1.Clinical Observation of Bevacizumab Combined with Chemotherapy in the Treatment of Advanced Gastric Cancer

OBJECTIVE:To investigate clinical efficacy and toxic and side effects of bevacizumab combined with chemothera-py in the treatment of advanced gastric cancer. METHODS:84 patients with advanced gastric cancer were randomly divided into combination group and chemotherapy group,with 42 cases in each group. Chemotherapy group received FOLFOX4 chemotherapy regimen(oxaliplatin+calcium folinate+fluorouracil);combination group was additionally given bevacizumab 7.5 mg/kg intravenous-ly,d1,on the basis of chemotherapy. A treatment course lasted for 21 d,and both groups received 3 courses of treatment. The short-term efficacy,the quality improvement of life,serum tumor marker before and after chemotherapy and the toxic and side ef-fects were compared between 2 groups. RESULTS:The effective rate in combination group(57.14%)was significantly higher than chemotherapy group(35.71%),improvement rate of life quality in combination group was higher than chemotherapy group,reduc-tion rate of life quality in combination group was lower than chemotherapy group,the differences were statistically significant(P<0.05);after chemotherapy,the serum CEA and CA199 levels were significantly decreased in both groups,the decrease of combina-tion group was more significant than that of chemotherapy,with statistical significance(P<0.05). The incidence of decrease in leu-cocytes,platelet and hemoglobin,periphery neurotoxicity,nausea and vomiting,liver and kidney function in combination group was higher than in chemotherapy,but there was no statistical significance (P>0.05). CONCLUSIONS:Bevacizumab combined with chemotherapy is effective in the treatment of advanced gastric cancer and don’t increase toxic and side effects.
2.Applying tuina to exterior-interiorly connected meridians for post-stroke upper limb spasticity

Objective:To observe the effect of applying tuina to exterior-interiorly connected meridians for post-stroke upper limb spasticity.
Methods:A total of 150 patients with post-stroke upper limb spasticity were randomly allocated into a treatment group (n=75) and a control group (n=75) by the random number table. Patients in the treatment group received tuina on exterior-interiorly connected meridians, whereas patients in the control group received standard rehabilitation therapy. The therapeutic efficacies in both groups were observed after 3 weeks of treatment.
Results:The total effective rate in the treatment group was 89.3%, versus 61.3% in the control group, showing a statistically significant difference (P<0.05). After the treatment, the muscle tones by the modified Ashworth scale (MAS) were significantly improved in both groups (bothP<0.05); and the improvement of muscle tone was more significant in the treatment group than that in the control group (P<0.05).
Conclusion:Applying tuina to exterior-interiorly connected meridians can obtain an exact efficacy for post-stroke upper limb spasticity.
3.Effect of Weiyanxiao Granule on Experimental Gastric Ulcer

To observe the effect of Weiyanxiao Granule (WG) in counteracting experimental gastric ulcer,acute lesion of gastric mucosa and to investigate its anti-inflammation and analgesic actions.Acute gastric ulcer models induced by stress or indomethacin, chronic gastric ulcer models induced by acetic acid and models with acute lesion of gastric mucosa induced by hydrochloride-ethanol or 0.2 mol?L -1 sodium hydroxide were used to observe the effect of WG in counteracting experimental gastric ulcer and acute lesion of gastric mucosa. Anti-inflammation action of WG was observed on mouse models with auricular swelling induced by dimethylbenzene, rat models with pedal swelling induced by white egg and rat models with granuloma induced by embedding tampon. 4.Intestinal absorption of forsythoside A by rat circulation in situ.

