1.In Vitro Antifungal Activity of Azone Against Dermatophytes and Candida Aibicans

OBJECTIVE:To observe the antifungal activity of azone against dermatophytes and candida albicans.METHO_ DS:The MIC of azone was determined by NCCLS M27,M38method and MTT assay.MFC was also determined.RESULTS:MIC and MFC of azone against dermatophytes were0.02%and0.08%respectively.CONCLUSION:Azone has significant action in inhibiting candida albicans and fungicidal effect on dermatophytes.
2.Effects of different penetration enhancer on penetration of ciclopirox olamine and salysic acid in percutaneous penetration

AIM: To study the enhancing effect of penetration enhancer on percutaneous absorption of antifungal agents. METHODS: Percutaneous penetration experiment was performed on device in vitro, and the deposit effects were observed with vivoperception. RESULTS: Combination of 1-menthol (1%) and azone (1%) showed the best effect on percutaneous absorption of cidopicox olamine, and 1-menthol (1%) had the best effect on percutaneous absorption of salysic acid. The compound penetration enhancer, consisting of 1-menthol (1%), azone (1%), propylene glycol ( 2.5%) and oleic acid ( 2.5%), had prominent action on absorption of cidopirox olamine through rabbit skins, and shorten the lag times obviously. CONCLUSION: L-menthol and azone can significantly enhance the percutaneous absorption of both salysic acid and cidopicox olamine. Combination of 1-menthol and azone can further increase the absorption.c
3.Effect of L-menthol on Two Compounds With Different Characteristics in Percutaneously Penetrating Isolated Rabbit Skin

OBJECTIVE:Indomethacinum(Ind) and diclofenac sodium(Dcs) were taken as model drugs to study the enhancing effect of l-menthol on percutaneous absorption of two compounds with different characteristics,and to probe into the effect of combined use of l-menthol with azone.METHODS:Percutaneous penetration experiment was performed on in vitro device and deposit effect was observed with vivoperception.RESULTS:l-menthol could significantly enhance the percutaneous absorption and shorten the lag times(T1ag) of both Ind and Dcs.The enhancing effects of l-menthol,used either alone or in combination with azone,were several times stronger on Ind than on Dcs.Combined use of two enhances was more effective in enhancing penetration and longer in T1ag than single use of l-menthol,especially for Dcs.The study on deposit effect showed that l-menthol could increase the penetration rate than control agent(P
4.Effect of metronidazole on percutaneous absorption of l-menthol through changing the skin character in rabbits

AIM: To investigate the effect of l menthol on percutaneous absorption of metronidazole through changing the skin character in rabbits. METHODS: Using rabbit skin with different character as transdermal barrier, and l menthol as transdermal enhancer, the absorbance of metronidazole was determined with a two cells diffusion apparatus in vitro, and its permeability was figured out. RESULTS: The transdermal action of l menthol on percutaneous absorption of metronidazole was very evident when the corneum was eliminated; l menthol and azone could increase the desposit function of metronidazole in the intact skin, but had no effect on the skin without corneum. The result also showed that the percutaneous absorption and intact skins desposit function of metronidazole had no difference between l menthol and azone. CONCLUSION: L menthol can increase the percutaneous absorption of metronidazole. The site of the action is mainly the corneum which has desposit function.

Objective To investigate the time-effect relationship and dose-effect relationship of the expression of adenovirus mediating Smad 7 gene regulated by irradiation via Egr-1 promoter in the lung of C57BL mice. Methods Recombinant adenovirus (AD.Egr-Smad 7) was made up through replication-defective adenovirus enclosed radio-inducible elements from the Egr-1 gene promoter and cDNA encoding Smad 7. Mice infected by intratracheal instillation with AD.Egr-Smad 7 were irradiated to their whole lungs after 24 hours. 324 mice were randomly divided into 6 groups, and the lungs were harvested at different time points following irradiation with single dose of 8?Gy to observe the time-effect relationship of Smad7 gene expression with different time intervals. 108 mice randomly divided into 3 groups were irradiated respectively by different single irradiation dose and the lungs were harvested 5 hours after radiation to evaluated the dose-effect relationship of Smad 7 gene expression with different radiation doses. The expression of exogenous Smad 7 was detected by Western blot analysis. Objective Objective To study the effect of adenovirus mediating Smad 7 gene regulated by radiation via Egr-1 on the primary tumor and lung metastasis in C57BL mice implanted with Lewis lung cancer.Methods The radio-inducible elements from the Egr-1 gene promoter were inserted upstream to a cDNA encoding Smad7 and integrated into a replication-defective adenovirus to generate recombinant adenovirus(AD.Egr-Smad 7).270 mice implanted with Lewis lung cancer in the hind legs were used and the experiment was started when the transplanted tumor diameter reached 0.8 to 1.0cm.Then three investigations were undertaken,each demanding 90 mice implanted with Lewis lung cancer respectively.To each group,90 mice models were randomized into 3 groups: the normal control group;the NS control group;and the implanted AD.Egr-Smad 7 group.Every 6 mice in each group were irradiated by different single dose to study the following: 1.The maximal and minimal diameters of the tumor were recorded to observe the tumor growth tendency,the tumor growth delay and the mice survival time,2.The incidence of lung metastasis two weeks after the radiation was recorded.3.The incidence of lung metastasis when the tumor volume was four times as large as that at the beginning of radiation was recorded.Results The adenovirus mediating Smad 7 gene expression regulated by irradiation via Egr-1 in C57BL mice implanted with Lewis lung cancer was able to inhibit the progression of the primary tumor and prolong the survival of the mice significantly as compared with the control group(P0.05).Conclusions The gene expression of AD.Egr-Smad 7 regulated by radiation is not risky in promoting the local progression and distant metastasis of Lewis lung cancer in mice.On the other hand,the gene expression of AD.Egr-Smad 7 regulated by radiation could inhibit the progression of the primary tumor and prolong the survival time of the mice significantly.It is safe,to some extent,of using AD.Egr-Smad 7 to block the signal transduction of TGF-?locally as a novel strategy for gene therapy aiming at the prevention of radiation-induced lung
7.Patterns of the first failure in completely resected stage ⅢA(N2) non-small cell lung cancer

