1.Effects of Moldavica Total Flavone on Vascular Endothelial Growth Factor, Intercellular Adhesinon Molecule-1 and Other Cytokines in Bronchial Asthmatic Rats

Objective To investigate the effects of moldavica total flavone on vascular endothelial growth factor (VEGF), intercellular adhesion molecule-1 (ICAM-1), nitric oxide (NO) and endothelin-1 (ET-1) in ovalbumin-induced bronchial asthmatic rats, and explore its mechanism. Methods A total of 60 SD rats was randomly divided into control group, model group, dexamethasone (1 mg/kg) group and moldavica total flavone low (90 mg/kg), medium (180 mg/kg) and high (360 mg/kg) dose group. Rats asthma model was established by ovalbumin challenge methods except the control group. After modeling, rats in control and model groups were intragastrically given distilled water, rats in medicine groups were intragastrically given moldavica total flavone and dexamethasone for 30 d. VEGF in lung tissue and ICAM-1 in bronchoalveolar lavage fluid (BALF) were detected respectively by ELISA. The content of ET-1 and NO in BALF were analyzed with radioimmunity method and nitric acid reductase method. Results The VEGF in lung tissue and ICAM-1, NO and ET-1 in BALF were decreased by moldavica total flavone at 180 and 360 mg/kg (P<0.05, P<0.01) in ovalbumin-induced bronchial asthmatic rats. Conclusion Moldavica total flavone is effective in adjusting abnormal increase of VEGF, ICAM-1, ET-1 and NO of bronchial asthma rats.
2.A prospective study of tubular stomach gastroesophagostomy on the life quality after with esophageal cancer esophagectomy surgery of postoperative patients

Objective To evaluate the application of tubular stomach gastroesophagostomy on the life quality after esophagectomy surgery.Methods From March 2011 to September 2011,66 cases of esophageal cancer patients were divided in two groups in randomized principles.Tirty-four cases in group of tubiform gastro surgery whose the stomach was cut into tubiform stomach in width of 4 cm.And 32 cases of conventional group and whose surgery method was adopted to maintain whole stomach but cut the cardia.And then,using statistical methods,life quality after esophagectomy (treatment related symptoms,general symptoms,daily life,social life,emotional activities) and the perioperative complications were evaluated in 3,6,12,24 months in comparison and analysis.Results Sixty-six surgeries of both group patients were successfully performed,tumor resection rate was 100%,and there was no intraoperative accident also no perioperative death.There was no statistical significance of perioperative complications of 2 groups (P > 0.05).Life quality of tubiform gastro group had improved apparently more than conventional group at the 3rd,6th,12th and 24th month after surgery(78.68 ±2.23 vs 65.94± 4.13,83.79 ± 2.40 vs 66.84 ± 5.86,86.41 ± 1.94 vs 67.09 ± 4.43,89.02 ± 1.71 vs 68.88 ±3.53,P<0.05).At each time point,there were statistically significant differences between tubiform gastro group and conventional group on treatment related symptoms,general symptoms,daily life,emotion activities (P <0.05).At the 3rd,6th month after surgery,tubiform gastro group and the conventional group had no statistically significant difference in the aspect of social life (P>0.05).In the conventional group,the evaluation of quality of life at each time point after surgery and before a point in time in the same group showed no statistical significance (P>0.05).Conclusion Tubiform gastro esophagectomy has a positive influence on life quality of patients of esophageal cancer after surgery prospectively.
3.Effect of Moldavica Total Flavone on Pulmonary Function in Ovalbumin-induced Bronchial Asthmatic Rats

Objective To investigate the effect of moldavica total flavone on pulmonary function and collagen type Ⅲ in lung tissue of ovalbumin-induced bronchial asthmatic rats. Methods A total of 60 SD rats were randomly divided into control group, model group, dexamethasone group, and moldavica total flavone low, medium and high dose group. Rat asthma model was established by ovalbumin challenge methods except the control group. The control and model group were intragastrically given distilled water, medicine groups were intragastrically given moldavica total flavone and dexamethasone for 30 d, once per day. After the last administration, the pulmonary function and airway responsiveness including breathing frequency (F), minute ventilation (MVb), peak inspiratory flow (PIFb), peak expiratory flow (PEFb) and enhanced pause (Penh) were measured by using non-invasive measurement system (Penh system, Buxco, USA). The collagen type Ⅲ in lung tissue were detected by ELISA. Results The moldavica total flavone at 180 and 360 mg/kg decreased F and Penh (P<0.05, P<0.01), increased MVb, PIFb and PEFb (P<0.05), and decreased collagen type Ⅲ in lung tissue (P<0.05, P<0.01). Conclusion Moldavica total flavone can relieve the airway hyperresponsiveness and remodeling, improve respiratory function of ovalbumin-induced bronchial asthmatic rats.
4.Effects of Capparis Spinosa Total Alkaloid on TypeⅢ Collagen Expression in Systemic Sclerosis Mice

