1.Efficacy of Ginkgo laminae on blood lipid, homocysteine and intimamedia thickness in recovery phase of cerebral infarction

Objective To observe the influence of Ginkgo laminae on serum lipid, serum level of homocysteine (HCY) and carotid intima-media thickness (IMT) of patients in recovery phase of cerebral infarction. Methods 100 patients in recovery phase of cerebral infarction were randomly divided into treatment group and control group, with 50 cases in each group. The control group were given aspirin enteric coated tablets 0.1g qn po., atorvastatin calcium capsule 20mg qn po.; the treatment group on the basis of the control group were added Ginkgo laminae, 1 tablet per time, three times daily. The blood liqid and HCY levels pre-treatment and 1, 3 and 6 months post-treatment and IMT value pre-treatment and 6 months post-treatment were collected. Results The triglyceride level had no significantly change in two groups. The high density lipoprotein cholesterol 3, 6 months post-treatment in treatment group was significantly higher than control group(P<0.05). The cholesterol and low density lipoprotein cholesterin 1 ,3, 6 months post-treatment in two groups significantly decreased compared with pre-treatment(P<0.05), while there was no significant difference between two groups at each time point. The cholesterol in treatment group had a smooth decreasing, while it had an increasing trend in control group 3 months post-treatment. The HCY 3 months post-treatment in treatment group was significantly lower than pre-treatment and control group 1, 3 and 6 months post-treatment (P<0.05). The IMT 6 months post-treatment in treatment group was significantly lower than control gorup(P<0.05). Conclusion Ginkgo laminae combined with atorvastatin for reducing lipid, which could raise high density lipoprotein cholesterol and reduce cholesterol smoothly and lower IMT, with the effective protection on vessel. The blood homocysteine level decreases after taking Ginkgo laminae for three months, which also could improve impaired endothelial function induced by high level of HCY and slow down the process of atherosclerotic plaque.

3.Analysis of the serum specific allergen-IgE in asthma patients with acute exacerbation

Objective To explore the change of the total level of lgE and specific allergen for acute exacerbation of bronchial asthma and to offer evidence for the prevention and cure of acute exacerbation of bronchial asthma.Methods The usual inhalant and ingested antigens were chosen in asthma patients,and the allergens were detected by ELISA in sera of acute exacerbation of bronchial asthma patients.Results Total positive rate of lgE was 92% (48/52),dust mites 61.2% (25/52),house dust 17% (9/52),shrimp and crab 15% (8/52),mould 9% (5/52).Conclusion The main allergen of acute asthma exacerbation is dust mites.House dust,shrimp and crab,and mould folloo.The rats of pollen or weed are very low.
4.Evaluating the change of left atrial function in the patients with paroxysmal atrial fibrillation underwent radiofrequency catheter ablation using echocardiography

Objective To evaluate the change of left atrial(LA) volume and function in patients with paroxysmal atrial fibrillation (PAF) after catheter ablation using real-time tri-plane echocardiography (RT-3PE) as well as quantitative tissue velocity imaging (QTVI).Methods Eighty-two PAF patients underwent first-time RFCA were included.Routine and tri-plane ultrasound images of 1 week before and 1 month,3 months,6 months after RFCA were stored for offline analysis.A range of LA structural and functional parameters were measured off-line using the EchoPAC workstation,including LA anteroposterior diameter (LADap),LA mediolateral diameter (LADml) and LA superior-infra diameter (LADsi),early and late diastolic transmitral flow velocity and ratio (E,A and E/A),the peak velocity of pulmonary vein and ratio (S,D,Ar,S/D),the maximum volume of the left atrium (LAVmax),the volume before LA active contraction obtained at time of the P wave on the surface electrocardiogram(LAVp),the minimum volume of the left atrium (LAVmin),LA passive emptying fraction (LAPEF),LA active emptying fraction (LAAEF),LA total emptying fraction (LATEF),LA expansion index (LAexpI),average peak velocity of six wall of the left atrium (Vs,Ve,Va).Results After 3,6 months,LADap decreased significantly(P < 0.05) ;3 months of follow up,peak A increased,E/A ratio decreased (P <0.05);6 months of follow up,peak A increased,E/A ratio reduced,peak S increased,peak D reduced,S/D increased(P <0.05) ;3 months of follow up,LAVmin reduced,LAAEF and LATEF increased (P <0.05);6 months of follow up,LA volume decreased,LAAEF and LATEF increased (P <0.05);3,6 months of follow up,the average Va increased(P <0.05).Conclusions LA reconstitution and function were recovering slowly 1 month after RFCA,and noticeable improvement can be observed 3 and 6 months after RFCA.
5.Peripheral Blood Motilin,Neuropeptide Y and Leptin Levels in Patients with Functional Dyspepsia

