Objective To explore the upstream signal transduction pathway of neutrophil elastase(NE)-induced mucin(MUC)5AC gene expression.Methods A549 cells were either incubated with NE alone or with DMTU,aprotinin or AG1478.The level of MUC5AC mRNA was measured with RT-PCR.The activation of epidermal growth factor receptor(EGFR) and signaling were assessed by measuring the release of epidermal growth factor(EGF) and the phosphorylation of EGFR.Results NE increased MUC5AC gene expression accompanied by an increase of EGF and phosphorylated EGFR.DMTU,aprotinin and AG1478 significantly inhibited MUC5AC gene expression.DMTU and aprotinin significantly decreased the level of EGF and phosphorylated EGFR.ConclusionNE can induce MUC5AC gene expression via EGFR signalling in A549 cells.The upper stream involves oxidants,activation of tissue kallikrein and EGF.
3.Interrelationship among urinary albumin elimination rate, serum adiponectin and blood glucose level in patients with type 2 diabetic nephropathy

Objective To investigate the interrelationship among urinary albumin elimination rate(UAER),serum adiponectin(ADPN)and blood glucose level in patients with type 2 diabetic nephropathy(DN). Methods A total of 89 type 2 diabetic patients and 30 healthy people as control were enrolled in the study.According to UAER,the diabetic patients were divided into three groups:normal group(35 cases,UAER<30 mg/24 h),microalbuminura group (32 cases,UAER 30-300 mg/24 h)and macroalbuminura group(22 cases,UAER>300 mg/24 h).Serum adiponectin was measured by ELISA.Body mass index (BMI),waist hip radio(WHR),fasting plasma glucose(FPG),2-hour plasma glucose(2hPG),glycated haemoglobin(HbAlc),blood lipid and UAER were measured routinely.Data were compared among groups. Results Serum adiponectin level was lower in diabetic patients than that in healthy control group(P<0.05).Among diabetic patients.serum adiponectin of macroalbuminura group was significantly higher than that of normal and microalbuminura groups[(67.74±14.89)vs(39.36±13.92),(54.38±10.14)ng/L,P<0.051.Multiple regression analysis showed 2hPG,HbAlc,BMI,disease course,UAER and DBP were closely associated with the level of serum adiponectin. Correlation analysis indicated serum adiponectin was positively correlated with age (r=0.255), disease course (r=0.405), HDL (r=0.501)and UAER (r=0.843); while negatively with HbAlc (r=-0.479), FPG (r=-0.436), 2hPG(r=-0.418),BMI (r=-0.479) and WHR (r= -0.531), all P<0.01. Conclusions Serum adiponectin is positively correlated with UAER and negatively correlated with FPG. Blood glucose level is one of the main factors affecting serum adiponectin. Strict controlling of blood glucose level is beneficial to higher level of ADPN.
4.Influence of foot bath with traditional Chinese medicine combined with foot massage on sleep quality of patients with insomnia

Objective To evaluate the combination of foot bath using tradirional Chinese medicine and foot massage on sleep quality of patients with insomnia. Methods 84 cases of insomnia patients were divided into the treatment group(44 cases) and the control group(40 cases)according to their willingness whether to accept foot bath and foot massage or not. The control group received routine care, the treatment group was given foot bath using traditional Chinese medicine and foot reflex zone massage at bedtime on the basis of the routine care. The treatment effect were evaluated after 10 days of continuous intervention. Pittsburgh Sleep Quality Index Scale (PSQI)was used to assess sleep quality of patients. Results The treatment efficacy and quality of sleep of the treatment group was significantly better than the control group. After treatment, sleep condition in the treatment group significantly improved than before. ConduMons The combination of foot bath using traditional Chinese medicine and foot massage can improve sleep quality in patients with insomnia.

6.Value of plasma ADMA in predicting cardiac structure and function of patients with chronic kidney diseases

Objective To explore the predicting value of plasma asymmetric dimethylarginine (ADMA) in cardiac structure and function of patients with chronic kidney diseases (CKD).Methods A total of 100 CKD patients were enrolled in this cross-sectional study.According to staging of the K-DOQI guideline,CKD patients were divided into five groups.Twenty healthy people were enrolled as control.Plasma level of ADMA was determined by reversed-phase high performance liquid chromatography.Cardiac structure and function were detected by color echocardiography.Results Plasma levels (μmol/L) of ADMA in CKD stage 3 to 5 patients (1.3318±0.4684,1.5712±0.4210,2.1093±0.7714) were significantly higher than those in CKD stage 1,2 patients (0.4387±0.2575,0.4809±0.2846) and healthy control (0.4611±0.1615) (all P< 0.01).Meanwhile,ADMA level of CKD stage 5 was significantly higher as compared to CKD stage 3-4.The left ventricular mass index (LVMI) was significantly higher in stage 4-5 patients than that in stage 1-3 and healthy control.Plasma ADMA was positively correlated with LVMI (r=0.476,P=0.028) and negatively correlated with left ventricular ejection fraction (EF) (r=-0.327,P=0.041).Multivariate stepwise Logistic regression analysis revealed plasma ADMA level was an independent predictors for EF (OR=0.984,P<0.01).Conclusions Plasma level of ADMA begins to increase in CKD patients of stage 3 and rises proportionally to the decline of renal function.Plasma ADMA level is useful in predicting the cardiovascular complications of CKD patients.
7.Survey on occupational hazards of nurses in the supply room

