1.Effect of triple therapy and sequential therapy on eradication of Helicobacter pylori in long-term aspirin use

Objective To analysis curative effect of triple therapy and sequential therapy for eradication of Helicobacter pylori in patients with long-term aspirin use.Methods 52 patients with long-term aspirin use who were diagnosed with helicobacter pylori infection were collected.All patients were divided into sequential group and triple group according to different drugs classified, each 26 cases in each group were given corresponding drug treatment, after the end of treatment, the serum pepsinogen, Hp-IgG antibody levels and Helicobacter pylori clearance rate were detected in all patients. Results After treatment, compared with the triple Group, in the sequential group, the levels of serum PG I and PG II were lower, and PGⅠ/PGⅡwas higher, and the differences were statistically significant (P<0.05);the serum Hp-IgG antibody levels were lower in the sequential group (P<0.05);the Hp clearance rate of patients in the sequential group was higher (P<0.05).Conclusion Compared with triple therapy, sequential therapy can lower serum PG, PG II and Hp-IgG levels, and improve the Hp clearance rate and clinical efficacy, have guiding significance to clinical.
2.Impacts of the Decoction of Fortifying the Spleen and Supplementing Qi on the cellular immunity of chronic Hepatitis B virus carriers

Objective To observe the impacts of the method of fortifying the spleen and supplementing Qi on the cellular immunity of chronic hepatitis B virus(HBV)carriers.Methods Asymptomatic HBV carriers group(ASC group,n=60)with HBV DNA positive were randomly divided into Fortifying the Spleen and Supplementing Qi decoction group(A group,n=30)and conventional treatment group(B group,n=30),the peripheral blood T lymphocyte subsets were detected with flow cytometey and compared with those of the healthy people(The normal control group,n=18).All the patients were treated,and 24 weeks after treatment the indexes were measured again.Results As compared with those of normal controls,the numbers of CD4+ T cells and CD8+ T cells were decreased significantly in HBV carriers(P<0.01);There was a statistical significance between the values before and complete the treatment with the decoction of fortifying the spleen and supplementing Qi(P<0.05 or P<0.01),and compared with conventional treatment group,there Was a statistical significance(P<0.05).Conclusion T-cell subset function was disturbed in the HBV carriers.The method of fortifying the spleen and supplementing Qi could improve the cellular immunity of carriers with chronic hepatitis B.
3.The histological changes of diabetic rats' skin and the effects on the percutaneous absorption of glucocorticoid.

To examine the histological changes of diabetic rats' skin and the effects on the percutaneous absorption of hydrocortisone (HC, a glucocorticoid), male Wistar rats were randomly divided into five groups: control group, diabetes one-week group (W1), two-week group (W2), three-week group (W3), and four-week group (W4), while each group contained 6 rats. Diabetes mellitus (DM) rat model was prepared with the method of streptozocin (STZ, 40 mg x kg(-1)) intraperitoneal injection. Abdominal skin was cut to carry out an in-vitro penetration experiment on an improved Franz diffusion cells, and phosphate buffer (PBS, pH 7.4) was used as receptor solution. The solution was analyzed with HPLC, and then the penetrating rate can be calculated. Meanwhile, rats' abdominal skins of different DM periods were HE stained and made into tissue slices to find if any histological changes occurred. The penetrating rate of control, W1, W2, W3, and W4 groups were 2.39 +/- 1.25, 3.22 +/- 1.72, 3.02 +/- 1.89, 3.63 +/- 2.02 and 5.00 +/- 3.36 microg x h(-1) x cm(-2), respectively. There was significant difference between the control and the W4 group (P < 0.05), but no significant differences were found between any other two groups (P > 0.05). The tissue slices showed that compared to the normal rats' skin, little change was observed in one-week DM rats' skin, but the skin of one-month DM rats' skin was observed thinner, and it became much thinner than that of rats with two-month diabetes, especially the epidermis. After making a rat into diabetic, the rats' skin goes through a pathological change, and this change is closely interrelated with the increase of the permeation of HC. Therefore, it is necessary to adjust the dose while some drug was applied on the skin in case of diabetes mellitus.
4.Regulating effect of oxymatrine on TGF-?1 in CCl_4-induced hepatic fibrosis rats

Aim To explore the anti-fibrotic effect of Oxymatrine(OM) on CCl4-induced liver fibrosis in rats and its modulation on the TGF-?1.Methods A hepatic fibrosis model was induced by CCl4.The levels of Alanine transaminase(ALT),aspartic transaminase(AST),type-Ⅰ collagen and TGF-?1 in plasma were detected by chemical Kit.The deposition of collagen was observed with H&E and Masson staining.Pathological changes were observed under light microscope in 8 randomly selected fields in each group.RT-PCR method was applied to detect the expression of TGF-?1 mRNA in hepatic tissue.Results The concentration of serum ALT,AST,type-Ⅰ collagen and TGF-?1 was significantly reduced in the middle and high dose treated groups compared with that of model group.A significant reduction of collagen deposition and rearrangement in OM-treated group was displayed in histopathological changes.The expression of TGF-?1 mRNA was considerably decreased in the treated animals.Conclusions Oxymatrine is effective in reducing the production and deposition of collagen in the liver tissue of experimental groups,and it has an obvious protective effect on model rats.It may be one of the therapeutic mechanisms to modulate the expression of TGF-?1 mRNA.
5.Observation on short-term therapeutic effect of zhenggan recipe in treating liver cirrhosis with alpha-feto-protein.

