1.Green tea in chemoprevention of prostate cancer

Green tea as a main beverage is consumed worldwide. In the last few years, .green tea has been shown to induce apoptosis and to inhibit the infiltration and metastasis of prostate cancer by in vitro and in vivo experiments. Epidemiologic investigations and clinical trials also have demonstrated the chemopreventive role of green tea in prostate cancer, such as reducing the possibility of carcinogenesis of high-grade prostatic in-traepithelial neoplasia, and is hopeful to be applied as chemopreventive agent of prostate cancer in the future.
2.Ethical Reflection on the Physio-psychic-social Assessment of Living Related Organ Transplantation

There is an ascending tendency in proportion of living related organ transplantation in nowadays China.However,the problems involved are more complicated than those involved in cadaveric and xenogenic organ transplantation.Therefore,a synthetic pre or post-operative physiological-psychic-social assessment to the donor and the receptor during living related organ transplantation is needed for a better understanding of the operations and a more reasonable application.Then not only the destination of the medical value can be achieved,but also the value of donor and receptor's lives can be fully respected.More importantly,our aim of developing and perfecting human life and eventually supplying better service for overall interests of human would be realized.
3.Progress on application of radiomics in precise treatment of non-small cell lung cancer

Radiomics utilizes the high-throughput extraction of large amounts of quantitative features from radiographic images,giving a comprehensive quantification of tumor phenotype.Thus it can provide complementary and interchangeable information for clinical usage,such as differentiating malignant nodules from benign ones,predicting response to treatment,identifying lymph node metastasis,improving individualized treatment selection and monitoring.The advantages of radiomics give it great potential in precise treatment.But much work needs to be done before it could be used in practice.It is imperative that a standard research procedure is needed to verify its reliability and clinical value via multi-central prospective clinical trials.
4.Effect of water storage on the flexural properties of three dual-cure core buildup composite resins

BACKGROUND:Hydrothermal condition of oral environment has an effect on the mechanical properties of dental core materials.
OBJECTIVE:To evaluate effect of water storage on the weight change rates and flexural properties of three dual-cure core buildup composite resins.
METHODS: Rectangular specimens (25 mm×2 mm×2 mm) were prepared from three dual-cure core buildup composite resins (LuxaCore Smartmix Dual, Para core and Clearfil DC Core). Specimens were stored in distiled water at 37℃ for 0, 1 day, 1, 2, 3 weeks and 1 month, respectively, weighted and then subjected to three-point bending test in a universal testing machine.
RESULTS AND CONCLUSION: Weight change rates of the three composite resins increased with immersion time; at the same immersion time, weight change rates of three composite resins ranked as the folowing order: LuxaCore Smartmix Dual > ParaCore > Clearfil DC Core. The flexural strength of LuxaCore Smartmix Dual decreased with immersion time. The flexural strength of ParaCore showed no significant change during 3 weeks immersion (P > 0.05), while it showed a significant decreasing after 1 month immersion (P < 0.05). There was no significant change on the flexural strength of Clearfil DC Core in the whole immersion process(P > 0.05). The flexural modulus of LuxaCore Smartmix Dual showed a decreased trend in the whole immersion process, but it showed that there was no significant change during the first 2 weeks(P > 0.05), while a significant decrease occurred after 3 weeks and 1 month (P < 0.05). Compared with original specimens, the flexural modulus of ParaCore and Clearfil DC Core showed a decreased trend, but this change was not significant (P > 0.05); the flexural properties of three composite resins showed a fluctuant decreased trend along with weight changes increased. To sum up, Clearfil DC Core showed the best water resistance among three composite resins.
5.Nosocomial Stenotrophomonas maltophilia Infection after Cardiac Surgery

