1.Clinical application for TME combined the double-stapling technique in low rectal carcinoma anus-preserving resection

Objective To evaluation the clinical implemented and primary experience of TME combined the double-stapling technique in low rectal carcinoma anus-preserving resection. Methods 22 cases of low rectal carcinoma (the distance between the inferior verge of rectal turnout and the anal verge were 4~7cm) were operated by TME combined the double-stapling technique. Results All of the 22 cases of TME combined double-stapling procedure were successful without operative mortality. Correlative complications including anastomotic leakage (3 cases), anastomotic stenosis (1 case). Temporary completely defunctioning loop colostomy 4 cases. Conclusions TME combined the double-stapling technique in low rectal carcinoma anus-preserving resection is feasibility and the tumour pathology and clinical stage should be considered. Mastering expert operation skill and adherence to sound surgical priciples is the key to prevent and to decrease postoperative complication.
2.Effect of the position of PORP on the middle ear transferfunction——a finite element model

OBJECTIVE To study the effect of partial ossicular replacement prothesis (PORP) on the middle ear transfer function.METHODS On a incus removed finite element model,a titanium columnar structure which link the malleus and the stapes was used to simulate the function of PORP to transmit acoustic pressure.The effect of placement of PORP on different positions of the manubrium,as well as the insertion of the cartilage between the artificial bone and the fascia on the middle ear transfer function were calculated.RESULTS When the PORP is placed on the neck of manubrium,mid-manubrium and headof malleus removed,the vibration amplitude of the umbo ofthe tympanic membrane is reduced in 200-8000 Hz,and thereduction range in 200-3200 Hz is more than that of 3200-8000 Hz.While the vibration amplitude of the stapes in the200-8000 Hz amplitude increased.The vibration amplitude is basically the same when placed on the neck of manubrium and mid-manubrium,while it slightly increased about 2-3 dB than the former two when the head of malleus removed.In 1000-8000 Hz.When putting the cartilage between the PORP and transplanted fascia,the vibration amplitude of the stapes footplate was slightly increased,but there was a slight difference with different cartilages.CONCLUSION The placement and material property of PORP play an important role in the middle ear transfer function.
3.Synthesis and properties of poly(hydroxyethyl methacrylate) hydrogel for IOL materials.

Poly (hydroxyethyl methacrylate) (PHEMA) hydrogel for intraocular lens (IOL) materials was synthesized by solution polymerization using 2-hydroxyethyl methacrylate (HEMA) as raw material, ammonium persulfate and sodium pyrosulfite (KPS/SMBS) as catalyst, and trietyleneglycole dimethacrylate (TEGDMA) as cross-linking additive. Effects of reaction time, temperature, dosage of catalyst and cross-linking additive on mechanical strength and the equilibrium water content (EWC) of the PHEMA hydrogel were systematically investigated and their structure and optical property were also characterized. The experimental results showed that the optimum conditions for preparing PHEMA hydrogel are: catalyst 0.5 wt%, cross-linking additive 1.0 wt%, reaction temperature 40 degrees C, reaction time 36 h. Under the optimum conditions, the tensile strength of PHEMA hydrogel prepared is as high as 0.57 MPa, hardness of Shore A is 23.0, EWC is over 40%, and light transmittance is over 97%.

4.Third generation dual-source CT in early differential diagnosis of intracranial hemorrhage and iodinated contrast medium after endovascular treatment of acute ischemic stroke patients

Objective To investigate the value of third generation dual-source CT in early differential diagnosis of intracranial hemorrhage and iodinated contrast medium after endovascular treatment of acute ischemic stroke (AIS).Methods Totally 78 patients with AIS underwent endovascular treatment were prospectively enrolled.Dual-energy CT (DECT) examination (80 kV/Sn150 kV) of the head was performed after treatment with a third generation dual-source CT scanner.Iodine overlay maps and virtual non-contrast images were post-processed.Taking conventional brain CT images obtained 24 48 h after AIS attack as references,the accuracy,sensitivity,specificity,positive predictive value (PPV) and negative predictive value (NPV) of DECT for identifying hemorrhage were computed respectively.Results Totally 31 patients with 53 foci of intracranial hyper-attenuation were finally enrolled.Among 53 foci,26 were correctly diagnosed as hemorrhage by DECT,23 were correctly diagnosed as iodinated contrast medium,while 1 calcification was misdiagnosed as hemorrhage combined with iodinated contrast medium,3 were misdiagnosed as contrast medium which showed delayed hemorrhagic transformation.The sensitivity,specificity and accuracy of DECT was 89.66% (26/29),95.83% (23/24) and 92.45% (49/53),PPV and NPV was 96.30% (26/27) and 88.46% (23/26),respectively.Conclusion The third generation dual-source CT is available in early and accurate diagnosis of hemorrhagic transformation after endovascular treatment of AIS,which can help clinicians to adjust the subsequent treatment strategies in time.
5.The value of automatic chromosome harvester in lymphocyte micronucleus test

Objective To explore the advantages of automatic chromosome harvester in lymphocyte micronucleus test. Methods The peripheral blood of 80 radiation workers was collected, then lymphocytes were harvested with an automatic chromosome harvester and manual methods. After Giemsa stained the slides, the micronucleus of the lymphocytes was analyzed under a microscope. Results For lymphocytes from the same batch of specimens, the time of the automatic harvester (185 min) was less than that of the artificial group (350 min). The micronucleus rate (1.04‰ ± 0.63‰), micronucleus cell rate (0.89‰ ± 0.69) and micronucleus positive rate (70.0%) that obtained by the harvester were all higher than manual method (0.60‰ ± 0.68‰, 0.51‰ ± 0.55‰, 48.8%), and the difference was statistically significant (P < 0.01). Conclusion The automatic chromosome harvester can shorten the experimental operation time, reduce the workload of laboratory personnel, and protect the physical and mental health of laboratory personnel. At the same time, it can increase the detection rate of lymphocyte micronucleus, which can provide more accurate detection data for the clinic.
