibiotics.
7.Study on expression of visfatin mRNA in abdominal omental adipose tissue and its relationship with blood lipid in type 2 diabetics

Objective To investigate the expression of visfatin mRNA in abdominal omental adipose tissue and its relationship with blood lipid in type 2 diabetes mellitus.Methods 161 type 2 diabetic patients were divided into two groups according to triglyceride ( TG ),high triglyceride group ( TG ≥ 1.7mmol/L) and normal triglyceride group(TG < 1.7 mmol/L).The expression of visfatin mRNA in abdominal omental adipose tissue was measured with Northern Blot.Visfatin plasma concentration,fasting plasma glucose (FPG),blood lipid profiles and other biochemical indicators were also measured.Results Comparing with normal triglyceride group,the diabetic patients in high triglyceride group had significantly increased levels of visfatin plasma concentration [ ( 129.07 ± 21.35)ng/mL vs ( 101.65 ± 15.23 ) ng/mL,t =2.295,P <0.05]and mRNA expression in omental adipose tissue( P <0.05).Visfatin plasma concentration was positively correlated with TG( β =0.592,P <0.05 ) and FPG( β =0.763,P <0.01 ).Visfatin mRNA expression in omental adipose tissue had no correlation with FPG,TG and other biochemical indicators.Conclusions The plasma visfatin level in T2DM patients may be associated with triglyceride metabolism.
8.Regulation of proliferation and apoptosis of K562 cells by changes of Ras-ERK pathway induced by simvastatin

Objective To investigate the changes in molecule levels of Ras-ERK signal pathway of K562 cells treated with simvastatin in vitro,and to illustrate that simvastatin inducing the changes in molecule levels of Ras- ERK signal pathway is involved in regulation of proliferation and apoptosis of K562 cells. Methods K562 cells,the chronic myelocytic leukemia(CML) cell lines,were cultured and treated with simvastatin in vitro and proliferation activity of K562 cells was detected by MTT. The changes of apoptosis rate and cell cycle of K562 cells were measured by flow cytometry(FCM). The molecular changes of Ras-ERK signal pathway were analyzed by RT- PCR in transcriptional level. Results The proliferation of K562 cells was inhibited by simvastatin,and G_0-G_1 arrested in K562 cells and significant apoptosis rate was observed with FCM. Most molecules of Ras- ERK signal pathway expressed differentially at transctiptional level. Conclusion Simvastatin probablely inhibit proliferation and induces apoptosis of K562 cells,depending on Ras-ERK signal pathway which is involved in cell proliferation and apoptosis.
9.Current status on informational self-management intervention of chronic obstructive pulmonary disease patients

Chronic obstructive pulmonary disease (COPD) is the leading death cause of pulmonary diseases, ranking the fourth of global death causes. Self-management program is able to gradually improve the Airways of COPD patients with ventilation, reduce the degree of lung failure, elevate the health status as well as promoting self-efficacy, which is the key process to effectively control COPD relapse. In recent years, more and more domestic and foreign academic employ the widely-used network technology and carry out a large number of information study on COPD self-management intervention. This article reviewed literatures about the intervention methods on self-management of patients suffered from COPD, aimed at providing a better basis for exploring more effective self-management.
10.Neuroprotection of Hypothermia in Full Term Neonates After Asphyxia: a Safety Study

Objective To study the safety of hypothermia in neonates with hypoxic-ischemic brain damage after asphyxia. Methods Twenty four full term newborns with Apgar score 0.05). Conclusions The results suggest that the selective head cooling in full term newborn has no significant adverse effect on cardiac function, kidney function and coagulation function.