This study is to investigate the effects of concentration, intestinal section, pH, paracellular route, substrate/inhibitor of enzyme (CYP3A) and proteins (P-gp, MRP2, SGL1) on the absorption of forsythoside A. The absorption of three concentrations (2.6, 5.2, and 10.4 microg x mL(-1)) of forsythoside A in different intestinal segments was studied with phenol red as the marker by rat circulation in situ. The results showed that the residue of forsythoside A with different concentrations had little significant difference from that obtained after perfusing via duodenum, jejunum, ileum and colon, which indicated that the absorption of forsythoside A was passive diffusion and had no difference in different segments of rat intestine. The residue of forsythoside A increased to 466.160 and 463.429 microg respectively when cyclosporine (4 microg x mL(-1)) or midazolam (50 micromol x L(-1)) was added to the circulation fluid, which showed significant difference compared to the control group (P < 0.05). Moreover, the residue of forsythoside A showed a tendency of increase with the increase of cyclosporine or midazolam. When digoxin (50 micromol x L(-1)) or EDTA (10 microg x mL(-1)) was added to the circulation fluid, the residue of forsythoside A decreased to 325.110 and 369.888 microg respectively, which showed significant difference as compared to the control group (P < 0.05). Besides, the residue of forsythoside A showed a tendency of reduction with the increase of digoxin or EDTA. However, there is no significant change in the absorption of forsythoside A when the different concentrations of mannitol were added to the circulation fluid. The results above indicated that the absorption of forsythoside A was mainly passive diffusion and involved paracellular route at the same time. In addition, the substrates of P-gp or CYP3A had dose-dependent effect on the absorption of forsythoside A.
5.Curative effect of posterior diskoscope discectomy for adolescent lumbar disc herniation

Objective To study the curative effect of posterior diskoscope discectomy for adolescent lumbar disc herniation.Methods Eleven adolescent patients with lumbar disc herniation were treated with posterior diskoseope discectomy,the Japanese orthopaedic association (JOA) lower back pain score was observed and compared before operation and after operation 3,6,12 months.Results The JOA lower back pain score were (15.45 ± 2.73),(27.18 ± 2.08),(28.18 ± 1.25),(28.54 ± 1.21) scores before operation and after operation 3,6,12 months,there was statistical difference in JOA lower back pain score between before operation and after operation 3,6,12 months (P < 0.01).The patients were followed up for 1-10 years,mean 7.9 years,excellent was in 8 cases,good was in 2 cases,general was in 1 case.Recurrence and iatrogenic lumbar instability were not found.Conclusions The posterior diskoscope discectomy with less trauma,less bleeding,faster recovery,and can achieve the same decompression objective with the traditional open surgical.So posterior diskoscope discectomy can be used as adolescent treatment of lumbar disc herniation.
6.Analysis of differentially expressed genes and metabolic pathways in human uveal melanoma

Background Studies showed that there exsits differential gene expression in human uveal melanoma (UM).However, the researching results are somewhat inconsistently abroad, while relevant literature is still less in China.Few domestic researches have reported the abnormalities of gene transcription level or the pathways of these genes.Objective This study was to compare the gene expression profiles between human UM and normal uvea tissues and analyze the metabolic pathways involved in these differentially expressed genes.Methods Four human UM samples were collected in Beijing Tongren Eye Center,and 4 pieces of normal uveal tissues from 4 donors served as controls.The expression of genes was detected with Human Genome U133 Plus 2.0 chip,and the expression profiles were compared between two groups.The biological functions and active pathways of the genes were analyzed by Gene Ontology Enrichment Analysis Software Toolkit (GOEAST).Results Compared with the normal controls,4 165 differential genes were screened in human UM (12.50%) ,including 1 236 up-regulated genes (3.71%) and 2 929 down-regulated genes (8.79%) ,in which the genes of raised more than 5-, 10-,50-and 100-fold were 113,21,1 and 1, respectively, and the genes of reduced by 50% ,90% ,98% and 99% were 1 053,422,33 and 5,respectively.The functions of these differentially expressed genes were associated with cellular differentiation and growth,development, cell adhension, immun response, transcriptional contol, signal transduction and anti-apoptosis.The metabolic pathways of differentially expressed genes included angiogenesis pathway, cell-cycle related protein kinase pathway and immune regulatory pathway (involving B lymphocytes and T lymphocytes).ConclusionsGene expression profiles are evidently different between human UM and normal uveal tissue.The variation of the gene profiles in human UM leads to the changes of multiple biological functions including angiogenesis and kinase pathway even immun system.It is implied that the pathogenesis of human UM is a comprehensive effect of multiple genes and biological pathways.

This article presents important application of intestinal transporters and metabolism enzymes on gastrointestinal disposal of the active ingredients of Chinese materia medica, and points out the importance of research on transport and metabolism of the active ingredients of Chinese materia medica in Chinese extract and Chinese medicinal formulae.

