1.Comparison of Ilizarov technique and open reduction with internal fixation in treatment of calcaneal fracture

Objective To compare the clinical effect of the treatment for calcaneal fracture by percutaneous reduction combined with Ilizarov stent and open reduction and internal fixation.Methods Data of 35 patients with closed calcaneal fractures from June 2012 to January 2014 were respectively analyzed.13 cases were fixed with poking reduction and Ilizarov support.Among them,there were 9 males and 4 females,aging from 20 to 51 years old (average,34.46± 10.21 years);Sanders type Ⅱ fractures in 2,type Ⅲ in 8 and type ⅣV in 3;Tscheme-Gotzen 2 degree of soft tissue injury in 9 and 3 degree in 4.22 cases were treated with open reduction and internal fixation.Among them,there were 15 males and 7 females,aging from 18 to 60 years old (average,33.59±12.07 years);Sanders type Ⅱ fractures in 3,type Ⅲ in 15,type ⅣV in 4;Tscheme-Gotzen 0 degree of soft tissue injury in 13 and 1 degree in 9.Results There was no significant difference in the anatomic parameters between the two groups before operation.Postoperative calcaneal B(o)hler angle of stent group (27.77°±2.86°) was less than that of the plate group (30.45°±3.45°),and Gissane angle of stent group (131.23°±3.92°) was larger than that of the plate group (127.82°±4.65°);axis angle of stent group (19.23°±2.20°) was less than that of the plate group (22.64°±5.14°);calcaneal width of stent group (33.61±3.43 mm) was less than plate group (36.05±3.26 mm),and the difference have statistical significance.The calcaneal length of the stent group (63.23±5.67 mm) was smaller than that in the plate group (63.59±4.58 mm),but there was no significant difference.The time from the injury to surgery in stent group (4.77±0.83 d) was less than that in the plate group (6.68± 1.84 d),the blood loss of stent group (45.00±6.04 ml) was significantly less than that of the plate group (292.95±43.66 ml).The fracture healing time of stent group (9.31± 1.38 week)was less than that of the steel plate group (9.82± 1.40 week),but there was no significant difference.About limb function American Orthopaedic Foot and Ankle Society (AOFAS) score,7 cases in the stent group were excellent,3 cases good,1 cases fair,2 cases poor;and 9 cases of the plate group were excellent,7 cases good,3 cases fair,3 cases poor,there was no significant difference.Conclusion For the Sanders Ⅱ / Ⅲ type calcaneal fractures and Sanders type ⅣV patients associated with soft tissue damage which are not suitable for open reduction and internal fixation in short term,Ilizarov technique is a good option.
2.Establishment of a normal adult’s pelvis three-dimensional finite element model based on CT image and reverse engineering method

BACKGROUND:Pelvis is the most complicated structure of human body, and many scholars have devoted themselves to its biomechanical study. However, it is difficult to construct the pelvic finite element model containing main muscles and ligaments, as a result of its irregular shape and complicated structure.
OBJECTIVE:To establish a normal human pelvis three-dimensional finite element model and provide digital model for the subsequent biomechanics analysis of pelvis.
METHODPelvic CT images of healthy male volunteers were stored in DICOM format and then put into Mimics 10.01 software for three-dimensional medical image reconstruction. The files were imported into reverse engineering software Geomagic studio 11 for surface processing and establishing surface model;then the surface model was introduced into finite element analysis software Ansys 14.0 for finite element meshing.
RESULTS AND CONCLUSION:A normal adult pelvis three-dimensional finite element model was developed, including 818 294 units and 149 290 nodes. Based on the CT images, we established pelvic finite element model, which have good accuracy, anatomical and geometric similarity, and could provide the basis for subsequent biomechanics analysis of pelvis.
3.Effects of different conserve time on physiological characteristics of allograft nerve transplantation

Objective To observe the effect of different conserve time in ultra deep cryopreservation (-196 ℃)on physiological characteristics of allograft nerve transplantation .Methods A total of 80 female Wistar rats were divide into three groups by random principle:derived group which 20 rats were sacrificed to get both sides of femoral nerves ;control groups which each group had 10 rats include group A for fresh autologous nerve transplantation ,group B for fresh allograft nerve transplantation ;experiment groups which each group had 10 rats including groups C ,D ,E ,F that transplanted after femoral nerves conserve in the -196 ℃ for 3 ,6 ,9 , 12 weeks respectively ,and the results of exterior appearance ,light microscope ,electron microscope were observed and electrophysi-ological test was conducted after transplantation .Results After 9 weeks transplantation :the physiological characteristics of group B was most affected ,followed by group C ,D ,E ,F ,group A was with minimal impact ;the result of electrophysiological test showed that groups A&B ,A&E ,A&F had significant difference(P<0 .05) .Conclusion The physiological characteristics of allograft nerve transplantation relate to the freezing time .
4.Observation on the Effect of Cefotaxime with Different Doses in Periopration Period on the Prevention of Postoperative Infection of Lung Cancer Resection