Background and purpose: The prognosis of completely resected stage ⅢA(N2) non-small cell lung cancer (NSCLC) remains a significant concern. The 5-year overall survival (OS) rates range from 10% to 30%. This study aimed to analyze the patterns of first failure in completely resected stage ⅢA(N2) NSCLC and to assess the actuarial risk of developing metastasis at different sites and to guild standard clinical practice. Methods: Patients withⅢA(N2) NSCLC who had undergone radical surgery in our hospital from Jan. 2005 to Jul. 2012 were retrospectively reviewed. The progression-free survival (PFS), the OS, patterns of first failure, the actuarial risk were analyzed. The cumulative incidence of first failure was determined using the Kaplan-Meier analysis. Results: Among 357 patients who met the eligibility criteria with completely resected stage ⅢA(N2) NSCLC, 5-year OS was 36.9%. There were 284 (77.6%) patients experiencing disease failure: 61 with local failure, 197 with local and distant failures, and 26 patients with local recurrence as the first failure. Brain, bone and lung were the main sites of distant failure as the first failure, while brain was the most common site. There were 67 patients developing brain metastases (BM) as the first site of failure. The median time of local failure as the first site of failure was 13.6 months, and the time to develop distant recurrence was 15.1 months. 92.5% BM developed in 3 years after the complete resection. Conclusion: As the first failure, the rate of distant failure was much higher than that of local failure in completely resected stage ⅢA(N2) NSCLC. Brain was the most common site of distant failure as the first failure. These results can be helpful in guiding standard clinical practice and evaluating the outcome of comprehensive treatment.
8.Analysis of prognosis and risk factors of patients with bronchopulmonary carcinoid tumors after surgical treatment

Objective To investigate the prognostic factors and survival of patients with bronchopulmonary carcinoid tumors (BPC) after surgical treatment .Methods The clinical data of 87 patients undergoing surgery for BPC from Jan .2002 to Dec .2008 were reviewed retrospectively .Kaplan-Meier method was used to analyze the survival of the patients .The risk factors such as age , gender ,smoking history ,histological type ,tumor size ,were analyzed by Cox proportional hazards regression model .Results The 1-,3-and 5-year overall survival rates were 85 .1% ,71 .3% and 63 .2% .Univariate analysis revealed that age (P=0 .016) ,smoking history(P=0 .007) ,histological type(P=0 .000) ,tumor stage(P= 0 .000) ,tumor size(P= 0 .006) lymph node metastasis(P=0 .000) ,surgery type (P= 0 .045) and postoperative chemotherapy (P= 0 .000) were prognostic factors .Multivariate analysis showed that histological type(P=0 .008) ,tumor stage(P=0 .000) ,lymph node metastasis(P=0 .033) were independent prognostic factor .Conclusion The survival rate of the BPC patient after surgical treatment is high ,histological type ,tumor stage and lymph node metastasis were independent prognostic factors .
9.Promoting Surgery Incision Healing Effect of Recombinant Human Acidic Fibroblast Growth Factor in the Treatment of Children with Open Fractures:A Multicenter, Randomized and Controlled Trial

Objective:To evaluate the promoting surgery incision healing effect of recombinant human acidic fibroblast growth fac-tor ( rh-aFGF) in the treatment of children with open fractures. Methods:A multicenter, randomized and controlled clinical trial was conducted to study the efficacy of rh-aFGF. Totally 120 cases of injured children (age<14y) were randomly divided into two groups, the treatment group (n=60) was given rh-aFGF washing during the operation and spraying after the operation, and the control group ( n=60) was treated with normal saline. Both groups were given traction, screw or Kirschner wire fixation. The healing time, healing status and delayed healing were observed and compared in the two groups. Results:Stage I healing rate, the complete healing time and delayed healing rate in the treatment group was 86. 6%, (20. 3 ± 5. 6)d and 3. 3%, respectively, while that in the control group was 56.7%, (23.4 ±6.2)d and 15.0%, respectively. The differences between the two groups were significant (P<0.05 or 0.01). Conclusion:Rh-aFGF can effectively promote wound healing and shorten the healing time. For all these positive aspects,rh-aFGF de-serves wider clinical application in postoperative rehabilitation after open fractures.