Background:The pathophysiology of functional dyspepsia(FD)is complicated and unclarified yet. Gastrointestinal hormone dysfunction may contribute to the development of FD. Aims:To investigate the correlation of different subtypes of FD with gastrointestinal hormone motilin(MTL),neuropeptide Y(NPY)and leptin(LEP). Methods:A total of 57 FD patients fulfilling Rome Ⅲ criteria were recruited and divided into epigastric pain syndrome(EPS)group(n = 24)and postprandial distress syndrome( PDS) group( n = 33 ). Ten healthy volunteers were served as controls. Fast and postprandial levels of MTL,NPY and LEP in peripheral blood were detected by radioimmunoassay. Results:Peripheral levels of fast MTL in EPS and PDS groups[(182. 90 ± 108. 57)pg/ mL and(145. 21 ± 67. 18)pg/ mL vs.(224. 47 ± 64. 55)pg/ mL,P < 0. 05],fast NPY in EPS group[(57. 40 ± 28. 75)pg/ mL vs.(90. 75 ± 49. 57)pg/ mL,P < 0. 01], and fast and postprandial NPY in PDS group[fast level:(38. 25 ± 20. 66)pg/ mL vs. (90. 75 ± 49. 57)pg/ mL,P <0. 01;postprandial level:(30. 26 ± 15. 12)pg/ mL vs.(65. 23 ± 54. 42)pg/ mL,P < 0. 01]were significantly lower than those in control group at same time points,especially the PDS group. Fast and postprandial levels of NPY were significantly lower in PDS group than in EPS group at same time points( P < 0. 05). No significant differences were observed in peripheral LEP levels among the three groups at any time points(P > 0. 05). Conclusions:The pathogenic mechanism of FD is related to the level of gastrointestinal hormones,and is different in EPS and PDS subtypes. Reduced MTL and NPY levels might be involved in the pathogenesis of PDS.
6.Pregnane Glycosides from Stems of Marsdenia tenacissima

Objective To study the chemical constituents from the stems of Marsdenia tenacissima.Methods The chemical constituents were isolated by various column chromatography and their structures were identified by spectral and chemical analysis.Results Two pregnane glycosides were isolated from the stems of M.tenacissima and identified as3-O-β-D-glucopyranosyl-(1→4)-6-deoxy-3-O-methyl-β-D-allopyranosyl-(1 →4)-β-D-oleandropyranosyl-11α-Otigloyltenacigenin B,named as tenacigenoside I(1)and 3-O-β-D-glucopyranosyl-(1→4)-6-deoxy-3-O-methyl-β-Dallopyranosyl-(1→4)-β-D-oleandropyranosyl-11α,12β-di-O-acetyltenacigenin B,named as tenacigenoside K(2).Conclusion Compound 1 is a new compound,1H-NMR and 13C-NMR data of compound 2 are reported for the first time.
7.Effect of carotid artery flow changes on the stability of plaque and typing characteristics of cerebral infarction

objective To observe the influence of hemodynamic changes caused by extracranial carotid stenosis on the plaque stability, and analyze the relationship of cerebral infarction classification,criminal artery stenosis degree and plaque stability to offer evidence for evalu-ating nosogenesis,clinical diagnosis,treatment and prevention of cerebral infarction. Methods CDFI,CTA were performed in 168 patients with acute cerebral infarction,some moderate and severe and occlusive carotid artery which were demonstrated by CTA and CDFI were further checked by DSA during convalescence. Totally 168 acute cerebral infarction patients were divided into 5 groups based on the criminal artery stenosis degree. To evaluate the plaque stability and flow field changes with CDFI and CTA,OCSP clinical classification and brain image clas-sification were finished according to CTA. Patients were classified into 4 subtypes,namely cortical infarction,basal ganglion infarction,corona radiata infarction and posterior infarction,according to the lesion distributions. Observe the changes of flow field and stability of plaque,and compare the relationship between different degree of stenosis and the classification of lesion of infarction. Results Among the 168 criminal arteries,there was 17. 9% of normal,22. 6% of mild stenosis,30. 9% of moderate stenosis,14. 8% of severe stenosis,and 14. 3% of occlu-sion,and the moderate stenosis proportion was the highest. When the stenosis degree was over 50%,it may lead to the changes of flow field, speeding up of blood flow,forming of turbulent flow,and increasing of plaque instability in which lipid plaque and admixture plaque was the highest. PACI is the commonest in all moderate groups. Admixture plaque is usually seen in severe stenosis and occlusion, while fibrous plaque is usually seen in mild stenosis. Conclusion Moderate carotid stenosis may lead to the increase of plaque instability as a result of the changes of flow field,and it may prone to artery-artery embolization.
8.Analysis of Off-label Use of Intravenous Drugs from 2013 to 2014 in Our Hospital