OBJECTIVETo observe the therapeutic effect of Zhenggan Recipe (ZGR) in treating liver cirrhosis with abnormal Alpha-fetoprotein (AFP), and to explore its mechanism.
METHODSSixty-four patients of liver cirrhosis with abnormal AFP were randomly divided into the treated group and the control group. All the patients received conventional combined Western medicinal treatment. Besides, additional ZGR was given to the patients in the treated group, one dose every day for orally taken in two times, 6 months as a therapeutic course. Changes in liver function (ALT, GGT, TBIL and ALB), AFP, T-lymphocyte subsets (CD4, CD4/CD8) and iconological examination were observed in the two groups before and after treatment to compared the efficacy.
RESULTSZGR could effectively lower the AFP level (P < 0.01), and improve the levels of CD4 and CD4/CD8 (P < 0.01 or P < 0.05).
CONCLUSIONZGR can effectively lower the level of AFP, its mechanism might be related to the improving of cellular immune function of the patients.

6.The clinical significance of ultrasonography combined with scrological detection in the screening of trisomy 21

Objective To investigate the clinical significance of ultrasonography combined with scrological detection in the screening of trisomy 21.Methods The clinical data of 2 281 pregnant women who have received prenatal ultrasound examination,scrological detection and prenatal diagnosis in our hospital from January 2014 to March 2016 were retrospectively analyzed.All the fetuses had ultrasound abnormal or/and 21-trisomy syndrome high risk with scrological detection.The detection rate of trisomy 21 was observed when ultrasonography combined with scrological detection.The detection rate was compared between the two methods respectively.Results Among 2 281 pregnant women who received prenatal diagnosis,68 fetuses with trisomy 21 were diagnosed by fetal karyotype,the detection rate was 2.98% (68/2 281);There were 879 cases of merely ultrasound abnormal,27 of them were trisomy 21,the detection rate was 3.07% (27/879);1 336 cases were merely high risk of 21-trisomy syndrome,34 of them were trisomy 21,the detection rate was 2.54% (34/1 336);There were 66 cases of both ultrasound abnormal and 21-trisomy syndrome high risk,7 of them were trisomy 21,the detection rate was 10.61% (7/66).The method of joint use is higher than the independent method,with statistically significant difference (P < 0.05).Conclusions Ultrasonography combined with scrological detection have good application value in the screening of trisomy 21 syndrome.
7.Research hotspots on hospice care based on co-word cluster analysis

Objective:To analyze the research hotspots of hospice care, and to understand the research status and development trend in this field, so as to provide reference for the development of hospice care in China.Methods:PubMed was retrieved systematically from June 2012 to June 2019, and the key words in the literature were taken as the research object. Bicomb and SPSS 23.0 software were used for co-word cluster analysis.Results:A total of 4 766 relevant literatures were retrieved and 46 high-frequency keywords were intercepted. Seven hot spots of hospice care were summarized through co-word cluster analysis: the demand for hospice care for terminal patients and its impact on their quality of life; the psychological status of caregivers and the social support they received; the upgrade of hospice care model; the promotion and Implementation of advance directives and advanced care plans; personnel's investigation and training on knowledge, belief and practice of hospice care; the best time for referral and treatment of hospice care; pain management of hospice care.Conclusions:In recent years, hospice care has attracted more and more attention, but there are relatively few studies in China. Through co-word cluster analysis of high-frequency subject words, we can realize the research hotspots of hospice care in the world, so as to provide references for domestic hospice care researchers.

Omentin-1 is one of the newly discovered Secretory protein,which is mainly synthesized and secreted by vascular stromal cells in visceral adipose tissue.Some related researches have shown that omentin-1 has effects of prevention and mitigation on pathological myocardial hypertrophy.With further explorations and clinical applications of omentin-1,it will provide new strategies and methods for the prevention and treatment of human pathological myocardial hypertrophy and heart failure.