Objective To investigate the effects of capparis spinosa total alkaloid on the pathological changes and the type Ⅲ collagen(COL?Ⅲ)expression in systemic sclerosis(SSc)mice. Methods Mice models with SSc were established by repeated local injection of bleomycin in BALB/c mice back. After administration of capparis spinosa total alkaloid ,the pathological changes of skin and lung tissue were observed ,and the COL?Ⅲ expression was detected by ELISA. Results Compared with the model group,the inflammation and fibrosis of skin and lung tissue were improved,and the level of COL?Ⅲ was markedly reduced by treatment of high dose capparis spinosa total alkaloid(P<0.05). Conclusion Cap?paris spinosa total alkaloid is effective in treating fibrosis of SSc.
5.Effects of Capparis Spinosa Total Alkaloid on MMP-9 and TIMP-1 Levels in Systemic Sclerosis Mice

Objective To investigate the effects of capparis spinosa total alkaloid on type collagen (ColⅣ - ),Ⅳmatrix metalloproteinase-9 (MMP-9), tissue inhibitor of metalloproteinase-1 (TIMP-1) and plasminogen activator inhibitor-1 (PAI-1) in bleomycin-induced systemic sclerosis (SSc) mice; To explore the effective mechanism of capparis spinosa total alkaloid on fibrosis of SSc. Methods Totally 90 BALB/c mice were randomly divided into control group, model group, penicillamine group and capparis spinosa total alkaloid low-, medium- and high-dose group. Mice models with SSc were established by repeated local injections of bleomycin in mice back, except for the control group. Mice in medication groups received external application with capparis spinosa total alkaloid cream;mice in penicillamine group were given penicillamine for gavage; mice in the control and model group received external application without substance, one time a day, for 60 days. The contents of MMP-9, TIMP-1 and PAI-1 in serum and Col- in skin tissue were dⅣ etected respectively by ELISA after the last medication. Results Compared with the model group, the levels of MMP-9 and ratio of MMP-9/TIMP-1 markedly increased and the levels of Col-Ⅳand TIMP-1 markedly decreased in medium and high- dose of capparis spinosa total alkaloid group (P<0.05, P<0.01). But the level of PAI-1 was not influenced (P>0.05). Conclusion Capparis spinosa total alkaloid is effective in treating fibrosis of SSc by adjusting imbalance of MMP-9/TIMP-1 and decreasing expression of Col-Ⅳ.
6.Analysis of the effects of da Vinci robotic surgery in the treatment of thymic diseases

Objective Although minimally invasive thoracoscopic surgery has unique advantages, there are still some difficulties in the complete resection of the thymus.Video-assisted thoracoscopic surgery has made up for the lack of thoracoscopy.This article summarize and analyze the clinical effects of DaVinci robotic surgery and thoracic surgery in the treatment of thymic diseases.Methods 116 cases of patients with thymic lesion were included in the study.According to the different treatment methods, 65 cases were included in da Vinci robotic group and other 51 cases were thoracoscopic group.Comparison was made in operating time, time of chest tube extubation, bleeding volume during surgery and amount of drainage within 24h after the operation between two groups.Results There were no statistical differences between robot group and thoracoscopic group in operating time(P>0.05).The time of pulling out the chest tube in robot group, postoperative days in surgical intensive care unit (SICU), and postoperative hospitalization days were significantly shorter than those in thoracoscopic group ([2.54±0.32]d vs [2.87±0.49]d, [0.75±0.04]d vs [0.81±0.06]d, [5.17±0.15]d vs [5.50±0.23]d, P<0.05).The bleeding volume during surgery and amount of drainage within 24h after the operation in robot group had a significant advantage over those in thoracoscopic group ([125.7±7.5]mL vs [136.6±8.2]mL, [117.38±11.45]mL vs [122.41±13.14]mL, P<0.05).Conclusion The da Vinci robotic surgery has the advantages of minimal trauma and rapid recovery, which makes up for the deficiency of thoracoscopy to a certain extent.
7.Effects of Capparis Spinosa Total Alkaloid on Transforming Growth Factor-β1/Smad4 Signaling Pathways in Systemic Sclerosis

