Objective To observe the effect of sinomenine(SN) in vitro on nuclear factor-?B(NF-?B) DNA binding activity and nuclear translocation of synoviocytes in collagen-induced arthritis(CIA) rats and explore its antiinflammatory mechanisms.Methods The experimental model of CIA rats was used and synoviocytes were collected.Cells were divided into five groups:normal control,CIA,CIA+10 ?mol/L methotrexate(MTX),CIA+50 ?mol/L SN,CIA+500 ?mol/L SN.Nuclear translocation of NF-?B p65 subunit and NF-?B DNA binding activity of synoviocytes were investigated by fluorescence labelling laser confocal scanning microscopy and electrophoretic mobility shift assay(EMSA) respectively.Results Compared to normal control,significant nuclear translocation of NF-?B p65 subunit was observed and NF-?B DNA binding activity was increased in synoviocytes of CIA rats(P

Objective To study the preparation and the in vitro and in vivo release profile of GH-Chitosan-Alginate microcapsules.Methods GH-Chitosan-Alginate microcapsules were prepared through impulsive electrostatic technique.The interrelated factors influencing the diameter and sphericity were studied through orthogonal experiments,and finally the statistic analysis made sure the optimum conditions to prepare microspheres.The morphology and size of the microcapsules were observed,and the content,encapsulation efficiency and recovery efficiency of the microcapsules were measured.Moreover,their in vitro and in vivo release experiments were carried out.Results The results showed that the diameter of needle was the most significant factor to the diameter of microspheres.The optimum conditions for the least diameter of microspheres were 450?m diameter of needle,2cm from needle tips to the gelation surfaee,1.5% alginate concentration,8ml/h speed of flowing-liquid and metal containers.The microcapsules had good sphericity morphology and distribution.The size of the microcapsules was in the range of 10-25?m with an average size of 47.93?m.The encapsulation efficiency and GH-load of the microcapsules were 94% and 11.24% respectively.The release kinetics of microcapsules was studied in false gastric and intestines juice.In false gastric juice,the GH of microcapsules was not released;in false intestines juice,it was released well,and TAM was completely released after about 12h.in vivo release profile made sure that the serum GH level of GH microcapsule group was at the highest value(98.59ng/ml) at 8h.The release profile was fitted well in both in vitro and in vivo conditions.Conclusion GH-Chitosan-Alginate Microcapsules have good morphology and sustained release effect.
3.Effect evaluation for comprehensive treatment of acute stage of stroke

OBJECTIVE: To explore the clinical evaluation system reflecting the superiority and characteristics of comprehensive traditional Chinese medicine (TCM) therapy for acute stroke. METHODS: A randomized controlled trial with single blind in various therapeutic centers was applied on the patients with the acute stage of hemorrhagic stroke due to hypertension, who were allocated to the trial group and the control group. The trial group accepted the general Western medicine therapy and differential treatment of traditional Chinese medicine based on stage classification. Patients in the control group were treated with the general Western medicine and the placebo of traditional Chinese herbal medicine. The treatment effect was assessed at the 7th day, 14th day, 21st day, and 28th day of post-treatment and after 3-month follow-up. The clinical evaluation system included the syndromes of TCM (ZH), Glasgow coma standard (GCS), nerve functional failure (NF), activity of daily living (ADL), Barther index (BI), quality of life index (QLI) and functional activities questionnaire (FAQ). RESULTS: Four hundred and four patients with acute stroke were included. There were 178 cases with yang-syndrome and 21 cases with yin-syndrome in the trial group (n=199), and there were 165 cases with yang-syndrome and 40 cases with yin-syndrome in the control group (n=205). The rates of recovery and obvious improvement after 3-month treatment in the trial group and the control group were 86.5% and 73.6% respectively. There was significant difference between the two groups (P<0.01). The response sensitivity of the effect items indicated that the sensitivity of GCS began to increase from the 7th day and was high at the 14th day and the 21st day for all patients. The response sensitivity of the NF scale was high for all patients at the 14th day, the 21st day and the 28th day. The BI scale and the QLI scale were sensitive to the patients with light and middle stage of stroke at the 21st day, and were sensitive to the patients with light stage of stroke after 3-month follow-up. The FAQ scale was sensitive to the light stroke at the 21st day and after 3-month follow-up. The ADL scale was sensitive to all patients at the 28th day and the patients with light stage of stroke after 3-month follow-up. The ZH scale was sensitive to all patients from 0 to 7d, the 14th day, and the 28th day. The principle components analysis indicated these 7 items could reflect the condition of stroke from 0-7d, the 21st day and after 3-month follow-up, and could be used to evaluate the therapeutic effect of hemorrhagic stroke. ZH scale and FAQ scale were more sensitive than other effect items at the 28th day. CONCLUSION: The seven items (GCSbNFbBIbQLIbFAQbADLbZH) have high sensibility to all patients in the acute stage of hemorrhage stroke especially at the 21st day. The ZH score will change in accord with the condition of stroke, and is appropriate to reflect the condition of stroke. It is believed that the seven items can form the system of effect evaluation in different stages of stroke, and the ZH scale is a very important item.

