1.Multidimensional status of family nursing assistants for elderly people with chronic diseases and disability in Beijing communities

Objective:To conduct a survey on the current situation of family nursing assistants for elderly people with chronic diseases and disability in the community from multiple dimensions such as personal information, work status, professional quality, difficult problems encountered in the nursing process, and solutions.Methods:From April to August 2022, a questionnaire survey was conducted among family nursing assistants of elderly people with chronic diseases and disability in six communities in Beijing using cluster sampling. We conducted a survey and analysis on the current situation of family nursing assistants for elderly people with chronic diseases and disability in the community from multiple dimensions, based on the presence or absence of professional qualification certificates for nursing assistants.Results:The study included 611 nursing assistants, aged (49.99±6.82) years, mainly composed of rural registered residence and education level below junior high school. Only 43.04% (263/611) of nursing assistants had professional qualification certificates for nursing assistants. Compared with those without professional qualification certificate for nursing assistants, those with professional qualification certificate for nursing assistants had statistical differences in gender, registered residence, education level, daily working hours, specific work content of care, monthly income, solutions to reduce the mobility of elderly nursing assistants, working years, feelings of caring for the daily health of elderly people with chronic diseases, psychological status, self-awareness about job, training to improve work skills, basic medical knowledge training, and reasons for not participating in training ( P<0.05). Nursing assistants reported a lack of medical and nursing knowledge and an urgent need for professional training and guidance from medical and nursing staff during the investigation of difficult issues encountered in their work. Conclusions:It is necessary to strengthen and improve the training of nursing professionals and basic medical knowledge of family nursing assistants for elderly people with chronic diseases and disability before and during work, which can help improve the level and quality of care provided by family nursing assistants.
2.Establishment of experimental model for saphenous artery perforator flap in mini pig

Objective To investigate the anatomy characteristics of saphenous artery(SA) and its perforator vessels in mini pig,and to provide a new perforator flap animal model.Methods Between May,2018 and August,2018,6 7-months-old mini pigs weight 25 kilograms were sacrificed by blooding under general anesthesia.The bilateral external iliac arteries were injected with a mixture of latex-lead oxide mixture and underwent CT-scanning to provide 3-dimensional reconstruction.The origin,diameter,courses and distribution of the SA and perforators of both hind limbs were observed.Results One of the hind limbs was abandoned due to the leakage of perfusion fluid.The other 11 hind limbs were available.The femoral artery passed through the medial femoral muscle and sartorius muscle in the middle of thigh,and run between the medial femoral muscle and gracilis muscle above the knee.Below the knee,it run on the surface of the medial leg muscles.A number of small branches were distributed in the tarsal joint and calcaneus at the medial malleolus,and there were abundant communicating branches with the posterior tibial artery.The average length of SA was (14.86±0.76) mm.The outer diameters of SA at initiative,medial tibial condyle and medial malleolus were(1.73±0.15) mm,(1.50±0.12) mm and (1.30±0.13) mm,respectively.There were a total of 79 perforators which were identified,with a mean of (7.09±1.16) perforators per hind limbs.And the average outer diameter was 0.10-0.78 (0.40±0.13) mm.Conclusion The SA constantly exist with good outer diameter.The number of perforator is abundant.SA can be used as a reliable animal model for studying and training of perforator flap.
3.Correlation Study of the Contents of Gentiopicrin and Loganic Acid with Appearance Traits of Gentiana scabra and Its Quality Gradation Criterion

