1.An investigation of response to SARS stress and mental health of nurse students during SARS prevalent period

BACKGROUND: SARS, as a kind of sudden stress source, make quite a few people present symptoms such as panic emotion, psychological allergic reaction, etc. There are also partial nurse students who worry about to be infected, fear to practice in clinical departments and live in an irritable mood, which interferes their clinical practice and affects both body and mind health.OBJECTIVE: Through the investigation of the nurse students' stress response and anxiety symptom to understand their psychological stress response scale and anxiety level during the prevailing period of SARS.DESIGN: An intersectional investigation with the nurse students doing clinical practice or probation apprentice as observative objects.SETTING: Department of Psychiatry, West China Hospital, Sichuan Univesity PARTICIPANTS: In May 2003, the nurse students of 2-4 grade doing clinical practice and probation in West China Nursing College of Sichuan University were selected as observative objects, all of whom had no record of mental illness or other severe disease, and volunteered to receive the investigation. 305 questionnaires were sent out and 299 valid questionnaires were taken back. They were 171 junior college students (47.2%) and 128undergraduates (42.8%) of 18-32 years old, average 21.51,and boy students held 4.6% while girl students 93.4%.METHODS: The crisis response questionnaire was used to investigate the nurse students' response scale to SARS crisis from three aspects such as cognition, emotion and behavior. The response grade is from 1 to 10.The total score scale is 3-30. 3 means no impairment, 3-9 means is a little impairment, 9-15 means slight impairment, 15-21 means moderate, 21-27 means significant, and more than 27 means severe. The self-rating anxiety scale (SAS) compiled by Zung in 1971 was mainly used to assess the frequency of the appearance of the symptoms defined in each item in the students being investigated, including 20 items and score in 4 grades: 1 indicates never (appear) or in a little time,2 indicates in a small part of time,3indicates in a large part of time,4 indicates almost in the whole or in the whole time. Only the matter of fact in the recent week was investigated in the above two questionnaires. The students were required to fill in the questionnaires separately on the spot, and the questionnaires were handed in as soon as they were filled out.MAIN OUTCOME MEASURES: The nurse students' response scale and anxiety level to SARS crisis on cognition, emotion and behavior.RESULTS: 305 questionnaires were sent out and 299 were taken back,with a back rate of 98%. ①Crisis response scale: in the three aspects of cognition, emotion and behavior of the nurse students, 21.7% of them had no impairment,56.8% had a little impairment, 13.8% had slight impairment,5.4% had moderate impairment ,1.7% had significant impairment and only 0.7% had severe impairment. The score of the undergraduates on their cognition, emotion and behavior and the total score of crisis response were obviously higher than those of the junior college students (P < 0.05).②Anxiety level: the nurse students' self-assessed result was that the total score was higher than that of the average normal Chinese [(39.89±11.98,29.78±0.46),(P<0.05)],the number of them with score higher than normal level made up 82.3% and there was no significant difference in scores on SAS between junior college students and undergraduates. ③Relation between crisis response and anxiety level: the analytic result of the relation between the stress response scale and anxiety level indicated that they had relativity (r=0.685 ,P <0.01).CONCLUSION: Facing the stress response to SARS crisis, the nurse students are impaired to some extent on their cognition, emotion and behavior.Their anxiety level are higher than that of the average normal Chinese and their psychological stress response scale has positive correlation with the anxiety level.
2.Effects of Porphyromnonas gingivalis lipopolysaccharide on the expression of RANTES and fractalkine in human urnbilical vein endothelial cells.

OBJECTIVEA study was conducted to investigate the effects of Porphyromonas gingivalis lipopolysaccharide (Pg-LPS) on the expression of regulated upon activation normal T-cell expressed and secreted (RANTES) and fractalkine in human umbilical vein endothelial cells (HUVECs).
METHODSHUVECs were incubated with different concentrations of Pg-LPS (200, 500, and 1000 ng x mL(-1)) for 1, 6, 12, and 24 h, respectively. Then real time quantitative polymerase chain reaction (RT-PCR) and enzyme-linked immunosorbent method (ELISA) were adopted to detect the protein levels and mRNA levels of RANTES and fractalkine.
RESULTSThe RANTES protein levels and mRNA levels, as well as fractalkine mRNA levels, were significantly higher in all experimental groups of 1, 6, and 12 h than in the control group (P<0.05), except the expression of RANTES mRNA in 200 ng x mL(-1) group of 12 h and RANTES protein in 200 ng x mL(-1) group of 1 h. The expression levels of RANTES mRNA and fractalkine mRNA were highest in 1000 ng x mL(-1) group of 6 h and were 4.88- and 6.20-fold higher, respectively, than those in the control group. The expression levels of RANTES protein, mRNA, and fractalkine mRNA decreased 6 h after stimulation, and were significantly higher than those in the control group (P<0.05) in the RANTES and fractalkine in HUVEC, and such expression is important in the development of atherosclerosis 500 ng x mL(-1) group of 24 h. There was a significant difference between the expression of fractalkine mRNA in 1000 ng x mL(-1) group of 6 and 12 h than in the control group (P<0.05).
CONCLUSIONPg-LPS infection might up-regulate the expression of RANTES and fractalkine in HUVEC, and such expression is important in the development of atherosclerosis.