Objective To evaluate the effects of capparis spinosa total alkaloid on transforming growth factor-β( TGF-β)/Smad4 signalling pathways in systemic sclerosis (SSc). Methods A total of 90 BALB/c mice were randomly divided into normal control group, model control group, penicillamine ( 125 mg·kg-1) group and Capparis spinosa total alkaloid low (225 mg·kg-1),medium (450 mg·kg-1) and high ( 900 mg·kg-1) dose group. Except for the normal control group,SSc mouse model was established by daily subcutaneous injection of bleomycin in the back of the mice.After the establishment of the model,Capparis spinosa total alkaloid emulsifiable paste was externally applied to Capparis spinosa total alkaloid group,ground substance was externally applied to the mice in normal control group and model control groups, and penicillamine was intragastrically administrated in the penicillamine group for 60 days,once daily.After the treatment,The expression of TGF-β1in skin tissue was detected by Western-blotting and the levels of actin / in receptor-like kinase / Smad4, nuclear factor-κB in skin tissue were measured by ELISA. Results The expression of TGF-β1was significantly decreased after administration of 225,450 and 900 mg·kg-1capparis spinosa total alkaloid, and the levels of ALK1 and Smad4 were significantly decreased after administration of 900 mg·kg-1capparis spinosa total alkaloid as compared with model control group (P<0.05 or P<0.01),but the content of NF-κB was not influenced (P>0.05). Conclusion Capparis spinosa total alkaloid can accommodate abnormal expression of TGF-β1/Smad4 signalling pathways in SSc.
8.A meta-analysis compare rapid rhino with merocel for nasal packing.

OBJECTIVE:
To assess the efficacy and adverse reaction of nasal packing materials Rapid Rhino and Merocel.
METHOD:
We searched the database PubMed, EMBASE, Cochrane Library, CBM, CNKI, VIP and WANFANG database on line by computer, and traced the related references. Randomized controlled trials(RCTs) of rapid rhino and merocel as nasal packing materials were included. The quality of the included documents was evaluated by the criterion of Cochrane handbook 5.1. The cochrane collaboration's Revman 5.1 software was used for data analysis.
RESULT:
Four RCTs involving 115 patients were identified. Meta-analyses showed that Rapid Rhino produced significantly lower pain and discomfort during insert of pack [MD = 1.37, 95% CI (0.13, 2.60), P < 0.05], whereas less pain and discomfort during removal of pack [MD = 2.88, 95% CI (2.34, 3.41), P < 0.01]. Rapid Rhino associated with a significantly slighter degree of fullness raised after insertion [MD = 1.15, 95% CI (0.75, 1.55), P < 0.01, and the same situation happened after 6 hours [MD = 1.15, 95% CI (0.75, 1.55), P < 0.01]. Rapid rhino caused to less reactionary bleeding when pack removal [MD = 0.26, 95% CI (0.12, 0.39), P < 0.01], rapid rhino was easier for the healthcare worker during insert and removal. There was no significant difference between two packs on the efficiency of hemostatic [OR = 1.00, 95% CI (0.38, 2.61), P > 0.05].
CONCLUSION
The application of Rapid Rhino caused less pain and fullness, leaded to slighter bleed than Merocel when insertion and removal. There was no significant difference between two packs on the efficiency of hemostatic when used for epistaxis or after routine nasal surgery.

Except for the blank control group,SSc mouse model was established by daily subcutaneous injection of bleomycin in the back of the mice.After establishing model successfully,Capparis spinosa total alkaloid emulsifiable paste was externally applied with Capparis spinosa total alkaloid group,ground substance was externally applied to the mice in blank control group and model control groups,penicillamine was intragastrically administrated in the penicillamine group for 60 days,once daily.After the treatment,the content of VEGF in skin tissue and ET-1,sVCAM-1 in serum were measured by ELISA. Results The levels of VEGF and ET-1 were significantly decreased in Capparis spinosa total alkaloid high dose group as compared with model control group(P<0.05, P<0.01),but the content of sVCAM-1 wasn't influenced(P>0.05). Conclusion Capparis spinosa total alkaloid is effective in adjusting abnormal expression of VEGF and ET-1 in SSc mice.
10.Construction and application of 3D printing model of endoscopic endonasal transsphenoidal surgical training

Objective The aim of our study was to establish an endoscopic endonasal transsphenoidal surgical training model, and to examine its application value. Methods The endoscopic endonasal transsphenoidal surgical training models were made.After assessments,the models was used for endoscopic operation training for 8 neurosurgeons with no experiences of endoscopic operation. Results The available operating space of this model is similar to the actual operating space in endoscopic endonasal transsphenoidal surgery. The egg fixed on the model has many layers of structure, such as eggshell, shell membrane, egg white and egg yolk, which constituted a good practice object. All students indicated that they could benefit from the training using this model. The skill in grinding the eggshell [(1.07 ± 0.221)cm2/min vs.(1.45±0.27)cm2/min, P<0.001]was significantly improved and the chance for shell membrane rupture (8 vs. 2, P=0.007) was significantly reduced in the last-time practice compared with the first-time practice (P<0.001). Conclusion The endoscopic endonasal transsphenoidal surgical training model is helpful for neurosurgeons to practice the basic operation of endoscopic surgery and to improve their surgical skills,and can be used repeatedly.It can be used in basic operation training before the training using cadaver cranium.