Objective To explore the safety and feasibility of guiding catheter passing through spasmodic vessels in patients undergoing percutaneous coronary intervention (PCI) via radial artery access by the aid of PCI guiding wire and balloon .Methods The clinical data of 33 coronary artery disease (CAD) patients undergoing PCI via radial artery access with radial artery or (and) brachial artery spasm ( group A ) were retrospectively analyzed .Among all these patients , guiding catheters were delivered through the spasmodic vessels successfully by the aid of PCI guiding wires and balloons .The clinical data of other 38 CAD patients having PCI during the same period performed by other operators via radial artery or ( and ) brachial artery approach and experienced vessel spasm were anlysed as the control ( group B ) .All patients in group B received conventional anti-spasm management during PCI .All vessel spasm was identified by angiography.For patients in group A , a diameter of 0.014 inch guiding wire was chosen to pass through the spasmodic vessel segment carefully and gently .The diameter of balloon should be chosen according to the diameter of guiding catheter .A balloon diameter of 2.0 mm and 2.5 mm was corresponded to 6F and 7F guiding catheter respectively .The balloon was advanced to the tip of guiding catheter , keeping a half in catheter and a half in vessel followed by inflating the balloon with a pressure of 8 atm.The balloon was kept inflated the guiding catheter was pushed in vitro carefully and slowly until the catheter passed through the spasmodic vessel segment .Then the balloon was deflated and pulled out together with PCI guiding wire . Exchanged a diameter of 0.035 inch wire and completed the positioning of guiding catheter .After finishing the PCI, radial or ( and) brachial angiography was performed again to observe if spasm disappeared and to determine if there any contrast medium exudation .For patients in group B , routine approach was applied including administration of nitroglycerine , diltiazem or nitroprusside etc . to relieve vessel spasm. Results The location of vessel spasm was similar in group A and group B ( P=0.150 ) , and the incidence rate of spasm in brachial artery was higher than that in radial artery in both groups .The chance of guiding catheter crossing the spasmodic vessel segment was significantly higher in group A than in group B ( 100%vs.39.5%, P=0.00).In patients whose guiding catheter could pass through the spasmodic vessel segment successfully , time spent in group A was shorter than in group B ( P=0.000 ) .The patient number which time spent was less than five minutes , five to 15 minutes and more than 15 minutes was 30 and 2 ( 90.1%vs.13.3%) , 3 and 7 ( 9.9% vs.46.7%) and 0 and 6 ( 0% vs.40.0%) in group A and in group B respectively.The incidence of forearm hematoma was lower in group A than in group B without statistical difference [6.1%(2/33) vs.18.4%(7/38), P =0.113].Conclusions It is safe and feasible for passing guiding catheter through spasmodic vessels during PCI via radial artery access by the aid of PCI guiding wire and balloon .
10.Expressions of miRNA-146a in peripheral mononuclear cells of children with systemic juvenile idiopathic arthritis and its clinical significance

Objective:To investigate the role of miR-146a in the pathogenesis of systemic juvenile idiopathic arthritis (sJIA) and its clinical significance.Methods:This article is a prospective clinical cohort study.Twenty-six patients with sJIA (14 cases of initial active group and 12 cases of stable group), 15 patients with multijoint juvenile idiopathic arthritis (JIA) and 15 patients with oligojoint JIA diagnosed in the Department of Rheumatology and Immunology of Anhui Provincial Children′s Hospital from June 2018 to December 2020 were enrolled.Twenty healthy controls from the out-patient clinic were also recruited.The expression level of miR-146a in peripheral blood mononuclear cells (PBMCs) of research objects was detected by real-time fluorescence quantitative polymerase reaction (qPCR), and the serum levels of interleukin (IL) - 6, tumor necrosis factor (TNF) - α and IL-1β in sJIA patients and healthy controls were detected by enzyme-linked immunosorbent assay.The expression levels of miR-146a in PBMCs and cytokines among different groups were compared by analysis of variance. Pearson correlation analysis was used to analyze the correlation of the relative expression level of miR-146a in PBMCs with clinical inflammatory indexes and serum cytokines in sJIA patients. Results:(1) The expression level of miR-146a in PBMCs of early sJIA patients was significantly higher than that in the multijoint JIA group and oligojoint JIA group (8.77±3.15 vs.4.40±1.59, 2.55±1.15, t=6.27, 14.23; all P<0.05). The expression level of miR-146a in PBMCs of sJIA active patients was significantly higher than that in sJIA stable patients (8.77±3.15 vs.3.63±1.37, t=10.27, P<0.05). There was no significant difference in the expression level of miR-146a between the sJIA stable group and healthy control group ( P>0.05). (2) The expression levels of IL-1β, IL-6 and TNF-α were significantly higher in sJIA active patients group than those in sJIA stable group[(58.56±17.47) ng/L vs.(26.32±10.54) ng/L, (73.72±11.16) ng/L vs.(23.20±9.12) ng/L, (70.93±19.97) ng/L vs.(24.25±9.49) ng/L, all P<0.05]. There was no significant difference in the expression levels of IL-1β, IL-6 and TNF-α between the sJIA stable group and healthy control group(all P>0.05). (3)The expression of miR-146a in PBMCs of sJIA patients was positively correlated with serum ferritin levels, platelets, erythrocyte sedimentation rates, C-reactive proteins, IL-1β, IL-6 and TNF-α( r=0.542, 0.433, 0.329, 0.306, 0.333, 0.342, 0.319, all P<0.05). Conclusions:miR-146a may be involved in the inflammatory process of sJIA disease.miR-146a can well distinguish sJIA from multijoint JIA and oligojoint JIA.TNF-α, IL-1β and IL-6 are involved in sJIA inflammatory responses.
