1.Surveillance of suspected adverse events following immunization in Wuyi County from 2018 to 2021

Objective:
To investigate the occurrence of suspected adverse events following immunization (AEFI) in Wuyi County, Zheijang Province from 2018 to 2021, so as to provide insights into safety assessment of vaccination. Methods The AEFI data in Wuyi County from 2018 to 2021 were collected from the AEFI Information Management System and Immunization Information Management System of National Immunization Information Management System in China. The reported incidence of AEFI and epidemiological characteristics of cases with AEFI were analyzed in Wuyi County from 2018 to 2021 using a descriptive epidemiological method.
Methods:
The AEFI data in Wuyi County from 2018 to 2021 were collected from the AEFI Information Management System and Immunization Information Management System of National Immunization Information Management System in China. The reported incidence of AEFI and epidemiological characteristics of cases with AEFI were analyzed in Wuyi County from 2018 to 2021 using a descriptive epidemiological method.
Results:
A total of 870 007 vaccine doses had been administered in Wuyi County from 2018 to 2021, and a total of 482 AEFI were reported, with annual mean reported incidence of 55.40/105. The incidence of AEFI associated with vaccines included in the national immunization program was significantly higher than those not included in the national immunization program (67.75/105 vs. 44.00/105; χ2=20.334, P<0.001). There were 435 individuals with general reactions (50.00/105 incidence), with high fever, local redness and swelling and solid nodes; 39 cases with abnormal reactions (4.48/105 incidence), including 87.18% allergic reactions; 7 cases with coincident syndrome and one case with psychogenic reaction; however, no vaccine quality or vaccination events were reported. The five vaccines with the greatest incidence of AEFI included 13-valent pneumococcal polysaccharide conjugate vaccine (269.11/105), diphtheria and tetanus toxoids and acellular pertussis, inactivated poliovirus and Haemophilus b conjugate vaccine (204.23/105), 23-valent pneumococcal polysaccharide vaccine (175.85/105), tetanus, diphtheria, acellular pertussis vaccine (128.39/105) and combined live vaccine for measles, mumps and rubella (124.36/105). The reported incidence rates of AEFI were 10.89/105, 49.88/105, 56.07/105 and 57.78/105 following oral, intradermal, subcutaneous and intramuscular administrations, respectively. In addition, AEFI were predominantly found to occur in patients at ages of 0 to <2 years (333 cases, 69.09%), and AEFI predominantly occurred <1 day following immunization (336 cases, 69.71%).
Conclusions
General reactions were predominant types of AEFI following vaccination in Wuyi County from 2018 to 2021, and the reported incidence of AEFI associated with vaccines included in the national immunization program was significantly higher than those not included in the national immunization program. AEFI mainly occurred among children under 2 years of age.

4.Study of the relationship between idiopathic thrombocytopenic purpura and HPA

ObjectiveTo observe the relationship between HPA-1—5,6,15 alleles and idiopathic thrombocytopenic purpura.MethodsPolymerase chain reaction-specific sequence primers(PCR-SSP) was used to genotype HPA-1-5,6,and 15 in the ITP group(n=25) and the health control group(n=25).The platelet adsorbed and free antibodies in the two groups were detected by ELISA and microcolumn gel immunoassay.ResultsThe allele frequencies of HPA-3 was significantly different between the ITP group and control group(P0.05).The platelet free antibodies were detected in 17 ITP samples(68%) and the adsorbed antibodies were detected in 21 ITP samples(84%).No platelet antibodies were detected in the control group.ConclusionThere was a possible association between the HPA-3 allele and ITP.The detection rate of platelet adsorbed antibodies is significant higher than that of platelet free antibodies.
5.Expression of Toll like receptors on peripheral blood mononuclear cells from patients with idiopathic thrombocytopenic purpura

Objective To study the level of the Toll like-receptor mRNA expression on PBMCs from patients with the idiopathic thrombocytopenic purpura.Methods We performed quantitative real-time reverse transcription-polymerase chain reaction(SYBR Green Ⅰ fluorescent dye combination technique)analyses of PBMCs from 30 idiopathic thrombocytopenic purpura(ITP) patients and 30 healthy subjects,to estimate the relative levels of TLR2,TLR4,TLR6,TLR7 and TLR9 mRNA expression.Results Relative expression levels of TLR2 and TLR7 mRNAs in ITP group(n=30)were significantly higher than those in health control group(n=30)(P0.05).Conclusion Expressions of TLR2 and TLR7 mRNA are up-regulated in patients with ITP,which may play an important role in the pathogenesis of ITP.
6.Bioinformatics Analysis of the Transcriptional Regulation of Peutz-Jeghers Syndrome Gene STK11/LKB1

Objective To map the approximate transcription initiation site of STK11/LKB1 and identify its potential promoter region. Methods All reported transcripts of STK11/LKB1 were colllected through searching online database. The “first exon”, “first intron” and 5' UTR upstream gDNA sequence of STK11/LKB1 were analysed by MethPrimer and FiretEF to predict the CpG island and possible transcription initiation site, respectively. The STK11/LKB1 gDNA sequence was also analysed by softwares such as PromoterInspector to predict the probable promoter region. Results Among all the reported transcripts of STK11/LKB1, 5'UTR of BC007981 was the longest. STK11/LKB1 gene was a typical CpG island associated gene. There was no exon in the upstream region of the “first exon”, and 5'UTR in BC007981 was close to transcription initiation site. Promoters were predicted in the BC007981 5' upstream 200~400bp region. Conclusion The 5'UTR of STK11/LKB1 approaches the transcription initiation site. The transcription initiation site of STK11/LKB1 gene in several hundreds bp region of 5'UTR upstream may be indentified. The data will benefit further research on the trascriptional regulation of STK11/LKB1 through experimental methods.