OBJECTIVE:To observe the effecacy and safety of using cefotaxime using different doses in perioprative period on the prevention of postoperative infection of lung cancer resection. METHODS:61 patients with non-small cell lung cancer who re-ceived lung cancer resection were retrospectively analyzed divided into cefotaxime 2 g group(26 cases) and cefotaxime 4 g group (35 cases)according to dosage. Cefotaxime 2 g group was treated with Cefotaxime for injetion 2 g 30 min before operation,add-ing into 0.9% sodium chloride injection 100 ml,by intravenous infusion,if the operation time was more than 3 h,cefotaxime 2 g was intravenously infused during operation and cefotaxime 2 g was intravenously infused after operation,interval of 12 h was re-quired for preoperative and postoperative medication time and the total medication time was no more than 48 h. Cefotaxime 4 g group was treated with Cefotaxime for injection 4 g(the usage was the same as cefotaxime 2 g group). Leukocyte,hemoglobin,al-bumin,infection rate and incidence of adverse reactions in 2 groups before and after operation were observed. RESULTS:There was no significant difference in the infection rates between 2 groups(P>0.05). Leukocyte,hemoglobin and albumin in 2 groups were significantly lower than before,the differences were statistically significant(P<0.05),however,there was no significant dif-ference between 2 groups(P>0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Small dose of ce-fotaxime has good prevention effect on the postoperative infection of non-small cell lung cancer,with good safety,which meets the principles of rational use of antibiotics.
5.Treatment of intraarticular calcaneal fractures using poking reduction combined with Ilizarov method

Objective To introduce the treatment of intraarticular calcaneal fractures with poor soft tissue using poking reduction combined with Ilizarov methods. Methods From September 2007 to September 2009,11 patients with the intraarticular calcaneal fractures were treated with poking reduction combined with Ilizarov methods,including 10 males and 1 female at average age of 33.6 years(range,21-50 years).Four patients had the left calcaneal fractures and seven with the right ones.According to Sander classification,there were seven patients with type II fractures,three with type III fractures and one with type IV fracture.The various extents of heel soft tissue injury were emerged in all the patients.Before operation,B(o)hler angle was 0°-12°(average 8°)and Gissane angle 70°-95°(average 80°).According to AO classification of close soft tissue injury,there were three patients with type IC2,six with type IC3,one with type IC5 and one with open soft tissue injury.The collapse of the articular surface was recovered by Steinmann pin which passed through the lateral calcaneal tubercle.The medial and lateral fracture fragments were corrected by using the tensed olive wires.The heighat and width were maintained by using the Ilizarov external fixation. Results The mean duration of follow-up was 13.3 months (range,6-20 months).All fractures were healed after mean duration for 8.1 weeks(range,7-10weeks).The postoperative B(o)hler angle was 20°-45°(average 33°)and Gissane angle 100°-118°(average 107°).According to Maryland Foot Score system,the result of postoperative function was excellent in seven patients,good in three and fair in one,with excellence rate of 91%. Conclusions For the intraarticular calcaneal fractures,the poking reduction combined with Ilizarov method is able to recover and maintain the normal calcaneal height and width and reduce postoperative wound infections and many other complications.specially for Sanders II or III with severe soft tissue injury in the open or closed calcaneal fractures.Master of surgical indications and operating poims can attain satisfactory clinical efficacy.
6.The treatment of severe open Pilon fractures

Objective To discuss the operative methods, timing and outcomes of severe open Pilon fractures. Methods From April 2003 to July 2008, 21 patients with open Pilon fractures were admitted. All the patients were type C fracture according to AO/OTA classification, including 17 cases of type C2 and 4 cases of type C3. According to Gustilo classification, there were 18 cases of type Ⅱ, 2 cases of type ⅢA, 1 case of type ⅢB. The patients were treated in two different ways: one-stage minimally invasive osteosynthesis for reconstruction of the articular surface with transarticular external fixation of the ankle (19 cases),which included 16 cases of Gustilo Ⅱ AO/OTA C2, 2 cases of type Gustilo Ⅱ AO/OTA C3 and 1 case of type Gustilo ⅢA AO/OTA C2. Formal open reconstruction of the articular surface by plating and external fixation was performed when the condition of soft tissue had improved (2 cases), which included 1 cases of type Gustilo ⅢA AO/OTA C3 and 1 cases of type Gustilo Ⅲ B AO/OTA C3. Results All the patients were followed up from 6 to 48 months, with the mean of 24 months. The outcome of reduction was evaluated by the Burwell-Charnley score. Anatomical reduction was found in 6 cases, function reduction was in 14 cases and poor reduction was 1 case. The heal-time ranged from 2.5 to 11 months, with the mean of 4.7 months. The delayed union occurred in 2 cases. The rate of delayed union was 9.5%. Two patients experienced skin superficial necrosis. There were superficial infection in 2 cases and deep infection in 1 case. The infection rate was 14.3%. Eight cases experienced early traumatic arthritis. The incidence of this complication was 38.1%.According to American Orthopedic Foot Ankle Society (AOFAS) scale for ankle joint, the result of the treatment was 66-94, with an average of 85.2. Conclusion It is important to have the optimal timing of surgery and reliable fixation according to fracture classification, the condition of soft tissue and time after injury. It is the key to evaluate the condition of soft tissue and protect the blood supply.
7.Designment and application of a novel device for microcirculation viviperception