OBJECTIVE To investigate the characteristics and antibiotics resistance of nosocomial infection caused by Stenotrophomonas maltophilia after open-heart operation.METHODS The clinical data of 324 patients with microbiologically documented nosocomial infection from Jan 2004 to Dec 2006 were retrospectively analyzed.RESULTS There were 34 infected cases caused by S.maltophilia,and accounted for 10.5% of all infections during the same period.All infected patients had serious original heart diseases,and received previous glucocorticoids and broad-spectrum antibiotic therapy.Thirty patients prolonged mechanical ventilation time for over 2 weeks.Results of susceptibility test showed that trimethoprim-sulfamethoxazole,ticarcillin-clavulanic acid and levofloxacin were the most active antibiotics,followed by ceftazidime and ciprofloxacin.S.maltophilia presented high resistantce to carbopenems and aminoglycoside antibiotics.CONCLUSIONS Nosocomial S.maltophilia infection is associated with the severity of preoperative underlying heart diseases,prolonged mechanical ventilation and longer period of broad-spectrum antibiotic therapy,especially carbopenems antibiotics.
6.Treatment of intraarticular calcaneal fractures using poking reduction combined with Ilizarov method

Objective To introduce the treatment of intraarticular calcaneal fractures with poor soft tissue using poking reduction combined with Ilizarov methods. Methods From September 2007 to September 2009,11 patients with the intraarticular calcaneal fractures were treated with poking reduction combined with Ilizarov methods,including 10 males and 1 female at average age of 33.6 years(range,21-50 years).Four patients had the left calcaneal fractures and seven with the right ones.According to Sander classification,there were seven patients with type II fractures,three with type III fractures and one with type IV fracture.The various extents of heel soft tissue injury were emerged in all the patients.Before operation,B(o)hler angle was 0°-12°(average 8°)and Gissane angle 70°-95°(average 80°).According to AO classification of close soft tissue injury,there were three patients with type IC2,six with type IC3,one with type IC5 and one with open soft tissue injury.The collapse of the articular surface was recovered by Steinmann pin which passed through the lateral calcaneal tubercle.The medial and lateral fracture fragments were corrected by using the tensed olive wires.The heighat and width were maintained by using the Ilizarov external fixation. Results The mean duration of follow-up was 13.3 months (range,6-20 months).All fractures were healed after mean duration for 8.1 weeks(range,7-10weeks).The postoperative B(o)hler angle was 20°-45°(average 33°)and Gissane angle 100°-118°(average 107°).According to Maryland Foot Score system,the result of postoperative function was excellent in seven patients,good in three and fair in one,with excellence rate of 91%. Conclusions For the intraarticular calcaneal fractures,the poking reduction combined with Ilizarov method is able to recover and maintain the normal calcaneal height and width and reduce postoperative wound infections and many other complications.specially for Sanders II or III with severe soft tissue injury in the open or closed calcaneal fractures.Master of surgical indications and operating poims can attain satisfactory clinical efficacy.
7.The treatment of severe open Pilon fractures

Objective To discuss the operative methods, timing and outcomes of severe open Pilon fractures. Methods From April 2003 to July 2008, 21 patients with open Pilon fractures were admitted. All the patients were type C fracture according to AO/OTA classification, including 17 cases of type C2 and 4 cases of type C3. According to Gustilo classification, there were 18 cases of type Ⅱ, 2 cases of type ⅢA, 1 case of type ⅢB. The patients were treated in two different ways: one-stage minimally invasive osteosynthesis for reconstruction of the articular surface with transarticular external fixation of the ankle (19 cases),which included 16 cases of Gustilo Ⅱ AO/OTA C2, 2 cases of type Gustilo Ⅱ AO/OTA C3 and 1 case of type Gustilo ⅢA AO/OTA C2. Formal open reconstruction of the articular surface by plating and external fixation was performed when the condition of soft tissue had improved (2 cases), which included 1 cases of type Gustilo ⅢA AO/OTA C3 and 1 cases of type Gustilo Ⅲ B AO/OTA C3. Results All the patients were followed up from 6 to 48 months, with the mean of 24 months. The outcome of reduction was evaluated by the Burwell-Charnley score. Anatomical reduction was found in 6 cases, function reduction was in 14 cases and poor reduction was 1 case. The heal-time ranged from 2.5 to 11 months, with the mean of 4.7 months. The delayed union occurred in 2 cases. The rate of delayed union was 9.5%. Two patients experienced skin superficial necrosis. There were superficial infection in 2 cases and deep infection in 1 case. The infection rate was 14.3%. Eight cases experienced early traumatic arthritis. The incidence of this complication was 38.1%.According to American Orthopedic Foot Ankle Society (AOFAS) scale for ankle joint, the result of the treatment was 66-94, with an average of 85.2. Conclusion It is important to have the optimal timing of surgery and reliable fixation according to fracture classification, the condition of soft tissue and time after injury. It is the key to evaluate the condition of soft tissue and protect the blood supply.
8.Comparative studies on cariogenic bacteria of the root surface before and after post-core crown restoration in aged people.