5.Protective Effects ofDasangju Mixture on Liver Injury Induced by Delayed-type Hypersensitivity in Mice

Objective To investigate the protective effects ofDasangju Mixture on the liver injury induced by delayed-type hypersensitivity in mice; To discuss its mechanism of anti-inflammatory-immunity.Methods After enterocoelia intravenous injection of cyclophosphamide was used, 2,4,6-picryl chloride was used twice for exterior coating for skin of abdomen to cause sensation of skin. Then percutaneous transhepatic puncture was used to attack for modeling. Male Kunming mice were randomly divided into blank group, model group, high-, medium- and low-dose treatment groups. The high-, medium- and low-dose treatment groups were given Dasangju Mixture for gavage, once a day for 7 days. ELISA was applied to determine the serum levels of aspartate aminotransferase (AST), alamine aminotransferase (ALT) and tumor necrosis factor (TNF)-α. Pathological features of liver tissue were observed by HE staining.Results Compared with the blank group, the levels of AST, ALT and TNF-α in the model group increased significantly (P<0.05); Compared with the model group, the levels of AST, ALT and TNF-α in the high-, medium- and low-dose treatment groups decreased significantly (P<0.05,P<0.01,P<0.001); Compared with the model group, high-, medium- and low-dose treatment groups can better alleviate topical inflammation infiltration and edema of liver tissue.ConclusionDasangju Mixture has certain protective effects on the liver injury induced by delayed-type hypersensitivity in mice.
6.Concordance of 2-dimensional and intelligent spatiotemporal image correlation echocardiography in measuring fetal aortic diameters

Objective To evaluate the concordance of two-dimensional echocardiography (2DE) and intelligent spatiotemporal image correlation (iSTIC) in measuring fetal aortic and aortic arch diameters during the second and third trimesters.Methods Data were collected by a prospective cross-sectional study of 140 normal singleton fetuses with the gestational age from 22 to 32 weeks.A total of 6 dimensions of the fetal aortic and aortic arch,including aortic annular diameter (AO),ascending aorta diameter (AAO),aortic arch diameter [AO Arch (INA to LCCA)],aortic arch diameter [AO Arch (LCCA to LSA)],aortic isthmus diameter and descending aorta diameter (DAO),were measured by two different methods.Concordance was assessed by comparing the measurements acquired by iSTIC with those determined by 2DE and depicted by Bland-Altman plots.Inter-and intra-observer variability was evaluated by the intraclass correlation coefficient (ICC) test.Results A total of 137 iSTIC volumes in 140 cases were found to be suitable for further analysis.Good correlation was observed in the measurements that determined by 2D or iSTIC (Pearson's R2 =0.977-0.983).There was no significant difference in the mean values of all the parameters that measured by two methods.Bland-Altman plot showed that the 95% limits of agreement (LOA) in AO,AAO,AO Arch (INA to LCCA),AO Arch (LCCA to LSA),aortic isthmus diameter and DAO were (-0.1260/+ 0.2299),(-0.1707/+ 0.2241),(-0.1547/+ 0.2190),(-0.1736/+ 0.2024),(-0.1514/+ 0.2039) and (-0.1485/+ 0.2228),respectively.The points in the outside of LOA were 5.11% (7/137),4.38% (6/137),5.11% (7/137),5.84% (8/137),4.38% (6/137)and 4.38% (6/137),respectively.Conclusions iSTIC has a good agreement with 2DE in measuring fetal aortic and aortic arch dimensions during the second and third trimesters.
7.Evaluation of right atrial function in patients with dilated cardiomyopathy and ischemic cardiomyopathy using two-dimensional speckle tracking imaging