The key technology of the device for the viviperception of the animal mesenteric microcirculation is to simulate the celiac environment in the device. The technical requirements of the device for microcirculation viviperception are that the observation box should be able to "keep warm, preserve moisture, continually perfuse, and fix the sample"; and the lighting should be "intense", "convergence", and "cool". After actual application, it was found that the newly designed and developed the device by research personnel of Wannan Medical College for the viviperception of the animal mesenteric microcirculation can meet the technical requirements, which is able to "keep warm, preserve moisture, continually perfuse, and fix the sample", and using LED lamp as the microscope light source is "intense", "convergence", and "cool". This device is ingenious and reasonable in design, stable in technology, convenient in operation, and competent in microcirculation viviperception. It solves the technical problem to simulate the celiac environment for mesenteric microcirculation viviperception. The device provides convenience to observe and study the microcirculation, which is worth to be applicated widely.
8.Screening and identification of the polypeptides specifically binding to the adhesion protein of Myco-plasma genitalium

Objective To screen and identify the polypeptides specifically binding to the adhesion protein of Mycoplasma genitalium(MgPa) by using the Ph. D.-12TM phage display peptide library for further understanding the biological function and the possible pathogenic mechanism of the MgPa. Methods The Ph. D.-12TM phage display peptide library was used for 3 rounds of biopanning with the purified recombinant MgPa ( rMgPa) as the given target. The phages were collected for amplification after biopanning. The single strand DNA of phage clones were extracted and purified by using the sodium iodide method for further se-quencing. ELISA, competitive binding assay and dot immunobinding assay were performed to analyze the specific binding of positive phages to rMgPa. Results A significant enrichment of phages was achieved after 3 rounds of biopanning. Eleven different phage exogenous sequences (P1-P11) were detected among the 38 phages randomly selected from the agar. Two core sequences were deduced according to the repeating times of amino acids among the 11 polypeptide sequences, which were V-H-W-D-F-R-Q-W-W-Q-P-S and D-W-S-S-W-V-H/Y-R-D-P-Q-T/S. Ten out of the 11 representative phages ( P1-P10 ) specifically combined with the rMgPa. Conclusion Two polypeptides specifically binding to rMgPa were successfully screened out, which provided the tool for further investigation on the biological function of MgPa and the pathogenic mecha-nism of Mycoplasma genitalium.
9.Features of Cervical Vertebra X-ray Examination and Transcranial Doppler in Children with Cervical Vertigo

Objective To analyze the features of cervical vertebra X-ray examination and transcranial Doppler (TCD) in children with cervical vertigo, and investigate the mechanism of the disease. Methods X-ray examination of cervical vertebra and TCD were performed on 80 children with cervical vertigo, and the features of the results were analyzed. Results The abnormal results of cervical X-ray examination mainly showed atlantoaxial joint or/and C2, C3 vertebral joint disorders, some of which combined with cervical instability, cervical curvaturestraightened and even reverse curvature, cervical scoliosis. The abnormal results of TCD showed the blood flow velocity in single or multiple branch of artery increased mainly, some of which decreased or became asymmetrial. Conclusion The cervical vertigo in children may result from various kinds of disorders of the atlas, axis or/and C2, C3 vertebrae.
10.Prognostic significance of neutrophil to lymphocyte ratio in patients with hepatocellular carcinoma

Objective To compare the clinical significance of peripheral blood neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR) in predicting prognosis of patients with hepatocellular carcinoma (HCC).Methods The clinical data of 661 patients with HCC were retrospectively analyzed.Routine peripheral blood test results were used to calculate the NLR and PLR,and the receiver operating characteristic (ROC) curves were drawn.Using the thresholds of NLR and PLR,the patients were divided into the low NLR group and the high NLR group,and the low PLR group and the high PLR group.Overall survival (OS) and disease free survival (DFS) were evaluated by the Kaplan-Meier method.Independent prognostic predictors were determined by the Cox proportional hazard model.Results The NLR and PLR thresholds were 2.790 and 99,respectively.Analysis of the ROC curves showed higher NLR and PLR were significantly associated with poorer OS and DFS (all P < 0.05).Multivariate analysis showed that NLR was an independent risk factor of OS and DFS (both P < 0.05).The results remained unchanged when the NLR was further analyzed by applying different cut-off values of 2.810 and 3.In subgroup analysis,NLR remained an independent factor of Barcelona Clinical Liver Cancer staging system (BCLC) 0/A/B (P < 0.05 for all measurements).Conclusion An elevated preoperative NLR could be a better prognostic predictor for HCC patients in comparison with PLR,especially for BCLC 0/A/B patients.