OBJECTIVETo investigate the changes in the quantity of colonizing Streptococcus mutans(S. mutans) and Actinomyces on the root surface plaque before and after post-core crown restoration of the mandibular first molars in the elderly patients.
METHODSA total of 30 elderly patients, each with one post-core crown restoration of the mandibular first molar, were randomly chosen to participate in the studies. Patients with mandibular first molars with post-core crown restoration and those with healthy contralateral mandibular first molars were divided into the test and control groups, respectively. Root surface plaques of the two groups were collected before tooth preparation, 72 h after preparation, one week after preparation, and one month after restoration. S. mutans, Actinomyces naeslundii (A. naeslundii) and Actinomyces viscosus (A. viscosus), were identified using colony morphology, biochemical techniques, and polymerase chain reaction (PCR). Plaque count was measured using microbial colony count.
RESULTSThe number of S. mutans and A. viscosus and A. naeslundii in the test group, which was statistically significant (P<0.05), increased 72 h after preparation. The quantities of S. mutans, A. viscosus, and A. naeslundii one week after preparation were significantly different (P<0.05). The plaque count of S. mutans, A. viscosus, and A. naeslundii in the test group decreased one month after restoration (P<0.05).
CONCLUSIONThe quantities of S. mutans, A. viscosus and A. naeslundii increase one week after preparation but decrease one month after restoration. The finding suggests that dentists should educate patients about plaque control during the early period after tooth preparation.

9.Modular flexible ureteroscope combined with holmium laser lithotripsy for treatment of renal calculi: report of 46 cases

Objective To evaluate the efficiency and clinical value of modular flexible ureteroscope combined with Holmium laser lithotripsy for treatment of renal calculi. MethodsA total of 46 patients with renal calculi were treated with modular flexible ureteroscope combined with Ho:YAG lithotripsy in our hospital from December 2009 to May 2011.There were 19 cases in middle and upper calyx,8 cases in lower calyx,15 case in renal pelvic,and 4 cases in multiple calyxes.The calculi diameter ranged from 6mm to 31 mm (average in 15 ± 7.9 mm).Flexible ureteroscope was used to find renal calculus under epidural anesthesia,and Holmium laser lithotripsy was used at a maximum energy of 12 to 20W (0.8 - 1.0 J/15 -20Hz).F5 or F6 double-J tubes and catheters were indwelled routinely postoperatively.-examine with KUB and removethe catheter 2 days after operation.Re-examine with B-ultrasonography or KUB 4 weeks after op-eration to evaluate the stone free rate.Residual stones ≥4 mm were regarded as clinical significance of residual stone fragments. Results 45 operations were successful except one case changed to percutaneous nephrolithotomy (PCNL) because of ureteral stricture.The operation time was 45 -150 min (average in 110.4 ±25.3 min).The patients were discharged from hospital in 2 -5 days(average in 3 days) after operation.The stone detection rate was 95.6% (44/46),and the stone free rate was 86.9% (40/46) after one operation. No severe complications such as ureteral perforation or hemorrhea occurred. Conclusions Modular flexible ureteroscope combined with Holmium laser lithotripsy is effective and safe for treatment ofrenal calculi,meanwhile it can decrease the high maintenance costs of integrated flexible ureteroscope.
10.Diagnosis and treatment of thrombosis of right hepatic artery after surgery for severe pancreatitis

The treatment for severe pancreatitis includes surgical and non-surgical methods,and the key points of treatment include surgical timing,surgical method selection and the management of postoperative complications.Hepatic artery thrombosis after surgery for severe pancreatitis is rarely seen,and few experiences in the diagnosis and treatment for this disease have been summarized.A patient with the course of severe pancreatitis of 10 years and suffered from 3 different kinds of diseases including thrombosis of right hepatic artery was cured by open surgery for 2 times and intervention therapy in the Affiliated Hospital of Hainan Medical College in October 2011.The treatment experience was summarized based on the clinical data of this patient.
