1.Clinical and pathological analysis of seven patients with limb-girdle muscular dystrophy 2B

Objective To analyze the clinical and pathological features of limb-girdle muscular dystrophy2B(LGMD2B).Methods The clinical and pathological features of 7 patients with LGMD2B were analyzed retrospectively.Results Seven patients had a slow onset,and progressive proximal muscle weakness,muscle atrophy,progressive,and incresed serum creatine phosphokinase; muscle biopsy showed different degree of muscle fiber degeneration,necrosis; stromal and inflammatory cell infiltration in muscle fiber; monoclonal antibody immunohistoehemical staining:showed expression of Dysferlin protein was not found in muscle cell membrane,Dystrophin,Sarcoglycans protein expression was normal.Monoclonal antibody immunohistochemical staining the proteins were expressed in normal cell membrane.Conclusions LGMD2B is a slow onset,progressive proximal muscle weakness,muscle atrophy.Histochemical staining on the basis of further immunohistochemical staining,to detect the membrane protein and Dysferlin protein expression,that is a necessary means to diagnose LGMD2B and inflammatory myopathies.

Objective Adverse reactions to Gd-DTPA contrast media should be noticed enough.Methods Three cases of adverse reactions to Gd-DTPA contrast media were studied, and literatures were reviewed.Results The rate of adverse reactions to Gd-DTPA contrast media in our hospital was 0.206%.Conclusion Although the rate of adverse reactions to Gd-DTPA contrast media is very low, this problem must be noticed enough with the medical quality’s developement.

AIM:To investigate the effect of ERK1/2/c-Fos signal pathway during angiotensin-(1-7)inhibiting proliferation of rat glomerular mesangial cell strain(GMCS)induced by angiotensin Ⅱ.METHODS:Rat glomerular mesangial cells(GMC)were co-cultured with angiotensin Ⅱ and different doses of angiotensin-(1-7).The numbers of GMC were evaluated by crystal violet staining.The amounts of p-ERK1/2 and c-Fos expressions were detected by Western blotting.RESULTS:Angiotensin-(1-7)showed its inhibitory effects on GMC number increasing induced by angiotensin Ⅱ as well as the amounts of p-ERK1/2 and c-Fos expressions in a concentration dependent manner.CONCLUSION:ERK/c-Fos signal pathway is involved in the inhibitory effects of angiotensin-(1-7)on angiotensin Ⅱ-induced GMC proliferation.
4.Modulation of Baroreflex Activity by Exogenous and Endogenous NO in SD Rats

Objective To assess the modulation of baroreflex activity by exogenous and endogenous nitric oxide (NO) in SD rats. Method The normal baroreflex sensitivity of SD rat was evaluated by intravenous (I.v.) administration of Phenylephrine (PE, 30 μg·kg-1) to elevate blood pressure. Exogenous and endogenous nitric oxide (NO) were respectively produced by I.v. Infusion of sodium nitroprusside (SNP, 10.2 μg·kg-1) and L-arginine (L-Arg, 25 mg·kg-1). And NG-nitro-L-arginine methylester (L-NAME, 10 mg·kg-1) was infused to inhibit the generation of NO. Meanwhile, baroreflex sensitivity fluctuations caused by PE were estimated. Result Compared with the normal state, L-Arg significantly reduced the baroreflex sensitivity (Gmax) and the slope coefficient τ of the blood pressure-heart rate curve [Gmax=-1.469±0.120 beats/(min·mmHg) vs. -0.664±0.080 beats/(min·mmHg), P<0.01; τ=0.0860.005 vs. 0.0490.007, P<0.05]. SNP also led to attenuation of Gmax and τ [Gmax=-1.277±0.392 beats/(min·mmHg) vs. -0.943±0.297 beats/(min·mmHg); τ=0.074豹0.027 vs. 0.071±0.023] but the changes were not significant. In contrast to L-Arg and SNP, L-NAME induced raise of Gmax and τ [Gmax=-0.705±0.231 beats/(min·mmHg) vs. -1.065±0.200 beats/(min·mmHg); τ=0.0450.011 vs. 0.0590.026], however, they were statistically insignificant. Conclusion The results demonstrated that NO imposes effect on baroreflex activity, and the behaviors of exogenous and endogenous NO are different. Endogenous NO may be involved in the regulation of autonomic nervous system via central nervous system or carotid sinus, and inhibits the baroreflex sensitivity.
5.Angiotensin-(1-7) inhibit the expression of c-fos in angiotensin II-induced proliferative glomerular mesangial cells

Objective To investigate the effect of angiotensin-(1-7) on the expression of cellular c-fos in angiotensin II -induced proliferative glomerular mesangial cells (GMC). Methods GMC were treated with angiotensin II and different dose of angiotensin-(1-7). GMC number were evaluated by crystal violet staining and the expression of c-foe were detected by western blot. Results Angiotensin-(1-7) inhibit angiotensin II -induced GMC proliferation as well as the expression c-foe in a concentration dependent manner. Conclusion c-fos is involved in the inhibiting effects of angiotensin-(1-7) on angiotensin II -induced GMC proliferation.
6.Application of non-humidified low flow nasal oxygen inhalation in ICU patients