Objective To evaluate right atrial function in patients with idiopathic dilated cardiomyopathy (IDCM) and ischemic cardiomyopathy (ICM) by using two-dimensional speckle tracking imaging (2D-STI).Methods Study population consisted of 31 patients with IDCM,30 with ICM and 30 healthy subjects.High frame rate two-dimensional images were recorded from the apical four chamber view.Right atrial global longitudinal strain (GLS) was measured using two-dimensional strain soft ware.Results Compared with the controls,left ventricular ejection fraction (LVEF),tricuspid annular plane systolic excursion (TAPSE),right ventricular fractional area change (RVFAC),right ventricular fractional shortening (RVFS) and tricuspid annular peak systolic velocity(S') decreased (P <0.05),while right ventricular myocardial performance index (MPI) increased in IDCM and ICM group.There were no significant differences for all above echocardiographic parameters between IDCM and ICM patients.Compared with the controls,right atrial GLS decreased significantly in patients with IDCM and ICM,even much lower in patients with IDCM (P <0.001).Conclusions Measurement of right atrial strain using 2DSTI could be used for the assessment of right atrial dysfunction in patients with ICDM and ICM.
8.Effort of Xiaoyu-Jiangzhi capsules on carotid artery atherosclerosis in Apolipoprotein E gene knockout mice

Objective:To investigate the effects of Xiaoyu-Jiangzhi capsules on blood lipid, carotid artery atherosclerosis (CAA) and plaque in apolipoprotein E knockout (ApoE -/-) mice. Methods:The ApoE -/- mice were fed with high-fat food to establish carotid atherosclerosis model. The ApoE -/- mice were randomly by weight divided into model group, Atorvastatin group, low- and high-dose Xiaoyu-Jiangzhi capsules group. The C57BL/6cnc mice were used as control group and fed with normal diet. The Atorvastatin group was given atorvastatin suspension 1.3 mg/kg, low and high dose groups were given Xiaoyu-Jiangzhi capsule suspension 325 and 975 mg/kg, and the control group and model group were given equal volume of distilled water. The mice were gavaged with 0.1 ml/10 g body weight, once a day, and the weight of mice was recorded weekly. After 12 weeks of continuous intragastric administration, the blood lipid and liver /body weight index of the mice were measured. Carotid arteries were sliced to conduct oil red O staining and VG staining for the pathological analysis. Results:After 12 weeks of drug administration, the weight of mice in the high-dose group was significantly lower than the model group. The level of TC (25.92 ± 4.21 mmol/L vs. 30.39 ± 4.67 mmol/L) and LDL-C (7.97 ± 2.14 mmol/L vs. 10.26 ± 1.97 mmol/L) in the high-dose group significantly decreased ( P<0.05), the level of HDL-C in the low and high-dose group significantly increased ( P<0.05). The pathological results showed that after 12 weeks of administration, the carotid artery lipid deposition blockage rate in the Atorvastatin group and the high dose group were significantly smaller than the model group( P<0.05), and no vascular plaque has been formed. Conclusion:The Xiaoyu-Jiangzhi capsules could reduce LDL-C, increase HDL-C levels, reduce the constriction of arterial stenosis and slow down the formation process of carotid plaque.
9.Study on changes in complete blood cell count parameters of patients with occupational silicosis