3.Comparative Study Between Bronchoscopy and Imageology of Endobronchial Tuberculosis

Objective To explore the relationship between the classification and imageology,the dynamic changes of bronchoscopy and CT features of endobronchial tuberculosis(EBTB).Methods CT findings and bronchoscopic findings in 48 cases with EBTB proved by sputum culture , bronchoscopic biopsy and surgical pathology were analyzed.The classification,dynamic changes and CT characteristics were also evaluated.Results Bronchoscopic results showed 13 of edematous-hyperemic type, 8 of caseous nocrosis type, 5 of fibrostenotic type, 2 of tumorous type, 6 of granular type, 7 of ulcerative type and 7 of mixed type. CT manifestations in different types were intersected,including thickened bronchial wall (64.6%) , bronchial stenosis (41.7%) , aerated bronchus sign ( 37.5% ) ,intra-luminal nodules ( 25% ), bronchial structure with obstructive atelectasis ( 37.5% ) and pneumonia (25% ) .The corresponding rate between bronchoscopy and CT was 83.3% in detecting diseases.19 cases were followed-up by bronchoscopy during the initial 1~5 months of treatment,and 12 cases (3 edematous-hyperemic,4 caseation,3 ulcerative,2 mixed type) were resolved or healed without sequelae,6 cases changed into the other type,4 cases changed into the fibrostenotic type, 2 cases remained in a fibrostenotic state in spite of drug therapy.28 cases were followed-up by CT during the initial 1~5 months of treatment, and 20 cases (6 edematous-hyperemic,4 caseation,3 ulcerative, 1 granular,2 fibrostegnotic 1 tumorous,3 mixed type) were improved,6 cases changed into the other type,2 cases turned into the fibrostenotic type, 2 cases did not improve despite antituberculosis chemotherapy.CT correlated well with fibrobronchoscope(88.5%) in predicting the therapeutic outcome of EBTB.Conclusion CT as a non-invasive method has high clinical value in observing the evolution , predicting the complications and evaluating the therapeutic effect of EBTB.
4.MR Features of Atypical Meningioma

Objective To analyze the MR features of atypical meningiomas,so that to improve the diagnostic accuracy of this tumor pre-surgery.Methods There were 12 cases of atypical meningiomas proved by surgery and pathology.All of these patients underwent plain and contrast-enhanced MR examinations.Results Totally 12 cases,included 2 cases of multiple meningiomas,2 cases of lateral ventriclar meningiomas,2 cases of completely calcific meningiomas,3 cases of cystic meningiomas and 3 cases of malignant meningiomas.Conclusion The external cerebral signs of the tumors are the important basis for the localizing diagnosis of atypical meningiomas.Atypical MR signs of meningiomas are of complimentary value in qualitative diagnosis.
5.Effect of nursing intervention on preoperative anxiety of patients with gynecological carcinoma

Objective To study the effect of nursing intervention on preoperative anxiety of patients with gynecological carcinoma.Methods 60 patients with gynecological carcinoma in our hospital who were to receive operations were randomly distributed into the control group and the intervention group with 30 in each group.The control group was given routine nursing,while the intervention group received nursing intervention based upon routine nursing.Indexes such as preoperative anxiety,sleeping quality and satisfaction degree to nursing care were evaluated within them.Results Through targeted nursing intervention preoperative anxiety of patients in the intervention group alleviated,their confidence enhanced and satisfaction degree to nursing care improved compared to those of the control group.Conclusions Targeted nursing intervention can reduce preoperative anxiety of patients with gynecological carcinoma and increase satisfaction degree to nursing care.
6.Effects of antidepressant agents on the neuropathic pain induced by oxaliplatin