Objective To investigate the afflication of non-humidified low flow nasal oxygen inhalation in the ICU patients. Methods Four hundred patients hospitalized in ICU and needing continuous low-flow oxygen inhalation for more than 24 h were randomly divided into experiment group and control group equally.The experiment group were managed with dried humidification bottle oxygen inhalation and the control group conventional humidified bottle oxygen inhalation.The two groups were compared in terms of 24 h respiratory reactions,bacterial contamination of the humidified bottle,complaints of patients about oxygen inhalation noises and time for nurses to change the humidified bottles.Results The two groups had no significant difference in adverse symptoms(all P>0.05).The rate of bacterial contamination in the experimental group was significantly lower than the control group(P<0.05),the noises were lower and the time for changing the bottle shorter(P<0.05).Conclusion Non-humidified low flow nasal oxygen inhalation in ICU patients is good for reducing the rate of bacterial contamination of humidified bottle and lowering workload and oxygen-inhaling noises.
7.Relation between cervical squamous cancer lesion among Uighur and Han ethnic women in Xinjiang with expression of PI3K,ERK,IGF-1R and ER protein

Objective To observe the expression levels of PI3K ,ERK ,IGF‐1R and ER in cervicitis and cervical squamous cancer tissues among Uighur and Han ethnic patients and their correlation .Methods The 90 paraffin embedding samples of cervici‐tis tissue( 46 cases for Han and 44 cases for Uighur) and 224 paraffin embedding samples of cervical squamous cancer tissue (36 ca‐ses for Han and 188 cases for Uighur) were collected and detected the protein expression levels by using immunohistochemistry .Re‐sults The positive expression rates of IGF‐1R and PI3K in cervical squamous cancer were 58 .04% and 92 .41% respectively ,which were higher than 13 .33% and 57 .78% in cervicitis tissue ,the positive expression rates of ER and ERK in cervical squamous cancer were 22 .32% and 68 .30% respectively ,which were lower than 63 .33% and 95 .56% in cervicitis tissue ;the positive expression rate of IGF‐1R and PI3K of cervical squamous cancer in Han and Uighur were 69 .44% ,88 .89% and 55 .85% ,93 .09% respective‐ly ,which were higher than 15 .22% ,54 .35% and 11 .36% ,61 .36% of cervicitis tissue ;the positive expression rate of ER and ERK of cervical squamous cancer in Han and Uighur were 13 .89% ,83 .33% and 23 .94% ,65 .43% respectively ,which were lower than 65 .22% ,93 .48% and 61 .36% ,97 .73% of cervicitis tissue respectively ;the expression of ERK in Uighur cervical squamous carci‐noma tissue was 65 .43% ,which was lower than 83 .33% in Han ,the difference was statistically significant (P<0 .01) .Conclusion PI3K ,ERK ,IGF‐1R and ER protein expression positive or deficiency is closely related to the occurrence of cervical cancer ,which may serve as the important biological indicators for detecting cervical cancer ,and the ethnic difference of ERK protein expression exists in cervical cancer .

9.Curative effect of ambroxol in large dose on ventilator-associated pneumonia in the elderly

Objective To investigate the curative effect of large dose ambroxol on ventilatorassociated pneumonia(VAP)in the elderly.Methods From January 2009 to January 2011,76patients with VAP treated by ambroxol were randomly divided into general dose group and large dose group(n =38 for each group).General dose group received 60 mg ambroxol in 250 ml normal saline,and large dose group received 500 mg ambroxol in 250 ml normal saline,intravenous drip,twice daily for 14 d.The other therapeutic measures in all patients were accorded to 2005 guidelines for the hospital-acquired pneumonia.Procalcitonin(PCT)and clinical pulmonary infection score(CPIS)were used to detect the degree of pulmonary infection.Treatment effects were described with duration of mechanical ventilation,rates of tracheotomy intubation and second endotracheal intubation.Results PCT concentrations and CPIS in large dose group during treatment of 5 d[(0.79±0.68)mg/L and (5.50±2.08)scores]and 14 d[(0.44 ± 0.36)mg/L and(5.42 ± 2.23)scores]were significantly lower than in general dose group at 5 d[(1.18±0.97)mg/L and(6.61±2.32)scores]and 14 d[(1.11±0.91)mg/L and(6.47±2.17)scores](t =2.065,2.189,4.220 and 2.090,P<0.05),and the level of PCT in large dose group during treatment of 14 d was decreased as compared with 5 d(P<0.05).The time of mechanical ventilation,rates of tracheotomy intubation and second endotracheal intubation in large dose group[(6.03± 1.87)d,7.9% and 10.5%]were declined as compared with general dose group[(7.24±2.72)d,26.3% and 31.6%](t=2.264,x2 =4.547,5.066,P<0.05).Conclusions Ambroxol in large dose for the treatment of VAP have better clinical efficacy than in general dose.