{L-End}Objective To study the changes of complete blood cell count parameters and its influencing factors in patients with occupational silicosis (hereinafter referred to as "silicosis"). {L-End}Methods A total of 354 silicosis patients were selected as the research subjects using judgment sampling method. The patients were divided into stage Ⅰ, stage Ⅱ and stage Ⅲ groups according to the stage of silicosis. Based on the course of the disease, they were divided into groups of ≤3, >3-≤6, >6-≤9 and >9-≤12 years. The peripheral blood of the patients was collected for complete blood cell count analysis, and the influencing factors of complete blood cell count were analyzed by multiple linear regression model. {L-End}Results The levels of hemoglobin and the average red blood cell hemoglobin in patients with silicosis at stage Ⅱ and Ⅲ groups were lower than those at stageⅠgroup (all P<0.05). The percentage and counts of neutrophils increased in patients at stage Ⅲ group (all P<0.05), while the percentage and counts of lymphocytes decreased (all P<0.05) compared with those in stage Ⅰand Ⅱ groups. The percentage of eosinophils in patients at stage Ⅲ was lower than those at stage Ⅰ group (P<0.05). The red blood cell count in the courses of silicosis >6-9 years group was lower (P<0.05), and the percentage of neutrophils was higher, while the percentage of lymphocyte was lower in the courses of silicosis >6-9 years group and >9-12 years group (all P<0.05) compared with the courses of silicosis ≤3 years and >3-6 years groups. The mean corpuscular volume of the courses of silicosis >6-9 years group and the neutrophil count of the courses of silicosis >9-12 years group increased (all P<0.05) compared with the courses of silicosis ≤3 years group. The results of multiple linear regression analysis showed that the silicosis stage and course of silicosis were influencing factors of erythrocyte count (all P<0.05), gender and age of first dust-exposure were influencing factors of hemoglobin level (all P<0.05), while age at diagnosis, duration of dust-exposure, age of first dust-exposure and comorbidities were influencing factors of neutrophil count (all P<0.05). Gender, comorbidities, smoking and silicosis stage were influencing factors of lymphocyte count (all P<0.05). {L-End}Conclusion There are differences in complete blood cell count parameters in patients with silicosis at different stages and courses of the disease. Silicosis stage, course of disease, gender, age, smoking,comorbidities, duration of dust-exposure and age of first dust-exposure were influencing factors affecting complete blood cell count in silicosis patients.
10.Genetic characteristics and prognostic influencing factors of middleˉhighˉrisk multiple myeloma patients stratified based on Mayo Stratification of Myeloma and RiskˉAdapted Therapy consensus guidelines

Objective To investigate the genetic characteristics and prognostic influencing factors of the middleˉhighˉrisk patients with multiple myeloma (MM) based on Mayo Stratification of Myeloma and RiskˉAdapted Therapy (mSMART) consensus guidelines. Methods A total of 179 hospitalized MM patients in Fujian Provincial Hospital from June 2009 to October 2017 were collected. Eventually, 49 patients were included except for the patients who were unable to perform mSMART stratification. According to the mSMART stratification criteria, the patients were divided into lowˉrisk group (24 cases) and middleˉhighˉrisk group (25 cases). The genetic characteristics of the two groups were analyzed to explore the relationship between mSMART stratification and clinical features. KaplanˉMeier method and logˉrank test were used to make survival analysis; logistic regression analysis was used to analyze the prognostic influencing factors in highˉrisk patients. Results The incidence of CSK1B gene amplification was the highest in the lowˉrisk group (41.7%, 10/24), while in the middleˉhighˉrisk group, the incidence of RB1 gene deletion was the highest (88.0%, 22/25). In the lowˉrisk group and the middleˉhighˉrisk group, there were no statistical differences in bone destruction, hypercalcemia, renal damage, anemia, β2 microglobulin abnormality, albumin abnormality, lactate dehydrogenase abnormality, and plasma cell ratio abnormality (all P> 0.05). Survival analysis showed that the median survival time of the middleˉhighˉrisk group was lower than that of the lowˉrisk group (23.19 months vs. 39.71 months, P= 0.043). Multivariate logistic regression analysis found that anemia and bone destruction were risk prognostic influencing factors for mSMART stratification as a middleˉhighˉrisk group (P= 0.044, P= 0.002). Conclusion mSMART stratification could indicate the poor prognosis for the patients with middleˉhighˉrisk, and the anemia and bone destruction are risk prognostic influencing factors for patients with middleˉhighˉrisk stratification.