OBJECTIVE: To establish the method for the content determination of gentiopicrin and loganic acid in Gentiana scabra, and to investigate the correlation of their contents with appearance traits and quality gradation criterion. METHODS: HPLC method was adopted. The determination was performed on Ascentis Express C18 column with mobile phase consisted of 0.1% phosphoric acid-acetonitrile (gradient elution) at the flow rate of 0.4 mL/min. The column temperature was 30 ℃, and detection wavelength was set at 240 nm. The sample size was 1 μL. Taking the length, number and diameter of fibrous roots as indexes, the appearance and morphological characteristics of G. scabra were studied. The relationship of gentiopicrin and loganic acid content with the appearance property of medicinal material was analyzed by SPSS 21.0 software. k-mean clustering analysis was carried out by using SPSS 21.0 software, and gradation standard for G. scabra was established preliminarily. RESULTS: The linear range of gentiopicrin and loganic acid were 0.5-3.0 μg/mL (r=0.999 9) and 0.05-0.50 μg/mL (r=0.999 9). The limit of quantification of gentiopicrin and loganic acid were 0.295, 0.289 μg/mL; the detection limit were 0.082, 0.081 μg/mL; RSDs of precision, stability, repeatability tests were all lower than 2%; the recovery rates were 97.56%- 102.23% (RSD=1.56%, n=6) and 97.58%-102.67% (RSD=1.86%, n=6). Correlation results showed that there was a significant positive correlation of the length of G. scabra, the number of roots, root diameter, with the contents of gentiopicrin and loganic acid. The order of affecting content was the number of roots >length >root diameter. k-means clustering analysis showed that 54 batches of G. scabra was divided into two categories; S4-S6,S13,S17-S23,S25,S28,S31-S34 were clustered into a category; S1-S3, S7-S12, S14-S16, S24, S26,S27,S29, S30,S35-S54 were clustered into the other category. The results of gradation showed that 54 batches of G. scabra could be divided into two grades, and the results were consistent with the cluster analysis. CONCLUSIONS: Established method is simple and stable, and can be used for simultaneous determination of gentiopicrin and loganic acid in G. scabra. The more fibrous roots, the longer the length, the thicker the root, the higher the content of gentiopicroside and loganic acid, the better the quality of G. scabra.
4.The application of Satir model team intervention model after radical colorectal cancer surgery

Objective? To discuss the application effects of Satir model team intervention model among patients after radicalcolorectal cancer surgery. Methods? From November 2017 to May 2018, 70 patients with colorectal cancer in Nanyang Hospital Affiliated to Zhengzhou University were selected by convenience sampling. All the research subjects were divided into intervention group and control group by random number table method, with 35 cases in each. The control group received routine nursing after operation, while the observation group received Satia model team nursing intervention on the basis of the control group. The Family Adaptability and Cohesion Evaluation Scale Ⅱ (FACES Ⅱ), the Social Avoidance and Distress Scale (SADS) and the Self-esteem Scale (SES) were selected to compare the effects. Results? The intimacy and adaptability scores of FACESⅡ in the observation group on the day after the intervention and one month after the intervention were significantly higher than those of the postoperative day and the control group, with statistical significances (P< 0.01). The SADS score of the observation group on the day after the intervention and one month after the intervention were significantly lower than those of the control group, with statistical significance (P < 0.01). The SES score of the observation group on the day after the intervention and one month after the intervention were significantly higher than those of the control group, with statistical significance (P< 0.01). Conclusions? The Satia model team intervention is helpful to improve family relationship, self-esteem and social reintegration of patients with colorectal cancer after radical operation.
5.Pelvic peritoneum reconstuction with barbed suture combined with irrigation and negative pressure drainage in laparoscopic rectal anterior resection for rectal cancer

Objective To evaluate pelvic peritoneum reconstruction with barbed suture and irrigation and negative pressure drainage in laparoscopy rectal anterior resection for locally advanced rectal cancer.Methods From 2014 to 2018 159 locally advanced rectal cancer patients treated by laparoscopy were derided into study group (n =76),and those received gravity drainage without pelvic peritoneum reconstruction (n =83).Results There were not significant difference in the operation time between the two groups [(165 ± 18) min vs.(160 ± 17) min,t =0.098,P =0.069].There were significant differences in postoperative drainage tube indwelling duration [(5.7 ± 2.4) d vs.(7.4 ± 3.7) d,t =3.309,P =0.001],intraabdominal infection (7% vs.17%,x2 =3.991,P =0.046) and anastomotic leakage (4% vs.13%,x/2 =4.280,P =0.039).There was no significant difference in anastomotic stenosis (5% vs.10%,x2 =1.088,P =0.297) and intestinal obstruction (9% vs.11%,x2 =0.117,P =0.732) between the two groups.Conclusion It is simple,safe and reliable to perform pelvic peritoneal reconstruction using barbed suture,combined with irrigation and negative pressure drainage,it can decrease drainage tube indwelling duration,reduce the incidence of intraabdominal infection and anastomotic leakage.
6.Anterolateral musculocutaneous flap for reconstruction of bone expose of lower leg amputation stump which on the verge of reamputation