Objective To observe the expression of apoptosis and invasion genes in different tissues of laryngeal lesions,and to detect the content of proliferation related protein and proliferation inhibition gene.Methods In our hospital,the clinical diagnosis of laryngeal carcinoma,precancerous lesions and vocal polyps in 88 cases of laryngeal carcinoma,surgical excision of lesion specimens cut from the amount of Pro apoptotic genes and promote invasion and proliferation related protein,gene the content of tissue proliferation inhibition gene detection.Results The laryngeal carcinoma group of Pro apoptotic genes nicotinamide adenine dinucleotide phosphate oxidase 1 (NOX1) and large tumor suppressor gene 1 (LATS1) were all significantly lower than lesion group and polyp group (P < 0.05).Laryngeal carcinoma invasion promoting gene tumour necrosis factor receptor associated factor 6 (TRAF6),and cellular FADD-like interleukin1βconverting enzyme inhibitory protein (c-FLIP) content were significantly higher than that of laryngeal pre cancerous lesions and vocal polyps (P < 0.05).The proliferation related protein Cyclin-dependent kinase 6 (CDK6) and E2F1 in laryngeal carcinoma were significantly lower than that in the lesion and polyp group,and cyclin D1 (CCND1),Bmi-1,and Livin contents were significantly higher than that in the lesion group and the polyp group (P < 0.05).The levels of differentiated embryo-chondrocyte expressed gene 1 (DEC-1),IκB kinase 16(IKK16),large tumor suppressor gene 1 (LAST-1),receptor-interacting proteins-1-(RIP-1),and c-myc promoter binding protein-1 (MBP-1) in the tissue of laryngeal carcinoma were significantly lower than those in the lesion group and polyp group (P < 0.05).Conclusions The laryngeal carcinoma invasion promoting gene was higher than that in other tissues,inhibition of gene content was lower than that of other tissues proliferation related protein gene,apoptosis and proliferation,apoptosis and invasion of lesions in gene expression level,proliferation related protein and proliferation inhibition of gene content and laryngeal squamous cell carcinoma were related to laryngeal disease prevention significance of detection.
9.Expression and prognostic significance of HPV16 E7 protein expression in laryngeal squamous cell carcinoma

Objective To explore the role of human papillomavirus (HPV) in squamous cell carcinoma of the larynx.Methods A cohort of 152 laryngeal squamous cell carcinoma (LSCC) patients,40 premalignant lesions of the larynx,and 40 vocal fold polyp patients was evaluated for the expression of HPV16 E7 protein by immunohistochemistry (IHC).E7 protein expression was correlated with clinicopathological features and outcomes.Results The expression of E7 protein in vocal fold polyp was negative,and it's positive expression in precancerous lesions and laryngeaI carcinomas was 2.5%,and 32.9%,respectively.There were no other significant clinical or demographic differences between pl6-positive and-negative cases.There was no difference in overall survival (OS) between E7-positive and-negative patients with 5-year survival of 79% and 66% for p16 positive and negative patients,respectively (P =0.581).There was no statistically significant difference in failure-free survival (FFS) with 5-year FFS of 57.8% and 42.3% for p16 positive and negative patients,respectively (P =0.579).Conclusions E7 expression is infrequent in laryngeal carcinomas.There are no statistically significant correlations between E7 IHC status and OS or FFS.
10.Comparative study of ultrasonography-guided optical imaging system and full-field digital mammography in breast neoplasms

Objective To compare the diagnostic values of US-guided optical imaging system (OPTIMUS) and full-field digital mammography (FFDM) in breast neoplasms. Methods Sixty-four patients suspected of breast neoplasms underwent OPTIMUS and FFDM before surgery. All patients were proved histopathologically. The accuracy of diagnosis was compared with chi-square test. Results Sixty-five masses were delineated by OPTIMUS and FFDM. The diagnostic accuracy, sensitivity and specificity of OPTIMUS were 86. 2% ( 56/65 ), 87.0% ( 20/23 ) and 85.7% ( 36/42 ), respectively. The positive likelihood ratio was 6. 09 and the negative likelihood ratio was 0. 175. The diagnostic accuracy, sensitivity and specificity of FFDM were 73.8% (48/65), 82. 6% ( 19/23 ) and 71.4% (30/42), respectively. The positive likelihood ratio was 2. 892 and the negative likelihood ratio was 0. 243. There were no significant differences between OPTIMUS and FFDM ( x2 = 3. 077 ,P > 0. 05). Conclusion OPTIMUS is similar and supplementary to FFDM for the diagnosis of breast neoplasms.