8.ELIMINATION OF FLUORINE IN URINE AS A PARAMETER OF FLUORINE OVERLOAD IN HUMAN BODY——ANALYSIS OF 2818 CASES

In order to find out a parameter for the medical diagnosis and therapeutic criteria of endemic fluorosis, the excretory level of fluorine in urine was determined between the patients of bone fluorosclerosis and those who lived in the district of endemic fluorosis. The results showed that both the patients and the normal people in the endemic area of fluorosis excreted more fluorine in urine than the others, because of drinking highly fluorina-ted water. Thus a positive correlation was found between the amount of fluorine in urine and the degree of fluorinated drinking water, while no significant difference was found between the patients and the normal individuals. The urinary output of fluorine fluctuated in a few cases of fluorosclerosis.From the data obtained we may, perhaps, conclude that the excretion rate of fluorine in urine may be considered as a parameter of fluorine overload in the body, but not as applicable to the medical diagnosis or treatment in fluorosis.
9.Effects of parthenolide on osteoclast differentiation of RAW264. 7 cell induced by receptor activator of ;nuclear factor κB ligand

Objective To study the effects of parthenolide on osteoclast differentiation of RAW264. 7 cell induced by receptor activator of nuclear factor κB ligand (RANKL). Methods The mouse macrophage RAW264.7 cells induced by RANKL was used alone as the control group, different concentrations of par-thenolide (0.5, 1, 2 μmol/L) were added to culture the RAW264.7 cells. On the third, fifth and seventh day, the tartrate resistant acid phosphatase (TRAP) staining method was used to detect osteodast-like cells and the cell number was count;the contents of tartrate resistant acid phosphatase (TRAP5b) in the Culture supernatant of each groups were detected by enzyme linked immunosorbent assay (ELISA) and the expression of osteodast marker gene alcitonin receptor (CTR) and matrix metalloproteinase (MMP)-9 in each groups were detected by realtime-polymerase chain reaction (PCR) on the seventh day. We use Chi-square test and t test to test the differences between groups by SPSS 17.0. Results In different culture conditions, RANKL could always induce the RAW264.7 cell differentiate into mature osteoclasts. Compared with the control group at the same time control group, on the third, fifth and seventh day, he number of mature osteoclasts induced were obviously decreased in groups adding different concentration of PAR; the number of induced osteoclasts decreased along with the increase of parthenolide concentration, on the seventh day in 0.5, 1, 2 μmol/L concentration PAR groups, the number of mature osteoclasts compared with the control group were descended 36.3%, 40.8%, 49.3%(t=7.758, 8.742, 10.56;P<0.05);the contents of TRAP5b in the culture supernatant were consistent with the cell counting results on the seventh day (P<0.05). The expression of CTR and MMP-9 by TRAP positive osteoclasts decreased along with the increase of parthenolide concentration, and the 2 μmol/L group was the lowest. Compared with the control group, there were statistically significant differences with the different PAR concentration groups 0.5, 1, 2 μmol/L (P<0.05). Conclusion Parthenolide can inhibit RANKL induced RAW264.7 differentiation into osteoclast cells, and the inhibition is dose dependent.
10.Determination of Valsartan in Human Plasma and Urine by LC-MS/MS and Its Pharmacokinetic Study

OBJECTIVE:To develop a method for the determination of valsartan concentration in human plasma and urine. METHODS:Plasma sample were acidified and extracted with diethyl ether for analysis,and urine sample was diluted directly for analysis. The samples were all determined by LC-MS/MS,and the separation was performed on a Aglient ZORBAX SB-C18 column with mobile phase consisted of acetonitrile and 0.1% formic acid (gradient elution) at flow rate of 0.2 ml/min. Ion transition was determined ESI ion source under multiple ion reaction monitoring with quantitative pair m/z 436.4→253.2 and qualitative ion pair m/z 436.4→291.3 for valsartan,and quantitative pair m/z 423.4→207.1 and m/z 423.4→180.2 for internal standard losartan. RE-SULTS:The linear range of valsartan were 4-5 000 ng/ml in plasma and 20-50 000 ng/ml in urine;the limit of quantification were 4 ng/ml and 20 ng/ml;plasma extraction recovery of valsartan were 61.21%-70.30%. The variation coefficient of internal standard normalized matrix effect were 3.20% and 11.21%. The within-day and between-day RSDs were no more than 8.34%. CONCLU-SIONS:The method is proved to be rapid and sensitive,and suitable for the determination of valsartan in human plasma and urine and pharmacokinetics study.