Objective To investigate the occupational hazards of nurses in the supply room,to determine what kind of occupational hazards is most dangerous,and accordingly put forward some suggestions of protective measures.Methods Totally 208 nurses from supply rooms of 4 general hospitals were surveyed with questionnaires.The data of occupational hazards were analyzed.Results Various kinds of occupational hazards happened among nurses in the supply room,and among all occupational hazards,physical damage was the most frequently happened.Conclusions The occupational protection awareness of nurses is relatively weak.They can not properly handle damages caused by occupational hazards,so sufficient attention should be given to it.
8.The effect of levocarnitine and Vitamin-E on oxidative stress state and the function of endothelial cells in maintenance hemodialysts patients

Objective To study the treatment effects of intravenous levocarnitine in combinaton wlth vitamin-E in the oxidative stress state,endothelial cell function and nutritional status of patients with maintenance hemodialvsis (MHD).Methods 83 patients with MHD were randomized to the control group (Group A),and three treatment groups:with levocarnitine (Group B),with Vitamin-E (Group C),with levocarnitine and Vitamin-E(Gmup D).Intravenous 1.0 g levocarnitine were administered to Group B,oral vitamin-E(200 mg,two times each day) to group C,and vitamin-E and levocarnitine were coadministratered to group D at the end of dialysis.The blood biochemical indicators of each group were measured respectively.At the same time,malondialdehyde(MDA),glutathione peroxidase (GSHPx) and nitricoxide (NO) expression were mesured.Results MDA of group A increased compared with pre-dialysis at the end of the 3rd month[(4.81±0.08)nmoL/L vs(4.62±0.06)nmol/L(P<0.05)].and GSHPx declined at the end of the 3rd month[(72.02±2.05)μmol/L vs(74.62±1.46)μmol/L(P<0.01)] Hemoglobin(Hb),and blood albumin (Alb) reduced without statistical significan (P>0.05);In group B and C.MDA declined more compared with Group A[(3.86±0.06)nmol/L,(3.81±0.19)nmol/L vs (4.81±0.08)nmoL/L,(P<0.01)],while the GSHPx and NO increased significantly[(92.11±1.62)μmol/L,(92.14±1.37)μmol/L vs(72.02±2.05)μmol/L;(64.24±1.72)μmol/L,(64.35±1.67)μmol/L vs (49.91±1.19)μmol/L(P<0.05)].In the 3rd month,Hb and Alb level rised obviously(P<0.05);Group D was better than Group B and C(P<0.01).Conclusions Levocamitine and Vitamin-E effectively improve oxidative stress and nutrition status and the function of endotllelial cells of the MHD patients.It probably slows atherosclerosis progress.Furthermore,the combined medication is better.

10.Tidal breathing pulmonary function test and clinical significance in infants with recurrent wheeze

Obiective To explore the dynamics of tidal breathing pulmonary function in infants with recurrent wheeze and its clinical signiifcance. Methods Eighty (80) infants with recurrent wheeze from October 2013 to February 2014 were enrolled and divided into asthma predictive index positive (n=25) and asthma predictive negative (n=55) groups, and another 20 healthy children were enrolled as control group. Tidal breath pulmonary function at the time of admission (acute phase), leaving hospital (remission phase), and a week after discharge (admission phase) were tested, the ratio of time taken to reach peak expiratory lfow to total expiratory time(TPTEF/TE)and ratio of peak expiratory volume to total expiratory volume(VPEF/VE) between groups were compared. Results From acute phase and remission phase to admission phase, TPTEF/TE, VPEF/VE were elevated in positive group and negative group showing signiifcant statistical difference between phases (P=0.000). In acute phase, TPTEF/TE, VPEF/VE showed no statistical difference (P>0.05) between positive group and negative group, when compared with control group, TPTEF/TE, VPEF/VE were signiifcantly lower in positive and negative groups than that in control group (P<0.05). In remission phase, TPTEF/TE, VPEF/VE in negative group were higher than that in positive group, but significantly lower than that in control group (P<0.05). In admission phase, TPTEF/TE and VPEF/VE in negative group and control group showed no statistical difference (P>0.05), but significantly higher than that in positive group (P<0.05). Conclusions Lung function impairment duration was longer in asthma predictive index positive children than in asthma predictive index negative children. The tidal breathing pulmonary function test can provide objective clinical indicators for infants with recurrent wheeze to predict asthma.