Objective To explore the curative effect of anterolateral musculocutaneous flap to repair the soft defect of the lower leg amputation stump which on the verge of reamputation, and preserved the length of leg or the knee joint to improve prosthetic limb function. Methods Four patients with soft defect and bone expose of proxi-mal leg amputation stump were enrolled in this study from April, 2013 to November, 2016. Each patient underwent stump revision using anterolateral musculocutaneous flap, including 3 free musculocutaneous flaps and 1 pedicle mus-culocutaneous flap. The patients were all male with the mean age of 20(8-48) years. The wound was treated debride-ment, and reconstructed on the second stage. Defect area was from 7.0 cm×6.0 cm to 25.0 cm×10.0 cm. Anterolateral musculocutaneous flaps was from 13.0 cm×10.0 cm to 28.0 cm×12.0 cm. The length of preservation tibia stump was from 5.2 cm to 9.4 cm. Results All patients were followed-up from 12 months to 37 months (average, 19 months). All the musculocutaneous flaps survived completely with a soft tissue texture, elastic, and very wear-resisting. The sensory return was determined to be S2in 2 patients, S3in 1 patient and S3+ in 1 patient. The 6-Minute Walk Test:grade Ⅳ in 2 cases, grade Ⅲ in 2 cases; Stanmore movement classification: gradeV in 2 cases, grade Ⅳ in 2 cases. Conclusion The anterolateral musculocutaneous flap maybe is an ideal flap to provid stable coverage for the stump of leg amputation which on the verge of reamputation.
7.Effects of strengthening out-of-hospital health beliefs on perceived well-being and behavioral change in elderly patients with colon cancer

Objective? To explore the effects of strengthening out-of-hospital health beliefs on perceived well-being and behavioral change in elderly patients with colon cancer. Methods? Totally 112 elderly patients with colon cancer admitted in Nanyang City Center Hospital from August 2015 to August 2017 were selected by convenient sampling and divided into two groups (n=56) based on the time of admission. Patients in the control group received conventional nursing care, while patients in the observation group received out-of-hospital health beliefs. Both interventions lasted for 6 months. The knowledge needs scale for cancer patients and the Health Promoting Lifestyle Profile-Ⅱ (HPLPⅡ) were used to evaluate the perceived well-being and healthy behaviors in the two groups before and after intervention. Results? The scores of psychological needs, technical needs, medication needs and rehabilitation needs and the total score of the knowledge needs scale for cancer patients in the observation group were higher than those before intervention and those of the control group after intervention. The scores of self-fulfillments, exercise, health responsibility, nutrition, pressure handling and interpersonal support and the total score of HPLP Ⅱ of the observation group were higher than those before intervention and those of the control group after intervention (P< 0.05). Conclusions? Strengthening out-of-hospital health beliefs can effectively improve the perceived well-being of elderly patients with colon cancer and enable them to develop healthy behaviors.
8.Endovascular treatment of ruptured intracranial vertebral artery dissecting aneurysm:correlation between location and outcome