OBJECTIVE:To provide a reference for reducing adverse events due to off-label use of drugs and promoting ratio-nal use of intravenous drugs in Renji Hospital Affiliated to Shanghai Jiaotong University School of Medicine. METHODS:The pre-scriptions of intravenous drugs in 42 wards of our hospital from Jul. 2013 to Jun. 2014 were reviewed,and then off-label use of drugs were analyzed,where statistics was made mainly to drug overdose,the use of drugs at the concentration beyond the standards (including concentrations too high and too low) and associated conditions. RESULTS:There were 622 irrational prescriptions,in-cluding 60 concerning off-label use of drugs,accounting for 9.6% of irrational prescriptions,in which 40 were involved with the concentration which was too high,accounting for the highest percentage(66.7% of prescriptions related to off-label use of drugs), 17 were related to drug overdose(28.3% thereof),and 3 were in relation to the concentration which was too low(5.0% thereof). CONCLUSIONS:Pharmacy intravenous admixture service may,through the communication between the pharmacist and the doc-tor,refuse to make up the above-mentioned prescription which was found to be irrational(including that involving in off-label use of drugs)or make up such prescription after it is modified according to the requirements as specified in the label to reduce the occur-rence of off-label use of drugs so as to promote rational drug use.
9.Feasibility of transplanting bone marrow mesenchymal stem cells into coronary artery

Objective To investigate the retainage rate,distribution,and emigration of the bone marrow mesenchymal stem cell(BMMSC) after transcoronary infusion and to further evaluate the feasibility of injecting BMMSCs into coronary artery.Methods BMMSCs were isolated,purified,expanded,and labelled with CM-DiI.The infarcted SD rat hearts were removed and perfused with Langendorff apparatus.The cells were injected into the aortic root and the fluid returning from the coronary system was collected and the labelled cells in the coronary effluent were quantitated with flowcytometry.At the same time,left ventricle function was recorded to evaluate the safety of this approach.In vivo study,the cells were then injected into clamped ascending aorta through a catheter inserted through the left ventricle into the aortic root.The hearts were harvested at different time points after cell transplantation to obtain the direct evidence of distribution and emigration of the implanted cells.Results Only 3%~5% of transplanted cells returned into the right ventricle and more than 90% cells retained in the heart after beinginjected into aortic root of Langendorff model of infarcted hearts of SD rats.Left ventricle function did not deteriorate after cell transplantation.The labelled cells were entrapped within the coronary capillary immediately after cell infusion,mainly in the normal area.After 24 hours some cells migrated through the capillary wall into interstitium of the heart.One week later we found that most survival cells located at the infarcted area and the border zone.Conclusion The majority of BMMSCs delivered by transcoronary infusion retained in the heart.BMMSCs can penetrate the vessel wall and home back to the interstitial compartment and the injured area in a few hours.
10.Expression and Purification of Enterovirus Type 71 Polyprotein P1 using Pichia pastoris system

Enterovirus type 71(EV71) causes severe hand-foot-and-mouth disease(HFMD) resulting in hundreds of deaths of children every year; However,currently,there is no effective treatment for EV71.In this study,the EV71 poly-protein(EV71-P1 protein) gene was processed and cloned into the eukaryotic expression vector pPIC9k and then expressed in Pichia pastoris strain GS115.The EV71 P1 pretein with a molecular weight of 100 kD was produced and secreted into the medium.The soluble EV71 P1 protein was purified by column chromatography with a recovery efficiency of 70%.The result of the immunological analysis showed that the EV71 P1 protein had excellent immunogenicity and could stimulate the production of EV71-VP1 IgG antibody in injected rabbits.We suggest that EV71-P1 protein is an ideal candidate for an EV71 vaccine to prevent EV71 infection.