Background and purpose:Oxaliplatin is a new cytotoxic platinum compound widely used in antineoplastic treatments.Peripheral neuropathy characterized by allodynia remains the most common way to limit the usage of oxaliplatin.Oxaliplatin-associated neuropathic pain is often resistant to standard analgesics.The effects of antidepressant agents such as desipramine and fluoxetine on chemotherapy-induced neuropathic pain were investigated so as to provide experimental evidence for clinical treatment.Methods:A single injection of oxaliplatin(30 mg/kg)intraperitoneal was injected into a test subject,a mouse that had chronic neuropathic pain.Using the von Frey filament as a touch stimulator,the mechanical withdrawal threshold(MWT)was measured when observing allodynia.The MWT was measured before and 1 h after the administration of desipramin and fluoxetine.Results:Desipramine and fluoxetine both have the potential to increase the MWT in mice with oxaliplatin-induced neuropathic pain.Pretreatment with antagonists such as an opioid receptor like naloxone could deepen their effects.Furthermore,when desipramine is combined with an opioid analgesic as buprenorphine,it causes an augmentation in the MWT.Conclusion:Antidepressants desipramine and fluoxetine antagonize oxaliplatin-induced neuropathic pain by inhibiting allodynia.Furthermore,the tricyclic antidepressing agent desipramine could enhance the effects of buprenorphine in subjects with oxaliplatin-induced pain,suggesting a synergistic effect for opioid analgesic.
7.Polymorphisms of MTHFR and MTRR among the Han women in Laiwu city of Shandong Province

Objective To explore the 5,10-methylenetetrahydrofolate reductase( MTHFR) and 5-methyltet-rahydrofolate-homocysteine methyltransferase reductase( MTRR) gene polymorphisms among the Han women in Laiwu City.Methods A total of 559 Han women were recruited.And their oral epithelial cells were collected to extract genome DNA in order to detect gene polymorphisms of MTHFR and MTRR using fluorescence quantitative PCR.Then the results were compared with those in other cities in China.Results The frequency of MTHFR 677CC,677CT and 677TT of Han women in Laiwu city was 14.3%,46.7%and 38.1%,respectively.The frequency of MTHFR 677TT among Laiwu women was significantly different to those of Zhenjiang, Wuhan, Kunming, Deyang, Huizhou, Qionghai (P<0.05).The frequency of AA,AC,CC gene type on MTHFR A1298C was 78.2%,19.7% and 2.1%,respec-tively, the frequency was significantly different to those of Zhenjiang, Wuhan, Kunming, Deyang, Huizhou, Qionghai (P<0.05).The frequency of AA,AG,GG gene type on MTRR A66G was 53.3%,38.8%and 7.9%,respectively. The frequency of MTRR 66GG was significantly different to that of Qionghai(P<0.05).Conclusion The MTHFR, MTRR polymorphism distribution of Han women in Laiwu City has the characteristic of region specificity,respectively.
8.Ktrans of dynamic contrast enhanced MRI in evaluation of anti-angiogenic effects on nude mice with orthotopic transplantation tumor model of gastric cancer

Objective To assess the feasibility of K value of dynamic contrast enhanced MRI (DCE-MRI) in evaluation of anti-angiogenic effects on nude mice with orthotopic transplantation tumor model of gastric cancer.Methods Nude mice with orthotopic transplantation tumor model of gastric cancer were randomly assigned to two groups:Treatment group (n =15),mice were given apatinib intragastrically for 18 days (100 mg/kg),and control group (n=15),mice were given ddH2O2 in the same manner.After 18 days,DCE-MRI was performed and K value was measured.Then the tumors were dissected from the adjacent tissues in order to detect the microvessel density (MVD) and vascular endothelial growth factor (VEGF) expression levels.MVD and VEGF expression level were compared between treatment group and the control group.Results MVD (x2 =4.89,P<0.05) and VEGF expression level (x2 =8.69,P<0.01) of treatment group were much lower than those of control groups.The Kt value of treatment group was significantly lower than that of control groups ([0.63±0.05]/min vs [1.66±0.23]/min,t=17.05,P<0.01).Conclusion The value of k in DCE-MRI can be utilized to assess the effects of apatinib on nude mice with orthotopic transplantation model of gastric cancer.
9.Expression and Purification of Enterovirus Type 71 Polyprotein P1 using Pichia pastoris system

Enterovirus type 71(EV71) causes severe hand-foot-and-mouth disease(HFMD) resulting in hundreds of deaths of children every year; However,currently,there is no effective treatment for EV71.In this study,the EV71 poly-protein(EV71-P1 protein) gene was processed and cloned into the eukaryotic expression vector pPIC9k and then expressed in Pichia pastoris strain GS115.The EV71 P1 pretein with a molecular weight of 100 kD was produced and secreted into the medium.The soluble EV71 P1 protein was purified by column chromatography with a recovery efficiency of 70%.The result of the immunological analysis showed that the EV71 P1 protein had excellent immunogenicity and could stimulate the production of EV71-VP1 IgG antibody in injected rabbits.We suggest that EV71-P1 protein is an ideal candidate for an EV71 vaccine to prevent EV71 infection.