Objective To investigate the correlation between the location of ruptured intracranial vertebral artery dissecting aneurysm (VADA)and the outcome after endovascular treatment. Methods Thirty-six patients with ruptured intracranial VADA undergoing endovascular treatment were enrolled retrospectively. According to the relationship between VADA and the location of the opening of posterior inferior cerebellar artery(PICA),they were divided into 3 groups:proximal to PICA group (n=13), distal to PICA group (n=13),and PICA involvement group (n=10). The demographic data, vascular risk factors, clinical features, imaging features, endovascular treatment mode, postoperative complications, and differece of the good outcome rate(defined as the modified Rankin scale 0-2)after 6 months of treatment were compared.Results Seven patients in the proximal to PICA group received reconstructive endovascular treatment (RET), and 6 received endovascular internal trapping(EIT);the good outcome rate was 100%(13/13).Seven patients in the distal to PICA group received RET,6 were treated with EIT;the good outcome rate was 84.6%(11/13).Four patients in the PICA involvement group received RET,and 6 received EIT,and 2 of them received contralateral vertebral artery retrograde PICA stenting combined with VADA segment and proximal vertebral artery coil embolization;the good outcome rate was 60.0%(6/10). There were significant differences in the overall outcome good rate among the 3 groups (P<0.05). The good outcome rate in the PICA involvement group was significant lower than that in the proximal to PICA group,and significantly lower than that in the proximal to PICA combined with distal to PICA group(P<0.05).Multivariate logistic regression analysis showed that only Fisher grade 3-4 was the independent risk factor for poor outcome (odds ratio 28.0, 95% confidence interval 1.71-458.82; P=0.020). Conclusions The surgical options of endovascular treatment for ruptured intracranial VADA needs to evaluate the relationship between the location of dissecting aneurysms and the PICA origin. The risk of endovascular treatment in patients with intracranial VADA involving the origin of PICA is higher and it may affect the outcome.
9.The relationship between serum cystatin C level and insulin resistance in elderly patients with type 2 diabetes

Objective To study the relationship between serum cystatin C(Cys C)level and the development of insulin resistance(IR)in elderly patients with type 2 diabetes mellitus (T2DM).Methods A cross-sectional survey research involving 757 elderly patients with T2DM was performed and patients were divided into groups according to Cys C and IR levels.Serum 1evels of fasting insulin (FBI),fasting blood glucose(FPG),fasting C-peptide (FCP),glycosylated hemoglobin A1 c (H bA1 c),homeostasis model assessment of insulin resistance (IR) (HOMA2 IR-C),homeostasis model assessment of insulin secretion,homeostasis model assessment of insulin sensitivity(HOMA2-% S-C),micro-albuminuria(mALB)and serum lipid were measured and compared among the groups.Possible influence factors were adjusted,and the correlation between Cys C levels and IR was analyzed.The influencing factors on IR were also analyzed.Results There were statistically significant differences in ages,course of T2 DM,FBI,FCP,HbA1c,urinary mALB,IR,insulin secretion,insulin sensitivity,morbidity rate of coronary heart disease,hypertension and diabetic nephropathy among the three groups(allP<0.05).Insulin sensitivity was decreased with the increase of Cys C level,while others were increased.Among 757 patients,the level of serum Cys C was positively correlated with FCP,HOMA2-IR C levels,and was negatively correlated with HOMA2-% S-C levels(P<0.05).Multiple logistic regression analysis showed that the higher level of Cys C was independent risk factors for IR in elderly T2DM patients(OR=3.41,95%CI:2.22-5.22,P<0.05).Conclusions Serum Cys C level is closely related with IR in elderly T2DM,and the elevated level of serum Cys C is one of independent risk factors for elderly T2DM.
10.Neoadjuvant therapy of regional intra-arterial chemoembolization combined systemic vein chemotherapy for locally advanced gastric cancer

Objective To investigate the efficacy and safety of preoperative regional intra-arterial chemoembolization combined with systemic vein chemotherapy in the treatment of locally advanced gastric cancer.Methods The clinical data of 78 locally advanced gastric cancer patients treated by radical D2 gastretomy from January 2010 to December 2012 were retrospectively analyzed.Patients were devided into treatment group receiving regional intra-arterial chemoembolization combined with systemic vein chemotherapy (n =36),and those with preoperative systemic vein chemotherapy (n =42) 3 to 4 weeks before surgery.Systemic chemotherapy with mFOLFOX6 was started after 5 weeks of surgery in the two groups.Results The incidence of toxicity reaction and postoperative anastomotic leakage were lower in treatment group than in control group,all P < 0.05.The 1-year recurrence and overall survival rate were similar between the two groups (all P > 0.05).The 3-year liver metastasis rate was higher in treatment group than in control group (x2 =4.063,P < 0.05).The 3-year survival rate was higher than that of the control group(x2 =4.396,P < 0.05).Conclusions Preoperative systemic vein chemotherapy combined with regional intra-arterial chemoembolization decreases toxicity reaction,anastomotic leakage and liver metastasis,while prolonging 3-year overall survival.
